Abstract

Background: Childhood and adolescent obesity is on the rise in Southeast Asia. Necessitating stronger actions from ASEAN governments to address this issue, because the significant financial and social welfare implications. Schools provide an effective platform for targeted interventions among children and adolescent groups. Objectives: This study provides an overview of school-based nutrition interventions implemented in ASEAN countries to address childhood and adolescent obesity. Methods: A literature review was conducted by searching multiple databases (Google Scholar, PubMed, ProQuest, ScienceDirect, and Web of Science) between April 1 and May 1, 2023. The search included computer-based searches and manual search in relevant journals. The articles considered were limited to publications from the past 10 years. Discussion: School-based nutrition interventions in ASEAN countries require further development, with attention to key factors such as intervention duration, sample size, and family involvement. Conclusions: Urgent large-scale research is needed to evaluate the effectiveness of existing school-based nutrition programs. Integrated and effective programs require support and collaboration from families, the private sector, and the government.

Abstract

Background: The recommended iodine intake for adult women is 150-300µg and for men <150µg, which functions in maintaining normal thyroid function. Hyperthyroid condition is characterized as increased synthesis or secretion of hormones by the thyroid gland. A family history of Graves' disease is the most frequently found risk factor, namely 60-80% worldwide. The prevalence of hyperthyroidism cases in Indonesia is around 6.8% Objectives: Comprehensively identifying risk factors and clinical manifestations of hyperthyroidism in case studies. Methods:  This research was carried out with a systematic review using two databases, namely Pubmed and Scopus. The literature search strategy was carried out using Convidence software by including predetermined inclusion and exclusion criteria. There were 11 selected pieces of literature published from January 2016 to October 2022 Discussion: A total of 11 case studies showed that women dominated cases of hyperthyroidism with a ratio of 3:1. Through a systematic review conducted by researchers, it shows that Graves' disease is the leading risk factor and clinical manifestations such as swelling of the thyroid gland, difficulty breathing, progressive weight gain, tremors, and heart palpitations are often found. Conclusion: Patients who are female and have a family history of Graves' disease are the most common risk factors found with clinical manifestations of swollen thyroid gland, difficulty breathing, progressive weight gain, tremors, and palpitations.

Abstract

Background: The knowledge, involvement, and self-confidence of nutrition professionals are highly required in the implementation of gene-based nutrition services. The limited number of nutrition professionals applying gene-based nutrition services poses a unique challenge in the developmental process of gene-based nutrition services in Indonesia. Objectives: This study aims to identify the association between the knowledge, engagement, and confidence levels of nutrition workers with gene-based nutrition services in Indonesia. Methods: This cross-sectional study was conducted in May-June 2023. The participants consisted of nutrition workers from five provinces in Indonesia. The instrument of this study used structured questionnaires encompassing socio-demographic information, knowledge, engagement, and confidence levels of nutrition workers distributed using the Qualtrics Survey platform. Data analysis employed the chi-square test with the help of Statistical Package for the Social Sciences (SPSS) version 25.0. Results: This study involved a total of 423 respondents. Findings indicated that 53.4% of nutrition workers possessed limited knowledge, 60.0% exhibited minimal engagement, and 51.8% held low confidence levels in gene-based nutrition services. Notably, a significant correlation was identified between knowledge and confidence in gene-based nutrition services (p=0.001; PR=2.0; CI=1.35-2.94). Additionally, a significant association was present between knowledge and engagement in gene-based nutrition services (p=0.001; PR=2.00; CI=1.34-2.96). Conclusions: This study highlights the association of low nutritionists' knowledge with low personal involvement and confidence levels to gene-based nutrition services in Indonesia. Nutrition professionals are encouraged to enhance their grasp of genetics and its relevance to nutrition through dedicated training, participation in seminars, and ongoing education initiatives.

Abstract

Background: Individuals possess unique genes that influence their susceptibility to diseases and their response to dietary interventions. The emergence of nutritional genomics has made it possible to identify disease predispositions and mitigate risks through gene-based dietary guidance. Consequently, understanding gene-based nutrition services is crucial for nutritionists, as it facilitates the integration of genetic insights into dietary recommendations. Objectives: This study aims to evaluate the familiarity of dietitians in Indonesia with gene-based nutrition services. Methods: Employing a qualitative approach with a phenomenological orientation, this study was conducted in DKI Jakarta, Jawa Barat, Jawa Tengah, Jawa Timur, and DI Yogyakarta. Purposive sampling was employed, involving a total of seven nutritionists. Data were gathered through in-depth interviews conducted both in person and online via Zoom meetings. Thematic analysis was carried out utilizing Nvivo 12.0. Results: The knowledge of nutritionists regarding nutritional genomics is limited. While they could explain fundamental concepts, their understanding lacked depth. Similarly, their comprehension of gene-based nutrition services was restricted, leading to uncertainties about their role in such services. Nutritionists generally expressed positive attitudes towards gene-based nutrition services, recognizing their significance and advocating for their provision by relevant agencies. There was a strong interest among nutritionists in offering gene-based nutrition services and a willingness to expand their understanding of nutritional genomics. Conclusions: The knowledge of nutritionists regarding gene-based nutrition services remains insufficient. There is a need for nutritionists to undergo training, attend seminars, and enhance their understanding of nutritional genomics in academic settings.

