Abstract

Background: Extraction has been known as a method for removing the tooth from its socket. Although extraction is frequent, the incidence of post extraction complication are commonly found, about 2,6-30,9%. Complications can inhibit wound healing and bone regeneration of socket. PRP is Platelet Rich Plasma containing growth factors that plays an important role in wound healing, induces bone regeneration and stimulates osteoprogenitor cell into osteoblast cell. Chitosan is a chitin derivate that extracted from the crab shells through deacetylation stage. Chitosan properties are osteoinductive, biocompatible, biodegradable, cell proliferation, antimicrobial, antioxidant, antitumor, and stimulates growth factors capabilities. Purpose: To analyze the combination of PRP and Chitosan gel to increase the number of osteoblast in post extraction wound of wistar rat teeth.Method: True experimental post-test only control group design was done in 28 wistar rats. Sample were divided into 4 groups: PRP gel, combination PRP and Chitosan gel, Chitosan gel and control group Povidone Iodine. Dextra Mandible incisors were extracted and treated accordingly. Number of osteoblast in post extraction socket was observed microscopically after 14 days using IHC staining. Data analyzed by using One Way ANOVA parametric test followed by Post Hoc LSD test.Results: There were statically signifcant difference between number of osteoblast in combination PRP and Chitosan gel group compare to PRP gel group (p=0,736), Chitosan gel group (p=0,402) and Povidone Iodine group (p=0,613).Conclusion: Combination PRP and Chitosan gel has a good effect in increasingnumber of osteoblast in wound healing post dental extraction

Abstract

Background: Wound healing is a complex biological process. Fibroblast is one of cell which is responsible for the wound healing process. Virgin Coconut Oil (VCO) have health benefit as antibacterial, antiinflamatory, antioxidant, and immunomodulatory. This research was purposed to know the influence use of VCO over the amount of fibroblast in wound healing after dental extraction.Method: Research quasi experimental with post test control group design and using 15 Rattus novergicus which were divided into three groups (control positive, VCO topical application, and VCO per oral application). Tooth removal was committed by using explorer, scalpel-blade, and arterial clamp. Then VCO applied everyday for 7 days. Day 7th Rattus novergicus were died, the socket after extraction were made into histological form, then the amount of fibroblast were counted with 5 field of views and made in average. Result: The number of fibroblast of the most numerous was VCO per oral ( 48,6 ± 49,8 ), VCO topical ( 36,2 ± 38,8 ), and control positive ( 33,4 ± 35,2 ). The results of the Saphiro Wilk test and Levene test acquired data normally distributed and homogeny, so it continued to One Way Anova and data showed there were significant difference among the groups of samples with value p<0,05.Conclusion: The conclusion of this research is VCO could increase the number of fibroblasts in wound healing after dental extractions

Abstract

Background: Chronic osteomyelitis mandibula is one of the complications from dental extraction. Inadequate wound handling can have an impact on the spread of infection in the surrounding tissue like nerve which results in facial nerve paralysis. The purpose is to present a rare case that facilitative nerve paralysis as a result of the spread of osteomyelitis infectionCase Management: A 69 years old woman with chief complains numbness onher lips accompanied by pus out beside the lower teeth. No sistemic disease. Panoramic radiograph showed abnormal bone-like sequester. Extraoral examination appeared the bluish color on the right cheek and there was right facial muscle paralysis. Debridement, sequesterectomy by general anesthesia and medication using ceftriaxone intravenous, ketorolac injection, multivitamin, and corticosteroid, physiotherapy for facial nerve paralyze, also.Discussion: Pathogenesis mandibular osteomyelitis involves contiguous spreadfrom an odontogenic focus infection. The bacteria produce an exotoxin, which, while unable to cross the blood-brain barrier, can have deleterious effects on thePeripheral Nerve System (Fasialis Nerve) in up to 75% of cases, with the severity of presentation correlating with the severity of the infection.Conclusion: Chronic mandibular osteomyelitis can spread the infection to around another anatomy oral cavity like facials nerves.

