Abstract

Aktivitas sekolah dan sosialisasi yang dilakukan oleh remaja mengakibatkan banyaknya waktu yang tersita dan hanya sedikit waktu yang disisakan untuk beristirahat ataupun tidur. Perubahan lingkungan, kemandirian dan perubahan pola tidur yang tajam biasa terjadi bagi siswa yang menjalani kegiatan pesantren, sehingga berpengaruh terhadap kuantitas dan kualitas tidur santri. Kekurangan tidur ini berhubungan dengan buruknya perilaku sleep hygiene. Penelitian ini bertujuan untuk mengetahui gambaran 8 aspek sleep hygiene, dan gambaran sleep hygiene berdasarkan latar belakang demografi santri. Penelitian ini menggunakan desain deskriptif kuantitatif dengan pendekatan waktu cross sectional study. Penelitian dilakukan pada 161 santri remaja usia 11-20 tahun di Pondok Pesantren Darul Hikmah Tanjungmekar Kecamatan Tanjungkerta Kabupaten Sumedang.Data dikumpulkan menggunakan kuesioner Adolescent Sleep Hygiene Scale serta dianalisis menggunakan nilai mean. Hasil penelitian ini menunjukkan lebih dari setengah jumlah responden memiliki praktik sleep hygiene yang baik (68,3%). Terdapat 6 aspek sleep hygiene santri dalam kategori baik, yaitu: aspek fisiologis, aspek perilaku penggugah, aspek kognitif emosional, aspek lingkungan tidur, aspek stabilitas tidur dan aspek penggunaan substansi. Adapun 2 aspek sleep hygiene dalam kategori buruk terdapat pada aspek tidur siang dan aspek rutinitas sebelum tidur. Memantau jadwal tidur santri agar tidak melebihi pukul 24.00, menyediakan ruangan terpisah bagi santri untuk belajar mandiri, pembiasaan sikat gigi sebelum tidur, pemantauan penggunaan rokok di lingkungan pondok, tidak menyediakan teh pada saat jam makan malam, dan pemantauan kegiatan santri diluar jam sekolah ataupun pondok pesantren merupakan hal yang harus pengurus pondok pesantren perhatikan.

Abstract

The failure of nurses in taking action related to their moral beliefs makes nurses' moral comfort not achieved and affects their dissatisfaction in their work, perhaps it also can result in nurses'  distress. The purpose of this study was to identify moral distress in the Emergency Department (ED) nurses. The study design used quantitative descriptive with a cross-sectional approach. the instrument used was the moral distress scale-revised (MDS-R) which measures the frequency and intensity of moral distress. The sampling technique used is total sampling. the sample population in this study was ED nurses in one type a hospital in Bandung, with a sample of 51 people. The results showed that the average of the total moral distress in emergency nurses was 50.72 (sd 43.846) and showed that moral distress in ed nurses tended to be low. the duration of nurse work experience is an individual factor that significantly influences moral distress in ed nurses. There is no significant difference in the mean moral distress score with the nurse's intention to quit his job. Qualitative research is needed to determine the causes of other moral distress and moral distress explanations can be given to nurses and students to increase the sensitivity and moral competence of Indonesian nurses.

Abstract

Health workers are people who have knowledge and skills through education and also devoted themselves in the health areas. The attitude of health workers has determined the services to the patient including patients with mental disorders. A negative attitude is one of the elements formed the stigma against ODGJ (mental disorder patients) who is the greatest barrier to the treatment and the development of effective treatments. This study aimed to identify the attitude of health workers to people with mental disorders at Puskesmas Jatinangor.The research used quantitative descriptive with the cross-sectional approach. The research used the technique of total sampling from the population. The sample was all health workers in Puskesmas Jatinangor total of 55 people. Data taken by using the instrument of Mental Illness: Clinician's Attitude Scale (MICA) developed by Gabbidon et al (2013). Data analysis used a descriptive analysis of the attitude in the form of the mean and the presentation. The higher the total score more and showed negative attitudes (stigmatization).The results showed that more than half of the total respondents had a positive attitude tend to value the mean 48.78 ± SD 8.993. This can conclude that health workers in Puskesmas Jatinangor had favored attitudes, accepted, and recognized ODGJ. It is recommended to all health workforce would maintain and increase positive attitudes towards ODGJ.

