Abstract

Sodium restriction effect on hospital readmission in patients with heart failure (HF) has been questioned for decades. Readmission related to low sodium intake recommendations should be changed as well as mortality. A literature review is needed to summarize the effect of low sodium, especially on readmission and mortality. This literature review aimed to summarize the prevalence of hospital readmission and mortality regarding low sodium intake in patients with HF. The searching process involved four databases; MEDLINE, Embase, EBSCO Health, Cochrane was explored for experimental studies of sodium restriction. Of 77 screened citations from 2000 to 2019 invested in patients with HF, four studies were included. Four studies from four databases were included and explained and it was found that hospital readmission was the outcome of implementing sodium restriction in patients with HF. Low sodium restriction (800 mg – 1800 mg/day) results in higher hospital readmission. Moreover, 1800 mg/day of sodium was followed by higher mortality and higher sudden death in patients with HF. Low sodium restriction did not lower hospital readmission as well as mortality of patients with HF. This article provides the reason, effect, and amount of sodium restriction in patients with HF. The recommendation from this literature review is low sodium restriction has no beneficial effect on readmission and mortality in HF conditions.

Abstract

ABSTRACT Heart failure (HF) as abnormal heart structure and function increases neurohormonal response and sympathetic activity resulting in a renin-angiotensin-aldosterone system (RAAS). Congestion as the result of RAAS, increase signs and symptoms of HF including thirst. Fluid restriction as the recommendation to reduce congestion is increased thirst in patients with HF.  The purpose of this study is to evaluate the effectiveness of ice cubes on thirst intensity, thirst distress, hospitalization, and mortality in patients with HF. This research implied quasi-experimental research, with one group pre and post-design. The sample recruited was 13 samples from outpatients and inpatients of a general hospital in Jakarta. Diagnosis must be HF by the cardiologist, over 18 years old, and in stable condition. Thirst intensity was measured by a visual analog score of 0-100, thirst distress was measured by thirst distress scale-heart failure, hospitalization, and mortality were measured by asking the patients or the family. There is a significant difference in thirst intensity before and after intervention (p 0.05); and there is also a significant difference in thirst distress before and after intervention (p 0.05). However, no difference in hospitalization and mortality after the ice cubes intervention. It is important for healthcare professionals or families to apply ice cubes sucking in patients with HF. The inpatients' patients are the most important to use the ice cubes to reduce thirst intensity and thirst distress due to thirst is higher in hospitalized patients with HF. Keywords: Ice Cubes, Thirst, Heart Failure

Abstract

Introduction: Since COVID-19 2019 became a global pandemic, the blended learning method has gained popularity, including in nursing education. A valid and trustworthy questionnaire is required in Indonesia to measure blended learning satisfaction. The purpose of this study was to evaluate the psychometric properties of Indonesian BLSS, a validated tool from Taiwan, among nursing students. Methods: This study employed a cross-sectional design to assess the validity and reliability of BLSS. The sample included 231 first-year nursing students from a university in Indonesia. The splitting sample method was utilized for exploratory factor analysis (EFA) and confirmatory factor analysis (CFA). The BLSS was translated using forward and backward translation. Three experts validated the content, while EFA and CFA investigated the structural validity. Reliability was assessed using Cronbach’s alpha. Results: The content validity index (CVI) of BLSS was 0.975. The mean age of the respondents was 19.26 (0.05), with the majority being female (85.07%). In EFA, one factor was retained based on cumulative variance, a scree plot, and parallel analysis. The CFA also showed one factor as retained. The factor loading of each item was greater than 0.5 both in EFA and CFA. This instrument has an internal consistency of 0.955 according to Cronbach’s alpha. Conclusion: The Indonesian BLSS is good, based on CVI, EFA, CFA and internal consistency analysis, which were used to measure satisfaction with blended learning. By measuring satisfaction following blended learning using BLSS, we hope that the learning process can be regarded as satisfactory and improved among nursing students.
