Abstract

Efforts to limit and regulate birth spacing as a goal to create quality families and healthy families, the government encourages people to use contraceptives as birth control. One of these contraceptives is injection contraception. Injecting contraception is the most widely used contraceptive tool by the people of Indonesia, is considered effective, practical, but has several side effects, the most worrying is that it affects blood glucose. Blood glucose levels in the human body that increase more than normal can be a symptom of diabetes mellitus. The purpose of this study is to determine differences in blood glucose levels in progestin injection and combined injection contraceptive acceptors. Observational research design using a cross sectional approach. The study population was all injectable acceptors. Samples were taken that met the inclusion criteria, namely respondents were willing to be examined, there was no history of diabetes mellitus (DM), less than 40 years of age, acceptors of at least 6-8 hours fasting. Sampling was carried out using consecutive sampling techniques totaling 50 acceptors (25 progestin injection acceptors, 25 combined injection acceptors). Data were analyzed by Witney mann test using α = 0.05. The results showed the mean fasting blood glucose for progestin injection acceptors was higher than for combination injections, the test results P = 0.008 <α = 0.05. The conclusions of the study show that there are significant differences in blood glucose levels in progestin-injecting acceptors and this combination means that progestin-injecting contraceptives have a higher impact on increasing fasting blood sugar levels compared with combined injection contraception. Suggestions for health workers to control blood glucose injector acceptors at acceptors who experience high glucose levels are recommended to replace non-hormonal contraceptive methods.

Abstract

Background: The percutaneous coronary intervention procedure (PCI) is an action to restore reduced coronary artery blood flow due to occlusion or blockage. Complex actions using sophisticated tools with the patient's condition fully aware can cause clients to experience anxiety, both before, during, and after the action. Many patients undergoing PCI experience this symptom of anxiety. These symptoms can lead to activation of the sympathetic nerves, changes in hemodynamic status, to the risk of arrhythmias during or after the procedure. Intervention to reduce anxiety hopefully can stabilize haemodynamic, avoid unwanted effects during and after the procedure, further can also improve the patient's quality of life.Purpose: This integrated literature review aims to find complementary interventions that can be carried out independently to overcome the anxiety experienced among patients undergoing PPCI.Methods: The compiled database was taken from the EBSCO, Scopus, Clinical Key, ProQuest, Science Direct, PubMed, and Cochrane databases, and published in 2015 - 2020 which were then matched with certain specific keywords to get the appropriate articles. A total of 11 selected articles were reviewed and the results were presented in a table. The results of 6 types of intervention were found, almost all of them were significant in reducing anxiety. Next, appraisal of the quality of selected articles is done by using The Critical Appraisal Skills Programme (CASP).Results: There were 11 articles that discussed 6 different forms of intervention in reducing anxiety, including physical therapy (2 articles), relaxation therapy (2 articles), cognitive therapy (2 articles), psychological therapy (1 article), and psychological therapy programs (combined) (1 article) and information / support / counseling (3 articles). Conclusion: This intervention in addition to reducing anxiety can also reduce stress, hemodynamic balance / physiological indicators, reduce pain, improve quality of life and coping mechanisms.

Abstract

Background: One form of service in the MCH program is services for pregnant women which consist of health services for pregnant women, maternal health, postpartum health, handling of obstetric complications, and contraceptive services, all of which aim to ensure protection for pregnant women and / or fetuses. This is closely related to efforts to reduce maternal mortality (MMR). Objectives: The research objective was to determine the priority of health problems, find the root of health problems and provide alternative solutions to problems in health care for pregnant women Methods: The research method used is descriptive research using secondary data sources to determine the priority of the problem by means of a Focus Group Discussion (FGD) involving decision makers at the Konawe District Health Office. The formulation of alternative solutions is carried out after the root causes are identified using Ishikawa or Fishbone diagrams. The priority of the root cause of the problem is carried out using the Capability Accessibility Readiness Leverage (CARL) method. Results: The results show that the low K1 and K4 coverage of pregnant women health services is the main priority problem. The root cause of the problem that must be prioritized to be resolved is that socialization to teenagers / prospective brides (catin) about marriage at a young age is still a major problem that must be followed up. The alternative that is given as a solution is to provide counseling using innovative and attractive media to make it easier to understand. Conclusions: The priority of health problems in the MCH Program of the Konawe District Health Office is the problem of health services for pregnant women with K1 and K4 coverage. The root of the problem of socialization among adolescents / catin about marriage at a young age is still lacking is the level of education, knowledge, employment, counseling media are less innovative and interesting, the frequency of counseling and the size of the area are not proportional to the available resources. An alternative solution is socialization to teenagers/catin about marriage at a young age is still lacking by using innovative and interesting counseling media so that it is easy to receive and understand the information conveyed.
