Abstract

The three parts of the ear are the external ear, middle ear, and inner ear. The ear contains cerumen, a natural material that protects the external ear canal. Proper ear hygiene and health are essential to prevent ear infections, ear ringing, hearing loss, and sudden deafness. In addition to improper ear cleaning, other factors might contribute to ear disorders, including the misuse of personal listening devices. SD Muhammadiyah Sapen students would be educated on the significance of maintaining ear health through this service project. This community service activity is conducted online via mini-webinars or counseling using zoom meetings, presentations, educational films, and brochures on how to maintain ear health. This extension is quasi-experimental, with a pre-and post-test design involving a single group, utilizing the Paired Samples T-Test model for data analysis. The activity commences with a pre-test administered to students, continues with the presentation of information, discussion, and question-and-answer sessions, and concludes with a post-test designed to assess students' level of knowledge and the activity's success. Twenty students participated in this community service event. Conclusion: There is a significant difference between the average pre-test and post-test scores with a p-value of 0.01 (0.05).

Abstract

Background: Foreign bodies in the larynx are reported quite rarely, with a prevalence of around 4% among patients, especially geriatrics, and represent an acute airway emergency with significant morbidity and mortality rate. Objective: This case report aimed to provide information on managing the extraction of foreign bodies and the accompanying problems. Discussion: A 49-year-old man with a history of wearing dentures was admitted to the emergency room. The patient complained of a hoarse voice upon waking up, painful swallowing, and a lumpy sensation in his throat, but he had no chest pain, cough, nausea, or vomiting. An indirect laryngoscopy examination revealed a foreign body lodged in the anterior rima glottis, impeding the mobility of the vocal folds. Under general anesthesia, emergency direct laryngoscopy, evacuation, and tracheoscopy were performed. It was observed that a foreign denture body was lodged in the larynx and was evacuated using grasping forceps. Post-evacuation, the vocal folds were swollen, with a false vocal fold hematoma, posterior subglottic laceration, and no active bleeding. Conclusion: Foreign bodies in the larynx must be prompted and gently removed because it leads to life-threatening. In suspected swallowed foreign bodies, plain radiographs are the initial imaging approach. Inhalational or intravenous induction is operated to administer the general anesthetic, along with the maintenance of spontaneous ventilation.

Abstract

Gallstone migration into the cystic duct aperture can prevent bile from exiting the gallbladder during contraction. Gallbladder wall strain increases due to a distinct form of pain (biliary colic). A more prolonged cystic duct obstruction can cause acute gallbladder inflammation (acute cholecystitis). Cholecystectomy is a popular treatment for gallstone-related illnesses. We present a 49-year-old male with heartburn, nausea, and vomiting for one month, worsening in the last week. On general examination, vital signs and urine output are expected. On clinical examination, he had tenderness on the upper right quadrant abdomen, and Murphy's sign was positive. An abdominal ultrasound (US) showed multiple cholelithiasis. The patient was performed elective laparoscopic cholecystectomy with pneumoperitoneum by Muyo hook. The patient was discharged on the third day after surgery. Recent research has highlighted this unusual method. In this unique and challenging situation, Muyo Hook may be an option.

Abstract

Chronic suppurative otitis media (CSOM) is a pathological disorder affecting the ear, characterized by a persistent and long-lasting middle ear infection accompanied by the absence of an intact tympanic membrane. Multiple measures can be employed to evaluate the quality of life among patients with CSOM. One such instrument is the chronic otitis media questionnaire-12 (COMQ-12), which has demonstrated satisfactory levels of validity and reliability. The utilization of the COMQ-12 questionnaire has the potential to improve the quality of life experienced by CSOM patients. This study aimed to evaluate the validity and reliability of the Indonesian adaptation instrument of the COMQ-12 in patients with CSOM in the Otorhinolaryngology department of multiple hospitals in Yogyakarta. The study was conducted from February to July 2023. The study comprised 48 participants, consisting of 16 males and 32 females. The adapted Indonesian version of the COMQ-12 instrument demonstrates validity and reliability in assessing patients' quality of life with CSOM, as evidenced by a statistically significant r-value of 0.2353. The internal consistency analysis reveals that Cronbach's α coefficient is 0.868. The Indonesian adaptation of the COMQ-12 demonstrates validity and reliability as an assessment tool for measuring the health-related quality of life in patients diagnosed with CSOM.