Abstract

Background: Yemen is one of the low-income countries, with 80% of the population living below the poverty line and almost half the population lacks adequate access to nutritious food. The low-birth-weight rate is 24.6%, and the overall infant mortality rate is 47 per 1,000 births. The risk of death of newborns varies depending on social, demographic, and economic factors.  Objectives: This study aims to evaluate the effect of the ongoing war on stunting prevalence among children under five years in Al-Rajaiya Health Center, Ash-Shamayatayn District - Taiz, Yemen. Methods: This study is an observational analytic with a cross-sectional design. It was conducted from January to June 2022, following purposive sampling gathered 254 participants. Results: The data revealed that the prevalence of severe stunting in 6- to 23-month-old children was higher in urban areas than in rural areas (45% vs. 43.7%); it was also higher in girls than in boys (58.4% vs. 30.3%). Furthermore, in 24- to 59-month-old children, it was higher in urban areas than in rural areas (50% vs. 12.5%) and higher in girls than in boys (42.9% vs. 19.6%) with p-value (0.040, 0.018, 0.001 and >.001) respectively. Conclusions: Findings demonstrated that the ongoing war has a significant effect on the stunting prevalence among children under five years. Early childhood undernutrition is linked to several adverse outcomes in adulthood, and it is plausible that undernutrition in early childhood may be a significant factor in explaining the health disparities faced by Yemeni children under five.

Abstract

Background: Central obesity causes various diseases. Abdominal fat is associated with metabolic disturbances. Dietary interventions targeting abdominal fat are reported to have health benefits. A plant-based diet is known to be beneficial in reducing abdominal fat. Objectives: This study aims to see the effect of a flexitarian diet on abdominal fat through waist circumference and SAD. Methods: This research used a quasi-experimental design with a pre-post-test control group design. The research sample size was 21 obese female students aged 19-25 in Semarang. Subjects were selected using the consecutive sampling method and divided into treatment groups (11 subjects) and control groups (10 subjects). The four-week flexitarian diet treatment group included three main meals and two snacks, while the control group was not given any dietary intervention. However, both groups (all subjects) were given nutrition and obesity education through lectures and leaflets. Anthropometric data on body weight, waist circumference, and SAD were taken before and after the intervention. Statistical analysis was performed using paired t-test, independent T-test, Mann Whitney, and ANCOVA. Results: Intakes between groups during the intervention showed statistical energy, fat, and fibre differences. SAD between the treatment group (-1.2±0.6cm) and the control group (0.2±1.5cm) showed a significant difference (p=0.010). There was no significant difference in waist circumference between the groups, but the decrease in the treatment group was more significant (-6.6±5.5cm). Physical activity did not affect changes in SAD (p=0.210), while diet treatment affected changes in SAD (p=0.010). Conclusions: Changes in SAD showed a significant difference between groups after intervention. Changes in waist circumference were insignificant between groups, but the decrease was more significant in the treatment group.

Abstract

Background: Fast and accurate detection of acute malnutrition is important to prevent death. The use of Mid Upper Arm Circumference (MUAC) can be an alternative but its accuracy is still being explored because many differences in research results are found. Objectives: This literature aimed to identify the accuracy (sensitivity and specificity) and optimal MUAC cut-off in detecting acute malnutrition in children under 5 years. Methods: Search for articles using Google Scholar and Pubmed with the keywords "MUAC” "children” "acute malnutrition”. Selection based on the question "How is the sensitivity and specificity of MUAC in detecting acute malnutrition in children under 5 years” with inclusion criteria articles for the last 10 years, fully accessible and free of charge. Excluding non-English articles and type review articles. Six articles were found that further review. Discussion: The sensitivity of the MUAC indicator by World Health Organization in detecting acute malnutrition is smaller than its specificity. The lowest sensitivity of MUAC in detecting severe (<11.5 cm) and moderate (11.5-12.5 cm) acute malnutrition is in Nepal, namely 13.6% and 21% respectively. Based on the results of analysis using the ROC curve in each article, it was found that the optimal MUAC cut-off varied in detecting cases of acute malnutrition. Conclusions: The MUAC cut-off accuracy recommended by the World Health Organization to identify severe and moderate acute malnutrition is not optimal. Therefore, further studies are needed in each country to determine the optimal cut-off that is appropriate for the region according to the age group and problem classification.

