Abstract

Background: Type 2 diabetes mellitus (T2DM) is a chronic metabolic illness characterized by persistent hyperglycemia, which raises the chance of macrovascular and microvascular complications. These issues are linked to increased platelet activation, aggregation, and thromboxane production.Objective: This study sought to assess the use of Mean Platelet Volume (MPV) and Platelet Distribution Width (PDW) as possible hematologic indicators for detecting vascular complications in people with T2DM. Methods: A retrospective cross-sectional research was conducted using the medical records of 141 T2DM patients treated at Sanjiwani Gianyar Hospital in 2023. Patients were split into two groups based on the presence or absence of vascular complications such as coronary artery disease, cerebrovascular disease, peripheral arterial disease, diabetic nephropathy, and diabetic retinopathy. MPV and PDW values were derived from routine Complete Blood Count (CBC) testing. The data were tested for normality, and any changes between groups were calculated using an independent samples t-test.Results: Patients with vascular complications had significantly higher MPV values (9.53 ± 1.30 fL) than those without complications (8.99 ± 0.96 fL), with a statistically significant p-value of 0. 018. However, the two groups' PDW values did not differ significantly (15.95 ± 0.44 vs. 15.91 ± 0.36; p = 0.582).Conclusions: MPV may be a useful and cost-effective hematological parameter associated with vascular complications in T2DM patients, but the clinical relevance of PDW is unclear and requires more research.

Abstract

Background: Plasmodium berghei is a rodent malaria parasite that infects mice and rats that mimics Plasmodium falciparum. It has a problem with standardized growing time and reaching high percentages of parasitemia, which allows optimum conditions in antimalarial research.Objective: This study aimed to analyze different outcomes from growing P. berghei ANKA (PbA) from frozen compared with fresh donors.Methods: The study involved 20 mice, with 10 infected using fresh and 10 using frozen P. berghei inocula. Blood smears were prepared daily for 7 days, fixed with methanol, stained with Giemsa, and examined every 24 hours at 1000× magnification. Parasitemia was calculated as infected erythrocytes per 1000 cells, recorded from initial parasite detection, and analyzed using a t-test.Results: Parasite growth in mice inoculated with fresh and frozen P. berghei blood was monitored through parasitemia levels over 7 days. In the fresh blood group, parasites appeared on day 2 (0.8%) and increased steadily to 11.3% by day 7. In contrast, parasites in the frozen blood group were first detected on day 3 at a low level (0.1%) and rose gradually to 5.0% on day 7.Conclusions:  In conclusion, this finding indicates the fresh blood contains more active and viable parasites, enabling them to rapidly replicate within the mice's bodies.

Abstract

Background: Bacterial infections are a major global health concern, including in Indonesia.  This has led to an increased effort to find new antibacterial compounds in natural resources. Bacillus sp. SA11, with reported antibacterial potential, was previously isolated from mangrove soil in Denpasar, Bali. This study further investigated its antibacterial activity.Methods: Secondary metabolites were extracted from Bacillus sp. SA11 liquid culture using ethyl acetate. The extract was then screened for antibacterial activity against Escherichia coli ATCC 25922, Streptococcus mutans FNCC 0405, Staphylococcus aureus ATCC 25923, and Klebsiella pneumoniae ATCC 70060 using the Kirby-Bauer method (three replicates). The chemical composition of the extract was analysed via GC-MS.Results: The ethyl acetate extract showed inhibition zones for E. coli, S. mutans, S. aureus, and K. pneumoniae of 11.7 ± 1.11 mm, 10.22 ± 1.5 mm, 8.7 ± 0.94 mm, and 7.2 ± 0.67 mm, respectively. GC/MS analysis identified 165 compounds, 20 of them have been  previously linked to antibacterial activities. The five most abundant compounds are n-hexadecanoic acid (6.91%), 2-butoxyethyl acetate (6.3%), Bis(2-ethylhexyl) phthalate (4.45%), Phthalic acid, di(2-propylpentyl) ester (4.45%) and Benzene, 1,3,5-trimethyl- (Mesitylene).Conclusion: These findings provide preliminary evidence that Bacillus sp. SA11, isolated from the mangrove ecosystem, has the potential to produce antibacterial compounds.

