Abstract

This study aimed to synthesize and evaluate the quantitative structure–activity relationship (QSAR) of a series of metronidazole–piperazine derivatives as potential antigiardiasis candidates. Electron-withdrawing (EWG) and electron-donating (EDG) substituents were modified on the piperazine nitrogen group to observe their effects on biological activity in silico. Descriptor analysis was performed using 12 physical, electronic, and topological parameters of the molecule. QSAR modeling was performed using multiple linear regression (MLR) using the Leave One Out (LOO) method. Model validation showed a Q² value of 0.9768, R = 0.99, and a low RMSEC, indicating the model is stable and suitable for use as a predictor. The final HKSA equation obtained is Y’ (log 1/C) = 15.620779 + 0.00125(chi1v) + 0.002787(dipole) + 32.525224(glob) + (-3.4999394) (mr)+ (-0.001002) (logP(o/w)) + 4.878564(PEOE_PC+) + (-1.619287) (PEOE_PC-) + 0.000150(pmi) + (-0.046015) (TPSA) + 0.000125 (Zagreb) + 1.025935(AM1_HOMO) + -0.907413(AM1_LUMO). Molecular docking results against the ferredoxin oxidoreductase receptor indicated that 2a was the ligand with the best binding affinity, thus predicting it as the most potential candidate for antigiardiasis compared to other compounds. Abstract

Abstract The stems of bird's eye chili (Capsicum frutescens L.) and curly red chili (Capsicum annuum L.), which are typically considered waste, actually hold compounds with potential for biological activity. Phytochemical screening has shown that stems of bird’s eye chili contain secondary metabolites such as alkaloids, phenolics, tannins, flavonoids, terpenoids, and saponins, while curly red chili stems contain alkaloids, steroids, phenolics, tannins, flavonoids, and saponins. The objective of this research to evaluate the effectiveness and determine the optimal concentration of both extracts as bioinsecticides against Aedes aegypti mosquitoes. Extraction was performed by maceration using 96% ethanol (ratio simplicial:solvent=1:10). The yield of bird’s eye chili extract was 7.45%, while curly red chili extract of 8.06%. The experiment consisted of five treatment groups with extract concentrations of 1%, 3%, and 5%; transfluthrin as a positive control; and distilled water as a negative control. Each group contained 25 Aedes aegypti mosquitoes. Mortality result indicated that both extracts had bioinsecticidal activity, although their effectiveness varied at different concentrations. The stems of bird’s eye chili extract showed the greatest effect at 5% concentration, with an LC₅₀ value of 4,177% at the 4th hour, while the curly red chili stems extract was most effective at 5% concentration, with an LC₅₀ value of 4,048% at the 5th hour. In conclusion, chili stem waste extracts demonstrate promising potential as effective and environmentally friendly botanical bioinsecticides for controlling Aedes aegypti. Abstract

Drug management in pharmacies is crucial to ensure the availability of appropriate medicines for the public. This study aimed to determine the procurement, storage, and use of NSAID analgesic drugs at Rizky Peni Farma Pharmacy in Plesungan in 2024. The research method used was a descriptive study with qualitative and quantitative approaches conducted retrospectively. Data were analyzed by comparing the results of observations with the Regulation of the Minister of Health of the Republic of Indonesia Number 73 of 2016 concerning Pharmaceutical Service Standards in Pharmacies. The results showed that drug procurement was carried out through Pharmaceutical Wholesalers (PBF) following appropriate procedures. Drug storage was conducted using the FIFO (First In First Out) and FEFO (First Expired First Out) systems. The highest use of NSAID analgesic drugs was found in tablet form, both generic (6,780 tablets) and branded (11,300 tablets). The most commonly used active ingredient was mefenamic acid, both in generic (2,020 preparations) and branded (4,510 preparations) forms. The most common generic NSAID analgesic was oxicam (2,880 preparations), while the most commonly used branded analgesic was mefenamate (4,510 preparations). The conclusion of this study indicates that medication management in pharmacies is in accordance with pharmaceutical service standards. Abstract

Hypertensive crisis imposes a substantial economic burden on healthcare systems. In Indonesia, rising service costs necessitate health economic evaluations to optimize hospital management. This study aims to analyze the direct costs of hypertensive crisis patients at RSUP Dr. Sardjito, Yogyakarta. A descriptive observational study with a retrospective cohort design was conducted on hypertensive crisis patients from January 2018 to July 2022. A total of 37 subjects were selected using a purposive sampling technique. Patients were divided into Calcium Channel Blocker (CCB) therapy groups (n=31) and non-CCB therapy groups (n=6). Cost components analyzed included medication costs, accommodation, inpatient / ER care, and radiodiagnostics. Statistical analysis used an independent sample t-test. The average total direct cost was IDR 3,282,080. The largest cost component for all subjects was inpatient/ER care costs of IDR 1,154,261 (35.17%). The average total cost in the CCB group was IDR 3,068,395, while in the non-CCB group it was IDR 4,386,119. Despite the cost difference, statistical tests showed no significant difference between the two therapy groups (p = 0.127). There was no significant difference in costs between the use of CCB and non-CCB therapy. These results can be used as a reference in budget planning and management of hypertensive crisis therapy in hospitals. Keywords:  CCB, Direct costs, Hypertensive crisis, Pharmacoeconomics

