Abstract

The use of e-Puskesmas is an effort to provide solutions to health service providers, namely Community Health Centers so that they can provide services to patients quickly; reporting activities to the Health Service are also faster due to the Online Reporting system, making it easier for previously manual activities to become digital, and of course save time at work. The E-Puskesmas application in Kota Metro is already running in 11 Community Health Centers. This research aims to determine the effectiveness of using the e-Puskesmas application in community health center services in the work area of the Dinas Kesehatan Kota Metro in 2023. This type of research is mixed research, namely qualitative followed by quantitative (mixed method). The population in this study determined the sources/informants using a purposive sampling technique of 20 people. This research was conducted on 6-11 August 2023. Data was collected using questionnaire filling, observation, and interview techniques. The results of qualitative research based on the results of interviews conducted with key informants at 11 Community Health Centers in Kota Metro regarding efficiency indicators in the effectiveness of using the e-Puskesmas application are less effective, this can be seen from the lack of adequate human resources in each Community Health Center which is not by their competence. The results of quantitative research based on the efficiency variable were 14 respondents (87.9%) with negative answers and 6 respondents (12.1%) with positive answers. Variable adequacy: 15 respondents (85.1%) answered negatively, and 5 (14.9%) answered positively. Alignment variable: 12 respondents (87.9%) answered negatively, and 6 (12.1%) answered positively. Responsiveness variable: 12 respondents (87.9%) answered negatively and 8 (12.1%) answered positively. Variable accuracy: 15 respondents (85.1%) answered negatively, and 5 (14.9%) answered positively. Suggestions should be made to first evaluate the puskesmas by related parties, regarding the preparation and availability of the puskesmas to implement the e-puskesmas application

Abstract

Background: Low utilization of ultrasonography (USG) examinations among pregnant women, at only 16.1% during the first antenatal care (ANC) visit and 18.7% during the last ANC visit, may hinder the early detection of pregnancy complications and contribute to the high infant mortality rate (IMR), which reached 34 cases in 2024 in Tulang Bawang Regency. This study aimed to analyze the determinants of USG examination utilization among pregnant women in Tulang Bawang Regency in 2025. Methods: This quantitative analytic study employed a cross-sectional design. From a population of 2,694 pregnant women in Tulang Bawang Regency in 2025, a sample of 267 respondents was selected using stratified random sampling from six primary health centers. Data were collected using questionnaires and secondary data sources. Data analysis included univariate, bivariate (Chi-square), and multivariate analyses using multiple logistic regression. Results: Of the 267 respondents, 141 (52.8%) adhered to ultrasound examinations. Knowledge (p=0.000), attitude (p=0.008), age (p=0.000), service availability (p=0.008), accessibility (p=0.000), and perception (p=0.013) were significantly associated with ultrasound examination adherence, whereas pregnancy risk status was not significantly associated (p=0.643). Age was identified as the dominant factor influencing ultrasound examination adherence (p=0.001; OR=3.660; 95% CI: 1.751–7.650). Conclusions: Ultrasound utilization among pregnant women was primarily influenced by age, accessibility, and knowledge. To improve ultrasound utilization as an innovation to strengthen ANC services, we recommend prioritizing screening among pregnant women aged <20 and >35 years, providing structured ultrasound education, implementing written scheduling systems, tiered reminders, and active follow-up for missed appointments. These efforts should be supported by flexible service hours and improved accessibility through scheduled transportation and mobile ultrasound services, subject to resource availability.

Abstract

The prevalence of smoking among adolescents aged 10-18 years in Indonesia continues to increase every year from 7.2% in 2013 to 9.1% in 2018. SKI data in 2023 shows that the number of active smokers is estimated to reach 70 million people, with 7.4% of them smokers aged 10-18 years. On the other hand, e-cigarette users among adolescents have increased in the last 4 years, rising from 0,3% in 2019 to 3% in 2021. The 2029 RPJMN mandates that the target prevalence of smokers aged 10-21 years is 6.9%. Meanwhile, Bappenas projects that the prevalence of adolescent smoking in Indonesia in 2030 can reach 16.96% if efforts to prevent and control smoking in adolescents are not carried out optimally. The impact of smoking behavior is not only on individual health, but also public health, potentially triggering chronic diseases, increasing health costs, and threatening the welfare of future generations, especially the Golden Indonesia in 2045. The purpose of this research is as an effort to plan a follow-up to the development of screening and efforts to quit adolescent smoking in schools that are more innovative and attractive to adolescents. This type of research is qualitative research with a case study research design using a policy alternative analysis matrix. There are 17 references used. The stages of the Systematic Literature Review are divided into 4 stages, namely Planning (designing review questions and planning methods), Data Collection (keyword search, title and abstract screening, filtering & assessment, data extraction), Analysis Stage (descriptive and thematic analysis), then ending with Synthesis (discussion). By synthesizing research results through a systematic review approach and presenting them in the form of actionable messages (policy brief and policy paper), more conclusive, comprehensive and balanced facts can be conveyed to stakeholders.

