Abstract

Hyperglycemia in patients with Diabetes Mellitus (DM) can lead to complications such as diabetic foot ulcers. Therefore, efforts are needed to prevent this chronic complication through proper foot care. This study aimed to determine the behavior of patients with type 2 diabetes mellitus (T2DM) in performing blood glucose monitoring and correct foot care. This study is a descriptive cross-sectional study, with a sample size of 125 patients with T2DM from the Community Health Center in Surabaya Indonesia. Data was collected with the use of a self-made questionnaire from May to October 2018. The variables of the study were patient characteristics, random blood glucose levels, blood glucose monitoring and foot care skills. The data analysis used was Spearman rho to determine the factors associated with foot care behaviors.  Fifty-six percent (56%) of the participants have poor behavior on foot care, 36 % have moderate and only 7.2% have good.  Factors that were significantly associated with foot care behavior were signs and symptoms (p = 0.023), blood glucose monitoring (p = .000), and random blood glucose levels (p = 0.040). The behaviors of patients with T2DM in blood glucose monitoring and foot care need to be improved to prevent complications from diabetic foot ulcers. Nurses must provide health education about the importance of regular foot care to patients and their families. The results of this study can be used to further improve the programs and services to prevent DM and delay the progression of chronic complications like diabetic foot ulcers at the community level.

Abstract

The goal of nursing intervention on chronic disease such as Hypertension is to improve the patient self-care, in which the Chronic care model could help to reach the best outcome. Research suggests the community, health workers, and regulations could improve the health. The purpose of this study was to apply nursing intervention based on chronic care model to medication adherence in hypertensive patients. This is quasi-experimental research used pre-post test control group design. 50 respondents of hypertension patients were recruited from Pucang Sewu, Pacar Keling and Tambak Rejo Surabaya Health Centres. The intervention was applicating nursing interventions based on the Chronic Care Model. The Mann Whitney U statistical analysis used to understand the effect of the intervention to the medication adherence in patients with hypertension. A total of 50 respondents who were divided into two groups, control and treatment, with each (n = 25). The distribution of characteristics in the two groups is homogeneous. Different test in the control group, there was no difference. The treatment group shown a significant difference in adherence before and after the intervention with p value 0.000 (p <0.05). This study found the reason of disobedient patient to medication, which is forget and fear to the side effect. The patient and the health care agree frequent reminder could be the solution to this problem. Chronic Care Model based nursing interventions can improve medication adherence in hypertensive patients by encourage the health care system to frequently remind patients.

Abstract

This pandemic period has multiple impacts on all aspects of life. Both economic, social, religious, and psychological. The psychological side is the great fear in the community as the virus spreads very quickly from the sick to the healthy. Changes in interpersonal relationships and in the economy are often followed by changes in politics, including the life of the nation and the state. Various everyday problems also need to be observed and solved with new approaches. The aim of this research is to examine the impact of counseling on the anxiety level of the surrounding community due to residents found to have positive COVID-19 infection. So that people can understand Covid-19 properly and don't experience anxiety even though there are local residents who are confirmed positive for Covid-19. This study was a quasi-experimental study with a pre-post test control group design. The purpose of this study was to analyze the effectiveness of counseling in reducing anxiety levels in the surrounding community due to residents indicated as positive for Covid-19. Test results showed a significant difference in the treatment group before (pre-test) and after (post-test) counseling (p-value 0.000). The test results between the treatment group and the control group also showed a significant difference in the post-test (p-value of 0.000). Anxiety or stress during this pandemic leads to an increase in concern for one's own health and that of the family. The situation is exacerbated by the fact that information arriving in bewildering abundance will increase public anxiety. There is a need for participation and collaboration between health care facilities and health care workers and health cadres to provide accurate information so that people are properly informed and can reduce their fears so that they are able to take appropriate action. There is a significant effect of counseling to reduce the anxiety level of the surrounding community due to residents who tested positive for Covid-19.
