Abstract

ABSTRAK                                                                                     Peningkatan jumlah penduduk lanjut usia (lansia) di Indonesia menimbulkan tantangan serius dalam bidang kesehatan masyarakat, salah satunya adalah kondisi frailty atau kerapuhan. Rumah Senja di Lombok Timur, yang menaungi lebih dari 100 lansia, belum memiliki sistem skrining rutin, dan kader pendamping belum mampu mngidentifikasi frailty secara mandiri. Program pengabdian masyarakat berbasis penelitian ini bertujuan meningkatkan kapasitas kader dalam mengidentifikasi frailty menggunakan instrumen fungsional sederhana. Metode pengabdian yang dilakukan adalah community-based research (CBR) dengan tahapan persiapan, pelaksanaan pendampingan, lalu monitoring dan evaluasi. Kegiatan dilakukan selama empat minggu, yang diikuti oleh 10 orang kader dan dihadiri oleh 49 lansia. Hasil pemeriksaan dengan pendampingan menunjukkan bahwa 28 lansia (57,1%) berada pada kategori frail, 16 lansia (32,7%) dalam kategori pre-frail, dan hanya 5 lansia (10,2%) tergolong robust. Pemahaman kader tentang frailty meningkat dari rata-rata 85 menjadi 87 poin, serta persepsi terhadap pencegahan melalui olahraga, nutrisi, dan dukungan sosial naik dari 85,7% menjadi 90,9%. Sebanyak 81,8% kader merekomendasikan skrining bulanan, sedangkan 18,2% menyarankan minimal dua kali setahun. Temuan ini menunjukkan prevalensi frailty di Rumah Senja cukup tinggi. Pelatihan kader dengan instrumen sederhana terbukti efektif meningkatkan kapasitas deteksi dini. Model skrining frailty berbasis komunitas ini memiliki potensi besar untuk dijadikan contoh di wilayah lain sebagai bagian dari strategi nasional mendukung penuaan sehat di Indonesia.Kata kunci: Frailty; Identifikasi; Instrumen Fungsional; Kader Kesehatan; Lanjut Usia. ABSTRACTThe increasing number of elderly people in Indonesia poses serious challenges in public health, one of which is the condition of frailty. Rumah Senja in East Lombok had more than 100 elderly individuals, currently lacks a regular screening system, and its field cadres are unable to independently identify frailty. This research-based community service project aims to enhance the capacity of cadres in identifying frailty using simple functional assessment tools. The method applied is community-based research (CBR) through stages of preparation, implementation of fascilitation, and monitoring and evaluation. The activities were conducted over four weeks, involving 10 cadres and 49 elderly participants. Screening results with guidance showed that 28 elderly (57.1%) were classified as frail, 16 (32.7%) as pre-frail, and only 5 (10.2%) as robust. Cadres’ understanding of frailty improved from an average of 85 to 87 points, and their perception of prevention through exercise, nutrition, and social support increased from 85.7% to 90.9%. A total of 81.8% of cadres recommended monthly screenings, while 18.2% suggested at least twice a year. These findings indicate a high prevalence of frailty at Rumah Senja. This community-based frailty screening model has significant potential to be replicated in other regions as part of a national strategy to support healthy aging in Indonesia. Keywords: Frailty; Identification; Functional Instrument; Health’s Cadres; Elderly.

Abstract

This article examines a mentoring program for Special Education Support Teachers (Guru Pendamping Khusus / GPK) aimed at improving competencies in behavior management and intervention for Children with Special Needs (CSN) at Buah Hati Learning Center in Mataram. The mentoring program focuses on strengthening GPKs’ practical skills in implementing effective, consistent, and scientifically grounded behavioral intervention strategies. The program seeks to enhance GPKs’ capacity to understand the behavioral characteristics of CSN, conduct systematic behavioral observations, analyze behavioral triggers, and apply positive behavior intervention strategies, while simultaneously reinforcing behavior management systems at the institutional level. The mentoring was implemented using a Participatory Action Research (PAR) approach, encompassing stages of needs identification, action planning, conceptual training, individual coaching, structured observation, and collaborative reflection. Data were collected through interviews, observations, field notes, and documentation of classroom practices. The results indicate a significant improvement in GPK competencies, particularly in the application of positive reinforcement, ABC (Antecedent–Behavior–Consequence) behavioral analysis, management of challenging behaviors, and the development of individualized behavior intervention plans. Positive impacts were also observed among CSN, including a reduction in maladaptive behaviors, improved ability to follow instructions, and the creation of a more conducive learning environment. At the institutional level, the program generated strategic recommendations, including the development of standard operating procedures (SOPs) for behavior management, strengthened supervision mechanisms, and the creation of internal behavior management modules.
