Abstract

Pemeriksaan CT Scan urografi merupakan pemeriksaan yang paling direkomendasikan oleh The European Society of Urogenital Radiology, karena mampu menghasilkan gambaran keseluruhan anatomi Tractus Urinarius dengan relatif cepat dan akurat. Pemeriksaan CT Scan urologi kontras dengan kasus kista ginjal memiliki persentase lebih dari 50% dari keseluruhan pemeriksaan CT Scan urologi tanpa kontras di Rumah Sakit PELNI. Penelitian ini bertujuan untuk mengetahui prosedur pemeriksaan CT Scan urologi kontras dengan kasus kista ginjal di Rumah Sakit Pelni. Penelitian ini dilakukan di Instalasi Radiologi Rumah Sakit PELNI. Penelitian ini bersifat deskriptif analitik dengan berdasarkan  informasi  yang diperoleh dari pengamatan di lapangan dan studi pustaka. Berdasarkan hasil penelitian, pemeriksaan CT Scan urologi kontras dengan kasus urolithiasis di Rumah Sakit Pelni dilakukan dengan persiapan puasa makan makanan berserat dan minum garam inggris agar tractus  digestivus tidak  terisi  udara  maupun  sisa  pencernaan sehingga dapat  meningkatkan visualisasi tractus urinarius beserta patologinya. Pemeriksaan  dilakukanmenggunakan pesawat MSCT Scan GE Optima 660 128 Slice dengan parameter scan interval 5mm, gantry tilt 0, SFOV large body, kV 120, mA 300, alogaritma standard plus, dengan scan type helical. Gambaran CT Scan yang dihasilkan berupa gambar potongan axial dari fase pre kontras, axial dan coronal dari fase arteri, nefrogram dan uretrogram, dan gambaran 3D saluran kemih yang dapat menampilkan kelainan dalam saluran kemih dengan jelas.

Abstract

Pada awal Desember 2019, wabah penyakit COVID-19 yang disebabkan oleh sindrom pernapasan akut parah Coronavirus 2 (SARS-CoV-2) terjadi di Kota Wuhan, Provinsi Hubei, Cina. Indonesia kali pertama mendapati kasus pada tanggal 2 Maret 2020 dan DKI Jakarta merupakan provinsi dengan jumlah tertinggi kasus COVID-19. Untuk itu, dibutuhkan satu sistem yang mampu mendukung dan saling bekerja sama gua menghadapi wabah pandemi COVID. Oleh karenanya, Tim PKM, selaku dosen yang memiliki kewajiban melaksanakan Pengabdian Kepada Masyarakat, pada kesempatan ini berupaya memberikan sosialisasi, edukasi, dan informasi terkini mengenai COVID-19 serta pencegahan dini dan kaitannya terhadap penggunaan alat elektromedik (termometer, tensimeter, tes gula darah, nebulizer dan oximeter) untuk pemantauan kesehatan dan perawatan sederhana di rumah di masa pandemi. Kegiatan ini dilaksanakan secara luring dengan mengundang perwakilan kecamatan Kebayoran Baru, kelurahan Cipete Utara, Ketua RW, Ketua RT, dan Satgas COVID-19 di wilayah tersebut. Turut hadir juga kader di setiap RT yang berhubungan langsung dengan masyarakat sehingga bisa memberikan edukasi setelah sosialisasi ini diberikan. Berdasarkan hasil survei, terdapat 20–25% peserta belum memahami fungsi alat elektromedik dan sekitar 57,5% peserta belum memahami penggunaan, penyimpanan, serta pemeliharaan alat elektromedik dengan baik dan benar. Selanjutnya, setelah sosialisasi diberikan, terlihat adanya peningkatan pemahaman peserta lebih dari 60% mengenai fungsi alat nebulizer dan oximeter. Kegiatan selanjutnya yaitu monitoring kader untuk menyampaikan serta memberikan edukasi ke masyarakat sekitar.

