Abstract

Stroke iskemik terjadi akibat penyempitan pembuluh darah sehingga sirkulasi darah terganggu. Angka kasus stroke iskemik di RS Bhayangkara Bondowoso mengalami fluktuasi dari tahun 2019-2023. Penelitian ini bertujuan melakukan klasifikasi penyakit stroke iskemik berdasarkan faktor risiko (usia, jenis kelamin, diabetes melitus, GDA, obesitas, hipertensi dan merokok), gejala (kelemahan ekstremitas, penurunan kesadaran, pusing, muntah, kejang, sesak, nyeri, kesemutan dan afasia) dan tindakan CT Scan kepala. Jenis penelitian ini kuantitatif menggunakan data primer. Sampel penelitian terdiri dari 385 rekam medis rawat inap menggunakan simple random sampling. Klasifikasi menggunakan algoritma KNN dengan supplied test. Hasil penelitian menunjukkan variabel yang menjadi faktor risiko penyakit stroke iskemik yaitu usia (65,8%), jenis kelamin (58,03%) dan hipertensi (63,73%). Sedangkan gejala yang sering dialami pasien stroke iskemik yaitu kelemahan ekstremitas (54,92%), pusing (53,89%) dan lemas (52,33%). Serta tindakan CT Scan kepala yaitu 98,96%. Klasifikasi ini menggunakan metode K-Nearest Neighbor dengan k=18 dan supplied test. Hasil akurasi menghasilkan performance dengan tingkat akurasi 92,2078%, Precision 86,05%, Recall 100%, F1 Score 0,9250 dan nilai ROC Area 0,9962. Nilai ROC area tergolong kategori Excellent Classification. Saran dalam penelitian ini yaitu melakukan perancangan sistem.

Abstract

Pencatatan yang efisien saat ini lebih ditekankan di segala bidang salah satunya di bidang kesehatan. Adanya era digitalisasi tidak terkecuali bidang kesehatan membuat sistem informasi digunakan untuk mempermudah petugas rekam medis di bagian filling dalam pengelolaan rekam medis yaitu retensi. Retensi atau depresiasi adalah tindakan memilah atau memisahkan berkas rekam medis yang masih aktif maupun yang sudah tidak aktif, dan mengurangi jumlah formulir di dalam dokumen rekam medis dengan menilai nilai guna dari setiap formulir. Berdasarkan hasil studi pendahuluan yang dilakukan pada bulan September 2022 di Puskesmas Tempurejo didapatkan bahwa belum pernah dilakukannya retensi rekam medis. Belum dilakukannya retensi mengakibatkan rekam medis rusak, serta menumpuk di lantai. Oleh karena itu, perlu adanya rancang bangun sistem retensi sehingga dapat meningkatkan kinerja petugas rekam medis di Puskesmas Tempurejo dalam proses retensi dan pemusnahan agar lebih efektif dan efisien. Penyusunan sistem dengan pendekatan metode waterfall melibatkan beberapa tahapan, seperti analisis dan penentuan persyaratan, perancangan sistem dan perangkat lunak, implementasi serta uji unit, dan akhirnya integrasi dan pengujian keseluruhan sistem. Tahap analisis kebutuhan dilakukan dengan teknik pengumpulan data wawancara langsung kepada pengguna. Tahap pemodelan sistem menggunakan flowchart, DFD dan ERD. Tahap pengujian sistem menggunakan metode uji black box yang berfokus pada uji fungsionalitas sebuah sistem. Hasil dari penelitian ini adalah sistem informasi retensi dan pemusnahan rekam medis yang dapat mempermudah kinerja petugas dan dapat meningkatkan mutu pelayanan di Puskesmas Tempurejo.

