Abstract

Isotretinoin is used to treat severe acne, and despite the therapeutic benefits, it has many side effects, including cheilitis on lips. This case report reviewed the importance of assessing the medical and pharmacological histories during anamnesis in the management of drug-related and angular cheilitis. Case: A 22-year-old female patient presented with complaints of dry, cracked, bleeding, and pain on the upper and lower lips, which occurred 7 days ago. Previous similar complaints were reported, although not severe. The patient had habits of lip licking, low water intake, and an irregular menstrual cycle. There was a history of using Roaccutane (isotretinoin) for acne treatment. Extraoral examination showed dry, desquamated lips with crusts, while intraoral examination had no abnormalities. Case management: The patient was prescribed NaCl 0.9%, Vaseline album, and hydrocortisone cream 1% twice daily. However, during treatment, angular cheilitis developed, prompting the prescription of miconazole cream 2% twice daily. One month later, all of the patient’s complaints were successfully treated with no subjective issues. Proper history taking and examinations were fundamental to making the right decisions and definite treatment plans.

Abstract

Kandidiasis oral merupakan infeksi jamur oportunistik yang paling sering terjadi pada pasien HIV/AIDS. Terdapat beberapa obat antijamur yang dapat digunakan dalam perawatan kandidiasis oral pada pasien HIV/AIDS, namun pemilihannya tergantung beberapa faktor, seperti perluasan lesi, keparahan penyakit, kondisi asam lambung, level supresi imun, farmakodinamik/farmakokinetik, interaksi obat, resistensi, dan tingkat kepatuhan pasien. Namun, kandidiasis oral refractory dapat terjadi yaitu kegagalan dalam merespon perawatan dengan antijamur. Pada kasus seperti ini, identifikasi Candida dari daerah orofaring dapat membantu membedakan penyebab kegagalan lain secara mikrobiologis. Pasien AIDS diketahui mengalami hypochlorhydria dan peningkatan pH lambung, sehingga berpotensi terjadi penurunan absorpsi dan bioavailabilitas beberapa obat antijamur. Pasien dengan kondisi ini dapat diberikan flukonazol oral karena tidak tergantung pada pH lambung, dapat diabsorpsi dengan cepat, serta memiliki bioavailabilitas tinggi, sehingga flukonazol lebih unggul dibandingkan golongan azol lain dan antijamur topikal seperti nistatin supsensi oral maupun klotrimazol troches. Larutan itrakonazol dan posakonazol dapat dijadikan terapi second-line yang dapat digunakan, selain itu peningkatan imunitas lokal dan sistemik merupakan hal yang penting dalam melawan infeksi Candida. Dengan demikian, berdasarkan tinjauan pustaka flukonazol lebih baik digunakan dalam perawatan kandidiasis oral pada pasien HIV/AIDS.

Abstract

Objective: The purpose article is to identify stress predisposing factors in patients with minor RAS.Methods: A 22-year-old woman came to Dental Hospital Faculty of Dentistry UI with complaints of canker sores pain in lower front gums for 4 days ago. Canker sores occur repeatedly every month, especially when there are lots of college assignments. Intraoral examination revealed a single ulcer, clear borders, oval-shape, a pink shallow base surrounded by erythema, size 3x2 mm in attached gingival, region 41-42, with tenderness. Screening for psychological conditions with DASS-21 questionnaire, obtained scores of severe anxiety level and stress. The patient was diagnosed minor RAS with stress predisposing factors.Results: Based on the subjective and objective examinations, the patient was diagnosed with cheilitis and minor RAS with possible predisposing factors to stress and anxiety.Conclusion: Stress can be a predisposing factor for RAS, so managing stress in patient is important for controlling recurrence.

Abstract

Objectives: This report describes 2 cases diagnosed with minor RAS treated with LLLT to relieve pain and promote the healing of the ulcers.Methods: A 21-years old female presented of ulcer on labial mucosa for about 4 days with VAS=5. The ulcer always appears 1-2/month before or on her menstruation period. A 21-years-old female presented of ulcer on right buccal mucosa for about 2 days with VAS=8. The ulcer always appears once a month. The predisposing factor in this case were trauma and stress.Results: We treat the patient with LLLT and educated the patients for management the predisposing factor. After treatment with laser, the pain decreased immediately. The lesions healed completely within 4-5 days and scoring of the Ulcer Severity Scores (USS) also decreased.Conclusion: Low level laser therapy is recommended for patients with minor RAS since it can relieve pain, reduce the healing period and lesion size and recurrence of the lesions.