Abstract

Penelitian ini dilakukan untuk mengevaluasi prosedur penggunaan dan pelepasan APD Covid-19, meliputi ketersediaan APD di bagian radiologi, pengetahuan radiografer, pelatihan tentang pemakaian dan pelepasan APD Covid-19, ketersediaan brosur/poster prosedur pemakaian dan pelepasan APD Covid-19, kepatuhan radiografer menjalankan SOP penggunaan dan pelepasan APD Covid-19. Desain penelitian analitik. Penelitian dilakukan pada bulan Mei-Desember 2021, dengan menyebarkan kuesioner melalui formulir online kepada radiografer di Indonesia, diperoleh data sebanyak 85 responen radiografer. Data diolah dengan analisis deskriptif univariat, dan bivariat dengan uji statistik chi square. Hasil penelitian menunjukkan dari 52 rumah sakit yang diteliti, ketersediaan APD_Covid19 berupa masker medik terdapat 2 Rumah sakit mengalami kekurangan, sedangkan 50 Rumah Sakit tercukupi bahkan sangat banyak tersedia. Masker N95 ada 2 Rumah Sakit yang tidak tersedia, yang mengalami kekurangan masker N95 sebanyak 3 Rumah Sakit Pelindung wajah terdapat 3 Rumah Sakit mengalami kekurangan. Radiografer yang bekerja di Level Perlindungan Tingkat 1 sebanyak 10 orang, Level Perlindungan Tingkat 2 sebanyak 34 orang, Level Perlindungan Tingkat 3 sebanyak 34 orang, dan Level Perlindungan tingkat 4 sebanyak 20 orang. Hasil Uji chi squre menunjukkan adanya dampak pelatihan penggunaan dan pelepasan APD Covid-19 dengan kepatuhan radiografer dalam menjalankan prosedur penggunaan dan pelepasan APD Covid-19. Adanya dampak pemahaman penggunaan dan pelepasan APD Covid-19 dengan kepatuhan radiogarfer dalam menjalankan prosedur penggunaan dan pelepasan APD Covid-19. Adanya dampak ketersediaan brosur/poster terhadap kepatuhan radigrafer dalam menjalankan prosedur penggunaan dan pelepasan APD Covid-19. Adanya dampak rumah sakit yang memiliki SOP terhadap kepatuhan radiogarafer dalam menjalankan prosedur penggunaan dan pelepasan APD Covid-19. Adanya dampak rumah sakit yang memberikan penjelasan terhadap kepatuhan radiogarafer dalam menjalankan prosedur penggunaan dan pelepasan APD Covid-19.

Abstract

Pediatric patients are generally uncooperative during radiology examinations. Radiographic images can become blurry due to movement. This will cause to repeat examinations, thereby significantly increasing radiation exposure, and it can pose significant risks to children, patient families, and radiation workers. The research aims to create and test the effectiveness of a fixation device equipped with Audio-Visual elements for pediatric chest radiography examinations in Anteroposterior and Lateral projections. The experimental method involves developing the fixation device in the Radiodiagnostic Department Laboratory of Poltekkes Kemenkes Jakarta II. The effectiveness of the fixation device is assessed through surveys and interviews involving 66 respondents at hospitals in Jakarta from January to July 2023 during pediatric chest examinations. The average results indicate that the device can be used in hospitals (3.27±0.63), it’s safe to use (3.00±0.74), and highly effective (3.17±0.67). The overall average value of 3.16±0.68, suggests that the fixation device is suitable for use in pediatric chest examinations. The obtained images from the Anteroposterior and Lateral projections optimally describe of lung organs, the heart, and blood vessels within the thoracic cavity. Suggestions for further development of the safe device include the addition of leg supports, pediatric chair can be moved forward/backward and patient restraints to prevent falling or movement

Abstract

Breast radiotherapy in breast cancer patients often uses CT landmark wires to pinpoint the area of ​​breast scars for appropriate treatment. Most commercial CT marker wires are expensive and made of high-Z materials that cause scratch artifacts. In hospitals, scar markers are made from nylon string with an iohexol contrast medium. This research aims to analyze a simulation of a CT examination in breast cancer cases using a string scar marker with iodine and analyze its characteristics. For this reason, the authors created a new breast CT scar marking wire using nylon rope combined with iodine contrast media in CT simulations of breast cancer cases for treatment planning. Three patients were scanned using a string scar marker developed with iodine for breast cancer in the hospital's CT simulator facility. As a result, using a computer console, the CT number showed this string scar marker had clear visibility and did not produce artifacts. In conclusion, this new string breast scar marker is fast, flexible, and inexpensive. This is beneficial for the treatment of breast cancer patients.

Abstract

Background: Cardiac MRI examination is relatively rare in Indonesia. The effect of the use of volume shim on moving organs, such as the heart is relatively unknown and noticed by radiographers and cardiologists. Purpose: To analyse the image quality of 4 chamber sections of Cardiac Magnetic Resonance Imaging with and without the use of shim volume on Steady State Free Precession (SSFP) sequences so that it can determine the most optimum 4 chamber images to maintain the diagnostic value by physician. Method: This research is designed through quantitative analytic approach with experiment method. The total samples used are 13 subjects ranging from 7 to 80 years old and 5 respondents and the sample undergone into a series of examinations of Cardiac Magnetic Resonance Imaging. Afterward, the 4 chamber sections in SSFP sequence were given different treatment, namely, with and without the use of shim volume. The result will be assessed by the respondents and then it will be done by non-parametric Wilcoxon Two-Sample Test and parametric Paired Sample T Test. Result: Thereafter, the result of statistic from the respondent assessment is that the quality of anatomy images has p=(0.113) whereas the p=(0.354) for the degree of artefact images and clarifies that the anatomy image quality and the degree of artefact is not too much different. Conclusion: The conclusion of the research is with and without the use of shim volume is not too significant to affect the quality of 4 chamber images.

