Abstract

Vitamin D is a micronutrient required by the body. Vitamin D has various functions, including its potential to prevent depression. There are studies suggesting that people with depression are at risk of vitamin D deficiency. This study discusses the correlation between vitamin D and depression.  This is a cross-sectional study designed to investigate the relationship between vitamin D levels and depression in elderly (≥ 60 years) residing at Bina Bhakti Nursing Home. Participants who meet the criteria underwent measurement of vitamin D levels and assessment for depression using the Geriatric Depression Scale (GDS). Spearman correlation is used as the statistical analysis to evaluate the relationship between variables. 93 respondents met the criteria with average age of 74.19 years (61-97 years). 77 respondents were female (82.8%) and 16 respondents (17.2%) were male. The mean vitamin D level was 9.78 ng/ml (1.07-25.6ng/mL) and the mean GDS score was 4.51 (0-15). The results of the analysis showed a significant negative correlation between vitamin D levels and GDS scale (-0.287, p-value=0.005). Low vitamin D levels are associated with increased GDS scores, indicating that it is important for the elderly to fulfill their vitamin D needs as an preventive measure against depression

Abstract

ABSTRACT Vitamin D deficiency is a public health problem that affects everyone regardless of their age. It is linked to various health problems, and one of them is sleep problem. Nearly 60% of elderly have sleep problems, with insomnia being the most frequently reported symptom. Insomnia can lead to physical, mental, behaviour problems, and increasing risk of having diabetes and cardiovascular disease. To find the association between vitamin D and insomnia in elderly. This research uses cross sectional study to find association between vitamin D and insomnia in elderly who live in Santa Anna’s nursing home. Respondents who met the inclusion criterias were measured for vitamin D and then filling out Insomnia Severity Index (ISI) questionnaire. Statistical analysis used is the Mann-Whitney test. Twenty-seven participants met the inclusion criteria, with the mean age of 75,59 (SD 7,42) years and vitamin D level 19,93 (SD 6,87) ng/ml. There was no significant difference in vitamin D level between non-insomnia and insomnia (p-value 0,979). However, from this study we found that lower vitamin D serum was associated with the increasing risk of insomnia. Vitamin D deficiency should be taken into account when treating elderly with sleep disorder. Health practitioners should consider Vitamin D supplementation as adjunctive treatment in sleep problems. Keywords: Insomnia, Elderly, Vitamin D  ABSTRAK Defisiensi vitamin D merupakan masalah kesehatan umum yang dapat terjadi pada semua orang tanpa memandang usia. Defisiensi vitamin D dihubungkan dengan berbagai macam masalah kesehatan, salah satunya adalah gangguan tidur. Gangguan tidur pada lansia merupakan masalah yang sering ditemui, dimana hampir 60% lansia mengalami gangguan tidur. Insomnia dapat menyebabkan gangguan fisik, mental, perilaku, dan meningkatkan risiko penyakit diabetes serta kardiovaskular. Meneliti hubungan vitamin D dengan kejadian insomnia pada lansia. Penelitian ini merupakan studi potong lintang yang mencari hubungan antara kadar vitamin D dengan kejadian insomnia pada orang lanjut usia di Panti Lansia Santa Anna. Responden yang memenuhi kriteria inklusi dilakukan pengukuran kadar vitamin D dan pengisian kuesioner Insomnia Severity Index (ISI) untuk insomnia. Analisis statistik dilakukan dengan menggunakan uji Mann-Whitney. Sebanyak 27 responden memenuhi kriteria inklusi dengan rerata usia 75,59 (SD 7,42) tahun, dengan kadar vitamin D 19,93 (SD 6,87) ng/ml. Hasil analisis statistik tidak mendapatkan perbedaan rerata kadar vitamin D yang bermakna antara kelompok dengan atau tanpa insomnia (p-value 0,979), meski demikian pada penelitian ini didapatkan bahwa defisiensi vitamin D berisiko meningkatkan terjadinya insomnia. Defisiensi vitamin D harus dipertimbangkan dalam manajemen lanjut usia dengan gangguan tidur. Suplementasi vitamin D dapat dipertimbangkan sebagai terapi tambahan pada lanjut usia yang mengalami gangguan tidur. Kata Kunci: Insomnia, Lansia, Vitamin D

Abstract

ABSTRACT Depression in the elderly is a multifaceted issue influenced by various physiological factors. This study investigates the correlation between muscle strength, calf circumference, Vitamin D, and Insulin-like Growth Factor 1 (IGF-1) levels and their collective impact on depression among the elderly.  This study aims to understand the correlation between muscle strength, calf circumference, Vitamin D, and IGF-1 levels on depression to develop targeted interventions to improve mental health outcomes in this demographic. This cross-sectional study involved 82 older people at RPTRA Krendang. Depression is measured using the Geriatric Depression Scale (GDS). Data were analyzed using Spearman's Rho to see the Correlation between Muscle Strength, Calf Circumference, Vitamin D, and IGF-1 on Depression in the Elderly. The study found most respondents were female (78%), with an average age of 74.34 years, and according to the Geriatric Depression Scale, the majority (59.8%) of respondents were normal. This study highlighted significant correlations between muscle strength, calf circumference, and decreased Vitamin D and IGF-1 levels with higher depression scores. These findings suggest that diminished physical health markers and nutrient deficiencies are associated with increased depressive symptoms in the elderly. Measuring muscle strength, calf circumference, Vitamin D, and IGF-1 levels is a good approach to understanding depression in the elderly that will lead to effective interventions by health workers and improve quality of life. Keywords: Calf Circumference; Depression; IGF-1; Muscle Strength; Vitamin D.

