Abstract

Background: Hypertension is a health problem that often occurs in society. Uncontrolled hypertension can trigger degenerative diseases such as heart disease and kidney failure. One of the therapies used to lower blood pressure is Ambon banana and sweet star fruit therapy. Subjects and Method: Research on elderly men suffering from hypertension in the Tarus Community Health Center working area used a Quasi-Experimental pretest–posttest with a Control Group research design. The number of samples in this study was 20 elderly men using a simple random sampling method. The dependent variable in this study was blood pressure, while the independent variable was the administration of Ambon banana juice, sweet star fruit juice, and a combination of Ambon banana juice and sweet star fruit juice. The measuring instrument used in this research is a questionnaire. The data analysis used in this research is Anova and Post hoc. Results: The average blood pressure in the elderly decreased after being given Ambon Banana Juice, Sweet Starfruit Juice, and a combination of Juices. The greatest reduction in blood pressure occurred in the group given Ambon banana juice (Mean= 140 mmHg; p=0.002), compared to sweet starfruit juice (Mean= 148 mmHg; p= 0.036), and these results were statistically significant. The juice combination also reduced the average blood pressure, but the results were not statistically significant (Mean= 156 mmHg; p= 0.454). Conclusion: The average blood pressure in the elderly decreased after being given Ambon Banana Juice, Sweet Starfruit Juice, and a combination of Juices. Abstract

Background: Musculoskeletal Disorders (MSDs) are a form of disorders that occurs in muscles, bones, joints, ligaments and tendons. Globally, Musculoskeletal Disorders are one of the most common health problems that occur in all ages and genders. Musculoskeletal pain is one of the most common problems in Musculoskeletal Disorders, which can cause various limitations in activity and participation. Musculoskeletal pain can occur in all ages and genders, including children and adolescents. Several studies show that musculoskeletal pain most commonly occurs in the vertebrae, trunk, neck, upper extremities and lower extremities. Musculoskeletal pain in MSDs can cause various negative impacts such as reduced learning motivation, academic failure and learning problems in student. The purpose of this study is to determine the effect of implementing a school exercise training program as a prevention of Musculoskeletal Disorders. Subjects and Method: This was  a quasi experiment with no control group. Total sample of 60 children was selected for this study. The dependent variables were spinal curve deformity, degree of forward head posture, and MSDs pain. The independent variable was school exercise program. Mean score before and after intervention were tested using Wilcoxon t test. Results: After implementing the school exercise program, spinal curve deformity (p=0.025), the degree of forward head posture (p=0.046), and MSDs pain (p=0.001) were all observed to decrease. Conclusion: Implementing exercise programs in schools can help prevent musculoskeletal disorders among school children.

Abstract

Background: The severity of COVID-19 can be evaluated by checking several routine hematological parameters. Routine hematology examinations have an important role in early detection, diagnosis, disease management and disease progression. This study aims to determine the relationship between hematological parameters and disease progression and severity in COVID-19 patients. Subjects and Method: This was a cross sectional study conducted at Royal Prima Hospital, Medan, North Sumatera, Indonesia, from January to February 2022. A total of 93 patients who had confirmed COVID-19 were selected using a simple random sampling technique according to the inclusion and exclusion criteria. The research was The dependent variables of this research were disease progression and severity of COVID-19 patients. The independent variables are hematological parameters based on hospital medical record data. Data analysis used the Chi-Square test and logistic regression. Results: 75 people (80.6%) COVID-19 sufferers had severe disease. NLR and D-dimer levels were significantly related to disease progression in COVID-19 sufferers (p<0.05). Leukocyte, lymphocyte, NLR, CRP, D-dimer levels are related to the severity of COVID-19 sufferers (p<0.05). Leukocyte levels have the strongest relationship with the severity of COVID-19 with an OR value of 1.798. Conclusion: NLR and D-dimer levels are significantly related to disease progression and leukocyte. Lymphocyte, NLR, CRP, D-dimer levels are related to the severity of COVID-19 sufferers.

