Abstract

Autism Spectrum Disorder (ASD) is a complex neurodevelopmental disorder, progressive disruption of brain development and synaptic connections, which causes impaired communication and behavioral disorders. However, the etiology is unknown. A damaged perinatal environment causes chronic nerve inflammation in genetically susceptible children of ASD, dysregulation of the immune system, which leads to ASD. The ratio of Neutrophil to Lymphocyte (NLR) as one of the markers of inflammation is frequently associated with inflammation-related psychiatric diseases and is potentially used as a diagnostic instrument. A cross-sectional study was carried out from June to November 2019 by using medical records of Outpatients with growth and development disorders and ASD and involving 69 children. The diagnosis of ASD was confirmed by the fifth edition Diagnostic and Manual of Mental Disorders criteria (DSM-V). A routine hematology test was carried out with a flow cytometry method. Diagnostic tests were performed in 43 children with developmental disorders (mean NLR 1.01±0.07) and 26 ASD children (mean NLR 1.02±0.06), with a cut-off value of 0.68. Sensitivity, specificity, positive predictive value, negative predictive value and accuracy of NLR were 84.62%; 27.9%; 41.51%; 25%; and 50.2%, respectively. Neutrophil to lymphocyte is sensitive to diagnose ASD but its accuracy is low. Immune dysfunction and complex neurodevelopmental disorders that are affected by chronic inflammation play a role in the occurrence of ASD. Other inflammatory markers are needed to improve the accuracy of laboratory tests to diagnose ASD.

Abstract

Background: Fasting is the voluntary act of abstaining from food, drink, or both, as well as from anything that breaks the fast for a certain time. In medicine, fasting has an important role in various diseases and health. Several studies have assessed the benefits of fasting in various physiological and pathological conditions, one of which is an appetite in obese and non-obese people. Objective: Writing an article in the form of a systematic literature review aims to analyze the effect of various fasting methods on appetite. Method: Systematic literature review method by taking 19 international journals, according to the specified keyword criteria. Results: From the results of the systematic review analysis, it was found that fasting affects subjective appetite by changing appetite scores and objective appetite by changing appetite regulation parameters such as leptin, ghrelin, PYY, adiponectin, and neuropeptide Y (NPY). Conclusion: It was concluded that fasting had positive benefits on appetite in both obese and healthy individuals. This systematic literature review is expected to be a reference for future researchers and further research can be carried out using a more varied literature search database.

Abstract

Background Chronic kidney disease (CKD) has become a global burden on the healthcare system and significantly impacts the quality of life of children with the condition. Objective To assess quality of life in children with CKD as well as its relationship with sociodemographic, medical, and psychosocial factors. Methods This cross-sectional analytic study was conducted from June to November 2021 at Dr. Moh Hoesin Hospital, Palembang. Children with CKD aged 2–18 years were included by consecutive sampling. Parents and patients were asked to complete the PedsQL™ generic score scale version 4.0 questionnaire. Results We assessed quality of life in 112 children with CKD from parents’ and children’s reports in the PedsQL™ questionnaire. Physical and emotional parameters had the lowest scores. Based on parental reports, quality of life was significantly associated with disease severity (P=0.002), behavioral disorders (P=0.007), and sleep disturbances (P=0.001). Based on the children’s reports, the factors significantly associated with quality of life were anemia (P=0.044), sleep disturbances (P=0.024), and behavioral disorders (P=0.002). Almost one-third of children with CKD had general impairment of quality of life, both from parental reports (32.1%) and children’s reports (33.0%). Conclusion Disease severity, anemia, sleep disturbance, and behavioral disorders were all associated with poorer quality of life in children with CKD.

Abstract

Background Systemic lupus erythematosus (SLE) is a chronic autoimmune disease that affects multiple organ systems, including the central nervous system. Depression is one of the neuropsychiatric manifestations of juvenile SLE . Objective To estimate the prevalence of depressive disorders in juvenile SLE and identify its potential risk factors. Methods This cross-sectional study was conducted in juvenile SLE patients at Dr. Mohammad Hoesin General Hospital, Palembang. Sociodemographic data and medications were recorded. Disease activity of SLE was assessed using the Systemic Lupus Erythematosus Disease Activity Index (SLEDAI). Physical activity was measured using the Physical Activity Questionnaire for Children (PAQ-C) and the Physical Activity Questionnaire for Adolescents (PAQ-A). All subjects were screened for depression using the Childhood Depression Inventory (CDI) questionnaire. Multiple linear regression analyses were used to determine risk factors for depressive disorders. Results We included 72 patients, of whom 67 (93.1%) were female. Mean age of the patients was 12 years 4 months (SD 2 years 6 months); most (67; 93.1%) were 10-18 years of age. Depression was found in 24 patients (33.3%). SLEDAI scores of ?3 (flare) were found in 54 patients (75%) and low physical activity was found in 62 (86.1%) patients. The prevalence of depression based on the Children's Depression Inventory was 33.3%. A SLEDAI score of ?3 and low physical activity were significantly associated with depression (P=0.009 and P=0.025, respectively). On multiple linear regression analysis, only SLEDAI score of ?3 remained significantly associated with depression (P= 0.017; OR 12.6; 95%CI 1.6–101.7). Gender, age, family economic status, father's education, mother's education, family history of depression, and duration of illness were not associated with depression. Conclusion A SLEDAI score indicating flare (?3) and low physical activity are significantly associated with an increased risk of depression. A SLEDAI score is an independent risk factor for depression when all other significant risk factors are considered.

