Abstract

Abstract Donggala district had successfully conducted Transmission Assesment Survey–1 despite two children found positive for Brugia malayi. This study aimed to determine the progress of the filariasis program in Donggala District, Central Sulawesi. It was conducted in Kabonga Kecil, Banawa Sub-District and Sabang, Dampelas Sub-District from February to November 2017. Data collection included finger blood surveys, detection of Brugia malayi DNA, mosquito surveys, and in-depth interviews. From 638 people tested for finger blood survey, none of them were positive for microfilaria. Twenty children were tested for Brugia malayi DNA and the results were negative. A total of 2.978 mosquitoes were caught from mosquito surveys which identified as Aedes, Anopheles, Armigeres, Culex, Mansonia, Aedomyia, Uranotaenia, and Coquillettidia. PCR examination results showed all mosquito negative for Brugia malayi. A comprehensive and integrated surveillance strategy with other programs that are cost-effective and sustainable must continue to be carried out therefore the elimination of filariasis in the Donggala district can be achieved. Abstrak Kabupaten Donggala lulus Transmission Assesment Survey-1 meskipun masih ditemukan dua anak positif antibodi Brugia malayi. Penelitian ini bertujuan untuk mengetahui capain program filariasis di Kabupaten Donggala, Provinsi Sulawesi Tengah. Dilaksanakan di Kelurahan Kabonga Kecil, Kecamatan Banawa dan Desa Sabang, Kecamatan Dampelas pada bulan Februari – November 2017. Kegiatan meliputi survei darah jari, deteksi DNA Brugia malayi, survei nyamuk dan wawancara mendalam. Sebanyak 638 masyarakat diperiksa darahnya hasilnya seluruhnya negatif microfilaria. Sebanyak 20 anak diambil sampel darah untuk diperiksa deteksi DNA Brugia malayi hasilnya negatif. Nyamuk tertangkap sebanyak 2.978 nyamuk dari genus Aedes, Anopheles, Armigeres, Culex, Mansonia, Aedomyia, Uranotaenia, dan Coquillettidia. Hasil pemeriksaan PCR menunjukkan seluruh nyamuk negatif Brugia malayi. Strategi surveilans yang comprehensif dan terintegrasi dengan program lain yang cost-effective dan berkesinambungan harus terus dilakukan agar eliminasi filariasis di kabupaten Donggala dapat tercapai.

Abstract

ABSTRACT First Transmission Assessment Survey (TAS-1) has been carried out successfully in Donggala District. The success was supported by epidemiological aspects, including data on the presence of vectors and the environment that is potential as a breeding places for mosquitoes. In order for Donggala District to achieve filariasis elimination formally, it is important to determine the existence of vectors and itspotential environment so as to continuously control them instead of other epidemiological interventions. A cross-sectional study was conducted to determine the presence of post-TAS-1 vector in Donggala District Activities include mosquito surveys and environmental surveys in two selected locations, namely Kelurahan Kabonga Kecil, Kecamatan Banawa and Sabang Village, Kecamatan Dampelas Donggala District. The results showed that 2,978 mosquitoes were caught from the genera Mansonia, Culex, Aedes, Anopheles, Armigeres, Uranotaenia, Coquilettidia and Aedomvia, the results of PCR examination showed mosquitoes were negative Brugia malayi. The mosquitoes” habitats were tree holes, ponds, rice fields, used goods, post -mining excavations, rivers, waterways, puddles, swamps, ponds, dug holes, springs, boats, water reservoirs, used tires, wells, coconut shells, and used cans. Monitoring and evaluation of program implementation by the local government, including routine vectors and environmental monitoring, must be continued as to maintain the elimination status of filariasis in Donggala District. ABSTRAK Transmission Assessment Survey pertama (TAS-1) telah dilakukan di Kabupaten Donggala dan dinyatakan lulus. Keberhasilan didukung oleh aspek epidemiologi di antaranya adalah data keberadaan vektor dan lingkungan yang potensial sebagai tempat perkembangbiakan nyamuk. Agar Kabupaten Donggala mencapai eliminasi filariasis maka keberadaan vektor dan lingkungan yang potensial dapat dikendalian disamping intervensi terhadap aspek epidemiologi lainnya. Studi potong-lintang dilakukan untuk mengetahui keberadaan vektor paska TAS-1 di Kabupaten Donggala. Kegiatan meliputi survei nyamuk dan survei lingkungan di dua lokasi terpilih yaitu Kelurahan kabonga Kecil, Kec. Banawa dan Desa Sabang, Kec. Dampelas Kabupaten Donggala. Hasil penelitian menunjukkan bahwa nyamuk tertangkap sebanyak 2.978 ekor dari genus Mansonia, Culex, Aedes, Anopheles, Armigeres, Uranotaenia, Coquilettidia dan Aedomvia, hasil pemeriksaan PCR menunjukkan nyamuk negatif Brugia malayi. Lingkungan habitat nyamuk yaitu: lubang pohon, kolam, sawah, barang bekas, bekas galian tambang, sungai, saluran air, genangan air, rawa, tambak, lubang galian, mata air, perahu, penampungan air, ban bekas, sumur, batok kelapa, dan kaleng bekas. Monitoring dan evaluasi pelaksanaan program oleh pemerintah daerah termasuk pemantauan vektor dan lingkungan secara rutin harus terus digalakkan agar status eliminasi filariasis di Kabupaten Donggala dapat dipertahankan.

