Abstract

"Discharge at Own Request (PAPS)" is one of the indicators used to assess the quality of inpatient care services. Based on direct interviews with several PAPS patients in the inpatient ward of a particular regional public hospital (RSUD), several factors were identified as triggers for their decision to leave, including the waiting time for treatment and the condition of the treatment room and walls, which appeared uncomfortable—especially for patients who did not yet have a BPJS National Health Insurance) card. The high incidence of patients discharging themselves at their own request (PAPS) has an impact on the hospital, including a potential decrease in service efficiency, as indicated by a reduction in the length of stay (LOS). This study aims to identify and analyze the influence of inpatient service quality on the decision of patients to discharge themselves at their own request (PAPS) at RSUD Mayjen H.A. Thalib, Sungai Penuh City, in 2025.This is a quantitative study using a cross-sectional study design. The research was conducted at RSUD Mayjen H.A. Thalib, Sungai Penuh City, from November 2024 to April 2025. The sample consisted of 125 patients hospitalized in the inpatient ward of RSUD Mayjen H.A. Thalib, Sungai Penuh City. The sampling technique used was accidental sampling. The research findings indicate a correlation between patient satisfaction—based on the aspects of tangibles, reliability, Responsiveness , assurance, and empathy—and the decision to discharge themselves at their own request (PAPS) at RSUD Mayjen H.A. Thalib, Sungai Penuh City, in 2025. Therefore, it is recommended that regular training be provided to all medical personnel and hospital staff to enhance their competence and ability to deliver optimal healthcare services.

Abstract

Tuberculosis (TB) is still a major health problem globally, due to serious health disorders in the community and the incidence of this disease increases every year. Currently, it is estimated that 1 in every 3 TB cases have not been detected by the Case Detection Rate (CDR) program, which is known from national data showing that CDR in Indonesia from 2012-2014 has decreased, namely case discovery in 2012 was 61%, and in 2014 the case discovery rate was 46%. The purpose of this study was to Analyze Factors Influencing the Implementation of the Pulmonary TB Patient Health Promotion Program at the Grandmed  Lubuk Pakam Hospital in 2024. This study is a qualitative descriptive study. The results of this study are that the implementation of the pulmonary TB patient health promotion program in hospitals is influenced by various interrelated factors, namely individual factors, the environment, health systems, policies, and health workers. Individual factors such as knowledge, socio-economic conditions, and treatment compliance determine the extent to which patients can accept and carry out health recommendations. Environmental factors, including social stigma and family or community support, also influence patient motivation and openness to health education. From the health system side, the existence of an effective information system and guaranteed drug availability are crucial technical supports. Meanwhile, the success of the program is highly dependent on management policies and the design of a structured and sustainable promotion program. The conclusion is that the role of health workers through quality communication and education greatly determines the effectiveness of information delivery and changes in patient behavior. Therefore, optimal implementation of health promotion requires a comprehensive and collaborative approach from all of these factors to support sustainable TB control efforts in hospitals.

Abstract

Occupational Safety and Health (K3) is an effort made so that workers always work safely and healthily in the workplace, and resources and production processes can be used safely and efficiently. The number of work accident cases in Indonesia in the period January-August 2024 was 278,564 cases, of which North Sumatra Province had 15,220 cases. Every company is required to implement an Occupational Safety and Health Management System (SMK3) that is integrated with the company's management system. The implementation of the occupational safety and health management system (SMK3) at Perumda Tirtanadi, North Sumatra Province has a good management system, this is proven by the certification audit process by the K3 management system audit institution by PT. Surveyor Indonesia and has received recognition from the Indonesian Ministry of Manpower. This study aims to determine five (5) principles, namely the determination of K3 policies, K3 planning, implementation of K3 plans, monitoring and evaluation of K3 performance, and review and improvement of K3 performance at Perumda Tirtandi. This study uses a descriptive qualitative approach through interviews. The interview results were processed using the NVIVO application. The results of the study are on the first principle of establishing K3 policies by establishing SOPs and annual work programs in accordance with national regulations, which must be informed to all employees. The second principle is K3 planning which emphasizes K3 culture through repeated socialization reflecting an ecological-social approach in health promotion. The third principle, namely the implementation of K3 in Perumda Tirtanadi shows a strong commitment to ensuring the use of PPE and compliance with K3 signs. The fourth principle, sustainability in SMK3 is reflected in the monitoring system of Perumda Tirtanadi, which involves visits from management and the K3 advisory committee. The fifth principle is a critical process that allows management to conduct objective assessments of the entire system. The study on the Implementation of the Occupational Safety and Health Management System (SMK3) requires socialization of SMK3 and considers K3 as something mandatory and a necessity when working.

