Abstract

Abstract

Background: Pneumonia remains the leading infectious cause of death in children under five globally. In Padang City, Indonesia, pediatric pneumonia cases increased sharply from 2021 to 2023, yet the environmental and host-related determinants driving this trend remain poorly understood in the local tropical urban context. Objective: This study aimed to analyze the correlation between physical environmental factors humidity, wind speed, temperature, rainfall, PM10, and PM2.5  and pediatric pneumonia incidence in Padang City from 2021 to 2023, and to spatially map host-related intervention coverage across sub-districts.Methods: An ecological study design was employed using monthly secondary data aggregated at the city level (n = 36 months). Pneumonia case data were obtained from the Padang City Health Office, meteorological data from BPS, and air quality data from the Environmental Agency. Pearson correlation and multiple linear regression were used for statistical analysis following assumption testing. Spatial overlay mapping was conducted using QGIS to examine the geographic distribution of cases in relation to exclusive breastfeeding, vitamin A supplementation, and complete basic immunization coverage. Results: A total of 5,428 cases were recorded (mean: 150.78 cases/month). Wind speed showed a significant positive correlation (r = 0.436, p = 0.008) and PM2.5 a significant negative correlation (r = −0.516, p = 0.003) with pneumonia incidence. PM2.5 was the most dominant predictor (R² = 0.463). Spatial analysis identified Kuranji and Lubuk Begalung as persistently high-burden sub-districts, consistently associated with inadequate host-factor intervention coverage. Conclusion: Wind speed and PM2.5 are significant environmental determinants of pediatric pneumonia in Padang City. Integrated strategies combining air quality monitoring, vitamin A supplementation, immunization programs, and strengthened Posyandu-based interventions are essential to reduce pneumonia burden in tropical urban settings.

Abstract

Background: Complete basic immunization (CBI) coverage in West Sumatra Province remains critically low, posing a significant public health challenge. Objective: This study aimed to identify determinants of CBI achievement among children aged 12–23 months in West Sumatra Province using nationally representative data from the 2023 Indonesia Health Survey (Survei Kesehatan Indonesia/SKI 2023).Methods: A cross-sectional study design was employed through secondary data analysis of the SKI 2023 dataset. The analytical sample comprised 609 children aged 12–23 months residing in West Sumatra Province. Variables examined included child characteristics (sex, birth weight), maternal factors (employment status, educational level, gestational age at delivery, KIA book ownership), health service factors (place of delivery, healthcare accessibility), family socioeconomic status, and history of adverse events following immunization (AEFI). Survey-weighted univariate, bivariate (chi-square test), and multivariate (binary logistic regression) analyses were performed. Statistical significance was set at p < 0.05.Results: Among 609 children aged 12–23 months, 87.8% (n = 534) had incomplete immunization status and only 12.2% (n = 75) had received complete basic immunization. The measles-rubella vaccine recorded the lowest coverage at 68.4%. In bivariate analysis, maternal employment (p = 0.045) and KIA book ownership (p = 0.001) were significantly associated with CBI status. In multivariate analysis, only KIA book ownership remained a significant independent determinant: children whose mothers did not own a KIA book were nearly five times more likely to have incomplete immunization (Adjusted OR = 4.86; 95% CI: 1.63–14.47; p = 0.005).Conclusion: KIA book ownership is the primary independent determinant of complete basic immunization achievement in West Sumatra Province. Targeted interventions should prioritize optimizing KIA book utilization, strengthening health education for working mothers, and addressing structural barriers to immunization services.