Abstract

Ecological studies on the vector species diversity usually focused on particular habitats and not a comparative study. Therefore, a research has been carried out to see the variation from one of many aspects which affects the condition of a habitat such as altitude. This study aimed to determine the Anopheles spp. species diversity between highland and lowland captured in the research of Vektora 2015-2016 in East Nusa Tenggara Province, Indonesia. This study is a further analysis of Vektora Research Data from 2015 to 2016 in East Nusa Tenggara Province. The population in this study is the whole Anopheles spp. in East Nusa Tenggara Province from 2015-2016. The sample is Anopheles species caught on Vektora. The variables used in this study are categorical variables of highland or lowland, Anopheles species and dominance index. This study results indicate that there is a significant difference in the number of Anopheles obtained per species between the lowlands and the highlands. The number of Anopheles found in the lowland is higher than in the highland. The lowland have more Anopheles species variation compare to the highland. Anopheles annularis is the most commonly Anopheles found in the lowlands whereas Anopheles vagus is the most commonly Anopheles found in the highlands.

Abstract

Indonesia is one of the countries in the world with high risk of malaria. Out of 496 districts in Indonesia, 396 districts is a malaria endemic area. 45% of the populations live in malaria endemic areas, such as the region of Sulawesi. This study aimed to determine the determinant for incidence of malaria in Sulawesi. Riskesdas data that have been implemented by the Agency for Health Research and Development in 2013 used in further analysis. The variables analyzed are characteristics, housing conditions, and the environment associated with the incidence of malaria. Data were analyzed by logistic regression. The results showed that there was a significant relationship between malaria by gender, education, occupation, home status, number of household members, window of the bedroom, the cleanliness of the dining room, ventilation of the living room, the type of floor in the house, house walls, ceilings, lighting source and taking preventive medication. The variables that most influence on the incidence of malaria in the region of Sulawesi is a type of home lighting. Integrated planning activities with relevant sectors, education about healthy homes and the restoration of housing through increased participation of the people needs to be done.

Abstract

Dengue Haemograffic fever is a vector borne disease which caused outbreaks and death. There is no applied vaccine until now, so the effort of prevention and control is to terminate chain of infection mosquito breeding. Factors which influenced the female mosquitoe to lay their eggsare type of container, color, water, temperature, water source, humidity and environment condition. This study was conducted to determine the preferences of Ae. aegypti mosquitoes to lay aggs in various colors ovitrap in the laboratory of Balai Litbang P2B2 Donggala, January until March 2015. Three repetitions with plastic cup black, blue, white, yellow and pink have been performed with water and filter pappers. 30 mosquitoes blood saturation included in the containers with various colors. The result showed that most of female mosquito laid their eggs in plastic cup black (53,2%). ANOVA analysis showed that the diversity of colors ovitrap produce different the number of eggs Ae. aegypti in each type of countainer color. It is recommended to use black ovitrap for controling populations of Ae, aegypti in environment with regular monitoring.

