Abstract

Introduction : A cataract is a cloudiness that occurs in the lens of the eye , which in normal conditions clear.Objective : To obtain the prevalence of cataract in children in hospital . H. Adam Malik Medan in 2012.Methods : This was a retrospective descriptive study . Data obtained from medical records of patients who were suffering from cataracts who went to the ophthalmology clinic of children RS H Adam Malik Medan in 2012 .Results : The age group of 4-6 years is the group (24.4%) age of majority , while the smallest age group 13-17 years ( 13.3%). Cataracts in children more dominating in boys is equal to 57.8% of girls. Based on the most lateralitas be unilateral , ie 64.4%, with the involvement of the right eye the most experienced , when compared with the left eye. While 35.6% occurred in children with bilateral cataracts . Nontraumatic cataract occurs at 53.3% . While traumatic cataract which was only 46.7% . Of the number of patients with cataract in children by 45 people, then the prevalence of cataract in children in RS Adam Malik in 2012 amounted to 0.55% .Conclusion : The prevalence of cataract in children in RS Adam Malik Medan in 2012 is 0.55% . Which can be used as a reference for the prevention and treatment of cataracts .Keywords : cataract in children; cataracts; nontraumatic cataract; traumatic cataract

Abstract

Introduction : Monosodium glutamate (MSG), a food additive that is still a controversial, where some states are safe, but partly it's harmful to health. High consumption of MSG will increase glutamate metabolism. It will increase free radical, which harmful to body. Honey as antioxidant can remove free radical. Giving honey in pregnant mice after giving MSG, will reduce the negative impact of MSG.The purpose of study was to determine the effects of honey therapy against fetal development disorder of mice and hepatic lesions induced by giving of MSG during pregnancyMethods : The study is purely experimental with a completely randomized design (CRD), which consists of five groups, there are one control group and four treatment groups. The control group was given distilled water only during gestation days 0-18. The first treatment were given MSG 4mg/g body weight/day, the second treatment were given honey 0.15g/20g body weight /day, and the fourth treatment were given MSG 0.15 g/20g body weight/day and honey 4mg/g body weight/day, as long as 0- 18 days of gestation. The third treatment groups were given MSG 4mg/g body weight/day on days 0-7 of pregnancy, and continued provision of honey 0.15g/20g body weight/day on days 8-18 of pregnancy. At gestational age 18 days, each treatment and control mice were killed by cervical dislocation and then dissected for liver and fetuses removalResult : Giving of high doses of MSG to mice during pregnancy, give embriotoxic effect, characterized by decreased the number of live fetuses up to 54.9% (P = 0.003) and increased the number of embrio resorption up to 58.86% (P = 0.02), and hepatotoxic effect compare control with P value = 0.041. Giving honey can reduce hepatotoxic and embriotoxic of MSG effect, particularly they are giving together will increase the number of live fetuses up to 100% and decrease the number of embrio resorption up to 0%.Key words : monosodium glutamate (MSG); fetal; development disorder; mice; pregnancy; hepatic lesion77 |

Abstract

Feeding skills are complex process that involves development of sensoric and motoric abilities. Feeding skills are influenced by anatomical, neurophysiological, and socio-cultural environment. Introduction of proper food that accordance with development of oromotor skills result in perfect development of feeding skill and also reinforce bonding between mother and child. Inappropiate feeding skills development will adverse bad impact on nutritional status, psychological development and the onset of feeding difficulties.Keywords : feeding skil; children