Abstract

Background: Leea indica, a large herbaceous plant traditionally used to treat diarrhea, has been studied for its medicinal potential. However, research on the antibacterial activity of its extract against Escherichia coli has not yet been widely reported.Objective: This study aimed to evaluate the antibacterial activity of Leea indica leaf extract against E. coli ATCC 25922 and to identify its phytochemical profile.Methods: Leea indica leaves were extracted using maceration with 96% ethanol as a solvent, followed by evaporation to obtain the crude extract. The antibacterial activity was tested using the well diffusion method at concentrations of 20%, 15%, 10%, and 5%. The inhibition zones were measured to determine antibacterial potency. Qualitative phytochemical screening was also performed to detect the presence of secondary metabolites.Results: The 20% concentration of Leea indica extract produced the largest inhibition zone (27.26 mm), categorized as a very strong antibacterial effect. Phytochemical analysis confirmed the presence of flavonoids, alkaloids, tannins, phenols, steroids, and saponins as secondary metabolites.Conclusions: This study demonstrates that Leea indica leaf extract exhibits strong antibacterial activity against E. coli ATCC 25922 and contains secondary metabolites that may support its traditional use for treating diarrhea.

Abstract

Background: Maternal anemia remains a primary global health concern due to its association with adverse pregnancy outcomes and the increased risk of iron deficiency anemia in infants, which may impair neurodevelopment and growth. Although iron supplementation is the standard therapy, its gastrointestinal side effects often limit treatment adherence. Therefore, Brazilian Spinach (Althernanthera sissoo), a plant rich in iron, flavonoids, and antioxidants, offers potential as a natural alternative.Objective: This study aimed to evaluate the hematopoietic effects of the ethanolic extract of Alternanthera sissoo on erythrocyte count, hemoglobin concentration, and hematocrit levels in female Wistar rats (Rattus norvegicus) induced with anemia.Methods: A quasi-experimental study using a completely randomized design (CRD) was conducted with three treatment doses (5, 7.5, and 10 mg/kg body weight). Hematological parameters (Erythrocyte count, Hemoglobin (Hb), and Hematocrit (HCT)) were analyzed using a Hematology Analyzer, followed by statistical testing with one-way ANOVA and Duncan’s Multiple Range Test (DMRT) at a 5% significance level.Results: The 10 mg/kg BW treatment produced the most significant improvement in erythrocyte count, Hb, and HCT levels compared to the other groups.Conclusions: Ethanolic extract of Althernanthera sissoo effectively enhances hematological indices in iron-deficient rats, showing strong potential as a natural nutraceutical supplement for preventing anemia during pregnancy.

Abstract

Background: Staphylococcus aureus is a pathogenic bacterium that causes skin infections and has been reported to develop resistance to antibiotics. Lactic acid bacteria, which can be isolated from tofu wastewater, have the potential to serve as natural antimicrobial agents through the production of bacteriocins, organic acids, hydrogen peroxide, and carbon dioxide. Objective: To evaluate the antibacterial activity of lactic acid bacteria isolates from tofu wastewater against Staphylococcus aureus. Methods: This descriptive analytic study used purposive sampling. Tofu wastewater samples were collected from 20 tofu factories in Klungkung District. Lactic acid bacteria were isolated on de Man, Rogosa, and Sharpe media, identified through catalase test, Gram staining, and fermentation type test, and tested for antibacterial activity against Staphylococcus aureus using the well diffusion method. Results: A total of 22 isolates of lactic acid bacteria were obtained, characterized as catalase-negative, Gram-positive, with bacilli and cocci morphology, and homofermentative properties. All isolates exhibited antibacterial activity against Staphylococcus aureus with an average inhibition zone diameter of 8.97 mm, categorized as moderate to strong. Conclusions: Lactic acid bacteria isolated from tofu wastewater demonstrated antibacterial activity against Staphylococcus aureus and have potential as natural antimicrobial agents.

Abstract

Background: The advent of mRNA vaccines has underscored the significance of nucleoside modifications in regulating innate immune recognition. Pseudouridine (Ψ) and its derivative, N1-methylpseudouridine (m1Ψ), are widely used to enhance the efficacy and safety of mRNA vaccines.Objective: This review examines how Ψ and m1Ψ alter the activation of key innate immune sensors, including RIG-I, MDA5, and endosomal Toll-like receptors (TLR3, TLR7, TLR8).Methods: A narrative review was conducted using PubMed, Scopus, and Web of Science databases, focusing on studies evaluating the molecular and immunological effects of Ψ and m¹Ψ on innate immune sensors.Discussion: Incorporation of Ψ or m1Ψ in mRNA markedly suppresses activation of TLR3, TLR7, and TLR8. Ψ-containing RNA avoids detection by TLR7/8 via two mechanisms: it resists endosomal nuclease digestion into immunostimulatory fragments and is poorly recognized by the TLR7/8 ligand-binding sites. m1Ψ similarly evades nuclease processing yet, unlike Ψ, can directly activate TLR8. In the cytosol, Ψ/m1Ψ modifications strongly reduce RIG-I signaling without impeding MDA5. Conclusions: By limiting innate PRR activation, these modifications increase the translation and stability of mRNA vaccines while reducing inflammatory interferon responses. This immune evasion is crucial for the high efficacy and tolerability of current mRNA vaccines; however, a trade-off exists between minimizing reactogenicity and the adjuvant benefits of innate stimulation.