Abstract

Pendahuluan: Kesehatan mental merupakan bagian penting dalam kesejahteraan individu, namun kesadaran masyarakat terhadap deteksi dini gangguan mental masih rendah dan stigma terhadap gangguan mental tinggi. Kota Bambu sebagai wilayah perkotaan memiliki risiko paparan terhadap tantangan lingkungan, sosial, dan ekonomi yang berpotensi meningkatkan risiko gangguan mental. Kegiatan pengabdian masyarakat ini bertujuan meningkatkan pengetahuan masyarakat dengan melakukan penapisan/skrining depresi, kecemasan,dan stres menggunakan instrumen Depression Anxiety Stress Scale-42 (DASS-42) dan edukasi. Kegiatan dilaksanakan pada Juni 2025 dengan melibatkan 168 peserta dari berbagai kelompok usia. Metode yang digunakan berupa pendekatan Plan-Do-Check-Act (PDCA) yang mencakup perencanaan, edukasi interaktif, skrining, analisis hasil, hingga tindak lanjut. Hasil skrining menunjukkan masih ada 16,1% peserta dengan kecemasan sedang dan 3% peserta dengan kecemasan sangat berat. Proporsi kecil depresi dan stres juga ditemukan pada berbagai tingkat keparahan. Kesimpulan: Dari hasil ini menggambarkan bahwa kecemasan masih ditemukan pada populasi perkotaan. Depresi dan stres juga tetap harus menjadi perhatian karena dapat mempengaruhi kualitas hidup dan produktivitas masyarakat. Edukasi terbukti dapat meningkatkan pemahaman masyarakat, sementara DASS-42 efektif sebagai instrumen deteksi dini. Implikasi kegiatan ini adalah perlunya skrining rutin, edukasi berkelanjutan, dan potensi integrasi dengan program Puskesmas.

Abstract

Schizoaffective disorder, depressive type, is a psychiatric condition characterized by the presence of psychotic symptoms within the context of mood episodes, particularly major depressive episodes. Clinical manifestations commonly involve hallucinations, delusions, and depressive symptoms such as low mood, reduced interest, sleep disturbance, decreased appetite, and suicidal ideation. The variability of presentation and the overlap between psychotic and affective symptoms may complicate both diagnostic assessment and therapeutic decision-making. This disorder is associated with significant functional impairment and an increased risk of suicide. We report a 22-year-old female who presented to the emergency department of a hospital in Jakarta with irritability, shouting, and aggressive speech, accompanied by auditory hallucinations described as whispering voices and visual hallucinations involving the image of her mother. Symptoms had been present for two months prior to admission and were associated with sleep disturbance, decreased appetite, weight loss, and suicidal ideation. Mental status examination revealed hypotymic mood, restricted affect, nonspontaneous speech, and the presence of both auditory and visual hallucinations. The patient was diagnosed with schizoaffective disorder, depressive type (F25.1). The patient was treated with a combination of risperidone, olanzapine, and sertraline to address both psychotic and depressive symptoms simultaneously. Although this approach is not considered first-line treatment in most clinical guidelines, it may be considered in selected cases with severe symptomatology and elevated suicide risk. The observed clinical improvement highlights the importance of individualized treatment planning, careful benefit–risk evaluation, and close monitoring of treatment response and tolerability.

Abstract

Hebephrenic schizophrenia is characterized by disorganized thinking, flat or inappropriate affect, and disorganized behavior. This condition typically appears in late adolescence to young adulthood and is associated with a poor prognosis due to early onset and social functioning. Severe disorganization can lead to persistent decline in social and occupational functioning, especially if treatment is inadequate or not sustained. A 20-year-old man came to the Emergency Department in Hospital accompanied by his family, complaining of unusual behavior. The complaints began in 2019, including disorganized behavior such as talking and laughing to himself without any apparent stimulus, irrelevant responses when spoken to, climbing roof tiles, jumping from the second floor, picking up cigarette butts along the street, taking money without permission, and leaving the house naked. The patient had undergone several months of psychiatric treatment in 2021, but therapy was discontinued. A mental status examination revealed severe disorganized thinking and behavior, flat affect, impaired attention and cognitive function, and auditory hallucinations characterized by talking and laughing to oneself. The patient was diagnosed with hebephrenic schizophrenia and received pharmacological therapy consisting of the atypical antipsychotics Clozapine 1 x 100 mg and Quetiapine 2 x 200 mg combined with Valproic Acid 2 x 500 mg, Lorazepam 2 x 1 mg, and Trihexylphenidyl 2 x 2 mg. Additionally, psychosocial interventions were provided, including psychoeducation. Severe disorganization in hebephrenic schizophrenia is a clinical challenge that requires a sustained multimodal approach encompassing pharmacological therapy and psychosocial interventions. Continuity of pharmacological therapy and psychosocial interventions is crucial for improving social functioning and preventing relapse.
