Abstract

Background: Hypertension is the disease with the highest number of cases among patients who visit primary care. Most hypertension is caused by a poor diet. One of the main causes of hypertension is poor food control. This study aimed to know the effect of diet control education intervention against the behavior changes in controlling the blood pressure for hypertensive patients. Methods: This study used the quasi-experimental design with pre-test and post-test with a control group. The intervention and control groups were chosen through the implemented criteria of inclusion and exclusion. The used sampling technique was a simple random sampling. There were 84 subjects of hypertensive patients. The subjects were divided into two groups, i.e., 42 patients (intervention group) and 42 patients (control group). The intervention was done for six weeks. Result: There were differences in dietary control behavior changes between the control and intervention groups. The intervention group had significant (p<0,05) change,1.41 while control group had decreased in score (∆) (0.47) where the value of p was not significant, (p>0.05). The average value of systolic blood pressure for the intervention group was 4.47 mmHg/5.17 mmHg, and the increase of the average value of systolic blood pressure was 3.63 mmHg/5.95 mmHg for the control group. Conclusion: Dietary control education influences blood pressure control for hypertensive patients.

Abstract

The difficulty of the community to understand health behavior is the main cause of poor physical activity behavior. The aim of this study is to explore health education barriers to controlling blood glucose patient with DM in primry health care. The Forum Group Discussion (n = 22) and interview (n = 5) methods were to explore the patient's desire to overcome educational barriers. Informants were patients with DM aged of 26-69 year-old from two community health center and two private health center in Sleman Regency, Indonesia. The questions were around barriers to physical activity programs for controlling blood glucose. The data were analyzed using Opencode. Boring educational training is the leading cause of physical activity and dietary control in patients with DM. Formal language in delivering material is the cause of patient boredom to take part in the training. Despise and feeling of anxiety to ask questions during the educational procedure that the patient feels. Also, in terms of health service providers, limited human resources have resulted in a lack of maximum education programs for patients with DM in Yogyakarta.Tense atmosphere and language that is difficult to understand have a negative impact on patient behavior to change physical activity behavior and controlled eating patterns in patients with DM.

Abstract

Hospital patients can respond to meet the expectations and realities of the health service they received. If it does not meet their expectations, there will be dissatisfaction. Consumer dissatisfaction is conveyed in the form of criticism and complaints, so good complaint management is required to provide an early warning system to anticipate serious safety incidents resulting in patient dissatisfaction. This includes complaints at St. Rafael Cancar General Hospital, Manggarai, East Nusa Tenggara, where most patients complained about the completeness of inpatient and outpatient services, resulting in a review of the Management of Complaint Handling in Health Services at St. Rafael Cancar General Hospital. This research was conducted on internal management and external patients at St. Rafael Cancar General Hospital, Manggarai, East Nusa Tenggara using qualitative descriptive method with observation and interview with public relations division and patients who have been treated at St. Rafael Cancar General Hospital. The results showed that the management of complaint handling at St. Rafael Cancar General Hospital has not implemented a tiered line of responsibility for complaints. The Public Relations Department often has a role as a complaint resolution party, even though its main function is as a mediator between customers and related units in the management of complaint handling at St. Rafael Cancar General Hospital. Supervision and documentation of complaints at St. Rafael Cancar General Hospital has not developed a Hospital Complaint Management Information System for the effectiveness of complaint resolution control. Patient evaluations showed that they were quite satisfied with the service and follow up on the complaints they have submitted, but there were some service facilities that they think still lacking.

This study explores health education barriers to controlling blood glucose in a patient with DM in primary health care. The Forum Group Discussion (n = 22) and interview (n = 5) methods were used to explore the patient’s desire to overcome educational barriers. Informants for the study were patients with DM aged between 26-69 years from two community health centers and two private health centers within Sleman Regency, Indonesia. The informants were then questioned about barriers to physical activity programs for controlling blood glucose. The data collected was analyzed using Opencode, and the results revealed that boring educational training is the leading cause of physical inactivity and dietary control in patients with DM. Furthermore, the use of formal language in delivering material is the cause of patient boredom to take part in the training. Also, limited health human resources have resulted in a lack of maximum education programs for patients with DM in Yogyakarta. Therefore, a tense atmosphere and the use of complex language have a negative impact on patients to change physical activity and control eating patterns.