Abstract

Waste with a certain concentration and quantity has a negative impact on the environment, especially for human health. Dominant environmental factors such as excreta disposal, drinking water sources, household waste water disposal channels, waste management play a role in the spread of diarrhea germs in toddlers. Diarrhea is one of the main causes of morbidity and mortality in children under five. The purpose of this study was to determine the relationship between household waste management and the incidence of diarrhea. The design of this research was cross sectional, by utilizing data from the 2018 Riskesdas activity, this was in accordance with the approval letter for the use of data issued by the Health Research and Development Agency Number: 11062001-118 dated June 22, 2020. Analysis of the relationship between waste management and the incidence of diarrhea in children under five had been carried out using logistic regression. The samples analyzed were 93,448 toddlers from Riskesdas 2018 data. The results of the analysis showed that the factors related to the incidence of diarrhea in toddlers in Indonesia were the majority male, the age of toddlers over two years old, handling and RT trash bins, and waste disposal sites. Handling household waste is the most dominant factor with the incidence of diarrhea in toddlers. Handling household waste is carried out in an environmentally friendly way by dumping it in a landfill, stockpiling or making compost, it is necessary for households to do so as not to be infested with flies which can be a source of transmission of diarrhea in children under five. Abstrak Limbah dengan konsentrasi dan kuantitas tertentu berdampak negatif terhadap lingkungan terutama bagi kesehatan manusia. Pengelolaan limbah rumah tangga merupakan pilar yang menentukan dalam kejadian diare pada balita. Faktor lingkungan yang dominan seperti pembuangan tinja, sumber air minum, saluran pembuangan air limbah rumah tangga, pengelolaan sampah,berperan dalam penyebaran kuman diare pada balita. Diare merupakan salah satu penyebab utama kesakitan dan kematian pada balita. Tujuan kajian ini untuk mengetahui hubungan pengelolaan limbah rumah tangga terhadap kejadian diare. Desain penelitian ini adalah cross sectional, dengan memanfaatkan data hasil kegiatan Riskesdas 2018, hal ini sesuai dengan surat persetujuan penggunaan data yang dikeluarkan oleh Badan Penelitian dan Pengembangan Kesehatan Nomor : 11062001-118 tanggal 22 Juni 2020. Analisis hubungan pengelolaan limbah dengan kejadian diare pada balita telah dilakukan dengan regresi logistik. Sampel yang dianalisis sebanyak 93.448 balita dari data Riskesdas 2018. Hasil analisis menunjukkan bahwa faktor yang berhubungan dengan kejadian diare pada balita di Indonesia yaitu mayoritas berjenis kelamin laki-laki, umurbalita di atas dua tahun, penanganan dan tempat sampah rumah tangga, serta tempat pembuangan tinja balita. Penanganan sampah rumah tangga merupakan faktor yang paling dominan dengan kejadian diare pada balita. Penanganan sampah rumah tangga dilakukan dengan cara yang ramah lingkungan dengan membuang di TPA, menimbun, atau membuat kompos perlu dilakukan rumah tangga agar tidak dihinggapi lalat yang dapat menjadi sumber penularan diare pada balita.

Abstract

Dengue Fever is a disease neglected tropical diseases are still a major problem in the field of public health, especially the tropics and subtropics. The spatial distribution of dengue fever cases and a location-based early warning system have not yet been developed properly. Modeling the spatial epidemiology of dengue is one of the applications of Geographic Information Systems (GIS). GIS can be used to determine the spatial patterns of temporal occurrence of dengue cases. The purpose of this study is to determine the spatial-temporal patterns of dengue cases based on the spatial statistical analysis in Palu 2011-2016. This study was an observational study with the cross-sectional design. . Samples of dengue cases were all reported from 2011 to June 2016 and were analyzed statistically using the averagenearest neighbor and space-time permutation. Estimation The results showed the spatial pattern of dengue cases from 2011- June 2016 tend to cluster. Clustering of dengue cases from 2011-2016 obtained two regions with significant clusters. The first cluster region has a p-value of 0.021.Time occurrence of dengue cases that have significant value from 1 March to 30 November 2011 with a total of 25 cases. Furthermore, for the second cluster showed a p-value of 0.037 with a span of the case from 1 May - June 30, 2013, with 17 cases. The main cluster locations or those that are spatially and temporally significant are located in six villages and become a priority in dengue controlling. The implementation of the eradication of dengue mosquito nests with the 3M plus movement and the one house one inspector movement was carried out intensively by prioritizing areas with significant clusters. Surveillance of cases and disease vectors should be improved and developed using GIS. Abstrak Demam Berdarah Dengue merupakan salah satu penyakit yang masih menjadi masalah utama di bidang kesehatan masyarakat khususnya negara-negara tropis dan subtropis. Distribusi spasial kasus demam berdarah dan sistem kewaspadaan dini berbasis lokasi sampai saat ini belum dikembangkan dengan baik. Pemodelan spasial epidemiologi DBD merupakan salah satu aplikasi dari Sistem Informasi Geografis (SIG). SIG dapat digunakan untuk menentukan pola spasial temporal kejadian kasus DBD. Tujuan dari penelitian ini adalah menentukan pola spasial-temporal kasus DBD berdasarkan analisis spasial statistik di Kota Palu Tahun 2011-2016. Penelitian ini merupakan penelitian observasional dengan menggunakan studi potong lintang. Sampel kasus DBD adalah semua yang dilaporkan dari tahun 2011 sampai dengan Juni 2016 dan dianalisis secara spasial statistik menggunakan average nearest neighbour dan space-time permutation. Estimation. Hasil penelitian menunjukkan pola spasial kasus DBD Tahun 2011- Juni 2016 cenderung mengelompok. Untuk pengelompokan kasus DBD Tahun 2011-2016 secara spasial-temporal didapatkan dua daerah dengan klaster yang signifikan. Wilayah klaster tersebut memiliki p-value 0,021 untuk wilayah pertama. Waktu kejadian kasus DBD yang memiliki nilai signifikan tersebut antara rentang waktu 1 Maret – 30 November 2011 dengan jumlah 25 kasus. Selanjutnya untuk klaster kedua didapatkan hasil p-value 0,037 dengan rentang waktu kasus 1 Mei – 30 Juni 2013 dengan jumlah 17 kasus. Lokasi klaster utama atau yang signifikan secara spasial temporal terdapat di enam kelurahan dan menjadi prioritas dalam pengendalian DBD. Pelaksanaan pemberantasan sarang nyamuk DBD dengan gerakan 3M plus dan gerakan satu rumah satu jumantik secara intensif dilakukan dengan memprioritaskan daerah dengan klaster yang signifikan. Surveilans kasus dan vektor penyakit harus ditingkatkan dan dikembangkan dengan memanfaatkan SIG.

