Abstract

Parijoto fruit is an herbal that is the potential to be used for the treatment of lowering blood glucose. The active compound in parijoto fruit can decrease glucose levels is flavonoids. The use of natural materials is limit because of their low solubility. It often failed in the clinical phase. The solution to this problem is that the parijoto fruit extract is formulated in the form of nanoparticles using chitosan and STPP as encapsulate materials. This study aims to determine the activity of nano chitosan parijoto fruit extract (NPFE) as a glucose-lowering agent by applying nanoparticle technology. PFE was made by ionic gelation reaction by mixing extract chitosan solution of 0.2% concentration, and NaTPP concentration of 0.1%. The characteristic of NPFE was particle size, PdI, and % transmittance. The effect of a glucose-lowering agent of NPFE tested by the Nelson Somogyi method. The particle size of the NPFE result is 269.3 nm, PdI value is 0.372, and the percentage of transmittance is 99.397%. The glucose-lowering effect of NPFE was more significant (EC50 <2 ppm ) than parijoto fruit extract (EC50 48,750 ppm) and quercetin (EC50 6,831 ppm). Nano chitosan parijoto fruit extract affects a glucose-lowering agent.

Abstract

Public health center X is one of the public health center with high outpatient loads in Malang City. On average, the daily amount of outpatients is 119. The high load of outpatient directly affects employee’s working load. One of the units considered susceptible to working load increase is the medical record unit at the registration department because they have to interact with all visiting patients. The study aimed to discover the need for medical record personnel at the outpatient registration department of public health center X using the Full-Time Equivalent (FTE) method. The study used a descriptive study design and a quantitative approach. The researchers utilized a total sampling technique with two medical record personnel at the outpatient registration department. The study instrument employed was the outpatient registration daily log of Public health center X. The study results show that the working load of medical record personnel at the BPJS patient registration department had an FTE index value of 2.24 > 1.28, categorized as overload. Meanwhile, medical record personnel at the non-BPJS patient registration department had an FTE index value of 0.96 < 0.99, categorized as underload. Therefore, it can be concluded that Public health center X requires two additional medical record personnel at the BPJS patient registration department, while the non-BPJS patient registration department did not require additional medical record personnel.

Abstract

Traditional medical plants are known to society long ago. Apart from easily obtained and inexpensive, it can cure diseases with few side effects than modern medicine. Papaya leaves were used not only because of contained various chemical compounds with pharmacological effects but also alkaloids, flavonoids, saponins, and tannins compounds indicated as anti-inflammatory. This study applied a quasi-experimental design to test the papaya leaves anti-inflammatory activity. The red blood cell stabilization method was employed because analogous to the lysosomal membrane affected the inflammatory process. Purposive sampling was used, creating papaya leaves extract of 50 ppm, 100 ppm, 200 ppm, 400 ppm, 600 ppm, and 800 ppm concentration, made 24 total samples by four times replication. Based on the red blood cell lysis inhibition, the anti-inflammatory activity was measured and was compared with the positive control (diclofenac sodium). The papaya methanol extract result showed the highest anti-inflammatory activity at 800 ppm of 74.29%. The most effective concentration was at 200 ppm of 62.19%. Tukey's test showed p1.000 ≥ 0.05, suggesting H0 was accepted. There was no difference between the anti-inflammatory activity of papaya leaf methanol extract and diclofenac sodium showing stabilization of red blood cell membranes, indicating papaya potentially as an anti-inflammatory.

Abstract

Babies born with Low Birth Weight (LBW) are more prone to experiencing malnutrition problems. Efforts to prevent malnutrition in infants and toddlers can be done through the Positive Deviance approach. Positive Deviance is an individual or group (family) based approach by identifying positive behaviors that allow them to find better solutions to their problems. The purpose of this study was to determine the experience of fulfilling the nutritional needs of infants with a history of LBW in Positive Deviance families. The research design employed in this study was qualitative with a phenomenological approach. The main informants were Positive Deviance mothers, and the supporting informants consisted of Positive Deviance families, non-Positive Deviance mothers, and village midwives. The sampling technique used was purposive sampling. Data collection was carried out through in-depth interviews and documentation studies. The results of this study reveal the experiences of families in fulfilling the nutritional needs of infants with a history of LBW including positive maternal behaviors. For example, Early Initiation of Breastfeeding (IMD), breastfeeding after childbirth, application of breastfeeding correct way to LBW babies, exclusive breastfeeding, complementary breastfeeding, and other efforts if the baby does not have an appetite. Thus, this study implies that positive habits of mothers and families can meet the nutritional needs of infants, especially in increasing the weight of LBW babies to achieve ideal body weight. Therefore, it is necessary to make efforts to make mothers and their Positive Deviance family role models in order to transmit positive habits to other families with babies in preventing malnutrition in infants with LBW history.

Abstract

Ocular hypertension (OH) is a risk factor in glaucoma and modifiable in its early stages. Goldmann applanation tonometer (GAT) is still the gold standard in measuring intraocular pressure (IOP) which is an important parameter in diagnosing and managing glaucoma. Several factors can affect the accuracy of the GAT, one of them is the thickness of the central cornea. This descriptive study was intended to describe the central corneal thickness (CCT) characteristics of the OH patients. The study was conducted at Eye Clinic - Sanglah General Hospital Denpasar which involved 32 patients with 46 eyes diagnosed with HO in 2018. The mean age of subjects was 43.98 (± 17.92), men had a larger proportion (68.1 %). Sixty-six per cent of patients were diagnosed with bilateral OH. In this study, the mean central corneal thickness in HO subjects was 573.81 ± 33.46 μm. The patients' median vertical cup to disc ratio at the time of diagnosis was 0.33, with a mean visual acuity of 0.85. The median value of IOP at the first time of examination was 24.47 mmHg.Central corneal thickness is known to have a positive correlation with IOP, thus affecting the accuracy of IOP measurements. The thicker central corneal thickness will lead to overestimation of the IOP and the thinner one will cause underestimation.