Abstract

Hipertensi adalah suatu keadaan dimana tekanan darah sistolik ≥140 mmHg dan tekanan darah diastolik ≥90 mmHg, dengan lifestyle modification sebagai salah satu tatalaksana non farmakologis utama yang dapat meningkatkan keberhasilan terapi. Penelitian ini mempunyai tujuan untuk mengetahui gambaran perilaku diet dan aktivitas fisik pasien hipertensi di Klinik Pratama Widuri Kabupaten Sleman. Penelitian ini menggunakan desain cross sectional dengan pemilihan sampel menggunakan metode simple random sampling. Total sampel sebanyak 84 responden. Kategori perilaku diet dihitung menggunakan kuesioner yang diadopsi dari Pennington Biomedical Research Center (2014), kategori aktivitas fisik dihitung menggunakan Kuesioner Baecke dan tekanan darah dihitung menggunakan spymomanometer yang telah dikalibrasi. Hasil penelitian menunjukkan pada kategori perilaku diet, responden yang memiliki perilaku diet baik sebanyak 13.1% dan responden yang memiliki perilaku diet kurang sebanyak 86.9%. Pada kategori aktivitas fisik, responden yang mempunyai aktivitas fisik ringan 70.2% dan responden yang aktivitas fisik sedang sebanyak 29,8%. Kemudian pada kategori tekanan darah, secara keseluruhan rata-rata tekanan darah pada responden adalah 153.59/90.71 mmHg yang terbagi menjadi 2 derajat yaitu hipertensi grade I (sistolik 140-159 dan/atau diastolik 90-99), hipertensi grade II (sistolik ≥160 dan/atau diastolik ≥100). Responden yang mempunyai tekanan darah tinggi grade II lebih banyak (52.4%) dibandingkan grade I (47.6%). Hasil penelitian menunjukkan bahwa perilaku diet dan aktivitas fisik pada pasien hipertensi di Klinik Pratama Widuri masih kurang dan memerlukan program Lifestyle Modification untuk meningkatkan lifestyle behavior dan keberhasilan terapi pada pasien hipertensi.

Abstract

The pattern of payment that is applied by Jaminan Kesehatan Nasional (JKN) in advanced health facilities (hospitals) is a prospective payment pattern. Prospective Payment pattern known as casemix is a grouping diagnoses and procedures with reference to clinical characteristics which is similar and resource used or maintenance cost is similar. The grouping is done by software groupers. Bagas Waras hospital in making diagnosis grouping sometimes experiences the negative difference (loss) between the real cost of hospital fees in accordance with the Indonesia Case Based Group (INA CBGs) cost package approved by Badan Penyelenggara Jaminan Sosial (BPJS).This study is to determine the cause of the change file BPJS claims that the negative consequences of coding problems after qualitycontrol of INA-CBGs coding in Bagas Waras hospital Klaten. This study was a qualitative research design action (Action Research). This study used a model of action research (Action Research) of S Kemmis and R McTaggart which is designed through two cycles of the procedure: (1) planning , (2) action (3) observation, (4) reflection  in each cycle. The research instrument used a questionnaire. Analysis of the data used qualitative analysis and quantitative analysis.The results showed that the frequency of claims BPJS start of the cycle I, II and III, there was a decreasing in the proportion of the, it is due to an error coding, Leght OfStay (LOS) extends, the cost of medical action that is excessive, and Excess Cost investigations. In improving the ability of diagnosis coding, Bagas Waras hospitalcontinuously strives to improve their ability to apply the Hospital Information System (SIRS).The cause of negative claim in a file BPJS after INA-CBGs control coding in Bagas Waras hospital  is the employees do not understand the overall coding system, and  do not run the monitoring and evaluation system of coding.  Keyword : Negative Claim of BPJS, Coding INA CBG's

