Abstract

Acne vulgaris is a skin disease, a chronic inflammation that affects the poly sebaceous glands, especially on the face and neck. One of the endogenous causes that influence the occurrence of acne vulgaris is a genetic and hormonal predisposition. Family history of acne is one of the risk factors in family genetics associated with the onset of the disease, the difficulty of therapy, and the risk for relapse. This research aimed to explain the relationship between genetic factors to the degree of acne vulgaris as one of the predisposing factors. This research was observational analytic, using purposive sampling as the sampling method and collecting data using a cross-sectional questionnaire. Statistical analysis between variables shows that age was not related (p = 0,790), while gender was not related (p = 0,066), the father's genetic history was related (p = 0,001), and mother's genetic history was not related (p = 0,390). In conclusion, among the factors that were analyzed, the father's genetic history was the only one which has a relationship in aggravating the degree of acne vulgaris.

Abstract

Mycosis is often misdiagnosed as Tuberculosis (TB) with negative sputum smear results or Tuberculosis recurrent. In the last two-decade, mycosis increases dramatically as HIV and immunocompromised incidence rates increased. Even though, the prevalence data of mycosis in TB patient is less researched, the identification using KOH often give a false negative result. To compare the result of fungal identification using KOH and PAS staining. This study is observational analytical using a cross-sectional design with 29 TB samples that have been diagnosed with Ziehl-Neelsen (ZN) stain positive in RSUD Dr. Soetomo. The sputum of every sample was colored with KOH and PAS and observed by the researcher and microbiology analyst. The results of this study were 22 sputa identified with Candida sp positive and 12 sputa with non-Candida positive using KOH staining. Meanwhile, using PAS staining, there were 25 sputa identified as Candida sp positive and 22 sputa as non-Candida positive. The congruency test between KOH and PAS has a fair result for both identifying Candida sp (κ = 0.298; significance = 0.069) and non-Candida (κ = 0.240; significance = 0.095). The identification of candida and non-candida in TB patients using KOH and PAS has a fair congruency result. The PAS staining has better identification results both in identifying candida and non-candida rather than KOH staining.

Abstract

Background: Foreign bodies in the larynx are reported quite rarely, with a prevalence of around 4% among patients, especially geriatrics, and represent an acute airway emergency with significant morbidity and mortality rate. Objective: This case report aimed to provide information on managing the extraction of foreign bodies and the accompanying problems. Discussion: A 49-year-old man with a history of wearing dentures was admitted to the emergency room. The patient complained of a hoarse voice upon waking up, painful swallowing, and a lumpy sensation in his throat, but he had no chest pain, cough, nausea, or vomiting. An indirect laryngoscopy examination revealed a foreign body lodged in the anterior rima glottis, impeding the mobility of the vocal folds. Under general anesthesia, emergency direct laryngoscopy, evacuation, and tracheoscopy were performed. It was observed that a foreign denture body was lodged in the larynx and was evacuated using grasping forceps. Post-evacuation, the vocal folds were swollen, with a false vocal fold hematoma, posterior subglottic laceration, and no active bleeding. Conclusion: Foreign bodies in the larynx must be prompted and gently removed because it leads to life-threatening. In suspected swallowed foreign bodies, plain radiographs are the initial imaging approach. Inhalational or intravenous induction is operated to administer the general anesthetic, along with the maintenance of spontaneous ventilation.

Abstract

Acute rheumatic fever is a nonsuppurative, immune-mediated consequence of group A streptococcal pharyngitis. Recurrent or severe acute rheumatic fever can cause permanent cardiac valve damage and rheumatic heart disease. Universally, the most common major manifestations during the first episode of ARF remain carditis and arthritis. Subclinical carditis now can fulfils a major criterion for ARF in all populations as in revised Jones criteria – AHA 2015. Many of the clinical features of ARF are non-specific, so a wide range of differential diagnoses should be considered. Primary prevention requires accurate recognition and proper antibiotic treatment of GAS pharyngitis. Prevention of recurrent attacks of rheumatic fever (secondary prevention) is the most cost-effective way of preventing further rheumatic heart disease (RHD). Penicillin remains the antibiotic of choice. Intramuscular penicillin is preferred as it is more effective than oral penicillin and results in better compliance.

Abstract

Congenital heart disease is a concerning disease among those affected since it affects the patient's quality of life. One of which is Ebstein Anomaly (EA), with a prevalence of 1 in 200,0000 births with a clinical characteristic of displacement of the tricuspid valve that causes atrialization of the right-side heart. Women of childbearing age are at risk because they especially have asymptomatic EA, compromised to hemodynamic consequences, and risk of maternal complications. A 25-year-old woman visited the emergency department with worsening shortness of breath since last week, during rest, accompanied by decreased physical activity tolerance, bilateral ankle edema, cyanosis to the lips, and nail bed. She had echocardiography and cardiac multi-slice computed tomography to identify EA before and after her first child's birth. She was well controlled with a loop diuretic, phosphodiesterase type 5 inhibitor, and beta-blocker but could not care for her child due to her condition. The patient was discharged after symptoms subsided, awaiting further interventional evaluation. Although EA is rare, it has high morbidity and mortality, especially in women of childbearing age since it may be asymptomatic during childhood. Therefore, early recognition of EA is probably necessary for women who are planning pregnancy.