Abstract

Sepsis is a life-threatening condition characterized by organ dysfunction resulting from a dysregulated immune response to infection. Sepsis is the leading cause of death from infection globally. Blood Urea Nitrogen (BUN) serves as a marker of kidney damage, and albumin is an indicator of fluid homeostasis. The BUN/albumin ratio can be used to predict mortality in infectious diseases, making it a widely studied and effective prognostic marker for mortality in sepsis patients. Analyzing the Blood Urea Nitrogen (BUN)/albumin ratio as a predictor of clinical outcome in sepsis patients. This retrospective cohort study used secondary data from laboratory results and electronic medical records of adult sepsis patients treated in intensive care units at RSUP Dr. Hasan Sadikin Bandung from January 2022 to December 2023. Patients were identified using ICD-10 code A41.9 and a Sequential Organ Failure Assessment (SOFA) score ≥2. Blood urea nitrogen and serum albumin levels were obtained from the first laboratory examination within 48 hours after ICU admission. A total of 139 patients who met the inclusion criteria were included. Statistical analysis was performed, with significance set at p<0.05. The subjects of this study included 139 sepsis patients, with a significant difference in the BUN/Albumin ratio between those who died and those who survived. The median BUN/albumin ratio was significantly higher in sepsis patients who died (16.58) compared to those who survived (9.16), p < 0.001. The analysis of the Receiver Operating Characteristic (ROC) curve showed an Area Under the Curve (AUC) of 0.814, indicating good predictive ability for mortality. The BUN/Albumin ratio with a cutoff of ≥10.1617 had a sensitivity and specificity of 73 and 71.8%, respectively. The Relative Risk in this study was 1.77, indicating that sepsis patients with a BUN/albumin ratio of ≥10.1617 had a 1.77 times higher risk of death compared to those with a BUN/albumin ratio of <10.1617. The BUN/albumin ratio is a significant predictor of mortality for determining clinical outcomes in sepsis patients. A higher BUN/albumin ratio is associated with increased mortality in sepsis patients, therefore the BUN/albumin ratio is useful as a prognostic marker of mortality in sepsis patients in clinical practice.

Abstract

The rapid expansion of social media use in the digital era has intensified the phenomenon of Fear of Missing Out (FOMO), influencing various aspects of adolescent behavior, including physical activity patterns. Jogging has emerged as a popular and accessible form of exercise among adolescents; however, participation is often driven by social trends rather than sustained health motivation. This study aimed to analyze the influence of Fear of Missing Out (FOMO) on healthy jogging behavior among adolescents in Metro City in 2025 using the Health Belief Model (HBM) framework. A quantitative cross-sectional design was employed involving 100 adolescents aged 15–24 years selected through purposive sampling. Data were collected using validated and reliable questionnaires measuring FOMO and healthy jogging behavior and analyzed using Pearson and Spearman correlation tests. The findings revealed a significant positive relationship between FOMO and healthy jogging behavior (r = 0.252; p = 0.012), as well as between FOMO and weekly jogging frequency (ρ = 0.205; p = 0.041). Duration of social media use was positively associated with both FOMO and jogging behavior, with a strong correlation observed for jogging behavior (ρ = 0.605; p < 0.001). These results indicate that FOMO may function as a digital social trigger that encourages adolescent participation in jogging activities.

Abstract

Mental health of pregnant women is an important aspect of efforts to improve the health status of mothers and children. South Lampung Regency still faces low achievement in mental health screening of pregnant women (22.27%). This study aims to determine the factors associated with the mental health status of pregnant women in South Lampung Regency in 2025. This study employed a quantitative approach with a cross-sectional analytical design. The study population consisted of all pregnant women attending nine selected community health centers in South Lampung Regency, with a total sample of 415 respondents selected using proportional random sampling. The dependent variable, namely the mental health status of pregnant women, was measured using the Edinburgh Postnatal Depression Scale (EPDS), which has been validated in the Indonesian population and demonstrated good reliability, with Cronbach’s alpha ranging from 0.78 to 0.85, indicating satisfactory internal consistency. A cut-off score of ≥10 was applied to classify pregnant women as being at risk of depression, as this threshold is considered sensitive for community-based screening in primary health care settings. The independent variables maternal age, gestational age, residence, spousal support, frequency of antenatal care (ANC) visits, and history of mental disorders were measured using a structured questionnaire. Data analysis was conducted using univariate, bivariate (Chi-Square test), and multivariate (multiple logistic regression) analyses. The results of the chi-square test showed a significant relationship between the age of the pregnant woman (p=0.000; OR=2.755), gestational age (p=0.014; OR=1.684), place of residence (p=0.004; OR=0.544), spousal support (p=0.000; OR=0.462), frequency of ANC visits (p=0.026; OR=0.625), and history of mental disorders (p=0.000; OR=3.595) with the mental health status of pregnant women. Multiple logistic regression analysis showed that the dominant factor associated with the mental health status of pregnant women was a history of mental disorders in the family (OR=4.453).  In conclusion, there is a relationship between the age of pregnant women, gestational age, place of residence, spousal support, frequency of ANC visits, and history of mental disorders with the mental health status of pregnant women. The dominant influencing variable is a family history of mental disorders.

Abstract

Background: The Chronic Disease Management Program is an integrated proactive health service involving patients, health facilities, and BPJS Kesehatan to improve care for individuals with chronic diseases, including type 2 diabetes mellitus (T2DM). This study aimed to analyze factors influencing the utilization of Prolanis among T2DM patients in West Pesisir Regency. Methods: This quantitative study employed a cross-sectional design with 149 respondents selected through stratified random sampling. Data were collected between February and March 2025 and analyzed using Chi-square tests and multivariate logistic regression. Results: Family support (p = 0.000), illness perception (p = 0.000), perceived needs (p = 0.014), cross-sector support (p = 0.017), and health facility availability (p = 0.004) were significantly associated with Prolanis utilization. Among these, family support was the most dominant factor (OR = 10.26), indicating that patients with low family support were over 10 times more likely to show low program utilization compared to those with strong family support. Conclusion: Family support is the key determinant of Prolanis utilization among T2DM patients. Strengthening family involvement may enhance participation in chronic disease management programs.