Abstract

Background: Dengue remains a significant health problem in Indonesia, including Batam. Because its symptoms resemble other infections, early laboratory testing is crucial. The non-structural protein one rapid diagnostic test (NS1 RDT) is widely used, but its accuracy depends on the day of illness, immune status, and viral serotype.Objective: To evaluate the diagnostic validity of NS1 RDT compared to RT-PCR, assessing the level of agreement between the two methods, and analysing the influence of clinical phase and serotype on the results of the examination.Methods: A retrospective analytic study used secondary data from 309 suspected dengue cases tested by NS1 RDT at primary care and confirmed by RT-PCR with serotyping at Batam Public Health Laboratory (2022–2024). Diagnostic performance was calculated from 2×2 tables; concordance was analysed with McNemar's test and Cohen's kappa.Results: The NS1 RDT showed 75.8% sensitivity, 81.2% specificity, 62.7% positive predictive value, 88.9% negative predictive value, and 79.6% overall accuracy, with moderate agreement with RT-PCR (κ = 0.54). NS1 positivity peaked on illness days 1–3 and varied by serotype, with the highest for DENV-3.Conclusions: The NS1 RDT showed good diagnostic validity with moderate agreement and sensitivity and high specificity in early dengue disease screening compared to RT-PCR, so the diagnostic performance of the NS1 RDT can be used in local epidemiological testing in Batam.

Abstract

Background: Nutmeg flesh (Myristica fragrans), known to contain flavonoid compounds with anti-inflammatory activity. Extract of nutmeg flesh can be formulated into nanoemulsion using the SNEDDS technique represents an innovative drug delivery method that enhances drug absorption in the body.Objective: This study aimed to determine the optimal formulation extract of nutmeg flesh (Myristica fragrans) nanoemulsion preparation using the SNEDDS technique as an anti-inflammatory.Methods: This research employs an experimental design, with data collection carried out through observation and measurement scales. Observations are conducted on the evaluation of the physical quality of the nanoemulsion formulation, as well as on the anti-inflammatory activity test using male white mice (Mus musculus).Results: The analytical findings confirmed the presence of flavonoids as secondary metabolites in nutmeg extract. The optimal formula consisted of VCO (11%), Tween 80 (73%), and PEG 400 (15%), with predicted values of viscosity at 347 mPas, transmittance at 97.8%, emulsification time of 5 seconds, and a desirability value of 1. The anti-inflammatory activity, based on the percentage of inhibition, showed a high result in the SNEDDS test group, with an inhibition rate of 90%.Conclusions: Our study revealed that the SNEDDS extract of nutmeg flesh contains secondary metabolites and exhibits strong anti-inflammatory activity.

Abstract

Background: Red ginger contains bioactive compounds such as gingerol and shogaol with known anti-inflammatory activity, making it a potential active ingredient for nanospray gel formulations. Objective: This study aimed to optimize a nano spray gel formulation containing fermented red ginger extract using the Simplex Lattice Design (SLD). Methods: An experimental laboratory study was performed. Formulation optimization was conducted using Design-Expert® version 13 with the Simplex Lattice Design (SLD) approach by varying the concentrations of Carbopol 940, Triethanolamine (TEA), and Tween 80. The responses evaluated were percent transmittance, pH, and viscosity. Anti-inflammatory activity was assessed in vivo in male mice using a posttest-only control group design. The treatment groups included sodium diclofenac as the positive control (KP), CMC-Na as the negative control (KN), 0.5% red ginger extract (EJM), 0.5% fermented red ginger extract (EFJM), and a nano spray gel containing 0.5% fermented red ginger extract (SNSG) Result: The optimized formulation achieved a desirability value of 0.948. The formulation exhibited a pH of 6.0 ± 0.06, transmittance of 99.1 ± 0.12%, and viscosity of 537.7 ± 2.08 mPa·s. Particle size analysis showed an average particle size of 14.35 nm and a PDI of 0.1012, confirming the successful development of a homogeneous nanodispersion system. The nano spray gel containing fermented red ginger extract demonstrated notable anti-inflammatory activity, with an AUC value of 0.0145 and an AIA of 80.67% after 1.5 hours of treatment, indicating effective inhibition of inflammation