Abstract

Introduction : Systemic Inflammatory Response Syndrome (SIRS) is a non specificreaction that could be caused by infection, inflammation, or acombination of both factors.Bacteremia detectionin SIRS patientsis a challenge forclinicians. Neutrophil-lymphocyte count ratio (NLCR) and C-reactive protein (CRP) could rapidly predict bacteremia.Method : Thiscross sectional study was conducted in Emergency Unit RSUPH.Adam Malik Medan from January to March 2016. Patients that met SIRS criteria had their NLCR, CRPand blood culture tested.Result : Of 48 samples, 21 (43.75%) were male and 27 (56.25%) were female.In this study, there was significant difference of NLCR from both groups, where NLCR in SIRS with positive blood culture was higher that SIRS with negative blood culture(38.16 ± 34.97vs9.68 ± 5.26, P=0.001). There wasa significant difference between CRP in both groups. CRP in SIRS with positive blood culture was higher than SIRS with negative blood culture. (5.89 ± 3.09vs 3.15 ± 1.89, p = 0.001).Conclusions : There is significant difference of NLCR and CRP in SIRS patients with positive and negative blood culture. Abstract

Neonatal sepsis is a clinical syndrome with symptoms of systemic infection and followed by bacteremia in the first month of life. Antibotic initial therapy empirically is often given concomittantly awaiting the results of blood cultures in neonates with suspected sepsis. The recommended initial treatment in general is a combination of ampicillin and aminoglycoside (gentamicin). Alternative to aminoglycosides is the third-generation cephalosporins (eg cefotaxime). This regimen was found safe and effective by appropriate duration of therapy. This therapy should be evaluated every 48 hours with a lingering risk of therapy associated with increased risk of necrotizing enterocolitis and death.Keyword : antibotic initial, Neonatal sepsis

Abstract

Background : Recent evidences showed correlation main class of lipoprotein with stroke and brain tumor. Stroke and brain tumor can caused by multifactor, and nutrition is one of their etiology.Objectives : This study aimed to investigate correlation between lipoprotein with stroke and brain tumor.Methods : This was a cross-sectional study, subjects with stroke (26 persons) and brain tumor (26 persons). All patients underwent physical and neurological examinations, neuroimaging and lipoprotein serum examinations. Results : Stroke subjects consisted of 17 (65.4%) women and 9 (34.6%) men, with mean age ±54.38 years. Mean HDL level was 38.96±15.66 mg/dL, mean LDL level was 131.69±32.37 mg/dL, mean VLDL level was 22.42±6.64 mg/dL. The LDL level were significantly correlated with stroke and had a moderate correlation (r=0.426, p=0.030). The HDL level were not significantly correlated with stroke and had a very weak correlation (r=-0.010, p=0.960). VLDL level were not significantly correlated with stroke and had a very weak correlation (r=-0.087, p=0.672). Brain tumor subjects consisted of 16 (61.5%) men and 10 (38.5%) women, with mean age ±46.46 years. Mean HDL level was 38.58±12.73 mg/dL, mean LDL level was 131.42±43.67 mg/dL, mean HDL level was 26.70±10.75 mg/dL. The LDL level were not significantly correlated with brain tumor and had a very weak correlation (r=-0.103, p=0.618). HDL level were not significantly correlated with brain tumor and had a very weak correlation (r=0.087, p=0.672). VLDL level were not significantly correlated with brain tumor and had a very weak correlation (r=0.031, p=0.881).Conclusion : There was a significant correlation between LDL level and stroke, but no significant corelation between HDL levels with stroke and VLDL levels with stroke . The HDL, LDL and VLDL levels were not significant corelation with brain tumor. Keywords : lipoprotein, HDL, LDL, VLDL, stroke,brain tumor

Abstract

Background : Aspiration pneumonia is the most important acute complication of stroke related dysphagia. Tube feeding is usually recommended as an effective and safe way to supply nutrition in stroke patients with dysphagia. On the cointains, many patients on continued to aspirate eventhough the used the nasogastric tube.The purpose of this study was to confirm that there werethe is correlation between nasogastric tube usege and incidence of pneumonia in acute stroke patient with dysphagia. Methods : This cross sectional study observed 60 acute stroke patients with dysphagia consist, 30 patiens using tube feeding and 30 were not using tube feeding, who stayed in Adam Malik General Hospital. Patients who admitted with pneumonia or other pulmonary infection or decreased level of conciussness were excluded. Patients were follow up in acute phase and pneumonia was diagnosed based on Center for Disease Control Criteria that were adopted by Indonesia Association of Lung Doctors.Results : In 30 acute stroke group patients with dysphagia who used nasogastric tube mean age is of 59 years old, 13(43.3%) females and 17(56.7%) males. While the other group who not do use nasogastric tube tube feeding were 30 acute stroke patients the mean age of 54 years old, 16(53.3%) males and 14(46.7%) females. There was no significant association with very weak correlation between Nasogastric tube usedwith pneumonia ( p=0591, r=0.063).Conclusion: Nasogastric Tube using does not have a significant with very weak correlation to the occurance of pneumonia. Abstract