Abstract

Background: Office bleaching is a treatment used to treat tooth discoloration, but it often results in tooth sensitivity. The desensitizing agent potassium nitrate and casein phosphopeptide amorphous calcium phosphate (CPP-ACP) can reduce tooth sensitivity because they work to inhibit nerve transmission, thereby preventing the sensation of pain. This study aimed to investigate the effect of potassium nitrate desensitization agents and CPP-ACP on tooth sensitivity after bleaching treatment.Subjects and Method: This was a meta-analysis with the following PICO format. Population= Patients who have undergone office bleaching treatment. Intervention= desensitizing agents potas-sium nitrate and CPP-ACP. Comparison: without desensitizing agent. Outcome: tooth sensitivity. Keywords used "Office bleaching" AND "Tooth sensitivity" OR "Dentine hypersensitivity" AND "Potassium nitrate" AND Casein Phosphopeptide Amorphous Calcium Phosphate" OR "CPP-ACP" AND "Randomized Controlled Trial" OR “RCT”. Studies published at PubMed, Springerlink, Cochrane Database, Hindawi, Elsevier, Google Scholar and Wiley Online Library databases, from 2013 to 2023. The selected articles were critically reviewed using the PRISMA checklist and analyzed using the Review Manager 5.4.1. The effect size used is the Standardized Mean Difference (SMD).Results: A total of 13 RCTs from Brazil, Spain, India, Pakistan, Turkey, Jordan, Iran, and the United States was selected for meta-analysis. Meta-analysis of 7 articles shows that the use of potassium nitrate desensitization agents can reduce tooth sensitivity by 0.20 units lower than without the use of desensitization agents (SMD= - 0.20; 95% CI= -0.37 to -0.03; p=0.020). Meanwhile, 7 articles show that the use of CPP-ACP desensitization agents can reduce tooth sensitivity by 0.46 units lower than without the use of desensitization agents (SMD= -0.46; 95% CI= -0.67 to - 0.24; p<0.001).Conclusion: The use of potassium nitrate desensitization agents and CPP-ACP reduces tooth sensitivity compared to without the use of potassium nitrate desensitization agents and CPP-ACP and is statistically significant. Abstract

Background: Medical students who get community-involved medical education will be better equipped to address the health needs of their communities by integrating public health services into their practice. The importance of public health disciplines for physicians needs to be reinforced in the medical curriculum. Many studies, findings, and discussions report on the importance of this integration. This study aims to identify, decipher, and map the available scientific evidence regarding the importance of public health integration in medical education. Subjects and Method: This was a scoping review study using PUBMED, ProQuest, SCOPUS, Google Scholar, and other related sources. The keywords used in article searches are as follows: "public health" AND "medical education" OR "support" OR "against" OR "proportion" OR "role". From the database used, 1,775 articles were obtained and those who met the criteria were 45 articles. Results: The results were presented in three sub-discussions, namely role, perception, and form of integration. 1) Role: doctors are able to facilitate individual health and address public health problems through health promotion, health policy, evidence-based medicine practices, improving the quality of health data and health services; 2) Perception: Both positive and negative percep­tions were reported in these integration-related findings; and 3) Form of integration: public health collaboration in medical education is carried out at the academic and professional stages of doctors. Conclusion: The importance of integrating public health principles into medical education is to encourage the formation of doctors who act as doctors in the curative department as well as public health experts who understand community problems and can address them holistically and comprehensively. Abstract

Background: Neck pain is a musculoskeletal disorder that occurs in the neck due to various factors such as incorrect ergonomic position when carrying out an activity. One intervention that can be carried out to improve the function of neck pain sufferers is by providing manual therapy. This study aims to determine the effectiveness of manual therapy for functional use in patients with neck pain. Subjects and Method: This was a meta-analysis study using PICO as follows, P=Patients with neck pain, I=manual therapy, C=non-manual therapy, O=functional improvement. The articles used in this research were obtained from several databases such as Google Scholar, NCBI, PubMed, Springer Link, Science Direct. The keywords used are "Randomized control trial" AND "neck pain" OR "cervical pain" OR "Cervicalgia" Upper cervical pain" AND "manual therapy" OR "Manipulation treatment" OR "thoracic manipulation in neck pain" OR "Cervical spine manipulation” OR “Passive therapy” OR “Ostheopathic” OR “neck pain rehabilitation” AND “Disability” OR “Functional” OR “Neck pain disability index”. The articles included in this research are full text articles with the study design used being a Randomized Controlled Trial. The full text review application uses PRISMA flow diagrams. Results: A total of 8 articles that have been analyzed come from Spain, the Netherlands, Minen­sota, Canada, Australia and Turkey. This study shows that there is an effect on neck pain sufferers who are given manual therapy intervention - 0.08 units of influence compared to non-manual therapy, this effect is not statistically significant on function and this effect has a statistically significant effect (SMD = - 0.08; CI 95% CI= -0.35 to 0.20; p= 0.590). Conclusion: The manual therapy has an impact on mobile range of motion of the joint in neck pain patients compared to those without manual therapy.

Abstract

Background: Neck pain (NP) is a musculoskeletal public health problem that is often experienced, especially by modern society, with a high prevalence that can cause disability. Neck pain can be treated with various physiotherapy modalities, one of which is manual therapy/manipulation of the cervical spine. This study aims to determine the effectiveness of manual therapy in reducing pain in neck pain patients. Subjects and Method: This research is a systematic review and meta-analysis study, using PICO as follows, P= neck pain patients, I= manual therapy, C= other than manual therapy, O= reduction in pain. The articles included in this research were articles taken from several databases including Google Scholar, PubMed, NCBI, Science Direct, Embase, and Springer Link between 2010 and 2023. The keywords used to search for articles were: "neck pain" OR " chronic neck pain” OR “cervical pain” OR “cervicalgia” OR “upper cervical pain” OR “nonspecific neck pain” OR “nonspecific chronic neck pain” AND “manual therapy” OR “mobilization” OR “musculoskeletal manipulation” OR “cervical manipulation ” AND “RCT” OR “randomized controlled trial” OR “randomized controlled trial”. This research analysis was carried out using the RevMan 5.3 application. The results of the meta-analysis were reported using PRISMA flow diagrams. Results: A total of 9 articles have been analyzed originating from Spain, Canada, Germany, Turkey, and Pakistan. The study showed that patients with neck pain who received manual therapy experienced -2.01 units lower pain than those who did not receive manual therapy (SMD=-2.01; 95%CI=-3.00 to -1.03; p=0.001). Conclusion: Manual therapy significantly reduces neck pain compared to usual care.