Abstract

Latar belakang: Standard precautions merupakan praktik pencegahan infeksi yang berlaku untuk semua perawatan pasien didalam memberikan layanan kesehatan namun beberapa penelitian menunjukkan pelaksanaan standard precautions belum optimal karena adanya hambatan yang dipersepsikan perawat.Tujuan: Penelitian ini bertujuan untuk mengidentifikasi hambatan yang dipersepsikan perawat dalam pelaksanaan standard precautions.Metode: Metode penelitian ini adalah deskriptif kuantitatif dengan pendekatan cross-sectional pada 37 perawat di Instalasi Gawat Darurat RSUD Sumedang dengan menggunakan teknik total sampling. Instrumen penelitian menggunakan kuesioner yang terdiri dari 48 pernyataan, mencakup lima tindakan standard precautions (hand hygiene, penggunaan APD, praktik injeksi yang aman, desinfeksi lingkungan, dan kebersihan pernafasan / etika batuk dan bersin) dan telah melalui uji content dan face validity. Data dianalisis dengan menggunakan distribusi frekuensi dan disajikan dalam bentuk persentase tiap faktor hambatan.Hasil: Hasil penelitian menunjukkan bahwa terdapat beberapa hambatan yang dipersepsikan oleh perawat pada tindakan standard precautions. Hambatan yang paling dirasakan yaitu peralatan yang membuat iritasi kulit pada hand hygiene (37,8%), keterbatasan peralatan APD (45,9%), keterbatasan dukungan dan pengawasan dalam pelaksanaan standard precautions pada praktik injeksi yang aman (16,2%), meningkatnya jumlah pasien pada desinfeksi lingkungan (32,4%), dan keterbatasan peralatan pada praktik kebersihan pernafasan / etika batuk dan bersin (10,8%).Kesimpulan: Berdasarkan penelitian ini, pihak rumah sakit harus mengupayakan peralatan hand hygiene yang mengandung emolien/pelembab dan jenis alcohol handrub n-propanol untuk mengatasi iritasi kulit. Memperkuat penyediaan sarana secara benar dan teratur untuk mengatasi keterbatasan peralatan. Memperhatikan kajian jumlah rasio perawat dan pasien untuk praktik kerja yang aman. Selain itu, membentuk komunitas praktik, mensosialisasikan reward atau punishment untuk  meningkatkan kepatuhan dalam pelaksanaan standard precautions.

Abstract

Gluten Free Casein Free (GFCF) is a diet recommendation to reduce hyperactivity in children with autism. Lack of adherence and knowledge can lead to improper dietary application which will result in remaining health problems and hyperactive behavior of autistic children. The purpose of this study is to examine knowledge and perception of parents towards GFCF diet in children with autism. This descriptive-quantitative study used a total sampling technique to 57 parents of autistic children in 3 Bandung District Extraordinary Schools. Data was obtained through questionnaire with validity ranging from 0.315-0.912 and reliability of 0.788 and 0.932. Data was analyzed using frequency and percentage in descriptive statistics. Results showed that some parents lacked of knowledge (n = 28), and 67% of subjects (n = 38) had a positive perception of GFCF diet. Positive perception includes perception of benefits, perception of susceptibility, perception of self efficacy, and perception of severity. In the perception of barrier, some subjects had negative perception. Based on the results, several parents had positive perception of GFCF diet, although the parental knowledge related to this diet was in the low category. That is likely because of parents have not received sufficient information related to GFCF diet. Therefore, health workers especially community nurses are expected to collaborate with nutritionists in making regular visits in order to provide parents of autistic children with health education related to GFCF diet in extraordinary schools.

Abstract

Penyakit Jantung Koroner (PJK) merupakan penyakit yang harus di kontrol seumur hidup. Salah satu cara untuk mengontrol PJK sekaligus menurunkan angka kekambuhan adalah dengan proses pembelajaran. Namun, kajian kebutuhan belajar pada klien PJK belum dilakukan secara spesifik, sehingga pendidikan kesehatan belum diberikan berdasarkan kebutuhan prioritas yang dibutuhkan klien. Penelitian ini bertujuan mengidentifikasi kebutuhan belajar klien dengan PJK di salah satu rumah sakit di kota Bandung.  Metode dalam penelitian ini adalah deskriptif kuantitatif. Sampel dalam penelitian ini adalah klien PJK yang menjalani rawat inap dan rawat jalan di salah satu rumah sakit di kota Bandung. 105 responden diambil dengan teknik consecutive sampling selama 30 hari. Pengumpulan data menggunakan instrumen A Turkish Version of the Cardiac Patients Learning Needs Inventory (TR-CPLNI). Analisa data menggunakan nilai mean per subvariabel. Hasil penelitian menunjukkan bahwa kebutuhan belajar yang dirasa sangat penting bagi klien PJK adalah kebutuhan tentang anatomi dan fisiologi jantung (= 4.41±0.71). Klien PJK kemungkinan tidak menerima informasi yang memadai tentang anatomi fisiologi jantung atau cara kerja jantung, sehingga perawat disarankan untuk memberikan pendidikan kesehatan berupa informasi anatomi dan fisiologi jantung.