Abstract

ABSTRACT Toddlers and pregnant women are a group that is vulnerable to contracting malaria because their immune systems are lower than healthy people. A cross-sectional study was conducted to identify the relationship of demographic and environmental characteristics factors to the incidence of malaria in vulnerable groups in Indonesia in 2018. A total of 2915 samples of vulnerable groups were collected at Riskesdas 2018, consisting of 2391 samples of toddlers and 524 samples of pregnant women. Toddlers 0.84% ​​(20/2391) and pregnant women 0.38% (2/524) were positive for malaria based on examination with RDT. Factors related to malaria incidence in children under five are gender, while in pregnant women are age, wastewater disposal, use of mosquito nets, use of electric mosquito repellent, and mosquito netting (p-value <0.05). Using Personal Protective Equipment against mosquito bites is necessary to protect infants and pregnant women from contracting malaria. ABSTRAK Balita dan ibu hamil merupakan kelompok rentan tertular malaria karena sistem kekebalan tubuh yang lebih rendah dibandingkan orang sehat. Studi cross sectional dilakukan untuk mengidentifikasi hubungan faktor karakteristik demografi dan lingkungan terhadap kejadian malaria pada kelompok rentan di Indonesia tahun 2018. Sebanyak 2915 sampel kelompok rentan dikumpulkan pada Riskesdas 2018 terdiri dari balita 2391 sampel dan ibu hamil 524 sampel. Balita 0,84% (20/2391) dan ibu hamil 0,38% (2/524) positif malaria berdasarkan pemeriksaan dengan RDT. Faktor yang berhubungan dengan kejadian malaria pada balita yaitu jenis kelamin, sedangkan pada ibu hamil yaitu umur, pembungan air limbah, penggunaan kelambu, penggunaan obat nyamuk elektrik dan kasa nyamuk (p-value < 0,05). Perlunya memproteksi balita dan ibu hamil agar tidak terular malaria melalui penggunaan Alat Pelindung Diri (APD) dari gigitan nyamuk.

Abstract

ABSTRACT Soil-Transmitted Helminths infection is still remain become a health problem in tropical and sub-tropical countries, one of which is Indonesia. The aim of the study was to determine the STH infection in Banawa and Dampelas sub-districts, Donggala district, Central Sulawesi Province. The study was conducted in February - November 2017 on 153 elementary school students. The collected feces were examined by the direct examination method. The results found infections of Ascaris lumbricoides, hookworm, and Trichuris trichiura. We also found Enterobius vermicularis and mixed infections. However, the logistic regression test showed there was no relationship between taking deworming medication and the incidence of worms in school childrenin Donggala Regency (p-value > 0,05). Based on the results it can be concluded that the prevalence of STH infection in both locations is still high. It is necessary to make efforts to treat school children and counseling on clean and healthy living behavior. ABSTRAK Infeksi Soil Transmitted Helminths masih menjadi masalah kesehatan di negara tropis dan sub tropis, termasuk di Indonesia. Tujuan penelitian adalah untuk menentukan tingkat infeksi STH pada siswa sekolah dasar (SD) wilayah Kecamatan Banawa dan Dampelas, Kabupaten Donggala, Provinsi Sulawesi Tengah. Penelitiandilakukan pada bulan Februari - November 2017 dengan jumlah sampel 153. Tinja yang terkumpul diperiksa dengan metode pemeriksaan langsung. Hasil penelitian ditemukan infeksi cacing gelang (Ascaris lumbricoides) cacing tambang (Hookworm), dan cacing cambuk (Trichuris trichiura). Selain itu ditemukan infeksi Enterobius vermicularis dan infeksi ganda. Namun uji logistic regression menunjukkan bahwa tidak ada hubungan antara minum obat cacing dengan kejadian kecacingan pada anak sekolah di Kabupaten Donggala (p-value > 0,05).Berdasarkan hasil penelitian dapat disimpulkan bahwa prevalensi infeksi STH di kedua lokasi masih tinggi. Perlu dilakukan upaya pengobatan anti cacing pada anak sekolah serta penyuluhan perilaku hidup bersih dan sehat.
