Abstract

Abstract: PT Pertamina (Persero) MOR V have responsibility to some areas, such as East Java, Bali, NTB, NTT, and East Timor. This company have been established information and system technology services. IT Marketing and Trading has responsibility for 6 information and system technology services. Maturity level for the last audit process were good enough, but there are problems that exists. The problem that happened in these services are lack of response to customer in respond to customer’s requests and lack of ERP and non-ERP services. Result from these problems is delays in management services called lack of services. The result is customer’s trust to IT M&T could be reduced due to this lack of service. Because of that this company needs to do continuous information and system technology audit that based on the 2014 audit which has been done by this company. Hopefully this audit can be used to know the cause from these complaint. This audit used COBIT 4.1 standard because control for problems existed in this company have been identified using COBIT 4.1 so this company can continue to implement and monitor the accepted main goal has been reached or not. Keywords: audit, system and information technology audit. Abstract

Inclusive school is a school which has regular students and also disability students. In Surabaya there were 70 inclusive schools, With so many the number of schools cause dinas difficulty in showing data are sprouting the capabilities of students inclusive , know the development of the total students inclusive. For teachers reporting documents to cause teachers must commutes collecting the document to Dinas Pendidikan. Based on of problems that emerged, can be given a solution that education authorities need an application that can assist in the reporting process of teaching and learning activities in schools and recaps in knowing the developmental ability of students inclusive. This program designed web based to solve problem. This application is designed Based on the results of the test system by using the black box and white box that has created the application can solve problems that occur by displaying developmental ability students - inclusive, is able to determine the amount of growth students - inclusive, helping teachers in accelerating the process reporting. Abstract

Abstract: The problem that occurred was at the time of demand for goods, which requests approval is done by signing the request form, so it requires the presence of the person concerned, but that happens the person concerned does not always be in place, as well as the absence of a reporting system requests the purchase of goods, supplies requests and stock supplies in warehouses that serve as a reference in the purchase of goods and supplies data summary of all the divisions that still have to be calculated again by the division of General Administration. In the research phase using System Development Life Cycle. The trials of the system and user involving several divisions, based on the results of such trials demand for information systems web-based purchases in STMIK STIKOM Surabaya able to produce design information systems that can manage the demand for the purchase of goods with a digital signature as the authentication approval as well as part of General Administration to determine report goods purchase request, request supplies, stock supplies in warehouses and supplies data summary of all the divisions.

Abstract:PT Qta Travelindo Jaya Surabaya is a travel agency company that has been supported by the application, but the application that is not met expectations, for the development of applications based only on immediate needs with different platforms. This resulted in no integration of data between parts, occurs redundancy of data, not optimal application usage, budget development of Information Systems and Information Technology (IS/IT) is not controlled, and the disjointed IS/IT with business objectives.In order to overcome the constraints mentioned above, it is necessary to Strategic Planning IS/IT. In drawing up the Strategic Planning IS/IT used method of Ward & Peppard so that IS/IT can be aligned with its business strategy. In this method the synchronization between the company's objectives with the aim of IS/IT. Besides Ward & Peppard method, there is a method of analysis or ancillary aspects, as follows: Strenghts, Weakness, Opportunities, and Threats (SWOT), Finance, Marketing, Operations, Human Resources, Industrial Markets, Competitors, Political Regulations, and McFarlan.The results of the Strategic Planning in the form of IT/IS document. This document contains the Application Portfolio namely Sales Information System (Invoice), Mobile Web Application Airlines plane and hotel, and webstite Gotravelindo. Documentation is made for three (3) years of the coming period and will be used as a recommendation IS/IT development be focused, IS/IT strategy aligned with business strategy, and can determine the competence of the resources needed in the IS/IT Qta Travelindo PT Jaya Surabaya

Abstract

Abstract: PT Goldfindo Intikayu Pratama is a company who works in furniture field. The company’s production is based on the customer orders. The average of the company’s transaction reaches about 3000 products, yet 31% of them delayed. It is caused by several things, which are the absence of scheduling method, the machine’s functions are not optimized yet by the company, and also order data recording with production scheduling are not integrate yet. This research results an application that can compare four methods of scheduling. The four methods are EDD, FCFS, SPT and LPT. The comparison of methods is aimed to get the best method that will be used by the company for scheduling. It requires some stages to make the application, the first is requirements analysis, system design, implementation, and the last are integration and testing. From this research, its result is about 96% which means that the application is feasible to use, and the user trial results are about 91% which means that this application can be accepted by the company.

