Abstract

Rheumatoid arthritis is a chronic autoimmune disease causing persistent joint inflammation, pain, stiffness, and progressive functional decline, particularly exacerbated among farmers by repetitive non-ergonomic labor. This case involved a 74-year-old male farmer from Pasuruan Regency, Indonesia, who presented with severe functional limitations. Assessment revealed throbbing joint pain rated 7 out of 10 on the Numeric Rating Scale, prolonged morning stiffness, fatigue with minimal exertion, slow and assisted gait, and dependence on family for ambulation and self-care. The priority of nursing diagnosis was Acute Pain related to inflammatory joint process and mechanical stress, per the Standar Diagnosis Keperawatan Indonesia, as uncontrolled pain was the primary barrier to physical function; activity intolerance was identified as a secondary diagnosis. The purpose of this case report was to evaluate the effectiveness of a nursing-led, non-pharmacological intervention topical ginger (Zingiber officinale) extract oil combined with massage in reducing acute pain and thereby improving functional capacity in a rural rheumatoid arthritis patient with limited access to pharmacotherapy. The intervention plan consisted of twice-daily application of ginger oil to knees and ankles with 10-minute gentle massage over three consecutive days, integrated with health education on activity pacing and light stretching. Following the intervention, pain intensity decreased to 3/10, ambulation improved from assisted to independent, and the patient resumed light farming and self-care tasks without frequent rest. No adverse effects occurred. These outcomes suggest that targeted pain management using locally available ginger oil may rapidly enhance activity tolerance in resource-limited settings. Further research is warranted to confirm efficacy and safety in larger cohorts.

Abstract

Schizophrenia is a serious mental disorder characterized by perceptual changes, including auditory hallucinations. Non-pharmacological interventions are needed to help patients manage these symptoms. This case report describes nursing care for two schizophrenia patients with auditory hallucinations using talking therapy. Assessment was conducted on two patients at the Social Rehabilitation Unit through interviews and observations. The nursing diagnosis established was sensory perception disturbance: auditory hallucinations. Planning involved implementing talking therapy interventions based on Indonesian Nursing Intervention Standards. The intervention was conducted once daily for three consecutive days, with each session lasting approximately 30 minutes. Results showed a decrease in hallucination frequency in both patients after three days of intervention. Patient one showed significant improvement in using conversation as a distraction, while patient two showed reduced response to commanding hallucinations. Talking therapy showed improvement in reducing auditory hallucinations by shifting focus from internal stimuli to external reality through social interaction.

Abstract

Schizophrenia is characterized by disturbances in thought processes, emotions, and perceptions. Among its symptoms, auditory hallucinations are the most frequently experienced by individuals with schizophrenia. If not managed appropriately, this condition can impair social functioning and increase the risk of destructive behavior. In addition to pharmacological therapy, non-pharmacological interventions such as rebuking and ear-closing techniques can be applied to help control emerging symptoms. This study aims to describe the implementation of nursing care for patients with schizophrenia experiencing sensory perception disorders, specifically auditory hallucinations, through the application of rebuking and ear-closing techniques. Methods: This study used a case study design involving two patients at UPT Rehabilitasi Sosial Pasuruan. Data were collected through interviews, observations, physical examinations, and documentation. The intervention consisted of rebuking and ear-closing techniques implemented once daily for three consecutive days, with each session lasting approximately 30 minutes. Outcomes were evaluated using indicators based on the Indonesian Nursing Diagnosis Standards (SDKI) and Indonesian Nursing Outcome Standards (SLKI), focusing on hallucination frequency, patients’ responses, and independence in controlling auditory hallucinations. After implementing SP1 (Implementation Strategy 1), the results showed that the intervention reduced the frequency of hallucinations. Quantitatively, the first patient demonstrated a 65% improvement in self-control ability, while the second patient showed a 75% improvement. The application of rebuking and ear-closing techniques showed potential benefits in helping patients control auditory hallucinations.

Abstract

Dermatitis is one of the most common integumentary disorders encountered in community nursing practice, and its burden is amplified among populations continuously exposed to environmental irritants such as seawater, wind, dust, and extreme humidity. Coastal communities sit at the upper end of this exposure gradient, yet affordable, theory-guided non-pharmacological interventions for this population remain sparsely evaluated. This study evaluated the effectiveness of standardized topical Virgin Coconut Oil (VCO) therapy, delivered through a structured nursing process guided by Orem's Self-Care Deficit Theory, on impaired skin integrity among coastal residents with contact dermatitis. A quasi-experimental, one-group pretest-posttest design, reported in accordance with the TREND Statement, was conducted over three consecutive days (12–14 May 2026) with two purposively selected participants, aged 35 and 60 years, in the coastal working area of the Gadingrejo Community Health Center, Pasuruan City, Indonesia. VCO was applied topically per a standard operating procedure while nursing support progressed from a wholly compensatory to a partially compensatory and finally a supportive-educative system. Skin integrity was assessed daily using SDKI/SLKI-aligned indicators covering pruritus, erythema, skin texture, lesion status, and local skin temperature. Both participants showed a consistent decline in pruritus and erythema, smoother skin texture, and reduced local skin temperature (37.4°C to 36.5°C in the older participant), with near-complete symptom resolution by the third day. Structured VCO therapy delivered through a theory-guided nursing process meaningfully restored skin integrity within 72 hours and moved participants toward independent self-care, supporting its viability as a low-cost, evidence-based community nursing intervention for coastal dermatitis.