Abstract

The hospital as one of the health service facilities, is part of the health resources that were needed in supporting the implementation of health efforts. The writing of patient diagnosis is the responsibility of the treating physician and should not be represented. The phenomenon found in the initial survey of Regional General Hospital Bagas Waras Klaten obtained many incomplete resume forms. The purpose of this research is to know the doctor's behavior on the filling of the in-patient resume sheet data at  BagasWaras HospitalKlaten. This research was a qualitative research with case study approach. Respondents in this study were DPJP Specialist Doctor, Medical Record Officer and Hospital Director. The data obtained from interviews, FGDs, observations and documentation and collected and grouped based on existing indicators, as well as based on existing facts and also on critical thoughts to obtain a weighted result. Based on the results of the doctor's observation through the medical resume sheet for 3consecutive months which had been submitted to the medical record, it appeared that medical resume filled by the doctor after the patient finished treatment was still low, from 217 files, which was incomplete as much as 125 with a percentage of 41%. It was due to lack of cooperation between doctors and nurses in filling medical resume. In addition, the incomplete medical resume was due to the nonoperation of the Standard Operational Procedure (SOP) which regulated the filling of medical resumes, the absence of regulations, sanctions for doctors who did not fill the medical resume so the doctors lazily fill medical resume. The doctor's behavior in completing the medical resume sheet is still low.  Keywords: Medical Resume, Behavior Physician

Abstract

Perilaku menetap merupakan faktor risiko terbesar terjadinya Diabetes Mellitus (DM) atau komplikasinya. Salah satu penyebab perilaku menetap adalah kurangnya pengetahuan tantang pentingnya aktivitas fisik dalam mengendalikan glukosa darah untuk mencegah terjadinya DM. Tujuan pengabdian masyarakat ini adalah untuk mengenalkan pentingnya aktivitas fisik menggunakan model edukasi berbasis pemberdayaan masyarakat untuk mengendalikan glukosa darah, terutama pada pasien DM. Jumlah peserta yang mengikuti kegiatan ini sebanyak 50 peserta, yang terdiri dari 29 masyarakat tidak terdiagnosis DM dan 21 masyarakat terdiagnosis DM. Hasil pengabdian masyarakat menunjukkan bahwa metode edukasia yang dilanjutkan dengan praktik aktivitas fisik dapat meningkatkan pengetahuan tentang pentingnya aktivitas fisik dalam menurunkan glukosa darah. Edukasi kesehatan dilakukan pada masyarakat yang menderita DM maupun masyarakat umum. Setelah edukasi kesehatan, dilanjutkan dengan diskusi kelompok dan latihan bersama. Simpulan pengabdian masyarakat ini adalah model edukasi program aktivitas fisik berbasis masyarakat dalam bentuk diskusi kelompok dan praktik langsung aktivitas fisik dapat meningkatkan pengetahuan tentang pentingnya mengendalikan glukosa darah. Saran, pemerintah dan institusi pendidikan perlu meningkatkan upaya preventif dan promotif untuk meningkatkan perilaku aktivitas fisik dalam upaya mengendalikan DM di wilayah kabupaten Sleman lainnya.

