Abstract

Inflammation of the synovial membrane of a joint is a common symptom of transient synovitis (TS), which is the medical term for a condition that leads to hip discomfort in juvenile patients.  The estimated annual incidence of TS is 0.2%, and the risk of developing the disease over one's lifetime is 3%.  Unfortunately, the specific pathophysiology of TS is still unclear.  Therefore, the particular treatment is still debatable.  There are several methods of treating TS, starting with bed rest in the hospital and using analgesics, steroids, skin traction, and joint aspiration.  Most of the time, transient synovitis is a self-limiting condition that heals independently and is managed conservatively by monitoring and rest.  Several medical therapy options, including analgesics, NSAIDs, and corticosteroids, speed up healing.  On the other hand, skin traction is also reported to provide beneficial therapeutic effects for TS patients.  Thus, with many therapeutic options, TS's "golden" management still needs further study.

Abstract

Propolis is an herbal medicinal plant that contains caffeic acid phenethyl ester. This natural polyphenol compound acts as an antioxidant and can reduce blood glucose levels and increase body weight. This study aims to determine the effectiveness of Gunung Lawu propolis in lowering blood glucose levels and increasing body weight in diabetic rats. The Wistar rats were induced with STZ 45 mg/kg BW and nicotinamide 110 mg/kg BW as diabetic model rats. Research subjects were taken randomly and divided into five groups: (1) Normal, (2) DM with no propolis, (3) DM 14 days + propolis 100 mg/kg BW/day (P1), (4) DM 14 days + Propolis 200 mg/kg BW/day (P2), and (5) DM 0 day + Propolis 200 mg/kg BW/day (P3). Propolis extract was given orally once a day for 14 consecutive days (in groups 3 and 4) and 28 consecutive days (in group 5). Blood glucose levels in the treatment group (P1, P2, and P3) decreased significantly (P1: 115.28 ± 4.7 mg/dL; P2: 98.36 ± 4.8 mg/dL; and P3: 87.36 ± 4, 2 mg/dL) compared with that in the DM group (272.07 ± 3.9 mg/dL). The body weight of the treatment group (P1, P2, and P3) increased significantly (P1: 180.60 ± 5.7 g; P2: 180.60 ± 4.2 g; and P3: 208.00 ± 5.1 gr) compared with that in the DM group (160.00 ± 3.2 gr). In conclusion, Propolis is effective in reducing blood glucose levels and improving (increasing) body weight in diabetic model rats.

Abstract

Plantar fasciitis in Indonesia, occur after 30 years of age, 77% of sufferers between 40 to 69 years old, men to women is 1:2. There are risk factors for plantar fasciitis include structural abnormalities, being overweight, age-related degenerative changes, activities that require prolonged standing and/or ambulation, and errors in training, especially in athletes. We present 20 case series of all patients diagnosed with plantar fasciitis who went to the Orthopedic Polyclinic in 2018 and 2019, they received conservative management with Calf Stretch workouts for 30 days and were evaluated with AOFAS Score. Calf stretch exercises in which the patient stands with the sore leg behind and both hands pushing against a wall, also known as the wall stretch. This exercise is performed 2 times a day with a duration of 10-15 minutes each session. This exercise is done routinely for 30 days. A simple, progressive training protocol consisting of strength training with heavy weights will produce better results in the first three months when compared to plantar-focused stretching and may aid in more rapid relief of symptoms on pain. Conservative management with calf exercises still provides a good outcome in patients with plantar fasciitis, based on the AOFAS score.

Abstract

Stroke is a metabolic illness that causes significant impairment in the working-age population. Disability develops due to the consequences of neurological deficiencies and the failure of the medical rehabilitation process. Post-stroke pain is one of the causes of this failure. In a post-stroke patient, pain is an unpleasant physical and emotional experience. In contrast, range of motion barriers might cause the medical rehabilitation procedure to fail. This paper aims to review the various methods of post-stroke pain management that can be used as an alternative therapy that helps post-stroke patients' rehabilitation. The PubMed database was used to search for different kinds of literature. The study includes clinical studies, pilot studies, and randomized control trials published between January 2015 and June 2023. The authors omit several publications to ensure that the final selection of papers includes only the most relevant and reputable sources of information on post-stroke pain, post-stroke pain management, and pain as a measure of outcome. There are 28 publications to be reviewed. The most prevalent cause of post-stroke pain was hemiplegic shoulder pain in numerous studies that have an impact on the post-stroke recovery process. In conclusion, the options for post-stroke therapy range from conservative rehabilitation to interventional therapy. Several innovative experimental rehabilitation treatment approaches have been studied. However, the findings do not outperform conventional rehabilitation treatment.

