Abstract

Abstract Pharmacoeconomic analysis has several methods, one of which is cost effectiveness analysis (CEA).  Cost effectiveness analysis is a pharmacoeconomic method to select and evaluate the most appropriate program or drug among treatment options that have the same goal. This method is done to find out which of the two treatments is the most cost-effective, besides that CEA is also useful to find out which treatment is more cost-effective than the two treatment alternatives chosen, especially in cesarean section patients. This research is a type of non-experimental observational research with a retrospective descriptive research design. The sampling technique in this study uses the puRp.osive sampling technique method, sample selection is carried out based on consideration of inclusion and exclusion criteria. The sample of this study was 28 patients consisting of 15 patients who used the prophylactic antibiotic ceftriaxone and 13 patients who used the prophylactic antibiotic seflotaxis. The results of the study showed that ceftriaxone antibiotic therapy is more cost-effective, with an Average Cost-Effectiveness Ratio (ACER) of Rp89.590 per day Keywords: Cost-effectiveness analysis, cesarean section patients, ceftriaxone, cefotaxime

Abstract

Mental disorders are the result of a long and complex psychosocial development process, influenced by family factors, the environment, and individual social conditions. Data from the 2007-2018 Riskesdas and the 2023 Indonesian Health Survey (IHS) show fluctuations in the prevalence of mental disorders in Indonesia, including in Lampung Province. Schizophrenia as a severe mental disorder and mental emotional disorders such as stress, anxiety, and depression are still major problems. Major challenges also arise in the form of the widespread practice of shackling and low access to mental health services. Although the coverage of administrative services is quite high, the quality of services and the success of long-term rehabilitation are not optimal. Low public literacy, lack of health resources, and limited facilities are significant obstacles. The integration of the Mental Health Law into the Health Law No. 17 of 2023 is expected to become a legal umbrella that strengthens an inclusive and sustainable mental health service system. Handling mental disorders needs to involve a cross-sectoral approach that is not only curative, but also promotive and preventive, to ensure the recovery of social functions and the overall quality of life of patients. Abstract

Abstract Katuk hutan leaf (Breynia cernua) are endemic plants of Papua traditionally used to treat cancer, smallpox and wounds. It contains various bioactive compounds such as alkaloids, flavonoids, terpenoids and tannins. Fractionation using n-hexane, ethyl acetate and ethanol-water solvents was performed to isolate specific bioactive components. This study aims to evaluate the antibacterial activity of n-hexane fraction, ethyl acetate fraction and ethanol-water fraction of Breynia cernua leaf against Staphylococcus aureus and Escherichia coli. Antibacterial testing was conducted using the disc diffusion method at concentrations of 250, 500 and 750 ppm. All fractions exhibited inhibition zones in the moderate to strong category against both bacteria. The highest antibacterial activity was observed in the n-hexane fraction at 750 ppm with inhibition zone of 10.2 ± 0.8 mm for S. aureus and 10.3 ± 0.8 mm for E. coli. ANOVA analysis showed a significant difference (p < 0.05) between the n-hexane fraction and the other fractions. These results indicate that the n-hexane fraction of Breynia cernua leaves has the most potent antibacterial effect. Abstract

Pneumonia is an acute infectious disease that affects lung tissue and is caused by bacteria, fungi, viruses, and parasites, leading to inflammation of the lung parenchyma and accumulation of inflammation in the respiratory tract. In Lampung Province, there was an increase in pneumonia case detection coverage, from 39.8% in 2020 to 40.6% in 2021. At RSUD Ahmad Yani in Metro City, Lampung, pneumonia ranks among the top 10 most common diseases. The purpose of this study was to evaluate the rationality and appropriateness of antibiotic use according to treatment guidelines in pneumonia patients. This study employed an observational descriptive-analytic design with a cross-sectional approach and data collection from patient medical records using purposive sampling. The samples consisted of inpatient records diagnosed with pneumonia infection at RSUD Ahmad Yani in Metro City during January–December 2023. Data were obtained retrospectively through medical record reviews of inpatients. Quantitative analysis of antibiotic use was conducted using the Anatomical Therapeutic Chemical/Defined Daily Dose (ATC/DDD) method, while qualitative analysis utilized the Gyssens algorithm. Statistical calculations were performed using SPSS software with Spearman’s correlation test. In this study, from 190 antibiotic prescriptions, the ATC/DDD value was 138.18 DDD per 100 patient-days. The correlation between the rationality of antibiotic use and length of hospital stay showed a Spearman correlation coefficient of 0.42, indicating a moderate positive correlation; however, the significance value of 0.567 indicated that this relationship was not statistically significant.   Keywords : ATC/DDD, Gyssens, Length of Stay (LOS), Pneumonia, Hospital