Abstract

Background: Tuberculosis infections are endemic diseases in Asian countries. Although the incidence is rare, tuberculous spondylitis manifests as a severe and life-threatening disease. This study aims to correlate the abnormal result of erythrocyte sedimentation rate (ESR) and magnetic resonance (MR) imaging findings. Subjects and Method: MR imaging of 60 patients with characteristics of spinal tuberculosis in 4-year 5 month period (January 2019 – May 2023) from Siloam Lippo Village is retrospectively analyzed and reviewed. Data were collected from Infinitt PACS and analyzed by SPSS. Dependent variables in this study are clinical suspicion for tuberculosis infection, erythrocyte sedimentation rate (ESR), and tuberculosis infection in different organs. Meanwhile, the independent variables in this study are multilevel vertebral involvement, multilevel disc involvement, abscess formation, and myelopathy. Results: Association with elevated ESR (erytrocyte sedymentation rate) are seen in 11 out of 31 patients aged >40 (OR=0.45; CI 95%= 0.16 to 1.26; p=0.120), 19 out of 37 patients with clinically suspected for tuberculosis infection (OR=1.98; CI 95% 0.68 to 5.78; p=0.210); 27 out of 55 patients with multilevel vertebral body involvement (OR=0.8; CI 95%= 0.12 to 5.17); p=1); 13 out of 30 patients with intervertebral disc involvement (OR=0.87; CI 95% 0.32 –to2.42); p=0.8); 20 out of 50 patients with abscess formation(OR=0.29; CI 95%=0.66 to 1.24; p=0.08); 5 out of 12 patients with tuberculosis infection on other organs(OR=0.84; CI 95%=0.24 – 3.04; p=0.8); and 5 out of 6 patients with myelopathy(OR=7.3; CI95%=0.79 TO 66.6; p=0.04). Conclusion: MR has an important role in detecting extrapulmonary tuberculosis lesions, especially in the spine. Elevated ESR results play important roles for physicians in identifying patients with the possibility of spondylitis TB.

Abstract

Background: Cerebral palsy is a disorder that affects the movement and posture of the brain of a developing fetus or baby. The prevalence is around 1-4 per 1000 live births globally, with 9 birth cases in Indonesia. CP children experience various sensory and motor disorders. Various interventions require a tool that can reliably detect the success of therapy. Electromyography (EMG) is an objective method for understanding muscle activity, helps in determining the muscles involved in movement, and provides related information about muscle activity in response to therapy. This study aimed to determine EMG in evaluating the results of therapy. Subjects and Method: A literature search was carried out using 2 databases, namely PubMed and Google Scholar regarding the use of EMG to assess or analyze physiotherapy interventions given to children with CP from the period 2018 - 2023. Using PRISMA as a writing rule. Results: 8 studies described the use of EMG in physiotherapy, 3 articles looked at muscle activity, 3 articles assessed muscle activation, 1 article assessed spastic responses, and 1 article looked at motor neuron excitatory pools. Conclusion: EMG helps physiotherapy in seeing muscle activity, muscle activation, seeing the spastic response of a muscle, and measuring the collection of excitatory motor neurons.

Abstract

Background: Gastric cancer is the third highest cancer mortality globally. This malignancy can result in emergency complications such as gastric perforation which results in pneumo­peritoneum. Currently, there are not many case reports which describe pneumoperitoneum in gastric cancer. In this report, we describe a gastric adenocarcinoma with pneumoperitoneum following jejunostomy feeding. Case Presentation: A 54 years old male presented with abdominal pain, black-colored feces, and body weight decrease as much as 22 kg within a month. The physical examination revealed anemic conjunctiva, distended abdomen with abdominal pain, and pale skin. On the lab examination, the hemoglobin level was 9.9 mg/dL. The peripheral blood smear showed signs of iron deficiency anemia caused by chronic process. An endoscopic examination and biopsy showed a gastric mass with well-moderately differentiated gastric adenocarcinoma. The three positions abdominal x-ray incidentally found that the patient had pneumoperitoneum. Results: Patient was diagnosed with Adenocarcinoma Gaster based on a gastric mass biopsy with hematoxylin-eosin staining. Then, the patient was treated with jejunostomy feeding. Thoracic and 3-position abdominal X-ray examinations revealed incidental findings of pneumoperitoneum and small bowel obstruction. Conclusion: Jejunostomy feeding in gastric cancer patient may result in pneumoperitoneum as a complication. Gastric adenocarcinoma followed by iron deficiency anemia from chronic process post jejunostomy feeding was found with pneumoperitoneum complication.
