Abstract

Osteoarthritis (OA) is a degenerative disease of the joints that involves the cartilage, joint lining, ligaments and bones causing pain and stiffness in the joints. The causes of osteoarthritis are multifactorial, including gender, obesity, genetic factors, race, family history, joint injury, strenuous physical activity, work, and repetitive movements of the same joints. Asthma is a chronic respiratory disease based on chronic inflammation which results in obstruction and hyperreaction of the respiratory tract as a result of the interaction between genetic, host, and environmental factors. Both osteoarthritis and hypertension are diseases that must be managed comprehensively because they are chronic. To apply the principles of family doctor medicine according to the problems found in patients in various aspects and carry out a holistic and comprehensive management that includes patient-centered, family approached, and community-oriented based on evidence-based medicine. Patient Mrs. I, 62  years, with complaints of knee pain since about 5 day ago. Knee pain is felt especially when the patient does strenuous activities or when in a squatting position and then stands up, and when the patient goes up and down stairs, but the complaint improves when the patient rests.  The patient was diagnosed with osteoarthritis.  The patient has a history of asthma since childhood and asked for asthma medication because he was worried that in the rainy season with cold weather his asthma could recur. Several factors can affect the patient's condition, namely internal and external risk factors. In this case, diagnosis and treatment have been carried out according to the latest theory and journals. After the intervention, there was a decrease in clinical symptoms and an increase in the knowledge of patients and their families.

Abstract

Depression can be caused by a variety of factors, including genetics, mental health, personality, mental health, unemployment, and intelligence. Another common cause of depression in children is lack of family support and proper encouragement. The problem solving process is a useful skill because it is a process of managing mental illness (both external and internal) and requires real effort and effort in the form of suppressing emotions. The problem solving process is the body's response to change, the need to solve problems, and internal and external damage to the mind. Basically, parents have a big role in controlling the growth and development of each child from birth to adulthood. Therefore, parents who ignore their children's needs, especially if the child experiences mental disorders (depression), leave the child empty and without support. Parents can play an advocacy role if they know what to do, including understanding the problems their children face, managing family relationships, and making contributions. a person's level of self-confidence. Mental illness is not something to be ashamed of or hidden, but it is an illness that requires proper treatment. Just like wounds on the hands and feet, wounds in the heart must also be healed. Parents promote good career opportunities for teenagers. The higher the parents' education, the easier it is to help children who are experiencing depression. Supporting children with depression is more than just raising awareness.

Abstract

Type 2 diabetes mellitus (T2DM) is a metabolic disorder characterized by impaired insulin secretion or hyperglycemia due to the inability of pancreatic β-cells to compensate for increased glucose levels in the blood. The pathophysiology of T2DM may result from the interaction of environmental and genetic factors. A person who has a family history of T2DM will experience genetic mutations that cause impaired insulin secretion and sensitivity. The genetic inheritance of the child is based on the parent alleles of both parents including some genes that have mutations. There are three types of genetic inheritance mechanisms described by parent of origin effects (POE), namely genomic imprinting, mitochondrial inheritance, and maternal intrauterine effects. The mechanism of impaired insulin secretion and sensitivity in someone with a family history of T2DM during puberty, which is also triggered by stress and hormonal imbalance, results in insulin resistance, especially significant when entering adolescence and supported by a family history of T2DM. In children with a family history of T2DM, it was found that pancreatic β-cell compensation was 30% lower compared to children without a family history of T2DM.

Abstract

Gout arthritis is an increase in blood uric acid levels above normal which causes an inflammatory reaction in the joints. Prolonged gouty arthritis can damage the joints, soft tissues and kidneys. Dyslipidemia is a common metabolic condition characterized by a level of cholesterol in the plasma that exceeds normal values. Total cholesterol level is correlated as a strong risk factor for cardiovascular disease. Therefore, application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient-centered and family problem-solving framework. so that treatment goals can be achieved, prevent complications that may occur, and improve the patient's quality of life. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from the patient's medical record. A Female patient, 55 years old, came with complaints of pain in both joints of the hands and feet and the back of the neck felt sore and tense. The patient has a history of high uric acid since the age of 36 years. In the knowledge assessment, the initial score before the intervention was 70 (patient), 50 (patient's family) and after the intervention became 100 (patient), 90 (patient's family). In terms of uric acid levels, uric acid levels decreased from 6.8 mg/dL to 6 mg/dL and blood cholesterol from 221 mg/dL to 188 mg/dL. In the aspect of diet there is a change in the type of food that is appropriate. In the aspect of physical activity there is a change in physical activity, sports 30 minutes 4 times a week. Enforcement of diagnosis and management of these patients has been done holistically, patient centered, family approach and community oriented based on several theories and the latest research. In the process of behavior change, the patient has reached the trial stage.

Abstract

Aging is a natural process experienced by all living beings, marked by physiological, psychological, and social changes in individuals over the age of 60, as well as a decline in functional capacity and structural changes in the body. Many elderly individuals reside in social care facilities, which, although providing care for the neglected or less fortunate elderly, often face negative views from society, especially those that adhere to strong family values, which can affect their quality of life. This study examines the culture and quality of life of elderly individuals living in social care facilities through a systematic review. Data were gathered from relevant articles using related keywords from various databases. The findings of this study indicate that elderly individuals residing in social care facilities tend to have lower quality of life, due to limited social interactions and health-related issues. Factors influencing the quality of life of the elderly include physical, social, psychological, and environmental aspects, which are often affected by health conditions, physical limitations, and the lack of social interactions in social care facilities. On the other hand, cultural values also play a role in shaping societal views of social care facilities, particularly in regions with strong family-oriented values. Recommendations to improve the quality of life for the elderly include increasing attention to both their physical and psychological health, while ensuring that cultural values are respected in elderly social care services.

