Abstract

Digital transformation in the health sector encourages the adoption of Electronic Medical Records (EMR) as an effort to improve the quality of service and efficiency of information systems. This study aims to analyze the level of completeness of EMR filling in the Outpatient Installation of Hospital X and identify the factors that influence it. The method used is quantitative descriptive with data obtained from 100 outpatient EMR documents during March–April 2025. The results show that only the patient identity component reaches 100% completeness, while other components such as anamnesis, actions, and therapy are still low (52%), followed by CPPT (65%), doctor's signature (67%), physical examination (77%), and diagnosis (90%). This incompleteness risks reducing the quality of service, patient safety, and validity of documentation. The main factor contributing to incompleteness is limited human resources, especially elderly officers who have difficulty adapting to digital systems. The study recommends intervention strategies such as needs-based training, intergenerational mentoring, and routine evaluation to improve compliance with EMR filling. In conclusion, the success of RME implementation does not only depend on the digital system, but also on the readiness of human resources, documentation culture, and management support in building quality health services.

Abstract

Patient adherence to treatment is an essential component in achieving successful therapy, especially in the management of chronic diseases that require long-term treatment and active patient involvement. Non-adherence can lead to therapy failure, increased complications, higher health care costs, and decreased quality of life. This study aims to analyze factors that influence patient adherence to treatment at Hospital X using a descriptive quantitative approach. The study sample consisted of 100 outpatients selected purposively and analyzed using cross-tabulation and chi-square statistical tests. The results of this study indicate that there are still respondents who are not compliant with treatment. Variables that show a significant relationship to compliance are education level (p = 0.034) and family support (p = 0.002). In contrast, gender (p = 0.859) and age (p = 0.640) do not have a significant relationship. These findings underline the importance of education-based interventions that are tailored to the patient's educational background, as well as active family involvement as an integral part of the care process. This study recommends a more holistic service approach, which not only emphasizes clinical aspects, but also considers social, psychological, and cultural factors of patients as important determinants of compliance behavior.

Abstract

This study analyzes the role of administrative staff in enhancing the effectiveness of the Hospital Management Information System (SIMRS) in recording the daily census of patients under the Health Service Guarantee (JPPK) at Hospital X. Employing a case study design with qualitative and quantitative methods, data were collected through observations and document analysis of census reports from January to March 2025. Results indicate a significant correlation between the accuracy, competence, and compliance of administrative staff and the effectiveness of SIMRS. An error rate of 2.96% was found, primarily caused by inaccurate input of consulting doctor data, medical procedures, and medication details. Medication changes were often due to stock limitations, requiring substitutions and approval from patients, which in turn demanded timely adjustments by staff. Errors in medication pricing occurred due to delays in updating price information in the SIMRS system. These issues point to a broader problem of limited coordination and delayed information flow between hospital units. The findings underscore the importance of administrative staff with adequate knowledge and skills in using SIMRS. Their performance significantly impacts the accuracy and efficiency of JPPK patient census recording, highlighting their strategic role in supporting integrated hospital information systems.

Abstract

This study aims to analyze the patient registration flow at the Eye Clinic of Dustira Hospital Cimahi, with a particular focus on the efficiency of service and the level of patient satisfaction. The background of this research lies in the increasing demand for quality and efficient healthcare services, especially in specialized outpatient clinics that serve high volumes of patients daily. The research was conducted using a qualitative descriptive method during the Field Work Practice (PKL) period, with data collection techniques consisting of direct observation of the registration process and semi-structured interviews with both administrative staff and patients. The findings show that the registration flow at the Eye Clinic has been arranged in a systematic manner, starting from the arrival of patients, queue number retrieval, identity verification, and input into the hospital information system. However, several issues were identified, such as long waiting times during peak hours and limited patient understanding of the available digital or online registration options. These factors were found to affect overall patient satisfaction. Despite the use of a structured system, the effectiveness of the registration process still depends on patient awareness and digital literacy. Therefore, the study recommends regular evaluation of the service workflow and the implementation of patient education programs regarding the use of digital registration tools. Improvements in communication, guidance signage, and staff support can further enhance service efficiency and ensure that patients receive timely and satisfactory care at the Eye Clinic.