Abstract

Alzheimer's attacks 24 million global population and dominates 60-80% of existing cases of dementia. It causes the accumulation of beta-amyloid (Aβ) plaques in the hippocampus and entorhinal cortex, resulting in decreased mass from the brain. Recent studies have shown that the manifestation of this disease is due to an overaccumulation of abnormal Aβ protein due to abnormalities in the APP and APOE4 genes. Point mutations in the APP gene will create the toxic form of Aβ protein, namely Aβ42, and the toxic APOE4 gene will accelerate the onset of Aβ42 deposition and pro-inflammatory activity that exacerbates the degenerative process of the brain. Gene editing as a potential definitive therapy was recently a concern by researchers. CRISPR-Cas9 repairs the APP gene and substitutes the APOE4 gene with APOE3 by modifying the gene's DNA sequence. Nano complex CRISPR-Cas9 tandem peptide lipid is a model for clinicians to target brain nerve cells. In vivo research on an Alzheimer's mouse model proved the potential of nano-complex-based peptides as carriers of CRISPR-Cas9 in brain nerve cells. This engineering technology offers satisfactory results with high precision, minimal side effects, and a relatively low price for long-term therapeutic effects and even a lifetime.

Abstract

ST-elevation myocardial infarction is a form of acute coronary syndrome that is the leading cause of death worldwide. Treatment with reperfusion therapy in the form of primary percutaneous intervention is the main treatment to reduce morbidity and mortality. However, reperfusion therapy is not recommended for patients presenting with symptoms onset of more than 48 hours. A 62-year-old male patient was diagnosed with anterior STEMI with symptoms onset of more than 48 hours based on complaints of anginal chest pain and shortness of breath. ST-segment elevation in V1-V4 and pathological Q in V1-V3 on ECG and increased hs-Troponin I was reported. The patient was not treated with reperfusion therapy based on treatment recommendations. The patient was given vasodilators, beta-blockers, statins, heparinization, and comorbid infection control in the cardiac care ward. Clinical improvement was obtained, and the patient was discharged after seven days of hospitalization and then was planned for Dobutamine Stress Echo during the follow-up visit. Conservative management and viability testing are the main options for patient management in STEMI with symptom onset of more than 48 hours without any complaints of chest pain, stable hemodynamics, and no life-threatening arrhythmia.

Abstract

Escherichia coli is the main pathogenic bacteria which causes diarrhea in humans and the fourth highest cause of death which occurs all age groups. Salmonella sp is the cause of typhoid fever and findings of 350-810 cases each year. Diarrhea and typhoid fever are closely related to the consumption of raw vegetables, such as lettuce. This study aimed to analyze the contamination of E. coli and Salmonella sp on some lettuces in Traditional Markets of Banyuwangi Regency. This research was an analytic descriptive study using a cross-sectional design at the Laboratory of Microbiology, STIKES Banyuwangi from 6 to 11 June 2022 using 6 lettuces as research samples which were selected by accidental sampling. All (100%) lettuce samples in this study were contaminated with >1,100 CFU/ml E. coli. All (100%) samples in this study were positively contaminated by Salmonella sp with various contamination number, such as: 0.82x104; 0.47x104, 1.90x105, 2.05x106, 1.00x105 and 1.25x106 CFU/ml, respectively. All six lettuces obtained from the Traditional Market of Banyuwangi Regency did not meet the microbiological quality specified by SNI, indicating that they were not suitable for  direct (raw) consumption.

Abstract

Pneumothorax occurs when the pleural space is filled with air due to trauma, iatrogenic injury, or underlying lung disease. If this air leak forms a one-way valve that causes air to be trapped, an uncontrolled increase in air volume can occur, causing lung collapse and shift of the mediastinum to the contralateral side, which can eventually cause respiratory and hemodynamic disturbances or even death. Here, we report a 46-year-old male patient who came to the hospital complaining of shortness of breath and chest pain radiating to the left shoulder. On physical examination, the patient had severe shortness of breath, tracheal deviation, decreased expansion and air entry of the left hemithorax, hyperresonance of the left hemithorax, and a shift of the punctum maximum of the heart sounds to the left sternal line. The chest x-ray performed on the patient confirmed the suspicion of a tension pneumothorax. A needle decompression in the 2nd intercostal space in the left midclavicular line failed to improve the tension pneumothorax. A chest tube insertion was performed which managed to improve the patient's clinical condition.

Abstract

Psoriatic arthritis (PsA) is a chronic, deforming arthritis associated with psoriatic skin lesions. Numerous patients with PsA carry other co-existing chronic diseases, adding to their overall disease burden and affecting the patient’s quality of life. Depression is a common illness known to coexist in about 20% of patients with PsA. Long-term inflammation conditions can make patients more depressed and make the treatment more difficult. Cushing Syndrome (CS) is a complication of long-term treatment due to the exposure of glucocorticoids given to turn the hypothyroid condition into hyperthyroid because hypercortisolism in humans lowers TSH secretion and TSH pulse amplitude. When PsA combines with depression and CS, it will create complex conditions and treatments. The complexity is all about how we control the disease activity of PsA and the vicious circle of an inflammatory process that is difficult to control. Conventional treatment will fail, and targeted therapy with monoclonal antibodies such as anti-IL-17 Secukinumab, is needed. Secukinumab as an anti-IL-17 will block the inflammation pathway from interleukin-17, decrease the inflammation process, and improve the symptoms of PsA. We report a patient with psoriatic arthritis and Hashimoto's thyroiditis (HT) with a major depressive episode with CS and subclinical hyperthyroidism successfully treated with Secukinumab.