Abstract

Hypertension rarely presents in isolation; a substantial proportion of affected adults also live with acute pain, most often occipital headache and neck tension, that erodes daily functioning and pushes patients toward unsupervised analgesic use. Community-based, non-pharmacological interventions capable of addressing this dual burden inside patients' own homes remain underdocumented, particularly when explicitly grounded in a nursing theoretical framework. This community nursing program examined the effect of guided autogenic relaxation therapy, structured around Orem's Self-Care Deficit Theory, on acute pain intensity, blood pressure, and self-care behavior among hypertension patients in a primary care catchment area. A quasi-experimental, one-group pretest-posttest design, reported following TREND and TIDieR guidance, was applied to three male patients aged 49-56 years with moderate acute pain (Numeric Rating Scale 4-5). Each participant received a standardized 15-minute guided autogenic relaxation session daily for three consecutive days at home, with pain, blood pressure, vital signs, and pain-related behavior recorded before and after every session. Mean pain intensity fell from 4.67 to 1.33, a 71.5% reduction; systolic pressure declined by 21 mmHg and diastolic pressure by 13 mmHg; and observable pain behaviors resolved by the third day. Two of the three participants began practicing the technique independently before the scheduled session on day two, consistent with Orem's transition from a partially compensatory to a supportive-educative nursing system. These findings support incorporating guided autogenic relaxation into home-based community nursing as an accessible, theory-consistent strategy for strengthening self-care agency in hypertension management.

Abstract

Background: Indonesian mental health service users increasingly negotiate biomedical psychiatric treatment alongside jamu, dukun consultations, and spiritual rituals, yet how this negotiation is enacted and disclosed outside the clinic remains poorly documented. Objective: To examine how Indonesian-language digital communities construct, coordinate, and communicate the integration of traditional healing with psychiatric care, and to derive implications for psychiatric nursing practice. Methods: An 18-week netnographic study (1 February–10 June 2026) of four Indonesian-language digital communities captured 1,563 threads and 9,847 posts from 2,489 participants who explicitly discussed integrating traditional healing with psychiatric treatment. Data were examined using reflexive thematic analysis. Results: Five interconnected themes emerged: epistemological pluralism, concealment and selective disclosure, temporal negotiation patterns, digital knowledge translation, and identity preservation through healing. Most participants (63.0%) concealed traditional healing use from every provider, while digital communities functioned as informal sites of herb–drug interaction surveillance and peer knowledge exchange. Conclusion: Traditional healing integration is better understood as a parallel, patient-managed care system than as covert non-adherence; psychiatric nurses require culturally responsive, non-judgemental assessment approaches that make this system visible rather than punishable

Abstract

Latar Belakang: Masyarakat pertanian menghadapi berbagai tantangan, seperti keterbatasan pemanfaatan pekarangan, ketidakpastian hasil panen, dan tekanan ekonomi yang dapat memengaruhi ketahanan pangan keluarga serta kesejahteraan psikologis. Budidaya ikan dalam ember (Budikdamber) merupakan inovasi yang berpotensi mendukung ketahanan pangan sekaligus meningkatkan resiliensi masyarakat melalui aktivitas produktif berbasis pekarangan. Tujuan: Kegiatan ini bertujuan meningkatkan pengetahuan dan keterampilan masyarakat mengenai Budikdamber serta mengeksplorasi kontribusinya terhadap penguatan resiliensi masyarakat pertanian. Metode: Pengabdian dilaksanakan di Dusun Badong, Desa Mojoparon, Kabupaten Pasuruan dengan melibatkan 18 peserta. Program menggunakan pendekatan pemberdayaan masyarakat melalui penyuluhan, demonstrasi, pelatihan intensif dan praktik Budikdamber secara langsung. Evaluasi dilakukan menggunakan pretest–posttest pengetahuan serta wawancara semi-terstruktur yang dianalisis secara tematik. Hasil: Proporsi peserta dengan pengetahuan baik meningkat dari 22,2% menjadi 83,3% setelah kegiatan. Hasil wawancara menunjukkan bahwa Budikdamber memberikan rasa nyaman, meningkatkan kepercayaan diri dan optimisme, memperkuat interaksi sosial, serta mendukung kemampuan peserta dalam menghadapi tekanan kehidupan sehari-hari. Kesimpulan: Program Budikdamber efektif meningkatkan pengetahuan dan keterampilan masyarakat serta berpotensi mendukung ketahanan pangan dan penguatan resiliensi masyarakat pertanian melalui aktivitas produktif berbasis lingkungan.