Abstract

Penelitian ini berfokus pada perbaikan sistem rawat jalan di Puskesmas Grujugan Bondowoso, yang masih mengandalkan proses manual. Beberapa masalah yang muncul yaitu sering kali KIB tidak dibawa oleh pasien lama saat berkunjung, menyebabkan petugas pendaftaran kesulitan dalam mencari dan menyediakan berkas rekam medis. Duplikasi nomor rekam medis juga sering terjadi, sehingga petugas kesulitan mengidentifikasi data pasien. Selain itu, penggunaan rekam medis manual pada pelayanan rawat jalan khususnya pada poli umum dapat menimbulkan ketidakmampuan dokter dalam pemberian pelayanan yang cepat. Penelitian ini memiliki tujuan merancang dan membuat sistem informasi rawat jalan berbasis web pada poli umum di Puskesmas Grujugan Bondowoso dengan menerapkan metode waterfall yang memiliki tahapan definisi kebutuhan, desain sistem dan perangkat lunak, implementasi dan pengujian unit, serta pengujian integrasi dan sistem. Hasil yang dicapai yaitu sistem informasi rawat jalan berbasis web pada poli umum di Puskesmas Grujugan Bondowoso yang memberikan kemudahan dalam pencatatan dan pencarian data pasien, mencetak KIB dan dilengkapi fitur mengirim e-KIB melalui WhatsApp, pencatatan data pemeriksaan pasien, mencetak hasil pemeriksaan, surat keterangan sehat/sakit, resep obat, membuat laporan kunjungan pasien, kunjungan poli, diagnosa pasien, penyakit tidak menular, laporan peresepan, laporan rujukan, 15 besar penyakit, dan data kesakitan.

Abstract

The national coverage rate for complete basic immunization in Indonesia is high. However, several regions in Indonesia have under-performed targets. Several factors contribute, including a lack of parental awareness of the importance of immunization, suboptimal program planning and management, and manual recording processes. These factors contribute to inaccurate decision-making. This research aims to create a dashboard system for monitoring and reminding toddlers about immunizations for decision support systems. The system development method used was the Scrum method. The results indicate that this system meets the standard requirements for immunization monitoring. Further development of the system can include the addition of a web service feature to facilitate access across multiple platforms.

Abstract

Immunization is one of the most effective public health interventions in preventing infectious diseases and reducing mortality and morbidity in the global population, with the potential to prevent 3 to 3.5 million deaths each year from vaccine-preventable diseases. In Low- and Middle-Income Countries (LMICs), including Indonesia, vaccination coverage for children under five years old still shows low numbers.  The low vaccination coverage is partly caused by the use of manual recording systems, which leads to delays in decision-making processes in immunization management. This study seeks to address this issue by developing a UI/UX prototype design for a toddler immunization information system. The proposed UI/UX design is equipped with features to monitor and record immunization activities in a particular region in real time, enabling effective, accurate, and integrated decision-making and implementation of immunization programs at both regional and national levels. The UI/UX design was developed using the User-Centered Design (UCD) method, which actively involves users throughout the process. Usability testing was conducted using the System Usability Scale (SUS) method. The results indicate that the designed UI/UX meets the standard requirements of public health centers and achieves satisfactory outcomes from prospective users. The contribution of this research is the proposed UI/UX design for immunization monitoring. In the future, this UI/UX design can serve as a foundation for the development of an Immunization Monitoring Information System.

Abstract

Diabetes mellitus is a chronic metabolic disorder characterized by high blood glucose levels, requiring early and accurate detection to prevent long-term complications. Machine learning is increasingly important in data-driven diagnostics, with the Naive Bayes algorithm widely used due to its simplicity, transparency, and efficiency. This study evaluates the classification performance of Naive Bayes for early diabetes screening using a clinical dataset containing incomplete and heterogeneous medical records. The pre-processing involved data cleaning, replacing missing values with the median, labeling patients based on a glyhb threshold ≥6.5%, preventing data leakage, and converting categorical variables into numerical form. Model training was performed with a 70:30 split, and performance was evaluated through accuracy, precision, recall, F1 score, and AUC. The classifier achieved an accuracy of 90.81% and an AUC of 0.919, outperforming standard baseline Naive Bayes implementations which typically report accuracies in the range of 76-78% on similar datasets. Despite this stability, the model showed varying sensitivity in identifying positive diabetes cases, largely due to class imbalance. Therefore, Naive Bayes is considered reliable as a preliminary screening method, but improvements through oversampling or cost-sensitive learning techniques are recommended to enhance recall and ensure more accurate patient identification in future clinical applications.