Abstract

Laparotomy is a surgery performed to open the abdomen, a form of major surgery by cutting the layers of the abdominal wall, risking causing infection of the surgical wound where antibiotics are given as treatment. Antibiotic resistance can lead to increased morbidity, mortality and health costs, therefore there needs to be an analysis for the use of antibiotics. The analysis of the use of antibiotics was carried out in the ICU room of the General Ahmad Yani Metro Lampung Hospital with the aim of analyzing how to use antibiotics qualitatively and quantitatively. The method used is descriptive with retrospective data collection from the period of January 1 to December 31, 2023. The qualitative analysis of the use of antibiotics used the Gyssens method and quantitatively with the ATC/DDD du 90% method. The sample of this study consisted of 52 patients after laparotomy surgery in the ICU room of General Ahmad Yani Metro Hospital Lampung, where based on the inclusion and exclusion criteria determined. Based on the results of the analysis of antibiotic use with the Gyssens method, 83.8% used antibiotics non-adherence (category 0) and 16.2% used antibiotics adherence (categories VIA and V). Analysis of the use of antibiotics using the ATC/DDD method obtained that Seftriaxone had the highest DDD with DDD/100 patients – 76.4 DDD per day and those in the DU segment of 90% were Seftriaxone and Metronidazole.    Keywords: ATC/DDD du 90%, Gyssens, Laparotomy, Quantitative, Qualitative

Abstract

Infection is one of the most common causes of disease in society. One type of infection caused by Staphylococcus aureus is a bacterium that causes skin infections in humans characterized by purulent abscesses. In addition, Salmonella typhi is also a pathogenic bacterium that can cause typhoid fever. Infections caused by microorganisms can be overcome by administering antimicrobials. One of the natural ingredients that has the potential as an antimicrobial is the petai belalang plant (Archidendron clypearia). This study aims to test the antibacterial activity of ethanol extracts and fractions of petai belalang leaves against Staphylococcus aureus and Salmonella typhi bacteria. This experimental research includes plants determination, sampling, making simplisia, fractionation, phytochemical screening, and antibacterial activity tests. Extracts and fractions were prepared in concentrations of 1%, 5%, 10%, and 15%, with positive control of amoxicillin 0.1% and negative control of DMSO 1%. The antibacterial activity test was performed using disc diffusion and pitting diffusion methods to measure the zone of inhibition. Data were statistically analyzed using the Kruskal-Wallis test. The results showed that in the disc diffusion method, the inhibition zone against Staphylococcus aureus was 7.3-10.8 mm, and against Salmonella typhi was 7.6-10.1 mm. In the well diffusion method, the zone of inhibition against Staphylococcus aureus ranged from 12.1-16.8 mm, and against Salmonella typhi it ranged from 12.5-16.8 mm. The ethyl acetate fraction obtained the largest inhibition zone with a diameter of 16.8 mm in the well diffusion method. Statistical analysis showed a significant difference in antibacterial activity on positive and negative bacteria with a substantial value of 0.005 in the disc method and 0.006 in the well method. Abstract

Self-medication is an act of treatment to overcome minor complaints with drugs independently without a doctor's prescription. Proper self-medication can be done with sufficient knowledge. Meanwhile, lack of knowledge can lead to unexpected impacts. The purpose of this study was to determine the level of knowledge of self-medication of SMA Negeri 12 Makassar students. This type of research is descriptive quantitative using stratified random sampling technique. Determination of the number of samples was carried out using the slovin formula involving 10th, 11th and 12th grade students of SMA Negeri 12 Makassar. The research was conducted in March 2025. The results of the study from 93 students revealed that 78.49% of SMA Negeri 12 Makassar students used analgesics and antipyretics. Mild complaints are the reason for the majority of self-medication (78.49%). The results of the students' knowledge level were 58.1% with poor knowledge. This indicates that students do not have enough understanding to medicate themselves properly.   Keywords: Behavior, High School Student, Knowledge Level, Medication, Self-Medication