Abstract

Background: HIV status self-disclosure is a critical determinant of transmission prevention, treatment adherence, and psychosocial wellbeing among people living with HIV/AIDS (PLWHA). Despite its public health significance, disclosure rates remain suboptimal across diverse populations, particularly among men who have sex with men (MSM) and non-MSM groups. Objective: This systematic review aimed to identify and synthesize factors influencing HIV status self-disclosure among PLWHA, with a specific focus on comparative differences between MSM and non-MSM populations. Methods: A systematic search was conducted across PubMed, Scopus, Science Direct, and Google Scholar, covering publications from January 2020 to December 2024. Study selection followed PRISMA 2020 guidelines using a predefined PICO framework. Two independent reviewers screened studies, and methodological quality was assessed using the Newcastle-Ottawa Scale (NOS). Data were synthesized narratively across three thematic domains. Results: From 1,125 initial records, 10 studies met the inclusion criteria (6 cross-sectional, 4 case-control). Three thematic domains were identified: psychological factors (stigma, HIV knowledge, self-acceptance), social factors (partner communication, social support, relationship quality), and structural factors (ART duration, residential setting, healthcare confidentiality). MSM faced distinctive barriers through dual stigmatization related to HIV status and sexual orientation, whereas non-MSM populations were more strongly influenced by gender-based power dynamics, economic dependency, and community-level stigma. Conclusion: HIV disclosure is a multifactorial process requiring differentiated, multi-level interventions targeting stigma reduction, social support strengthening, and improved healthcare confidentiality across both population groups.

Abstract

Background: HIV/AIDS remains a major public health challenge in Indonesia. Dharmasraya Regency, West Sumatra, ranks second in HIV prevalence among districts in the province (0.018%), with cases increasing from 3 new infections in 2023 to 8 cases by October 2024. The specific pattern of risk factors driving this upward trend in Dharmasraya remains insufficiently documented. Objective: This study aimed to identify risk factors of individual characteristics, sexual behavior, and health history associated with HIV/AIDS incidence in Dharmasraya Regency. Methods: An analytical study using a non-matching case-control design was conducted from March to May 2025. A total of 117 participants were recruited from the HIV/AIDS Information System (SIHA 2.1): 39 laboratory-confirmed HIV-positive cases (total sampling) and 78 HIV-negative controls (systematic random sampling, 1:2 ratio). Data were collected through structured, interviewer-administered questionnaires. Bivariate analysis was performed using the Chi-Square test (α = 0.05), with crude odds ratios (OR) and 95% confidence intervals (CI) calculated for each variable. Results: Five variables were significantly associated with HIV incidence: risky sexual practices (OR = 44.24; 95% CI: 9.83–199.02; p < 0.001), non-heterosexual orientation history (OR = 5.57; 95% CI: 2.40–12.93; p < 0.001), history of sexually transmitted infections (OR = 5.47; 95% CI: 2.06–14.50; p = 0.001), family history of HIV/AIDS (OR = 5.33; 95% CI: 2.08–13.70; p = 0.001), and male gender (OR = 2.62; 95% CI: 1.12–6.09; p = 0.039). Age, education level, occupation, marital status, and condom use showed no statistically significant association. Conclusion: Risky sexual practices, non-heterosexual orientation history, STI history, family history of HIV/AIDS, and male gender are significant risk factors for HIV incidence in Dharmasraya Regency. Targeted prevention programs addressing behavioral risk factors and stigma-free healthcare access are urgently recommended. Future research should employ multivariate methods to identify independent risk factors.

Abstract

The LINTAS Diarrhea program for toddlers was carried out by the Andalas Health Center but only achieved 76% of its target in 2022. This research aimed to evaluate how the program was implemented at the health center in 2023. A descriptive qualitative approach was applied, involving nine informants chosen through purposive sampling. The study took place from November to July 2024. Data collection methods included in-depth interviews, field observations, and document analysis. Data were then processed through reduction, display, and conclusion drawing, and validated using source and method triangulation. Although the program referred to the Indonesian Minister of Health Regulation No. 1216 of 2001, not all staff were aware of the policy. Staff members often faced multiple roles and heavy workloads. Funding and infrastructure remained inadequate. Furthermore, setting of program targets and delivery of preventive education had not been optimal. The program was not fully aligned with the LINTAS Diarrhea Program Pocket Book, and delays occurred in recording and reporting activities. Although monitoring and evaluation were carried out, overall program execution was still lacking. Therefore, improvements are needed, including better funding, additional personnel, improved facilities, and stricter adherence to program guidelines for optimal diarrhea prevention in toddlers.