Abstract

Background: Adolescence, specifically among female is a high-risk period for weight gain and the incidence of being overweight. Lifestyle is considered a significant contributing factor to overweight in female adolescents. Objectives: This study aimed to examine the association between lifestyle, particularly daily activity and eating habits with the incidence of being overweight among female adolescents in Tasikmalaya City, West Java. Methods: A cross-sectional design was used with 275 subjects from seven state junior high schools selected by proportional random sampling. The data collected included demographics, lifestyle comprising daily activity, eating habits, as well as the incidence of being overweight, measured using the Body Mass Index-Age-Z score (BAZ). The association between lifestyle and overweight was evaluated using binary logistic regression. Results: The results showed that out of 275 subjects, 51 were overweight (18.5%) and 224 (81.5%) had normal weight. The risk factors for being overweight included sleeping less than seven hours a day (p=0.017; OR=3.020; 95%CI=1.218-7.486), mild physical activity (p=0.008; OR=19.251; 95%CI=2.170-170.790), breakfast frequency less than 3 times a week (p=0.033; OR=2.418; 95% CI=1.074-5.443) and frequent snacking habits (p=0.000; OR=15.022; 95% CI=5.922-43.846). Conclusions: Several lifestyles were found to significantly impact the incidence of being overweight among female adolescents, thus  suggesting the need to make lifestyle  modifications.

Abstract

Background: Community-based health insurance is increasingly integrated into the Indonesia health system. Health protection is one of the dimensions in the Special Index for Stunting Management (IKPS). The Indonesia National Health Insurance, regulated by Social Health Insurance Administration (BPJS Health), has stunting handling programs. Stunting is less likely to occur in toddlers who have health insurance. Objectives: To explore the use of by BPJS Health for handling stunting in the stunting locus area. Methods: Qualitative research using a case study was conducted in the Wori Primary Health Service. Snowball sampling methods and recruit 9 participants: health service and village staff, a mother with a stunting toddler, and cadres. All interviews, were audio-recorded and transcribed verbatim. Thematic analysis was used to and using OpenCode software. Results: Even though there was local health insurance, the regional government kept supporting BPJS Health ownership for stunting toddlers. They get referral services to pediatricians for further screening, but this cannot be carried out due to the low ownership of by BPJS Health. The determining variables were the absence of a toddler's National Identity Number, the drawn-out procedure, and the mother's perception of the risk of stunting. Because of the lower knowledge barrier and accessibility to other healthcare facilities, they thought it was sufficient to treat stunting at the Primary Health Service level. Conclusions: The benefits of BPJS may be seen in the stunting handling program at the Wori Primary Health Service, however, there is a need for coordination of various components to inform it.

Abstract

Background: Non-communicable diseases (NCDs) constitute significant causes of global mortality, with their prevalence continually rising each year. The development of nutrigenetic science and gene-based nutrition services plays a vital role in mitigating NCDs, despite the lack of awareness among many individuals.  Objectives: This study aims to investigate the relationship between education levels and the level of interest in gene-based nutrition services among Indonesians.  Methods: A cross-sectional study design was conducted online using the Qualtrics Survey during May and June 2023 in DKI Jakarta, West Java, Central Java, East Java, and DI Yogyakarta. The cluster sampling technique was employed. Structured questionnaires were designed, covering sociodemographic information and interest levels, and were validated and tested for reliability. Data analysis involved chi-square testing.  Results: Indonesians exhibited a substantial interest in gene-based nutrition services (92.3%). Most participants (89.7%) expressed willingness to recommend nutrigenetic testing to their families. Additionally, 97.5% of participants agreed that gene-based nutrition services offer numerous benefits (97.5%), and a significant proportion was open to paying more for such services (62.4%). A large percentage of Indonesians demonstrated interest in undergoing nutrigenetic testing to ascertain disease susceptibility (95.9%). However, no significant association was observed between education levels and the interest in gene-based nutrition services in Indonesia (p=0.134, OR: 0.551, CI=95% (0.27-1.11)).  Conclusions: The study findings suggest that the educational background of Indonesians does not significantly impact their interest in gene-based nutrition services. However, a majority of Indonesians display interest and recognize the potential benefits of gene-based nutrition services, particularly in the context of preventing NCDs.