Abstract

Pengadaan obat di rumah sakit merupakan aspek penting dalam menjamin kelancaran pelayanan kesehatan kepada masyarakat. Dengan kemajuan teknologi dan regulasi pemerintah, pengadaan obat kini dilakukan secara menyeluruh melalui sistem e-Purchasing menggunakan e-Catalogue. Penelitian ini bertujuan mengevaluasi pengadaan obat e-Catalogue dan non e-Catalogue sebelum dan sesudah penerapan kebijakan e-Catalogue tahun 2023 di RSUD dr. Dradjat Prawiranegara Serang. Penelitian ini menggunakan pendekatan observasional analitik dengan data retrospektif tahun 2022-2023. Data obat e-Catalogue diperoleh dari LKPP dan obat non e-Catalogue dari SIMRS. Analisis dilakukan secara univariat dengan metode deskriptif yang ditampilkan pada gambar dan tabel. Hasil menunjukkan bahwa secara keseluruhan pengadaan obat melalui e-Catalogue pada tahun 2023 meningkat dan pengadaan non e-Catalogue menurun. Sedangkan pengadaan obat generik meningkat dari 66,62% menjadi 70,20% dan pengadaan obat non e-Catalogue nama dagang meningkat dari 51,81% menjadi 64,67%. Waktu tunggu pengadaan obat e-Catalogue pada tahun 2023 tercatat lebih lama, yaitu sekitar 103 hari, dibandingkan obat non e-Catalogue sekitar 17 hari. Namun, waktu tunggu obat e-Catalogue pada 2023 lebih cepat dibandingkan tahun 2022 yang mencapai 121 hari. Terdapat delapan penolakan obat (50% karena persediaan, 25% syarat minimum, 12,5% harga, 12,5% negosiasi). Kesimpulan pengadaan obat e-Catalogue dan non e-Catalogue periode 2022-2023 dipengaruhi oleh implementasi kebijakan pengadaan obat tahun 2023

Abstract

Pada tahun 2022, BPOM telah menemukan kandungan bahan berbahaya dalam kosmetik yang dapat menimbulkan bahaya terhadap kesehatan. Penemuan tersebut didominasi dengan bahan pewarna yang dilarang, yakni Merah K3 dan Merah K10. Alternatif dari pewarna sintetis dapat menggunakan pewarna alami. Salah satu pewarna alami yang dapat digunakan pada sediaan blush on stick ialah bubuk buah delima. Metabolit sekunder dari buah delima ialah antosianin. Warna merah dari buah delima berasal dari antosianin sianidin. Tujuan dari penelitian ini ialah untuk mendapatkan formulasi blush on stick powder buah delima (Punica granatum L.) dengan variasi konsetrasi 0%, 10%, 15%, dan 20%, serta melakukan evaluasi yang meliputi uji organoleptik, homogenitas, keretakan, daya poles, pH, dan iritasi. Penelitian ini merupakan penelitian eksperimental. Hasil penelitian yang diperoleh menunjukkan bahwa blush on stick powder buah delima memiliki peningkatan warna mulai dari putih kekuningan, merah muda keputihan, merah muda terang hingga merah muda gelap; berbau khas oleum rosae; dan memiliki bentuk yang solid. Semua konsentrasi memiliki susunan yang homogen, memiliki daya poles yang baik, memenuhi persyaratan pH, serta tidak adanya iritasi yang ditimbulkan pada 15 orang panelis. Pada uji keretakan, seluruh formula tidak memenuhi persyaratan keretakan, didapat hasil yang pecah pada tiap sediaan.

Abstract

Inflammation of the hair follicles and sebaceous glands causes acne, a common skin disorder often triggered by bacterial infections such as Propionibacterium acnes. This study aims to determine the antibacterial activity of an ethanolic extract of purple passion fruit seeds against Propionibacterium acnes. The extraction process was carried out by maceration using 96% ethanol. The antibacterial activity of the extract was tested using the disk diffusion method (Kirby-Bauer) at concentrations of 10%, 20%, and 30%, with positive control (clindamycin) and negative control (1% DMSO) for comparison. Antibacterial activity was measured based on the inhibition zone diameter. The data obtained were then statistically analyzed using a one-way ANOVA test with a 95% confidence level. The extract yield was 5.78% and contained chemical compounds such as flavonoids, alkaloids, tannins, saponins, and terpenoids. The antibacterial test results showed inhibition zones for the 10%, 20%, and 30% extract concentrations and the positive control (clindamycin) of (6.45 ± 1.86) mm (moderate activity), (10.38 ± 0.29) mm (strong activity), (12.21 ± 1.21) mm (strong activity), and (17.78 ± 0.55) mm, respectively. Statistically, there was a significant difference in antibacterial activity between the samples (p-value < 0.05). In conclusion, the ethanolic extract of passion fruit seeds possesses antibacterial properties effective against Propionibacterium acnes..   Keywords: antibacterial, seed, Passiflora edulis Sims, Kirby_bauer, acne
