Abstract

Treatment of cardiovascular diseases tends to be expensive and requires long-term therapy. Investigation about the effect of pumpkin seeds on prevention of endothelial dysfunction of blood vessel has never been reported yet, accordingly, bodies of research are required to know its effectiveness. This study aimed to analyze the expression of Vascular Cell Adhesion Molecule (VCAM) in male Wistar rats (Rattus norvegicus) with hypercholesterolemia supplemented with pumpkin seeds extract (Cucurbita moschata Duch). This was an experimental laboratory study with randomized post-test control group design. The independent variable was pumpkin seeds extract, and the dependent one was expression of VCAM. Data analysis employed chi-square test to test the difference between control and treatment groups. Study results showed that there was significant difference (p=0.000) between VCAM expression in group-K2 (positive control) as compared to group-K1 (negative control) and group-K5 (treatment group-dose 3) α=0.001, whereas no significant difference was found between that of as compared to group-K3 (treatment group-dose 1) α=0.125 and group-K4 (treatment group-dose 2) α=0.05. The study showed reduction of VCAM expression in treatment groups consisted of male Wistar rats with hypercholesterolemia supplemented with pumpkin seeds (Cucurbita moschata Duch) extract. Abstract

Diabetes mellitus is a chronic disease that requires long-term management. There is a tendency in type 2 diabetes mellitus patients to use herbs as a complement to medical treatment. The purpose of this study was to analyze the use of herbs as a complement to medical treatment in reducing blood sugar levels of T2DM patients. This type of research is quasi-experimental, with 40 research subjects divided into 4 groups. Determination of research subjects by purposive sampling in accordance with established inclusion criteria. Data analysis using paired T test. The majority of respondents was women (77.5%) and had no genetic history (75%). The duration of type 2 diabetes mellitus  majority was >5 years 72.5%). Data on random blood sugar levels before (p=0.098) and after (p=0.113) intervention were normally distributed and homogeneous p>0.05 (0.839). The treatment group (groups 2, 3, and 4) showed a significant decrease in random blood sugar levels as the control group (1) with a significance value <0.05. Giving herbs has a tendency to reduce random blood sugar levels when used as a complement to medical treatment. Bitter melon shows the effect of decreasing random blood sugar levels greater than the provision of bay leaves and cinnamon.

Abstract

Diabetes mellitus type 2 patients who have a tendency to use herbs, are based on the patient's health belief that herbal medicine is a natural ingredient that has efficacy and is safe. This study aimed to explore health beliefs as predictors of herbal use as a complement to diabetes self-care. A cross-sectional study was conducted on 230 patients with type 2 diabetes mellitus who had been clinically diagnosed, received medical treatment and used herbs. The data was collected using a questionnaire at seven community health centers (Puskesmas) in Surabaya City. Data analysis using Partial Least Square. The indicator of health belief variables has a loading factor value>0.5: perceived vulnerability and severity (0.820), perceived benefits (0.739), perceived barriers (0.822), sense of self-efficacy (0.695). Health beliefs have a significant effect on the use of herbs as a complement to diabetes self-care, the value of T>1.97 (T>10.07). Health beliefs consisting of perceived vulnerability and severity, perceived benefits, perceived inhibition, and perceived self-efficacy were strong predictors associated with the use of herbs as a complement to diabetes self-care. Nurses as part of health workers must pay attention to patient health beliefs in providing education to patients.

Abstract

Introduction: Hypertension in elderly is triggered by the thickening and stiffness of arterial wall due to ageing processes. One attempt to prevent the complications of hypertension is by change their lifestyle, such as perform a low-salt diet. The implementation of a low-salt diet on elderly should be adjusted with their behavior and habits. TPB is one of theoretical attempt to identify the relationship between elderly’s attitude and their behavior, and to emphasize the importancy of intention as determinant of behavior, focused on cognitif and rational decission making process. This study was aimed to analyze the improvement of low salt diet behavior based on TPB on elderly with hypertension.Method: This research was used descriptive design with observational analytic method. Population were elderly with hypertension recorded at Puskesmas Pucang, Kota Surabaya. Samples were 32 respondents, taken randomly. Data were collected by using questionnaires and direct interview. Data were then analyzed by using Partial Least Square (PLS).Result: The results had showed that intention to implement a low-salt diet was built by attitudes toward behavior, subjective norms, and perceived behavioral control (PBC) on elderly with hypertension. A low-salt diet behavior was built by intention to implement a low-salt diet on elderly with hypertension.Discussion: These results can be used as a model to improve low salt diet behavior on elderly with hypertension.

