Abstract

Psoriasis is a skin disease that can affect all ages. Many people think that skin diseases do not endanger human life, but psoriasis can be complicated into a more serious disease. This study aims to determine the clinicopathological characteristics of psoriasis at Prof. Dr. I G. N. G. Ngoerah Central Hospital Denpasar in 2011-2021. This study used a cross-sectional design with total sampling from secondary data at the Anatomical Pathology Installation of Prof. Dr. I G. N. G. Ngoerah Denpasar. Of the 75 samples obtained, the most cases of psoriasis were psoriasis vulgaris (54.7%) which was dominated by male patients (70.7%), where most cases occurred in the age group of 31-40 years (24.0%). Most of the lesions were found locally, especially on the body area (54.7%). Of all medical record data that included family history (21.3%), all cases stated that there was no family history of psoriasis. Most of patients’ main complaints were red spots (57.3%). The majority of patients had a match between the clinical diagnosis and the clinicopathological diagnosis (73.3%). These findings can provide insight into the clinicopathological characteristics of psoriasis so that it can be detected as early as possible and given appropriate treatment. It is necessary to carry out further analytical research to look for the relationship between various variables.

Abstract

ABSTRACT CRC precursor lesions are abnormal tissue growths in the colon and rectum that are not controlled but do not have the capacity to invade surrounding tissues. Good management if delayed in the long term causes the lesion to develop into colorectal carcinoma, a malignant condition that occurs in the colon and rectum. The purpose of this study was to determine the clinicopathological description of pre-cursor colorectal carcinoma lesions at Sanglah Hospital Denpasar in 2018- 2020. The study used secondary data by cross sectional (cross sectional) with a total sampling technique using data from the patient's medical record at the Anatomical Pathology Installation at Sanglah Hospital Denpasar. The number of data obtained during the study period as patients diagnosed as precursor TRC lesions were 55 samples which were then processed using SPSS ver 26 software. The results of the study obtained a profile of the clinicopathological characteristics of pre-cursor TRC lesions at Sanglah Hospital Denpasar in 2018- 2020 the most findings were adenoma low grade dysplasia as many as 26 cases (47.3%), occurring in the age group 60-69 as many as 15 cases (27.3%), experiencing clinical manifestations of blood during defecation with a total of 18 cases (26.9%), and most commonly found in the rectum as many as 29 cases (52.7%). This finding is useful because it can provide insight into the profile of the clinicopathological characteristics of pre-cursor TRC lesions at Sanglah Hospital Denpasar in 2018-2020. Further analytical research is needed to find the relationship between various variables. Keywords: profile, precursor lesion, colorectal carcinoma

Abstract

Background: The genes cadherin 4 (CDH4), Kirsten rat sarcoma viral oncogene homolog (KRAS), V-Raf murine sarcoma viral oncogene homolog B (BRAF), and microsatellite instability (MSI) each play a role in the development of colorectal cancer (CRC). CDH4 gene is located in chromosome 20q13.33 and its amplification or gain is the earliest mutational event found in the majority of CRC and colon polyps. This study aimed to identify copy number alteration in the CDH4 gene and its association with the clinical profile of CRC patients, including gender, age, tumor location and differentiation, frequency of BRAF and KRAS mutations, and MSI status. Methods: The DNA was extracted from 50 tumors and adjacent normal tissues based on the manufacturer’s instructions. Detection of MSI status was carried out by pentaplex polymerase chain reaction (PCR) and PCR products were size separated by capillary electrophoresis (CE) using an ABI 310 Genetic Analyzer. Both KRAS and BRAF mutations were identified by PCR and sequencing while CDH4 copy measurement was measured using TaqManTM Copy Number Assay. Results: Our findings showed 22 (44%) samples with no changes in the copy number of CDH4 gene. Interestingly, 21 (42%) cases had an amplification of the CDH4 gene or CDH4 gene gain, and seven (14%)  cases decreased in CDH4 gene expression or CDH4 gene loss. We found an association between changes in the CDH4 gene and gender (p=0.001). However, there was no association between changes in the CDH4 gene and age (p=0.979), tumor location (p=0.145) and differentiation (p=0.648), the frequency of BRAF (p=0.171) and KRAS mutations (p=0.184) and MSI status (p=0.923). Conclusions: Copy number alteration in CDH4 gene in CRC patients and this alteration is significantly associated with gender. Further studies with a larger number of samples are essential to confirm this result and to identify the cause of CDH4 copy number alteration and its biological significance.

Abstract

Introduction: GISTs usually occur in older adults with a median age of about 60–65 years. The incidence rate between men and women is equal (1:1). The most common location of GISTs is the stomach which accounts for 60%, followed by the jejunum and ileum around 30%, duodenum 5%, and colorectal 5% and a small percentage of cases occur in the esophagus, appendix, gallbladder, and extraintestinal areas such as the mesentery, omentum, and retroperitoneum. Case Presentation: A 59-year-old male, suffered from a lump under the umbilicus 3 months before admission to the hospital. The results of the ultrasound examination showed a mass in the superior area of the bladder, and the mass did not appear to infiltrate the intestines, bladder, umbilicus, and surrounding organs. The biopsy tissues showed spindles and round-to-oval morphology with eosinophilic cytoplasm. Mitoses 65/50 High Power Field In addition, immunohistochemistry examination showed positive stains in DOG 1, CD117, SMA, CD 34, and negative for S100.Conclusion: Extragastrointestinal stromal tumor is a rare tumor. Based on, clinical, imaging, histopathological, and immunohistochemical examination, this case was diagnosed as an extragastrointestinal stromal tumor, spindle cell type.

Abstract

Introduction: Leprosy is a chronic disease caused by Mycobacterium Leprae. According to the WHO, leprosy is one of the communicable diseases that affect the skin, peripheral nerves, and upper mucosa, and it can cause physical disabilities, if not properly handled. This study aims to determine the clinical and pathological profile of leprosy at Prof. dr. I. G. N. G. Ngoerah Central Hospital Denpasar, in 2017 - 2022. Metods: The study used a descriptive research design with total sampling from secondary data obtained from the Pathology Anatomy Laboratory of Prof. dr. I. G. N. G. Ngoerah Central Hospital Denpasar. The sample collected during the study period included 63 samples of patients diagnosed with leprosy, which were then processed using SPSS software version 26. Result: The highest number of cases was found in patients age group of 19-64 years, accounting for 55 cases (87.3%), predominantly in male patients with 36 cases (57.1%). The most common main complaint was red patches, with 43 cases (72.8%) presenting erythematous skin lesions, specifically plaques, in 24 cases (42.8%). Conclusion: Based on classification, leprosy was more frequently grouped into two classifications: WHO classification and Ridley Jopling classification. According to the WHO classification, the majority of patients were found to have the PB type, accounting for 35 cases (55.6%), while based on the Ridley Jopling classification, the BT type was more prevalent, with 21 cases (33.3%). This study is beneficial as it provides insights into the clinical and pathological profile of leprosy. Further analytical research is needed to explore the relationships between various variables