Abstract

Beberapa penelitian telah dilakukan untuk mengevaluasi kadar 25(OH)D pada pasien penyakit Parkinson. Namun, hasil penelitian ini tidak konsisten. Penelitian ini melakukan meta-analisis terhadap kadar 25(OH)D pada pasien dengan penyakit Parkinson. Database PubMed digunakan untuk melakukan pencarian literatur secara sistematis. Meta-analisis dilakukan dengan menggunakan model efek acak untuk menghitung perkiraan efek gabungan. Tujuh belas artikel dimasukkan dalam penelitian ini. Meta-analisis mengungkapkan bahwa pasien dengan penyakit Parkinson memiliki kadar 25(OH)D yang lebih rendah dibandingkan dengan kontrol. Temuan dari artikel prospektif dan retrospektif sebagian besar konsisten. Besarnya respon nocebo dalam uji coba terkontrol acak yang dirancang paralel pada penyakit Parkinson sangat penting dan harus dipertimbangkan dalam interpretasi hasil keamanan dan dalam desain dan interpretasi uji klinis masa depan.

Abstract

Child development disorders can affect the next stage of growth and development and can have an impact on the future. Good and proper care for children who are in the growth and development period will help children to grow healthy and be able to achieve their optimal abilities so that they can contribute better to society. The main objectives of this community service are to improve the skills of Early Childhood Education (ECE) teachers to detect early growth and development disorders, reduce the number of advanced growth and development disorders, and optimize child growth and development with early intervention. The method of service carried out in this activity is in the form of training on the use of the PRIMAKU application for 1000 ECE teachers in Palembang City and assessed using pre-test and post-test questionnaires. The results of the counseling showed an increase in results in two benchmarks, an increase in participant knowledge based on the results of the pre-test and post-test correct answers by 17.03%. Furthermore, it is expected that PAUD teachers who have attended the training can apply the use of the PRIMAKU application for early detection of growth and development disorders or delays in early childhood.

Abstract

Obesitas pada remaja merupakan masalah kesehatan yang semakin meningkat, dengan dampak serius terhadap metabolisme dan risiko penyakit kronis. Puasa Ramadan diketahui dapat mempengaruhi pola makan dan metabolisme energi, tetapi pengaruhnya terhadap kadar Neuropeptida Y (NPY) sebagai regulator nafsu makan masih belum banyak diteliti, terutama pada remaja obesitas dan overweight di Kota Palembang. Penelitian ini merupakan studi observasional kohort yang melibatkan 80 remaja laki-laki obesitas dan overweight yang melakukan puasa dan kelompok kontrol yang tidak puasa. Pengukuran kadar NPY dilakukan sebelum dan setelah puasa Ramadan menggunakan metode ELISA, sementara nafsu makan subjektif diukur dengan kuesioner Adult Eating Behaviour Questionnaire (AEBQ). Analisis data dilakukan menggunakan uji statistik univariat dan bivariat. Hasil penelitian menunjukkan bahwa puasa Ramadan secara signifikan meningkatkan food avoidance dan menurunkan food approach pada kelompok obesitas dan overweight (p<0,05). Namun, tidak ditemukan perubahan bermakna pada kadar NPY sebelum dan setelah puasa Ramadan (p>0,05). Simpulan yang didapat adalah puasa ramadan memiliki efek signifikan terhadap pola nafsu makan subjektif pada remaja obesitas dan overweight, tetapi tidak secara signifikan mempengaruhi kadar NPY. Temuan ini menunjukkan bahwa puasa Ramadan dapat menjadi intervensi awal yang baik dalam mengubah kebiasaan makan, meskipun mekanisme molekuler yang mendasari efek ini memerlukan penelitian lebih lanjut.

Abstract

ABSTRACT. Folate binding protein (FBP) is a protein in breast milk that plays a role in the regulation and bioavailability of folic acid. In contrast to cow's milk FBP, information about breast milk FBP is still limited. This research aims to determine the isolation and purification methods of breast milk FBP and the molecular weight of breast milk FBP. The sample in this study was 1000 mL of breast milk. Breast milk was prepared in several stages to yield whey. Isolation and purification of FBP from whey were carried out in stages, salting-out, ion exchange chromatography, and affinity chromatography. Whey salting-out with 95% saturation of ammonium sulfate could precipitate folate-binding proteins. This precipitate showed three peaks on DEAE chromatography. Peak II DEAE 95% was thought to be a negatively charged folate-binding protein. Peak II DEAE 95% also showed the presence of two peaks on affinity chromatography. It was believed that Peak II AF 95% was a pure folate-binding protein. Peak II AF 95% showed the presence of a single band on SDS-PAGE and western blot. This indicated that the folate-binding protein was 100% pure. FBP can be isolated from breast milk by the salting-out method using 95% ammonium sulfate, DEAE chromatography, and affinity chromatography. FBP from breast milk has a molecular weight of approximately 37 kDa. The final level of FBP isolated from breast milk is approximately 0.022 mg/mL. The successful isolation of FBP from breast milk provides an opportunity to use it to understand the clinical role of FBP in increasing folic acid levels in both breast milk and infant serum, as well as to develop methods for determining folic acid levels in these fluids.