Abstract

Hyperglycemia in patients with Diabetes Mellitus (DM) can lead to complications such as diabetic foot ulcers. Therefore, efforts are needed to prevent this chronic complication through proper foot care. This study aimed to determine the behavior of patients with type 2 diabetes mellitus (T2DM) in performing blood glucose monitoring and correct foot care. This study is a descriptive cross-sectional study, with a sample size of 125 patients with T2DM from the Community Health Center in Surabaya Indonesia. Data was collected with the use of a self-made questionnaire from May to October 2018. The variables of the study were patient characteristics, random blood glucose levels, blood glucose monitoring and foot care skills. The data analysis used was Spearman rho to determine the factors associated with foot care behaviors.  Fifty-six percent (56%) of the participants have poor behavior on foot care, 36 % have moderate and only 7.2% have good.  Factors that were significantly associated with foot care behavior were signs and symptoms (p = 0.023), blood glucose monitoring (p = .000), and random blood glucose levels (p = 0.040). The behaviors of patients with T2DM in blood glucose monitoring and foot care need to be improved to prevent complications from diabetic foot ulcers. Nurses must provide health education about the importance of regular foot care to patients and their families. The results of this study can be used to further improve the programs and services to prevent DM and delay the progression of chronic complications like diabetic foot ulcers at the community level.

Abstract

The goal of nursing intervention on chronic disease such as Hypertension is to improve the patient self-care, in which the Chronic care model could help to reach the best outcome. Research suggests the community, health workers, and regulations could improve the health. The purpose of this study was to apply nursing intervention based on chronic care model to medication adherence in hypertensive patients. This is quasi-experimental research used pre-post test control group design. 50 respondents of hypertension patients were recruited from Pucang Sewu, Pacar Keling and Tambak Rejo Surabaya Health Centres. The intervention was applicating nursing interventions based on the Chronic Care Model. The Mann Whitney U statistical analysis used to understand the effect of the intervention to the medication adherence in patients with hypertension. A total of 50 respondents who were divided into two groups, control and treatment, with each (n = 25). The distribution of characteristics in the two groups is homogeneous. Different test in the control group, there was no difference. The treatment group shown a significant difference in adherence before and after the intervention with p value 0.000 (p <0.05). This study found the reason of disobedient patient to medication, which is forget and fear to the side effect. The patient and the health care agree frequent reminder could be the solution to this problem. Chronic Care Model based nursing interventions can improve medication adherence in hypertensive patients by encourage the health care system to frequently remind patients.

Abstract

Hypertension is a chronic disease that has earned the title of the silent killer because its symptoms are often without complaint. Many people with hypertension are unaware of their disease for a long time, as the appearance or recognition of symptoms of the disease takes some time. Patients realize their disease after experiencing complications. The target is expected to have good knowledge and understanding of the care and prevention of hypertension patient emergencies, can apply in their daily lives as an effort to manage their disease in order to obtain optimal blood pressure, continue to carry out routine controls to the Puskesmas and take medicine regularly, consume appropriate food in the right amount and do regular and sufficient physical activity. Activities were carried out from June to August 2022, targeting 75 people and health cadres. The method used in the implementation of activities in the form of lectures and questions and answers about healthy living behavior to prevent emergencies in hypertensive patients, demonstrations of measuring blood pressure, BMI, determining salt requirements, regulating daily diet, and hypertension exercises. The media used are modules on healthy living behaviors to prevent emergencies and demonstrations of hypertension exercises. The results and outcomes of community service that have been carried out are almost all (86.7%) female participants, most (52.1%) are ≥61 years old, almost all (93.3%, 97.3%) are Muslim and Javanese, most (58.7%) in the family have no history of hypertension, most (64%) systole pressure ≥ 140 mmHg, most (56%) diastole pressure ≥ 90 mmHg and most (50.7%) cholesterol levels ≥ 200 mmHg. The results of this community service can be used as a motivation to increase community participation in making efforts to prevent and treat hypertension emergencies early in the Pucang Sewu, Pacar Keling, and Tambak Rejo Puskesmas areas, becoming a starting point for the realization of residents who are able to control blood pressure by regulating diet, obeying taking medication, and hypertension exercises and doing regular activities and materials to improve the performance of the Puskesmas while increasing the range of health services for the Puskesmas. Abstract