Abstract

Background: Osteoradionecrosis (ORN) post dental extraction is post dentalextraction complication with post radiation cancer theraphy. Objective : to present rare case, ORN post dental extraction with post radiation Ca nasopharing therapy 3 years ago.Case Management: A 54 years old patient reported to the dental out-patient department with a chief complaint of pus discharge from right buccal since post dental extraction 6 months ago. He gave a history of a nasopharing Carcinoma with histopatology as squamous cell carcinoms 3 years ago and radio therapy but no surgery. Intraoral examination, exposed necrotic bone found from right lower retromolar area 46 with pus discharge. Radiographic view was likely squester. Local surgical debridement and the sequestrectomy was undertaken with general anaesthesia. Antibiotic injection treatment was ceftriaxon 2x 1gram, infus metronidazol 3x500 mg and ketorolac 3x1 ampul, the patient was treated for 3 days and educated to maintain his oral hygiene with povidone iodine gargle.Discussion: Osteoradionecrosis (ORN) is late effect of radiation therapy thatresults in irreversible tissue death, which is clinically observed as bony exposure for more than 3 months duration. The mandible is affected more often than the maxilla or any other bones of head and neck region. The incidence of ORN in the mandible is reported to be between 2% and 22% and most often affects the body of the mandible. Ideal time is one year minimal post radiotherapy to get maximal vascularization for optimal healing. But immunity factor and radiation doses can trigger emergense ORN.Conclusion: Need time consideration, clinic analysis and pathologys before doing dental extraction for post radiotherapy cancer cases to prevent ORN.

Abstract

Background: Ivory palm water (Cocos capitata) has a high potassium content. Potassium ion can depolarized excited dental nerve on dentin hypersensitivity. This study was to determine the effect of water gel ivory palm against dentine hypersensitivity by the anxiety level of mice using RGS (Rat Grimace Scale).Method: This research has been through ethical clearance which researchmethod was quasy experimental in vivo. The sample was 16 Wistar rats (Rattus novergicus) male 4-5 months with 4 mice in each group. Group I as a positive control. Group II as the ivory palm water gel 10% treatment group. Group III as the ivory palm water gel 70% treatment group. Group IV as a negative control. Research data analysis used Kruskal Wallis test with p <0.05 then continued by Mann Whitney test.Result: Kruskal Wallis test obtained significant difference between the four study groups (p <0.05). Mann Whitney test results in comparison either between positive control group and ivory palm water gel 10% and 70%, and comparison group of ivory palm water gel 10% and 70% weren’t significantly different.Conclusion: This study conclusion there was no significant difference between ivory palm water gel 10 % and 70 % in reducing anxiety level in male Wistar rats with dentinal hypersensitivity

Abstract

Background: The number of cleft lips or palatal cases is still very high in Indonesia, the growth of cleft lips or palatal cases was 1: 700. This prevalence varies for each region in Indonesia. The condition and composition of saliva in children affect the condition of dental caries, including protein statherin, histatin -1, and calcium minerals.  Statherin has the strongest interaction with calcium hydroxyapatite, Histatin-1 has an antimicrobe and antifungal effect and calcium is an imported mineral component The purpose of this study was to analyze the children's saliva of cleft lips with or without cleft palatal non- syndromic against the risk of caries. Material and Method: This study used an Observational Analytic method with a Post Test Only Control Group Design. Saliva samples were taken by the pipetting method, saliva samples were taken from 20 children with cleft lips with or without palate and 20 normal children, aged 3-17 years, with good general health. Analysis levels of Statherin, Histatin-1, and Calcium minerals used the ELISA Method. Statistical analysis used ANOVA. Result and Discussion: The results showed that the average level of statherin for children with a cleft lip was 0.65 µg / ml, whereas for normal children was 1.1 µg / ml, the results of the analysis of histatin-1 salivary protein showed an average of 0.86 ± 0.50 at cleft children and 1.01 ± 0.71 for normal children, mean calcium levels in saliva for cleft children is 1.09 ± 0.09 mmol / L and normal children were 1.34 ± 0.13 mmol / L. ANOVA analysis for the three salivary components were found to be p = 0.001, which means that there were significant differences between the average levels of statherin, histatin-1, and calcium in cleft lips children with or without cleft palate and normal children. Conclusion: The levels of statherin, histatin-1, and salivary calcium can be early diagnostic risk dental caries in children with cleft lips/palate nonsyndromic, to optimize management of preventing dental caries on clefts cases.

