Abstract

Introduction: Breast cancer is one of the most common cancers and is often diagnosed at a late stage, especially in Indonesia. This highlights the need for cost-effective treatment options. ACT (Adriamycin, Cyclophosphamide, and Taxane) and ACTH (ACT plus Herceptin/Trastuzumab) are used chemotherapy regimens, but studies comparing their cost-effectiveness are still limited. This study aimed to evaluate the cost-effectiveness of ACT versus ACTH for breast cancer treatment using tumor volume reduction. Method: A retrospective cost-effectiveness study was carried out at Dr. M. Djamil Hospital by comparing direct medical costs and tumor volume changes in breast cancer patients treated with ACT or ACTH regimens. Effectiveness was based on the average change in tumor volume, and cost-effectiveness was evaluated using the Incremental Cost-Effectiveness Ratio (ICER). Results: The mean tumor volume reduction in the ACT group was 2820.85 ± 11190.42 cm³, and significantly higher at 9273.50 ± 60101.33 cm³ in ACTH, although the difference was not statistically significant (p = 0.614). The average direct medical cost for ACTH was IDR 67,138,579, compared to IDR 13,005,027 for ACT. The ICER calculation resulted in IDR-838,935 per cm³ of tumor reduction, while the negative ICER might suggest cost savings, it does not imply that ACTH is a dominant strategy indicating that ACTH incurred a higher cost with less effectiveness per unit of tumor reduction than ACT. Conclusion: The high associated cost leads to a negative ICER, suggesting that ACT may be more cost-effective in this setting. These findings are critical for optimizing chemotherapy choices within limited healthcare budgets.

Abstract

Low public awareness of routine health monitoring and knowledge of proper medication use remains a significant problem, leading to increased adverse drug reactions and a decline in health status. Furthermore, a lack of health literacy and high self-medication practices also increase the risk of inappropriate medication use. To address this need, this activity combines health screening and medication education as an integrative approach to improving public health awareness and drug literacy. This study used a quasi-experimental one-group pre-posttest design and was conducted on 24 October 2025, at SDN 07 Ganting, East Padang Panjang, involving 34 participants. Activities included blood pressure, glucose, cholesterol, and uric acid screenings; medical consultations; and medication education and counseling. The majority of participants were women (80%), with a dominant age range of 41–59 years (50%). Mean blood pressure was 129.1 ± 25.0/86.26 ± 13.60 mmHg, glucose 132.41 ± 74.5 mg/dL, cholesterol 214.7 ± 33.8 mg/dL, and uric acid 5.6 ± 1.8 mg/dL, indicating the presence of risk factors requiring monitoring. Knowledge levels were in the good category at both pre-test (90%) and post-test (93.3%), with no significant difference (p = 0.541). This activity contributed to strengthening drug literacy through direct education linked to screening findings, enabling participants to understand their personal health risks. This integrated screening-education approach shows potential as a more effective community intervention model in increasing public knowledge and awareness of health and the safe use of drugs.

Abstract

Pendidikan literasi di era global menuntut pendekatan yang tidak hanya teknis, tetapi juga filosofis dan kontekstual. Trisenta Pendidikan Ki Hajar Dewantara — Ing Ngarsa Sung Tulada, Ing Madya Mangun Karsa, dan Tut Wuri Handayani — menjadi kerangka etis-pedagogis yang dapat memperkuat pembelajaran literasi di sekolah. Studi ini menggunakan metode studi pustaka dengan analisis terhadap dokumen kebijakan literasi, teori literasi, serta kontribusi pemikiran Prof. Anas Ahmadi dalam bidang sastra dan budaya lokal. Hasil kajian menunjukkan bahwa keteladanan guru dalam literasi, partisipasi siswa, serta dukungan emosional dari lingkungan sekitar sangat memengaruhi efektivitas pembelajaran literasi. Selain itu, pengintegrasian teks sastra dan folklor lokal memberi ruang bagi pendekatan literasi yang lebih humanis, kontekstual, dan membumi. Diperlukan sinergi antara nilai-nilai Trisenta dan strategi literasi nasional untuk membentuk generasi yang literat secara kognitif, emosional, dan budaya.