Abstract

Monitoring child growth and development during the First 1,000 Days of Life is critical for preventing stunting and developmental delays. Despite national policies mandating routine monitoring, current practices in Indonesia remain largely manual and fragmented, limiting real-time data access and early intervention. This gap highlights the need for an integrated, real-time digital monitoring tool that connects parents and health workers.This study aims to develop GROWTH (Global Real-time Observation of Well-being, Tracking, and Health), a web-based system that integrates physical growth and developmental monitoring for children aged 0–72 months. The system was developed using the Scrum framework, involving parents and midwives in the requirements elicitation process. System functionality was evaluated through black-box testing using simulated child growth and development data, as well as its usability was assessed by end-users (parents and midwives). The results indicate that all core features functioned as expected, including growth chart visualization based on WHO Anthro standards and age-appropriate developmental screening. The GROWTH system offers a practical digital solution to support timely decision-making by parents and healthcare workers, with potential for integration into national child health information systems.

Abstract

Incomplete filling of medical records (KLPCM) is an indicator of the quality of nursing care. Citra Husada Hospital experienced an 83.5% medical record incompleteness rate in the fourth trimester of 2022, with the highest form of incompleteness occurring in the pre-surgical assessment at 59.68%. Furthermore, the KLPCM review process still relies on Excel, which presents several drawbacks, such as non-specific columns, a lack of user-friendliness, and the need for high precision due to small input fields. As a result, the KLPCM review process is time-consuming and prone to input errors. Based on these issues, a KLPCM information system is needed at Citra Husada Hospital in Jember. The system development method used is the Prototype method, with data collection techniques including interviews, observation, and documentation. The system is web-based, developed using the PHP programming language with the Laravel framework and a MySQL database. The system's features include user management, doctor management, form and form content management, patient management, KLPCM medical record analysis, reports, and incompleteness notifications. The system testing results, conducted using black box testing, showed that all functionalities work properly.

Abstract

The medical record at Umbulsari Jember Healthcare are still conducted manually leading to incomplete coding processes for patient diagnoses, with the completeness percentage falling below 100%, and the time required to provide medical records exceeding 10 minutes. This research aims to design and develop an electronic outpatient medical record system for Puskesmas Umbulsari Jember based on a web platform. Data collection was carried out through observation, interviews, documentation, and brainstorming. The system development method used is the prototype method, which includes the stages of requirement identification, prototype development, adjustment to user preferences, system development, and further adjustments. The first stage involves problem analysis, revealing that medical records are still handled manually. The next stage is prototype development, including the creation of Flowcharts, Data Flow Diagrams (DFD), Entity Relationship Diagrams (ERD), and interface designs. The system development process begins with gathering tools such as software, the Bootstrap 4 CSS Framework, the CodeIgniter Framework, and Visual Studio Code, followed by the system design phase. Once the system is complete, testing is conducted using the Black Box Testing approach, which shows that all system functions operate correctly

Abstract

The activity of borrowing and returning medical records at the Semboro Jember Community Health Center experienced several obstacles, including the failure to implement a tracer to mark files that would leave the storage room, and the recording of borrowing and returns was still carried out manually in the expedition book. The aim of this research is to design a website-based electronic tracer system. The system development method uses waterfall. The result of this research is the creation of a website-based electronic tracer which has advantages including that police officers as loan officers can borrow medical record files by looking at the patient's name or medical record number to be borrowed and can print an electronic tracer accompanied by a QR Code to be handed over to the officer. medical record filling section. Meanwhile, medical record officers can add patient data, officer data, doctor data and verify the return of files that have been returned by police officers or loan officers, return notifications and late charts.