This pandemic period has multiple impacts on all aspects of life. Both economic, social, religious, and psychological. The psychological side is the great fear in the community as the virus spreads very quickly from the sick to the healthy. Changes in interpersonal relationships and in the economy are often followed by changes in politics, including the life of the nation and the state. Various everyday problems also need to be observed and solved with new approaches. The aim of this research is to examine the impact of counseling on the anxiety level of the surrounding community due to residents found to have positive COVID-19 infection. So that people can understand Covid-19 properly and don't experience anxiety even though there are local residents who are confirmed positive for Covid-19. This study was a quasi-experimental study with a pre-post test control group design. The purpose of this study was to analyze the effectiveness of counseling in reducing anxiety levels in the surrounding community due to residents indicated as positive for Covid-19. Test results showed a significant difference in the treatment group before (pre-test) and after (post-test) counseling (p-value 0.000). The test results between the treatment group and the control group also showed a significant difference in the post-test (p-value of 0.000). Anxiety or stress during this pandemic leads to an increase in concern for one's own health and that of the family. The situation is exacerbated by the fact that information arriving in bewildering abundance will increase public anxiety. There is a need for participation and collaboration between health care facilities and health care workers and health cadres to provide accurate information so that people are properly informed and can reduce their fears so that they are able to take appropriate action. There is a significant effect of counseling to reduce the anxiety level of the surrounding community due to residents who tested positive for Covid-19.

Abstract

PENGETAHUAN IBU TENTANG GIZI SEIMBANG PADA ANAK USIA SEKOLAH DI MI PLUS AL- ISLAM DAGANGAN MOTHER'S KNOWLEDGE ABOUT BALANCED NUTRITION IN SCHOOL-AGE CHILDREN AT MI PLUS AL-ISLAM DAGANGAN ABSTRAK Gizi buruk dan overweight merupakan salah satu permasalahan kesehatan di beberapa negara berkembang termasuk Indonesia. Masalah tersebut tidak hanya terjadi pada balita, tetapi juga pada anak usia sekolah. Salah satu penyebab terjadinya gizi buruk dan overweight adalah tidak seimbangannya asupan nutrisi yang dikonsumsi anak. Penyebab tersebut dapat terjadi salah satunya karena pengetahuan ibu yang kurang tetang gizi seimbang pada anak usia sekolah. Penelitian ini bertujuan ntuk mengetahui pengetahuan ibu tentang gizi seimbang pada anak usia sekolah di MI Plus Al-Islam Dagangan. Desain penelitian yang digunakan adalah deskriptif. Populasi dari penelitian ini adalah ibu siswa kelas 1 sampai kelas 4 di MI Plus Al-Islam Dagangan sebanyak 203 ibu, dengan besar sampel 148. Sampel diambil dengan quota sampling. Data dikumpulkan dengan kuesioner dengan jumlah 10 soal dan diolah dengan analisa presentase serta disajikan dalam bentuk distribusi frekuensi dan persentasi. Hasil penelitian menunjukkan pengetahuan ibu tentang gizi seimbang pada anak usia sekolah  dengan kriteria kurang terdapat 10 ibu (6,8%), kriteria cukup 38 ibu (25,7%), dan kriteria baik 100 ibu (67,5%), dari total 148 ibu. Hasil penelitian ini menununjukkan usia ibu mempengaruhi tingkat pengetahuan ibu, pendidikan mempengaruhi tingkat pengetahuan ibu, dan pekerjaan tidak mempengruhi tingkat pengetahuan ibu. Diharapkan untuk ibu yang memiliki pengetahuan cukup dan kurang dapat meningkatkan pengetahuannya dengan mengikuti sosialisasi atau penyuluhan tentang kesehatan anak yang dilakukan di Puskesmas Dagangan. Bagi ibu yang sudah memiliki pengetahuan baik, diharapkan dapat melaksanakan pemenuhan kebutuhan gizi seimbang agar anak tumbuh dan berkembang dengan optimal. Kata Kunci: Pengetahuan, Ibu, Gizi Seimbang  ABSTRACT Malnutrition and overweight are health problems in several developing countries, including Indonesia. This problem does not only occur in toddlers, but also in school-age children. One of the causes of malnutrition and overweight is an imbalance in the intake of nutrients consumed by children. One of these causes can occur due to mothers' lack of knowledge about balanced nutrition in school-age children. This study aims to determine mother's knowledge about balanced nutrition in school-age children at MI Plus Al-Islam Dagangan. The research design used is descriptive. The population of this study were 203 mothers of grade 1 to 4 students at MI Plus Al-Islam Dagangan, with a sample size of 148. Samples were taken by quota sampling. Data was collected by means of a questionnaire with a total of 10 questions and processed by percentage analysis and presented in the form of frequency and percentage distributions. The results showed that 10 mothers (6.8%) had sufficient knowledge about balanced nutrition in school-age children, 38 mothers (25.7%) had sufficient criteria, and 100 mothers (67.5%) had good criteria, of the total 148 mothers. The results of this study indicate that the mother's age affects the mother's knowledge level, education affects the mother's knowledge level, and work does not affect the mother's knowledge level. It is expected that mothers who have sufficient and less knowledge can increase their knowledge by participating in socialization or counseling about child health which is carried out at the Dagangan Health Center. For mothers who already have good knowledge, it is hoped that they can carry out the fulfillment of balanced nutritional needs so that children grow and develop optimally.