Abstract

Hospitalization is essential in Bima Hospital. Inpatient services are service activities to patients in the hospital, occupying a bed for the purposes of observation, visit doctor, administration of fluid and drugs. In the period 2000 through to 2012 Party in particular parts of the hospital inpatient having problems processing of patient data.  Search history of patient data, frequent loss of patient data. Cost overruns the process of final payment patient. Another problem faced by the hospital is difficult to make regular monthly report.To overcome the obstacles in the hospitalization, the hospital needs  information system  to registration patient , the estimated cost of care, loss of medical record patient and can be make report for executive, patients and Kemenkes. With this requirement it needs to make hospitalization information system in the RSUD Bima.

Abstract

Ard Motor is a company engaged in the sale of car spare parts. The company offers customers two ways of payment are cash payments and loan payments. In the process of granting credit, the company assesses the credit worthiness from the customer’s debt, the amount of credit purchases will be proposed, and the business value. But the problem for the company is many customers are already overdue its payment. This is due partly intentional factor customers in payment. Besides making lending decisions are subjective only give a decision worthy or not.Overcoming these problem, therefor the application to determine credit worthiness was made. The parameter that used is the number of purchase transactions, the amount of debt, purchase frequency and value of character to assess the customer, In order for credit granting ratings become not subjective therefor in this application fuzzy logic methods are used to support the decissions. The results from evaluation it can be concluded that the application can generate a percentage value of credit worthiness so it can be more accurate, and the application can determine the priority lending who was worthy so that companies can provide credit payment to the right customers.Keywords : Credit, Feasibility Assessment of Credit Applications, Fuzzy Logic

Abstract

PT Goldfindo Intikayu Pratama is a company engaged in the production of furniture, such as furniture for household and office supplies. The company's main processes namely, marketing and sales, purchasing, and production. Problems faced by the company is the systems and information technology (SIT) that failed to implement because it can not help in running the business processes of the company, so it can not align business processes with existing SIT. The failure in the implementation of the SIT cause the company suffered losses in developing SIT. Efforts are needed to improve the situation is to make strategic planning SIT, it aims to be able to align the company's business processes with SIT. Therefore, this study uses the stage Anita Cassidy and using analytical methods such as value chain analysis, SWOT, Five Forces Model, Portfolio McFarlan, Owners Estimate that can help identify and analyze SIT that is consistent with the company's business. The results of this study in the strategic planning documents STI which includes 12 SIT projects, the total estimated cost of Rp 192 900 061 SIT, SIT portfolio, there are directives SIT developments and there is a SIT roadmap for the next 3 years starting from 2017 to 2019.

Abstract

Abstract: Mother and child hospital is a hospital that provide health services and medical services for the surrounding community. Health services and medical provided to the public is not optimal, this is because lack of the number of health workers that led to the average waiting time patients who do not meet standard prescribed by health department. The process of planning needs analysis of health workers in RSIA currently only reference to the rules of health minister regulation no. 340/MenKes/PER/III/2010 about a minimal amount of health to be owned and not consider their workload. While ideally in determining the amount of health workers have to consider their workload. It takes an appropriate solution that is using a method of WISN (Workload Indicator Staff Need) in the process of planning needs analysis of health workers based on their workload and can give a total number of optimal health workers needed.                 Results from the trial of research and evaluation of this final project research is that produce an application of planning needs analysis of health workers using methods WISN (Workload Indicator Staff Need) which can facilitate the user in determining the amount of planning needs of health workers required