Abstract

Patient satisfaction is a key indicator in assessing healthcare quality, especially in hospital pharmacy services which play a vital role in treatment effectiveness. Hypertension is a major chronic illness requiring strict medication adherence to prevent complications such as heart disease, stroke, and kidney failure. This review examines the relationship between outpatient hypertension patients' satisfaction with pharmacy services and their medication adherence. Findings highlight that satisfaction dimensions such as reliability, empathy, responsiveness, tangibles, and assurance significantly shape patient perceptions. Higher satisfaction correlates with increased adherence. Continuous education, empathetic interaction, efficient service systems, and clear pharmacist communication are essential. Quality pharmacy services not only improve patient outcomes but also support long-term hypertension control.

Abstract

Intrauterine infection is an inflammatory condition involving various components of pregnancy, including the amniotic fluid, placenta, fetus, amniotic membranes, and decidua. One of the main mechanisms contributing to this infection is ascending infection from the lower genital tract, in which microorganisms such as Ureaplasma, Escherichia coli, and Streptococcus agalactiae ascend into the amniotic cavity. Imbalances in the vaginal microbiota, such as in bacterial vaginosis (BV), exacerbate the risk of ascending infection, by replacing the protective Lactobacillus dominance with anaerobic pathogens. This condition can lead to various obstetric complications, including preterm labor, preterm rupture of membranes, and increased maternal and neonatal morbidity and mortality. Studies have shown that changes in the vaginal microbiota can have significant impacts on reproductive health. Processes such as menstruation, sexual activity, and invasive medical interventions can alter the composition of the microbiota, affect vaginal pH, and reduce the dominance of Lactobacillus, thereby increasing the risk of intrauterine infection. In addition, uterine pumps play a role in the transfer of microorganisms from the vagina to the uterine cavity, further increasing the risk of infection. With the advancement of molecular technology, the understanding of vaginal and uterine microbiota is growing, especially in relation to the mechanism of intrauterine infection. Microbiota imbalance not only contributes to inflammation, but also alters immunological signaling pathways, reduces the barrier function of the endometrial epithelium, and allows pathogens to cross the placenta. Therefore, microbiota-based approaches, such as the use of Lactobacillus probiotics, as well as education about reproductive health, are potential strategies in preventing complications associated with intrauterine infection.

Abstract

The use of personal protective equipment (PPE) is an essential effort in preventing occupational health and safety risks, especially for farmers working in the plantation sector. This study aims to analyze the use of PPE among oil palm farmers in Simpang Pematang Subdistrict, Mesuji Regency. The research employs a cross-sectional approach involving 105 oil palm farmers selected through purposive sampling. Data were collected using questionnaires and analyzed univariately. The findings show that the level of compliance with PPE usage is still low, with only 48.6% of farmers consistently adhering to PPE usage procedures. The use of specific PPE items such as masks and protective glasses is particularly low, at just 13.3% and 5.7%, respectively. The main barriers to PPE use include farmers' lack of understanding about its importance, limited availability of PPE, and misconceptions about its comfort. This study concludes that strategic efforts are needed to improve PPE compliance through education, outreach programs, and the provision of adequate PPE.

Abstract

Hyperglycaemia or high levels of glucose in the blood is the hallmark of diabetes mellitus. Diabetes mellitus type 2 (DMT2) significantly impacts bone health, leading to a condition known as diabetic bone disease, characterized by decreased bone mineral density and an increased risk of fractures. This condition arises from metabolic disturbances triggered by hyperglycemia, insulin resistance, and inflammatory factors that compromise bone structure. Research indicates that over 35% of DMT2 patients experience reduced bone density, with approximately 20% meeting osteoporosis criteria. The pathophysiology involves several mechanisms, including the accumulation of advanced glycation end-products (AGEs), which hinder bone formation and increase resorption. Insulin deficiency further exacerbates this issue by impairing osteoblast function and reducing the synthesis of key bone matrix proteins like collagen and osteocalcin. Treatment options such as metformin have shown potential in improving bone health, while other medications like sulfonylureas and SGLT-2 inhibitors may have detrimental effects on bone density. Therefore, careful management of diabetes is essential to mitigate the risks of osteoporosis and fractures in patients with DMT2. Ongoing research is needed to deepen the understanding of diabetic bone disease and optimize treatment strategies to protect bone health in these patients.

Abstract

Low Back Pain (LBP) is one of the most common musculoskeletal disorders encountered in primary healthcare settings. The etiology of LBP is diverse but generally associated with poor posture, heavy or repetitive physical activity, degenerative processes, and psychosocial factors. In Indonesia, the prevalence of LBP is estimated to range from 7.6% to 37% of the general population, with a high proportion linked to physically demanding occupations. Diagnosis of LBP involves a comprehensive history-taking, systematic physical examination, and consideration of supporting investigations if red flags such as fractures, infections, or malignancies are suspected. Identifying the pain source, duration, characteristics, and distribution is crucial in establishing the diagnosis and determining the underlying cause. In clinical practice, a holistic and evidence-based approach by primary care or family physicians is highly recommended. This should include the assessment of internal risk factors such as advanced age and degenerative disease, as well as external factors like a history of strenuous work and physical inactivity. The case of Mr. S, a 73-year-old male, illustrates chronic LBP with worsening symptoms over the past month. The pain radiated to the right buttock and lower limb, leading to functional impairment and limitations in daily activities. A thorough evaluation using the biopsychosocial model and multidisciplinary management—including pharmacotherapy, patient and family education, targeted physical exercises, and lifestyle modifications—resulted in significant symptom improvement and increased patient awareness of the condition.
