Abstract

The decline in outpatient visits is a major challenge for regional hospitals amidst increasing competition in healthcare services. This study aims to analyze the implementation of the 7P marketing mix (Product, Price, Place, Promotion, People, Process, and Physical Evidence) in increasing outpatient visits at Mayjen HM Ryacudu Regional General Hospital in Kotabumi, Lampung. The study used a qualitative approach with a case study method. Data were collected through in-depth interviews with management, medical personnel, and patients, which were then validated through source triangulation, field observation, and documentation. Then, they were analyzed using data reduction, data presentation, and conclusion drawing techniques. The results show that price and place elements are the hospital's main strengths due to affordable rates and strategic location. However, limited pricing flexibility due to government regulations limits the function of price as a differentiation tool. People and process elements play a significant role in shaping patient perceptions of service quality, while promotion has not been optimally utilized. A SWOT analysis places the hospital in a position that requires aggressive market penetration through service digitalization and physical facility modernization to restore public trust and increase visits by BPJS and general patients. This study emphasizes the importance of integrating all elements of the 7P marketing mix synergistically to increase patient interest in visiting regional hospitals.

Abstract

This study aims to analyze the effect of the Hospital Management Information System (HMIS) on the effectiveness and efficiency of outpatient services and its implications for employee performance at Rumah Sakit Sehat Terpadu Dompet Dhuafa, Bogor City. The study employed a quantitative approach with a survey method involving employees directly engaged in outpatient services and HMIS utilization. Data were collected through questionnaires and analyzed using descriptive analysis and path analysis with SPSS version 24. The results indicate that HMIS contributes to service effectiveness (β=0.605; R²=36.6%), reflected in improved patient data accessibility, recording accuracy, and service speed. HMIS also contributes to service efficiency (β=0.603; R²=36.4%), evidenced by reduced patient waiting times and accelerated administrative processes. Service effectiveness and efficiency jointly contribute to employee performance (R²=26.5%), although only efficiency demonstrates a meaningful partial contribution. HMIS exerts a direct effect on employee performance (β=0.526) as well as indirect effects through effectiveness (0.126) and efficiency (0.242), yielding a total effect of 0.894. It is concluded that optimal HMIS implementation contributes to improving outpatient service quality and employee performance.

Abstract

Hospitals are under increasing pressure to improve service quality while maintaining financial sustainability under Indonesia’s National Health Insurance (JKN) reimbursement system. Clinical Pathway (CP) implementation and Length of Stay (LOS) are considered important strategies for improving cost efficiency, yet their financial impact remains inconsistent across healthcare settings. Previous studies have primarily examined the influence of Clinical Pathway implementation on service quality or Length of Stay separately. In contrast, evidence regarding the combined effects of Clinical Pathway compliance and Length of Stay on hospital cost efficiency, particularly in Type D hospitals, remains limited. This study aimed to analyse the effects of Clinical Pathway compliance and Length of Stay on cost efficiency among inpatients at Marsudi Waluyo Hospital, Malang Regency, using Gross Profit Margin (GPM) as the indicator of cost efficiency. A quantitative study with an observational, analytic, and retrospective design was conducted using secondary data from medical records, Clinical Pathway forms, hospital information systems, billing records, and casemix data. The study included 361 inpatient care episodes selected through total sampling. Multiple linear regression was employed to examine the relationships among variables, and bootstrap analysis with 1,000 resamples was applied to enhance the robustness of the estimates. Clinical Pathway compliance significantly affected Gross Profit Margin (β = −0.341, p = 0.029), while Length of Stay also showed a significant negative effect (β = −10.385, p < 0.001). Simultaneously, both variables significantly influenced Gross Profit Margin (F = 30.800, p < 0.001), explaining 14.7% of its variance (R² = 0.147). Length of Stay demonstrated a stronger influence on cost efficiency. This study also contributes empirical evidence on hospital cost efficiency in a Type D hospital and provides practical guidance for hospital managers in developing evidence-based cost-control strategies.

Abstract

Through this research, it is expected to obtain empirical evidence about the influence of Educator Resource Management and Principal Leadership on School Quality in Private Madrasah Aliyah, Babakan Jamanis Parigi Pangandaran Islamic Boarding School Foundation. The object of research in this research is Madrasah Aliyah Private Islamic Boarding School Foundation Parigi Pangandaran, because based on the observations of researchers that there is a phenomenon of educators who do not have an academic degree and there are still many who have not been certified. The research method used is descriptive and verification analysis with data collection techniques using questionnaires distributed to educators using total sampling technique (saturated) because the population is only 25 people. The results of descriptive analysis prove that educator resource management, principal leadership and quality the school is classified as very good, the results of the verification analysis "there is a strong positive relationship between educator resource management and principal leadership on school quality".

Abstract

Patient safety incidents remain a major concern in hospital services because they may affect clinical outcomes and public trust. Open disclosure is increasingly recognized as an ethical and communication-based response after patient safety incidents, yet its implementation remains inconsistent across healthcare settings. This study aimed to examine the concept and implementation of open disclosure, its relationship with patient trust, and its role in reducing medical disputes, complaints, and patient conflicts. This systematic literature review was conducted using Scopus, PubMed, and Google Scholar. The target population was peer-reviewed studies on open disclosure in healthcare contexts. Studies were selected through systematic searching and PRISMA-based eligibility screening using the keywords “open disclosure” AND “patient safety”. From 563 identified documents, 14 studies published between 2021 and 2026 met the inclusion criteria. The independent variable was open disclosure, while the dependent variables were patient trust, complaints, conflict resolution, medical disputes, and patient responses. Data were measured through structured extraction based on PICO and eligibility criteria, then analyzed using narrative synthesis. Among the 14 included studies, 6 were qualitative, 4 cross-sectional, 2 mixed-method, 1 quasi-experimental, and 1 retrospective observational. Most studies were conducted in Australia (n=3), followed by Ireland and multi-country studies (n=2 each). Open disclosure supported patient trust through honest, timely, empathetic communication, apology, and patient or family involvement. Full disclosure increased forgiveness (P<0.010), while partial and full disclosure increased intention to return for treatment (P<0.050). Complaints increased by 4.2% (p<0.010), whereas medicolegal cases decreased (CAGR=4.8%; p<0.050), with 88.6% resolved without legal proceedings. Open disclosure is an important hospital response strategy for restoring patient trust and reducing escalation of medical disputes. Its effectiveness depends on structured implementation, non-punitive culture, and healthcare professionals’ communication competence.
