Abstract

Stunting is a chronic nutritional problem that affects physical growth, cognitive development, and the productivity of future generations. Prevention efforts need to be carried out upstream, including among adolescents who will become future parents. This community service program aimed to empower adolescents in preventing stunting risk factors through knowledge enhancement, nutrition education, and health screening. The program was implemented at Bali Dewata Health Vocational School in June 2025, involving five lecturers and two students from STIKES Wira Medika Bali. The activities included coordination with the school and primary health center, pretest, health education sessions, health screening (blood pressure measurement and medical history), distribution of educational booklets, and a posttest to evaluate knowledge improvement. A total of 61 students participated in the program. The results showed a significant increase in knowledge about stunting prevention, with the proportion of students in the “good” category rising from 50.8% to 88.5%. Health screening also revealed a small number of students with anemia, a condition that may contribute to future stunting in their offspring. This program highlights the crucial role of adolescents as agents of change in stunting prevention through healthy lifestyles, balanced nutrition, and peer education. With the active involvement of schools, health centers, and students, the program successfully improved adolescent awareness of stunting issues and produced outputs in the form of scientific publication and educational media. Continuous monitoring is planned to ensure program sustainability.

Abstract

Electronic Medical Records (EMR) are designed to improve service quality, documentation accuracy, and administrative efficiency, including reimbursement through the Indonesian Case-Based Groups (INA-CBGs). However, incomplete medical assessments remain a challenge, potentially leading to service gaps and reduced hospital claims. This qualitative descriptive study was conducted at Dharma Yadnya General Hospital, Denpasar, which has fully implemented EMR since 2024. Data were obtained through in-depth interviews with five informants: two medical record officers, the head of the emergency nursing unit, the head of the inpatient nursing unit, and the head of the medical record department. Data analysis used the 3M framework: Man, Method, and Material. From the Man aspect, heavy workloads of doctors and the lack of administrative support staff often caused delays and incomplete documentation. From the Method aspect, although Standard Operating Procedures (SOPs) and system reminders existed, monitoring relied mainly on manual follow-ups via WhatsApp groups, limiting effectiveness. From the Material aspect, the EMR system was user-friendly and supported by weekly vendor maintenance, but occasional network disruptions and insufficient administrative staff still hampered real-time documentation. Incomplete medical assessments in EMR directly affect INA-CBGs claims, especially when comorbidities, complications, or procedures are not recorded, resulting in under-coding and reduced reimbursement. Strengthening human resources, enforcing SOPs, and improving system support are essential to ensure documentation completeness, service quality, and financial sustainability

Abstract

Abstract Tuberculosis under-reporting in private healthcare facilities creates substantial economic burden that has not been systematically quantified in Indonesia, despite private sector handling 47% of TB cases in Denpasar with discrepancy rate reaching 29.1% between Tuberculosis Information System (SITB) and BPJS Healthcare P-Care. This study aimed to analyze and quantify financial losses due to TB under-reporting in Denpasar private clinics using economic burden approach from provider perspective. Descriptive quantitative study with single case study design and mixed-method approach was conducted at Klinik Pratama Karya Prima during August-December 2025. Data collection included comprehensive economic surveys, in-depth interviews with 5 key informants, SITB data extraction (2022-2024), financial document audits, and 16-hour time-motion study observation. Bottom-up costing analysis was applied to calculate cost components including direct medical costs, direct non-medical costs, indirect costs, and hidden costs. The study revealed significant financial losses of Rp 1,419,500 per unreported TB suspected case, with total annual cost of Rp 1,510,000 while revenue only Rp 90,500 per case (16.7-fold gap). Main cost components included diagnostic examinations (52%), hidden costs (19.9%), and staff time allocation (18.2%). Projection for 147 private clinics in Denpasar reached Rp 17.42 billion annually with estimated 3,260 TB cases systematically unreported. The massive gap between capitation payment system and actual cost requirements demonstrates fundamental misalignment justifying urgent investment in integrated reporting systems and improved incentive mechanisms to optimize TB program economic efficiency in private sector.

Abstract

Downtime in the Electronic Medical Records (EMR) system at Udayana Level II Military Hospital has reduced the effectiveness of outpatient services. System disruptions in the registration and admission units have led to long queues and increased the risk of medical errors.  This study aims to analyze the causes of this downtime using the 5M management approach (Man, Money, Material, Machine, Method). A descriptive qualitative method was employed. Data were collected through direct observation and in-depth interviews with key informants in the relevant units. Interview guidelines were used as instruments to explore operational constraints in each of the 5M elements. The results revealed that downtime was caused by limited digital competencies of staff (Man), outdated computer specifications (Machine), and network disruptions due to adverse weather conditions (Material). Additionally, inefficient manual transition workflows (Method) and bureaucratic obstacles in the implementation of the IT budget (Money) were identified as critical contributing factors contributing to the disruptions. That the dominant factors causing downtime lie in the Man, Machine, and Material aspects, supported by the Method and Money factors; therefore, integrated improvements are needed to enhance system reliability and service quality. It is recommended that management streamline the bureaucracy surrounding equipment repairs, upgrade hardware, provide UPS systems, and offer self-mitigation training to staff to ensure uninterrupted service.