Abstract

Abstract Pharmaceutical wholesalers (PBF) play a strategic role in the drug distribution chain, ensuring the safety, quality, and authenticity of medicines circulating in society. Compliance with Good Drug Distribution Practices (GDDP) is a legal obligation regulated by the Head of BPOM Regulation. This study aims to analyze the factors influencing GDDP implementation compliance among pharmaceutical wholesalers in Cirebon City. An observational analytic method with a cross-sectional approach was employed using a structured questionnaire distributed to 52 respondents from 12 PBF in Cirebon City. Variables studied included human resources, facilities and infrastructure, documentation systems, management commitment, and regulatory oversight. Data were analyzed using Chi-Square test (α = 0.05) followed by logistic regression for multivariate analysis. The results indicate that all variables had a significant relationship with GDDP compliance (p < 0.05). Human resource competency was identified as the most dominant factor. This study recommends strengthening continuous training for PBF personnel and increasing the intensity of regulatory supervision to improve GDDP compliance. Abstract

Abstract Skin inflammation is a common health problem requiring effective management through safe and stable topical formulations. This study aimed to formulate and evaluate the physical characteristics of a topical gel containing a combination of Aloe vera and menthol as a potential anti-inflammatory agent. The gel was prepared using xanthan gum as the base with varying concentrations of Aloe vera powder 200x (0.25%, 0.5%, and 0.75%) and a fixed concentration of menthol (0.5%). The formulations were evaluated for organoleptic properties, homogeneity, pH, spreadability, and stability. The results showed that all formulations exhibited good physical characteristics, were homogeneous, had pH values within the physiological skin range, and demonstrated acceptable spreadability. Statistical analysis indicated that variation in Aloe vera concentration had no significant effect on pH but significantly affected spreadability. Stability testing revealed that the formulations remained relatively stable, although slight color changes occurred at elevated temperatures. These findings suggest that the Aloe vera–menthol gel has potential as a natural-based topical formulation. Keywords: Aloe vera, formulation, gel, menthol, stability

Abstract

Abstract Free radicals can cause skin cell damage and accelerate premature aging; therefore, natural antioxidants are needed and can be developed into topical cosmetic preparations. Bottle gourd leaves (Lagenaria siceraria (Molina) Standl.) are known to contain secondary metabolites with potential antioxidant activity. This study aimed to formulate bottle gourd leaf extract into a body scrub, evaluate its physical quality, and determine its antioxidant activity using the ABTS method in vitro. This experimental study included plant identification, phytochemical screening, and the formulation of a body scrub with extract concentrations of 3%, 5%, and 7%. The results showed an extract yield of 17.31% and the presence of alkaloids, phenolics, flavonoids, tannins, saponins, and triterpenoids. Physical evaluation indicated that all formulations met the requirements of semisolid preparations, with pH values of 6.97–7.37, viscosity of 2646–9596 cPs, spreadability of approximately 5 cm, and good stability. Antioxidant activity testing showed that the positive control, ascorbic acid, had an IC₅₀ value of 2.95 ppm, while the extract and body scrub did not exhibit IC₅₀ values at the tested concentrations. This may be due to low levels of active compounds or interactions with the formulation matrix; therefore, further studies using higher concentrations or different methods are recommended. Abstract

Abstract This study investigates the use of a 96% ethanol extract of palm mushroom (Volvariella volvacea) as a source of vitamin D₂ in a capsule dosage form using the wet granulation method to improve stability. The aim was to formulate and evaluate granule and capsule characteristics in accordance with pharmacopeial standards. This experimental study used three formulations (F0, F1, and F2). Evaluations included organoleptic properties, moisture content, flow rate, angle of repose, weight uniformity, disintegration time, and vitamin D₂ content. The extract yield was approximately 10%. Granules met the requirements for moisture content and angle of repose, but flow rate decreased with higher extract concentration. Capsules met the requirements for weight uniformity and disintegration time (3–8 minutes). The highest vitamin D₂ content was found in F2, while F1 showed the best balance between physical properties and active content. In conclusion, wet granulation produced capsules that met quality requirements, with F1 as the optimal formulation. Keywords:  Vitamin D2, Palm mushroom, Wet granulation, Capsules, Formulation

Abstract

Drug management is an important aspect and a focus in the Pharmacy due to its relation to the drug supply in hospitals. Having a good drug management system is an effort to enhance health services in the community, particularly in pharmaceutical services. However, in practice, managing medications is not an easy task and many issues arise. Pharmaceutical services are divided into two categories: clinical pharmacy services and management of pharmaceutical preparations. The management of preparations begins with selection, planning, procurement, storage, distribution, control, recording and reporting, as well as destruction, which is carried out in pharmaceutical facilities such as pharmacies, community health centers, clinics, and hospitals. This research aims to review the effectiveness of the Hanlon method in determining priorities for medication management issues in hospitals. The research method employed involves reviewing several sources including journals and articles into a narrative review format. The results of this study indicate that the Hanlon method is effective to prioritize medication management issues found in hospitals. Abstract

Bleeding complications are a significant contributor to hospitalization in cardiovascular patients on warfarin therapy; however, the contributing factors remain incompletely characterized. This study aimed to describe herbal use patterns, evaluate clinical outcomes (average length of stay/ALOS, rehospitalization, ICU transfer, and mortality), and examine bivariate associations between potential risk factors and bleeding events in hospitalized cardiovascular patients receiving warfarin at RSUD Kabupaten Tangerang. Data were analyzed using JAMOVI version 2.6.2, encompassing univariate and bivariate analyses. A total of 132 patients were included: 44 (33.3%) with bleeding and 88 (66.7%) without. The most frequently consumed herbs were garlic (28.95%), ginger (23.68%), and turmeric (21.05%). Clinically, bleeding was associated with a mean LOS of 3.70 days, rehospitalization rate of 22.7%, ICU transfer of 2.3%, and mortality of 2.3%. Chi-square analysis identified significant associations with herb use (p<0.001; OR=460), INR ≥3 (p<0.001; OR=8.33), and hypofibrinogenemia (p<0.001; OR=179). Herb use, INR ≥3  and hypofibrinogenemia have a significant association with bleeding events in cardiovascular patients undergoing warfarin therapy.