Abstract

ABSTRAKKegiatan operasional sebuah klinik yang berjalan sesuai dengan ketentuan yang telah ditetapkan dalam peraturan perundang-undangan dan kebijakan pemilik mendukung tercapainya visi dan misi klinik. Tujuan dari kegiatan pengabdian masyarakat ini adalah meningkatkan kegiatan operasional Klinik Pratama Al-Ikhlas Muhammadiyah Borobudur untuk memperbaiki kinerja klinik yang selama beberapa tahun terakhir mengalami penurunan. Metode yang digunakan adalah studi kasus menggunakan observasi partisipatif dengan mensinergikan kegiatan pengabdian masyarakat dosen Magister Administrasi Rumah Sakit Universitas Muhammadiyah Yogyakarta dengan kegiatan operasional Klinik Al-Ikhlas Muhammadiyah Borobudur (Klinik) bekerja sama dengan Ranting Aisyiyah Borobudur 3 dan Cabang Borobudur. Hasil dari pengabdian masyarakat adalah peningkatan kegiatan operasional klinik dalam memberikan pelayanan kesehatan kepada masyarakat terutama warga Muhammadiyah dan Aisyiyah Borobudur dan warga masyarakat yang berada di sekitar klinik. Hasil dari pengabdian masyarakat yang lain adalah peningkatan motivasi dari sumber daya manusia klinik untuk memperbaiki kinerjanya sehingga klinik dapat menjalankan fungsinya dengan baik. Kata kunci: peningkatan; kegiatan operasional; kinerja; klinik pratama.  ABSTRACTThe clinic's operational activities that work according to the provisions that have been set in regulations and stakeholder policy can support the clinic to achieve the clinic’s vision and mission. This community service aimed to improve the operational activities of Klinik Pratama Al-Ikhlas Muhammadiyah Borobudur, especially in improving the clinic’s performance which decreases in recent years. The method applied was a case study using participatory observation by synergizing both the community service of the master of Hospital administration lecturers, Universitas Muhammadiyah Yogyakarta, and clinics operational of Klinik Pratama Al-Ikhlas Muhammadiyah Borobudur which collaborate with Aisyiyah Borobudur Branch 3 and Borobudur Branch. The result of this community service was the improvement of the clinic’s operational activities in providing health service to the community, especially the residents of Muhammadiyah dan Aisyiyah Borobudur and also the residents around the clinic. Besides, another result of the community service was the enhancement of the motivation of clinical human resources to improve the clinical performance, so the clinic can carry out its function properly.                                                              Keywords: improvement; operational activities; performance; pratama clinic.

Abstract

Momentum peningkatan kualitas dan perawatan kesehatan yang terjangkau untuk semua masyarakat telah melahirkan Universal Health Coverage (UHC). Sebagai bagian dari strategi Indonesia untuk mencapai tujuan UHC, investasi besar telah dilakukan untuk meningkatkan akses kesehatan bagi masyarakat miskin, sehingga pelaksanaan berbagai skema asuransi kesehatan ditujukan untuk masyarakat miskin dan hampir miskin dalam Jaminan Kesehatan Nasional (JKN). Pelayanan kesehatan tidak lagi terpusat di rumah sakit atau faskes tingkat lanjutan, namun pelayanan kesehatan harus dilakukan secara berjenjang, dimulai dari pelayanan kesehatan tingkat pertama, tingkat kedua dan tingkat ketiga. Fenomena yang dihadapi selama pelaksanaan program JKN diantaranya berhubungan dengan sistem rujukan berjenjang yang masih belum optimal dilihat dari adanya peningkatan progresif pasien di rumah sakit, baik karena kendala yang berhubungan dengan kompetensi dokter umum di faskes tingkat pertama, sarana prasarana, ataupun berhubungan dengan aturan-aturan dari sistem rujukan itu sendiri. Hal ini dianggap perlu dipelajari dan diteliti dalam rangka memperbaiki dan mengefektifkan pelayanan kesehatan baik di tingkat pelayanan primer dan khususnya di tingkat pelayanan sekunder dalam hal ini di Rumah Sakit Umum (RSU) Ampana.. Penelitian dilakukan dengan menggunakan pendekatan kualitatif. Inti dari penelitian ini adalah untuk menganalisis kecenderungan tingginya angka rujukan pasien BPJS dari PPK I ke PPK II di Kabupaten Tojo Una-Una. Pelaksanaan rujukan di lima puskesmas terpilih di Kabupaten Tojo Una-Una, menunjukkan tiga dari lima puskesmas tersebut memiliki angka rujukan yang tinggi, dimana rasio rujukan lebih dari 15%, meliputi Puskesmas Ampana Timur 27,3%, Puskesmas Ampana Barat 24,28%, Puskesmas Marowo 11,6%, Puskesmas Ampana Tete 25,76%, dan Puskesmas Wakai 0,65%. Dari analisis rujukan diatas, dapatlah kita lihat bahwa penyebab tingginya angka rujukan disebabkan oleh faktor Sumber Daya Manusia (SDM), fasilitas atau sarana prasarana, dan aturan terkait sistem rujukan berjenjang dari PPK I ke PPK II belum diterapkan dengan baik.