Iron deficiency and lead poisoning often occurs simultaneously causing anemia and showed severe anemia. Conditions of iron deficiency anemia increases the incidence of lead poisoning due to lead and iron have the same receptors that Divalent Metal Transporter 1 (DMT 1). Prevention that children do not suffer from iron deficiency anemia is one way to prevent lead poisoning. In children with BLL above 45 mg / dL, first managed lead poisoning with chelation agents then overcome iron deficiency anemia.Keyword : iron deficiency, lead poisoning, anemia, children

Abstract

Introduction : Colorectal cancer is the third most common cancer in the world with nearly 1.4 million new cases diagnosed in 2012. The incidence rate of men in higher than women in all countries. Colorectal cancer patients undergoing elective surgery more than emergency. The aim of the study was to determine the clinical features and pathological cases of colorectalcancer that were performed emergency surgery and elective at RSUP Haji Adam Malik from January 2011 to December 2013.Method : This study is a retrospective desriptive study design with secondary data from medical record of patients with colorectal cancer who had performed emergency and elective surgery at Adam Malik Hospital from January 2011 to December 2013.Result :From 40 cases underwent emergency surgery there were 22 male and 18 female, and from 139 cases undergoing elective surgery there were 77 male, 62 female. In the case of emergency 38 cases with clinical fincing of obstruction and 2 cases with peritonitis. For elective cases, 65 cases with clinical finding of defecating blood mucus, 41 cases of constipation and 33 cases with a palpable mass in the abdomen. Histopathological features in emergency cases, 22 cases with well differentiated adeno Ca and in elective cases, 81 cases with well differentiated adeno Ca.Conclusion: The most clinical features in emergency cases with obstruction were 38 cases, while for elective cases with clinical picture defecating blood mucus were 65 cases. The most histopathologic features in emergency cases with well differetiated Ca were 22 cases and in elective cases were 81 cases.Keywords : Colorectal cancer, clinical pathology, emergency and elective surgery

Abstract

Introduction: Determination of Hormonal receptors is of utmost importance in planning therapy in patients with breast cancer. Breast cancer ussualy develops in perimenopausal and postmenopausal women and only several cases before age 25. Endogenous excess also plays role in the onset of breast cancer,  particularly in women with prolong reproduvtive periode, nulliparae, and those first delivery at an older age. The aim of the study was to assess the difference expression of estrogen receptors (ER) and progesterone receptors (PR) premenopausal and postmenopausal in breast cancer patient.Method :  This study  was analitic cross sectional included 245 patient treated at Oncology Surgery Division, Departemen of Surgery Medical Faculty USU – RSUP H. Adam Malik, between Januari 2013 until Mei 2015. ER and PR status was determine by Immunohistochemistry (IHC) from paraffin block.Result : From this researh the result of the premenopausal patients 137 (55.9%), postmenopausal 108 (44.1%). The frequency of premenopausal with ER + and PR +, 33 (24.1%) and postmenopausal 25 (23.1%). There was no significant different between ER and PR expression, tumor size and differentiation from histopathology grading between premenopausal and postmenopausal patient (p>0.05) in this research. And also in this research there was significant comparison between ER + PR + (Hormone dependent carsinoma) with premenopausal and postmenopausal with p value 0.043.Conclusion : From this research the hormonal expression receptor of ER and PR is higher in the premenopausal patient than the postmenopausal patient, but have no significant result in statistic.Keywords :  Estrogen Receptor, Progesteron Receptor, Premenopausal, Postmenopausal.