Abstract

Anxiety is a global psychiatric phenomenon, while poor sleep quality remains a pressing public health issue. The arduous medical education journey exposes students to prolonged stress, potentially exacerbating anxiety and sleep disturbances. This study investigates the correlation between anxiety and sleep quality in Hang Tuah Faculty of Medicine students, bridging the pre-clinical and clinical phases. Employing an analytical observational approach with a cross-sectional design, this study enrolled students from pre-clinical (batches 2020, 2021, 2022) and clinical years (batches DM45, DM46, DM47). Among the 409 approached participants, 296 met the inclusion criteria after excluding 113 samples. Data collection utilized online questionnaires with acquired informed consent. Sleep quality was evaluated via the Pittsburgh Sleep Quality Index, generating scores from 0 to 21. This study showed significantly robust evidence of an association between anxiety and sleep quality (p-value: 0.0001; r: 0.342) across both pre-clinical and clinical students. This study underscores a direct relationship: elevated anxiety levels correspond to diminished sleep quality. Such findings resonate with the prevailing challenges in medical education and their impact on students' mental and physical well-being. Implementing targeted interventions is crucial to address these concerns. In conclusion, this study underscores the undeniable link between anxiety and sleep quality among Hang Tuah Faculty of Medicine students, encompassing the years 2017 to 2022. The implications are far-reaching, emphasizing the necessity of interventions that enhance both psychological well-being and sleep quality. By recognizing and addressing these challenges, medical institutions can foster healthier academic environments and better equip students for their medical careers.

Abstract

Tuberculosis (TB) is the main risk factor for chronic pulmonary aspergillosis, especially in patients with residual cavitary lesions after completion of TB therapy. The body mass index (BMI) is one of the established markers in predicting the mortality of CPA. However, the data regarding BMI profiles of CPA patients in Indonesia is still limited. We evaluated the BMI profiles of CPA patients among post-TB patients. Patients diagnosed as CPA based on clinical, radiology and the Aspergillus IgG test. 50 patients were included with a mean age of 35 years; 13 patients met the criteria of CPA. The overall median of BMI in these patients was 21. The median BMI in the CPA group was 19,3. The lowest (19,8) median BMI value was observed in the positive Aspergillus IgG group (11,5 – 30 mg/L) with a minimum BMI of 14,7 and a maximum BMI of 29,2. The differences in median BMI across CPA and non CPA groups and Aspergillus IgG level were not statistically significant. However, there is a trend that lower BMI were distributed among patients with positive Aspergillus IgG. More than one third of CPA patients in this study were classified as underweight. Future study is necessary to better depict the BMI profiles in larger scale of populations of CPA in Indonesia in order to achieve better diagnosis and management of CPA.

Abstract

The most common worm infections are those caused by soil-transmitted helminths (STH) infection. Soil contamination is one of the causes of helminth transmission to humans. Poor environmental sanitation and personal hygiene facilitate the spread and transmission of STH. Jember is a district where the majority of the population lives in rural areas with poor sanitation access. This study aims to determine the correlation between soil contamination and incident of STH infection in Sukokerto Village, Sukowono District, Jember Regency. This cross-sectional analytic observational study was conducted from August 2022 to February 2023. The total of samples in this study was 44 fecal and soil around the house. The data obtained from the laboratory examination showed that the total fecal positive of STH was 11 or 25%, and soil was 13 or 29.5%. The results of the Chi-Square test analysis obtained ρ<0.05 (ρ=0.036, OR=4,457). The results showed a significant relationship between soil contamination and the incidence of STH infection in Sukokerto Village, Sukowo District, Jember Regency