Abstract

Background: The problem of impacted teeth is often asked by high school students. A student conselor must be provided with knowledge related to impacted teeth and their impact. The purpose of this study was to determine the level of knowledge of student conselors about impacted teeth.Method: Questionnaires were distributed to 40 student conselors, before and after counseling, then the differences were analyzed using SPSS.Result: Based on statistical calculations, the p value of 0.000 (p <0.05) means that there was a significant difference between the knowledge of student conselors before and after counseling. Conclusion: there was a difference in tooth impaction knowledges between before counseling and after counseling

Abstract

ABSTRACTBackground: The healing of wounds in the lip region is impeded by the fact that chewing food and speaking can exert pressure and attract the wound. Rosella flowers were chosen because they contain anti-inflammatory flavonoids, anthocyanins, tannins, and saponins, as opposed to chemical medications for wound healing that often induces negative effects. Purpose: This study's objective was to assess the influence of 15% roselle flower extract (Hibiscus sabdariffa L) in a paste formulation on the expression of IL-6 and TNF Alpha during the incision wound healing phase. Method: The method is experimental analytic research with post-test-only control group design. There were two groups: control group and the treatment group. Each group contains 3 Rattus Norvegicus. They were incisions in the upper labial mucosa using blade with a length of 5 mm and a depth 1 mm. Roselle flower extract (Hibiscus sabdariffa L) 15% was applied two times per day for three days in treatment group. After the treatment, the rats were sacrificed to check the expression of IL-6 and TNF Alpha using the Immunohistochemistry method. Result: The results of the analysis of the ANOVA-test show that for TNF Alpha expression it has a significant value (p) of 0.054 or p > 0.05, meaning that there is no significant difference. Meanwhile, the IL-6 expression obtained a significant value (p) of 0.009, meaning that there was a significant difference between the control and treatment groups. Conclusion: The study concluded that 15% rosella flower petal extract in a paste reduced IL-6 expression during the inflammatory but not TNF Alpha. Keyword : Roselle flower; Interleukin 6 (IL-6); TNF Alpha, Oral mucosa; Wound healing

Abstract

Background: Periodontitis is the inflammation of periodontal tissue that is characterized by the migration to the apical epithelial jungsional with clinical signs of an increase in probing depth. Periodontitis is one of the oral manifestations in patients with DM. CRP is a protein alpha-globulin produced in the liver and increased levels of inflammatory process in the event. In diabetic patients increased levels of CRP resulted in periodontal tissue destruction. The purpose of this study was to determine the profile of CRP levels in patients with type 2 diabetes mellitus with periodontitis. Method: This research was conducted using analytical observational research methods. The samples used were type 2 diabetes patients with periodontitis and the control group of patients with type 2 diabetes without periodontitis in Sultan Agung Islamic Hospital . All of samples were examined periodontal index, levels of CRP and oral hygiene. Result: Mann Whitney's test show significance p=0.000, which means there are significant differences between groups. Spearman-Kendall's correlation show that relationship between CRP and periodontal index p=0.000 with a correlation coefficient 0.875 means p<0.05 there was that a significant relationship exists between them and the relationship is strong . In relation OH and periodontitis index p=0.030 with a correlation coefficient of 0.650 means p<0.05 there was that a significant relationship exists between them and the relationship strong . Conclusion: It can be concluded that CRP levels in patients type 2 diabetes with periodontitis is higher than the patients type 2 diabetes without periodontitis. And there is a strong relationship between levels of CRP with periodontitis index , and also a strong relationship between periodontitis and OH index .

Abstract

Background: Child anxiety is a worries that comes from unknown sourceand most important factor that caused the problems of child visits to check the condition of oral health to dentist. Music applied as distraction to relieve anxiety, music can provide comfort and relaxation that becomes way to lowering psychological anxiety. Purpose of this study is to determine effect of instrumental music of Kitaro and traditional music of Javanese Langgam on the level of child’s anxiety before dental treatment.Method: Quasi-Experimental research with pretest-posttest design contol group was conducted on 74 samples were divided into 2 groups: instrumental music of Kitaro and traditional music of Javanese Langgam. Anxiety levels were measured by Wong Baker Face Scale questionnaire and pulse. Hypothesis was tested by Wilcoxon test on Wong Baker Face Scale questionnaire and paired sample t-test on measurement of pulse. To determine child’s anxiety level differences between two music were analyzed by Mann-Whitney test.Result: Wilcoxon test results on both music showed p=0.000 and results of paired sample t-test on both music showed p=0.000 this showed there were significant difference between before and after treatment. On Mann-Whitney test showed p=0.096 at Wong Baker Face Scale questionnaire and p=0.006 at pulse measurement, this showed there were significant difference between two groups.Conclusion: From these results concluded that there were difference effect of instrumental music of Kitaro and traditional music of Javanese Langgam in anxiety levels of children before dental treatment.