Abstract

Elderly is one of the most vulnerable groups to nutritional problems. Elderly people should maintain nutritional status at optimum conditions, so they can help their physical condition in the process in adjusting to the changes they experience. The purpose of this study was to determine the description of the nutritional status of the elderly at Posbindu Caringin Health Center. The research method was descriptive using a cross-sectional approach.  The number of respondents was 125 people that chosen using purposive sampling technique. Data collection using a Mini Nutrition Assessment questionnaire and analyzed using percentage and frequency distribution. The study found that the elderly had normal nutritional status (68.0%), the risk of malnutrition (27.2%), and malnutrition (4.8%). The conclusions in this study are the nutritional status of the elderly in Posbindu Pusingin Caringin, most of the elderly have normal nutrition. Community nurses are expected to be able to use this information in determining appropriate interventions and prevention related to the nutrition problems of the elderly.

Abstract

Prevalence of  cancer is estimated will increase in the next two decades. Therefore, there is a challenge for health provider to encounter treatment and caring for the patients. Especially, the cancer patients face several problems not only physical but also psychological, emotional, spiritual and social cultural aspects.This study explored the evidence-based practice on community-based palliative cancer care. Literature study is done by making a summary of published articles related to the question. The searching method used several electronic databases such as Google Scholar, Proquest, and PubMed. Articles under the keywords of “Palliative Cancer Care”, “Community”, and “Nursing” reach as much as 1.804. The inclusion criteria for this literature review were articles that have been peer-reviewed, are in full-text, in either English or Indonesian, and publication year from 2008 to 2018. Meanwhile, the exclusion criteria include those that do not follow a standardized structure of an article (consisting of Abstract, Introduction, Method, Result, Discussion, Implication, and Reference), are in the form of a review, and whose content does not answer research questions.Results: The United States of America is on the highest place regarding palliative care service, following by community-based palliative cancer implementation in Europe. Asian countries had been applying palliative care service, integrated with national health care system. In the Middle East countries, palliative care program ranks the lowest, but in implementation, they have discreetly performed community-based palliative care. In Africa, it is not the main focus in the field of health. Palliative care for cancer patients that is potential for development in Indonesia is that of family-based.Conclusion: Community-based palliative care is a variant of palliative treatment long applied and being developed in many countries in the world. In continents such as America and Europe, the implementation of palliative care ranks the highest place. In Indonesia, it is done partially and only available in hospitals or non-governmental organization. In the level of community, family-based palliative care can be developed by involving trained family members.

Abstract

The incidence of childhood cancer in Indonesia increased annually. The successful treatment of childhood cancer remains low. One of the factors that influence cancer care was parent’s self-efficacy. Adequate parent’s self-efficacy would improve the quality of care in children with cancer, however, limited studies examine factors associated with parent’s self-efficacy. The aim of this study was to identify factors associated with parent’s self-efficacy in order to undergo cancer treatment. The type of research was descriptive analytic. The population was parents with cancer children. This study was conducted in childhood cancer shelter home community, 40 parents were chosen using the consecutive sampling technique. Self-efficacy was measured using a questionnaire. Data were analyzed with univariate and bivariate analysis. The results showed that 23 (57.5%) of parents have higher self-efficacy. There were significant relationship between enactive mastery experience (p =0.011), vicarious experience (p=0.030), verbal persuasion (p=0.003), the nature of the task faced (p=0.022), external incentives (p=0,009), the status or role of the individual in the environment (p=0.024) and education level (p=0.031) with parent’s self-efficacy. While the physiological state (p=0.284), parent’s sex (p=0.277), parent’s age (p=0.513), and parent’s culture (p=0.174) were not significantly associated with parental self-efficacy in the treatment of childhood cancer. Based on these results, it was very clear that parents who have higher self-efficacy are expected to carry out their duties better, so nurses should provide opportunities and support for families to demonstrate the capabilities and competence of the family to meet the care needs for child cancer.

Abstract

Bleeding is the main cause of maternal mortality in the world, the mortality rate due to bleeding about 30.3% in Indonesia in 2013. Dealing bleeding during and after labor is very important for health professionals including nurses. Research on the management of postpartum hemorrhage is still limited. The purpose of this literature review was to determine the management of postpartum hemorrhage from various countries. The method, this literature review involved articles that published in 2008-2018. The articles searched from several databases, including ProQuest, 32 articles, PubMed 22 articles, and Google 21 scholar articles. Keywords for article search included postpartum hemorrhage, nurse, prevention, treatment, management, and intervention. 75 articles were assessed for the quality using the JBI instrument (Joanna Briggs Institute), and finally, 10 articles were found and met the inclusion criteria. The results show that there are two types of actions to deal with postpartum hemorrhage including direct treatment and indirect treatment. The direct treatment includes bimanual compression, maneuvering techniques, balloon tampons, and tools resembling a butterfly shape for bimanual compression. Indirect actions include training of health workers, initiation of early breastfeeding and ice packs. Conclusion, effective management both directly and indirectly is able to overcome postpartum hemorrhage. Health workers are expected to master effective ways to deal with postpartum hemorrhage.
