Abstract

Abstrak Tuberkulosis Paru dalah penyakit infeksius yang ditakuti oleh masyarakat karena sumber penularan yang cepat dari bakteri tahan asam (BTA). BTA adalah suatu basil aerobic tahan asam yang merupakan bakteri yang terdiri dari lapisan lilin dan asam lemak mikolat, lipid yang ada bisa mencapai 60% dari berat dinding sel. Kelompok BTA terdiri dari: Mycobacterium Tuberculosis, Mycobacterium africanum, Mycobacterium bovis dan Mycobacterium Leprae. Keluarga sangat penting untuk dilibatkan secara aktif untuk melakukan pencegahan penularan dalam rumah.Tugas dari keluarga adalah melakukan perawatan bagi anggota keluarga yang sakit dengan mencegah penularan. Penyakit Tb Paru akan semakin lama pengobatannya bila disertai dengan komorbid DM. Keluarga harus membuat pencegahan/ pengobatan lain untuk dapat membuat penderita sembuh dari kedua penyakit tersebut, sehingga tidak memperburuk kondisi penderita. Penelitian ini bertujuan untuk menggambarkan bentuk upaya penanganan dan pencegahan pada klien tuberkulosis paru dengan komorbid DM yang sedang menjalani proses pengobatan di Kota Salatiga. Penelitian menggunakan metode kualitatif dengan mendeskripsikan bagaimana upaya penanganan dan pencegahan dan penanganan Tb Paru yang disertai dengan komorbid DM dapat dilakukan oleh anggota keluarga klien. Background: The number of older adults who have poor nutritional status is still often found in nursing homes in Salatiga City. Lack of appetite is the leading cause of poor nutritional status. Even though the nursing home has provided a healthy food menu for each older person, comfort food can be used to increase appetite so that the nutritional status of the elderly improves. This research aims to describe perceptions of comfort food to elderly caregivers in providing food at the Salatiga Nursing Home. The research method is qualitative with a descriptive approach. The criteria for participants are caregivers or caretakers of older adults aged 30-60 years who work in nursing homes to provide food and can still communicate. The research participants were ten caregivers or administrators of the three nursing homes in Salatiga City, Central Java nursing homes. The research results were five themes: first, food menu management, selection of food menus for the elderly, preparation of menu lists in nursing homes made by the caregivers, and asking the elderly about their food preferences. Both elderly eating habits and nutritional status, older adults have the habit of not finishing food if it does not suit their taste. Third, caregivers often feel sad and disappointed when older people do not finish their food. The fourth is the meaning of comfort food, which is everyone's favorite food from childhood. Fifth, comfort food is needed to manage food in nursing homes. Conclusions show that comfort food is understood as a term for favorite foods or favorite foods for caregivers in nursing homes but has not yet been used as an approach in the nutritional management of the elderly.

Abstract

Latar Belakang: Penyakit Paru Obstruktif Kronik (PPOK) merupakan salah satu penyakit paru dengan prevalensi tinggi dan menjadi penyebab lebih dari tiga juta kematian di seluruh dunia pada tahun 2015. Berdasarkan data rumah sakit, jumlah pasien PPOK di RS Emanuel Banjarnegara terus meningkat dari tahun ke tahun, yakni sebanyak 212 pasien pada 2019, 604 pasien pada 2021, dan 656 pasien pada 2023. Tujuan: Menganalisis faktor risiko yang berhubungan dengan peningkatan kejadian PPOK di RS Emanuel Banjarnegara. Metode: Penelitian ini menggunakan metode kuantitatif dengan desain cross-sectional. Analisis data dilakukan menggunakan uji chi-square dan analisis multivariat. Penelitian dilaksanakan di RS Emanuel Banjarnegara pada Oktober–November 2024 dengan populasi seluruh pasien PPOK yang pernah atau sedang dirawat. Sampel penelitian berjumlah 86 responden. Hasil: Terdapat hubungan signifikan antara peningkatan kejadian PPOK dengan usia (p=0,000), jenis kelamin (p=0,018), riwayat penyakit pernapasan (p=0,026), dan tingkat aktivitas fisik (p=0,023). Sementara itu, riwayat merokok tidak menunjukkan hubungan yang signifikan terhadap peningkatan kejadian PPOK (p=0,230). Kesimpulan: Peningkatan kejadian PPOK di RS Emanuel Banjarnegara dipengaruhi oleh faktor usia, jenis kelamin, riwayat penyakit pernapasan, dan tingkat aktivitas fisik, sedangkan riwayat merokok tidak berpengaruh secara signifikan.

Abstract

The recovery of Tb patients depends on medication adherence. Drug side effects, duration of treatment, health control, and social stigma towards Tb patients influence adherence. The assumption that Tb is a hereditary disease, a curse, and challenging to cure makes patients unmotivated to take medicine and reluctant to go to health services because of shame. These factors are at risk of increasing drug resistance rates and hindering the recovery process in Tb patients. The purpose of this study was to describe drug compliance in patients with pulmonary Tb in West Sumba. This study used a descriptive qualitative method with a case study research design and an indepth interview technique. The sample selection was made by purposive sampling with the age 20-45 criteria. Data analysis was done by reducing, presenting, and drawing conclusions. The results showed that drug adherence behavior in TB patients was influenced by treatment routines, barriers during treatment, and emotional, informational, and financial support from the social environment. According to the community, Tb is a curse, contagious and dangerous disease, not a cursed one. Therefore, the results of this study should serve as a basis for health workers and health policymakers in the West Sumba region to design health promotion programs on the importance of social support on treatment adherence of Tb patients so that Tb patients can maintain treatment adherence.

Abstract

The increasing number of older people in Indonesia impacts the care process. Culturally, the eldest in Indonesia are cared for by other family members. On the other hand, several older people live in nursing homes during their old age. While in the nursing home, the eldest are cared for by health workers or nursing home administrators physically, psychologically, and spiritually to maintain or improve their quality of life. However, the elderly living in three retirement homes in Salatiga City are at risk of non-communicable diseases such as hypertension (70%) and high cholesterol (23%), loneliness, and sleeplessness (41%). The purpose of the community service is to provide group activities therapy to older people living in nursing homes. The activities included cognitive exercises (drawing, writing) and psychomotor (brain gymnastics and song movements). The service results showed that the eldest expression was happy and eager to follow the activities. The involvement of students also reduces the loneliness of the eldest, who have not received visits from their families for a long time. Group therapy activities can train cognitive and psychomotor skills and reduce feelings of loneliness in the elderly.