Abstract

Nasal irrigation with saline is frequently used as an alternative therapy for sinonasal diseases since it effectively reduces sinonasal symptoms and improves the quality of life when measured by various subjective instruments. The mechanisms of action of saline irrigation remain unclear, but some hypotheses attribute the direct physical cleansing of thick mucus, allergens, and pollutants. This study aimed to determine the effect of nasal saline irrigation on the nasal mucosa using the modified Lund-Kennedy endoscopic score as the objective evaluation. This pre-post-experimental study was conducted at PKU Muhammadiyah Gamping Hospital Yogyakarta, Indonesia, from May to June 2023. The study included 18 individuals presenting with sinonasal symptoms. Participants meeting the inclusion and exclusion criteria who provided informed consent were trained in nasal saline irrigation. Patients were assessed at baseline and one week after nasal saline irrigation using endoscopy, TNSS, and MCC time. Results revealed that the mean of endoscopic scores before and after saline irrigation were 6.89±1.1 and 5.33±1.4, respectively (p-value 0.006),  while the mean of TNSS scores before and after saline irrigation was 6.17±2.3 and 4.28±2.6, respectively (p-value 0.006). Thus, nasal saline irrigation significantly improves the endoscopy and TNSS scores, but not the MCC time.

Abstract

Hemorrhoids are the third most frequent gastrointestinal condition. The symptoms of hemorrhoid illness may include pain, itching, bleeding, soiling, and swelling. This study aimed to examine the impact of treatment on hemorrhoid patients’ quality of life. 36 patients with hemorrhoids between 18 to 60 took part in this longitudinal study before and after treatment between April and August 2022. Health-related quality of life (HRQOL) before and after treatment is 0.037<0.051. Lower quality of life in terms of essential health is indicated by the health scale modified for hemorrhoid disease symptom score (HSHD) since the score before treatment is lower than the score after treatment. The p-value thus shows a significant impact both before and after treatment. The research on hemorrhoid patients showed that the patient’s quality of life increased after treatment.

Abstract

Allergic rhinitis (AR) is a typical chronic condition commonly affecting the upper airway. Allergic rhinitis is marked by nasal congestion, rhinorrhea, pruritus, sneezing, and postnasal drip syndrome (PND), which includes eye itching and erythema. This study aims to assess the validity and reliability of the Indonesian version of the knowledge, attitude, and practice (KAP) of the allergic rhinitis questionnaire. The study employed a cross-sectional design. The study included patients with allergic rhinitis who were 18 or older and visited the otorhinolaryngology clinic between January and February 2024. The questionnaire was generated and disseminated using a Google form, and the statistical analysis was performed with SPSS 22.0. The correlation coefficient (r) between all questions on the knowledge dimension was consistently high, ranging from 0.302 to 0.828. The items on the attitude dimension had a strong positive association with the total score, as seen by the high correlation coefficients (all r = 0.962). The practice dimension also exhibited high correlation coefficients, ranging from 0.273 to 0.810 in total score. The questionnaire's validity and reliability have been established, confirming that the Indonesian version is recommended for future studies.

Abstract

Circumcision is a common surgical operation that entails excising the foreskin from the penis, primarily to improve hygiene and prevent the accumulation of smegma. This practice is widely observed worldwide, especially within Muslim and Jewish populations. However, despite its advantages, circumcision may lead to several complications, such as excessive bleeding and infections. A healthy 13-year-old male presented for circumcision due to grade 2 phimosis, seeking to improve his personal hygiene. The procedure was executed using a combination of diode laser technology and cyanoacrylate skin glue, which minimized tissue trauma and provided effective wound closure. When circumcision is performed utilizing advanced methods such as diode laser and cyanoacrylate glue, it is regarded as a safe and effective intervention. This approach offers considerable benefits in terms of recovery duration, cosmetic results, and overall patient comfort, while also maintaining low rates of complications.