Abstract

Background: Routine pelvic radiograph examination commonly performed in diagnostic radiology services is a pelvic examination of anteroposterior projection (AP). This results in direct exposure to ionizing radiation to the internal organs in the lower abdomen, especially the reproductive organs. Purpose: Optimizing the dosage of AP projection pelvic radiographs. Method: Research is conducted quantitatively and used experimental approach with an analysis of 15% mAs reduction from the standard mAs value on AP projection pelvic examination on radiation dose and image quality using anthropomorphic phantoms. The radiation dose analysis uses the exposure index indicator. In contrast, the image quality uses the Visual Grading Analysis method, then it was continued with the Kappa Cohens test to determine the level of agreement between 2 respondents. Result: The exposure factor with the mAs value reduced by 15% from the standard exposure factor on the AP projection pelvic radiography affected the decrease in the value exposure index. The EI value generated from the optimization exposure factor shows a decrease to 300 EI, which means the dose produced is only half of the standard exposure factor dose but still produces an acceptable image, as evidenced by the VGA score. As an assessment of image quality. In this study, there was no significant difference with range VGA scores 1,7. Besides that, the test had the Kappa Cohens level of agreement with a value of 0.5-0.8. Conclusion: All images resulting from the 15% mAs value reduction variation can be used as a diagnostic assessment.

Abstract

Radiation accidents pose a significant risk, often stemming from inadequate calibration of radiation output, procedural lapses, and faults in radiation monitoring systems. Addressing these risks is pivotal, particularly in radiotherapy, where the precise functioning of linear accelerator (Linac) devices is crucial. This study delves into the quality control procedures applied to the Linac radiotherapy device within the Department of Radiotherapy at Pasar Minggu District Hospital. Drawing upon the renowned reference standards set by the AAPM Task Group 142 and BAPETEN Regulation No. K2N.2/MT-08, this research, conducted in November 2022, rigorously evaluated the mechanical, dosimetry, and safety aspects of the Linac device. The examination encompassed critical elements, including gantry angles, optical distance indicators, laser precision, collimator angles, light field size, and photon and electron beam outputs. Safety features such as door interlocks, audiovisual indicators, and radiation alert systems were scrutinized. The analysis reveals a reassuring finding: the Linac device at Pasar Minggu District Hospital adheres commendably to tolerance limits specified by reference standards across all measured parameters, indicating robust performance in mechanics, dosimetry, and safety. This meticulous quality control regimen has proven highly effective in ensuring the device's operational integrity and safety, affirming its reliability for precise radiotherapy treatments.

Abstract

Salah satu penyinaran radioterapi yang menjadi pilihan di berbagai rumah sakit di Indonesia adalah penyinaran kanker otak dengan teknik 2 Dimensi. Di Instalasi Radioterapi RSUD Prof. Dr. Margono Soekarjo menggunakan teknik 2 Dimensi dengan alat teleterapi Cobalt-60 untuk kasus kanker otak. Sehingga penatalaksanaan tumor otak dibutuhkan sebagai rujukan dasar penting untuk dilaksanakan. Penelitian ini dilakukan dengan tujuan mendeskripsikan teknik penyinaran radiorerapi dari teknik penyinaran radioterapi dengan kasus kanker otak dengan menggunakan alat teleterapi cobalt-60. Jenis penelitian ini bersifat deskriptif kualitatif yaitu studi kasus dengan melakukan observasi di Instalasi Radioterapi Rumah Sakit Umum Daerah Prof. Dr. Magono Soekarjo, Purwokerto dari tanggal 1 s/d 29 Mater 2020 dengan mengambil sampel sebanyak 1 orang pasien. Hasil dari penyinaran radioterapi kanker otak dengan menggunakan cobalt-60 adalah radiasi dengan metode 2 dimensi dengan dua lapangan penyinaran yaitu dengan sudut 90° dan 270°. Dosis total radiasi yang diberikan ke pasien sebesar 60Gy dengan fraksi sebanyak 30 kali. Pada penelitian ini tidak menggunakan alat verifikasi untuk mendapatkan posisi target yang sebenarnya.

Abstract

Several previous studies have shown that without dose evaluation and monitoring, there is a potential for deterministic radiation effects in 3D-CRT procedures. This study is expected to close the gap by conducting a more comprehensive evaluation of the 3D-CRT technique for breast cancer patients. The research method is quantitative analysis with a study sample of 20 breast cancer patients. Data processing was carried out by conducting statistical tests for the distribution value of organs at risk (OAR) against the Radiation Therapy Oncology Group (RTOG) guidelines and analyzing the conformity index (CI) and homogeneity index (HI) values according to the values recommended by the International Commission Radiation Unit (ICRU). The results of this study obtained OAR doses for the lungs and heart are still within the tolerance value limits set by RTOG p-value <0.05. then for CI and HI there is a deviation p-value <0.05 that there is a difference between the values obtained and the ICRU recommended value. This is due to the location of the target or tumor adjacent to the OAR, to keep the OAR value in accordance with the specified limit value.