Abstract

A total number of people living with HIV reached since it was first reported. Indonesia is on the brink of rapidly worsening AIDS epidemic. Oral health was a frequent problem among HIV-infected person. Opportunistic oral lesions and infections were often one of the first manifestations of HIV. The aim of this literature review was to discuss complication risks and management undergoing invasive dental procedures. Thereby, dentists have been expected to increase awareness of dental treatment needs for HIV-infected patients by pay attention to applied standard procedure. The complications of HIV-infected patients were reported minor, self-limiting, non-life threatening and readily treated. Treatment planning for the patient with HIV follows the same sequences as with other patients priorities are to assuage pain, restore function, prevent further disease and consider esthetic results. We concluded that no different complication risks between HIV-infected and non-infected. Dentists can provide care for HIV-infected patients with assessing the current immunosuppression stage, intake systemic medications, opportunistic infection potential and minimize contagion possibility to dental care professional or other patients.

Abstract

Background: The use of mouthwash to maintain oral hygiene has been carried out for many years. However, long-term use of mouthwash can cause side effects such as tooth discoloration, burning sensation and the death of normal oral flora. The use of herbal plant, such as Moringa oleifera lam has been studied as an alternative medicine because of its pharmacological properties and it is hoped to have minimum side effect. In this systematic literature review, we included the studies that have been investigate the antimicrobial capacity of Moringa oleifera lam as a basic knowledge to develop a preparation for oral hygiene maintenance.Method: A comprehensive literature search was performed utilizing four different electronic databases: PubMed, Scopus, Cochrane library, and EBSCO. Articles included in this study were published from 2013 to September 2023, with broad search criteria comprising MeSH-terms and free-text keywords Moringa Oleifera, Oral Antimicrobial, and Mouthwash.Result: At the beginning our search resulted in 17 articles, with 14 articles remaining after removal of duplication. We exclude five studies that do not meet the PICO criteria, resulting seven articles which included to this study.Conclusion: According to the literature review that has been carried out, Moringa oleifera lam has antimicrobial capacity which allows it to be used as a material in medicinal preparations. Further research, both in vitro and clinical research, will determine the effectiveness of Moringa oleifera lam in suppressing microorganisms.

Abstract

Leprosy has a high incidence of physical complications that will impact directly the physiological, economic, and social status of patients. Indonesia has a high burden of the disease, and several leprosy settlements are still spread in the country. Neglasari Village, Sitanala, Tangerang serves as one of the places of residence of people who have had leprosy. A health service initiative known as Identifikasi Tanda-Tanda Mata, Ekstremitas, dan Kulit pada Kusta (KATAMATAKU) was launched as a collaboration of health services among multi-departments (ophthalmology, dermatovenereology, and medical rehabilitation). Sitanala has a relatively high incidence of people who have had leprosy with disabilities of the hand, foot, and eye. As a continuation of this health service and combined with the efforts to improve the welfare of leprosy patients, in November 2019, a multidisciplinary program titled KATAMATAKU Universitas Indonesia, was conducted. This program aimed to determine the demographic data regarding the health, psychological, social, and economic status of the leprosy population at Sitanala The project consisted of a collaborative anti-stigma program by the Faculty of Psychology, Public Health, Social and Political Sciences, Cultural Sciences, Administrative Sciences, and Vocational Educational Program; thematic health program, which supports the improvement of physical abilities and empowerment of former leprosy patients, by the Faculty of Medicine, Dentistry, Nursing, and Pharmacy; thematic economics program, which aims to increase the economic capacity of the leprosy community, by the Faculty of Economics and Business, Mathematics and Natural Sciences, and Engineering. This program enabled the construction of a multidimensional management model, in which every aspect plays important roles to improve the patients’ quality of life.