Abstract

Sweet potatoes and corn are local food ingredients with potential as alternative substitutes for rice. Traditionally, apem cakes are made using rice flour as the main ingredient. Partially replacing rice flour with sweet potato flour and corn flour is expected to optimize the utilization of these local ingredients and improve the nutritional value of the resulting apem cake. This study aimed to analyze the effect of using purple sweet potato flour and corn flour on the moisture content and protein content of apem cake. The study employed a completely randomized design (CRD) with four formulations of rice flour, purple sweet potato flour, and corn flour, namely A (100%:0%:0%), B (50%:15%:35%), C (50%:25%:25%), and D (50%:35%:15%). Moisture content was measured using the gravimetric method, while protein content was analyzed using the titrimetric method. Moisture content data were analyzed using Welch’s ANOVA, and protein content was analyzed using One-Way ANOVA followed by Tukey’s HSD test. The results showed that the highest moisture content was 41.42% in the formulation with 15% purple sweet potato flour and 35% corn flour, and the lowest was 40.76% in the 100% rice flour formulation. Welch’s ANOVA indicated no significant difference in moisture content among treatment groups (p = 0.378; p > 0.05). The highest protein content was 4.51% in the 100% rice flour formulation, and the lowest was 4.09% in the formulation with 35% purple sweet potato flour and 15% corn flour. ANOVA results indicated a statistically significant difference in protein content (p = 0.001; p < 0.05). In conclusion, the use of purple sweet potato flour and corn flour in apem cake did not significantly affect moisture content but significantly influenced protein content. Further investigations should focus on optimizing the formulation to evaluate the influence of the treatment on product quality and storage stability.

Abstract

Birth length has been used as an indicator of infants' nutritional status, reflecting the results of linear fetal growth during pregnancy. Meanwhile, the nutritional status of pregnant women can be assessed using the mid-upper arm circumference (MUAC). In Indonesia, MUAC is primarily utilized to classify chronic energy deficiency (CED) and has not been explored as a predictor of infants' nutritional status based on birth length. This study aims to analyze the relationship between MUAC and birth length. A retrospective cohort study was conducted involving 55 pregnant women aged 20-35 years who attended prenatal check-ups and gave birth between January and December 2018. Data were collected from patient records at the Maternal and Child Health Unit of Pulokulon I Community Health Center in Grobogan Regency in July 2019. Pregnant women with incomplete data, those diagnosed with anemia, and babies born prematurely were excluded from the analysis. Data were analyzed using the Rank-Spearman correlation test with SPSS version 16. The analysis of maternal demographic characteristics revealed that only maternal age had a significant impact on the baby's birth length, with a Pearson correlation test showing p = 0.04. The average maternal mid-upper arm circumference was 26.01 ± 3.45 cm, while the average birth length was 48.76 ± 1.49 cm. The study concluded that there was no significant relationship between maternal mid-upper arm circumference and birth length (p-value = 0.931). Further research is needed across various community health centers, involving a larger sample size and additional maternal variables, to determine the effectiveness of mid-upper arm circumference as an indicator for predicting infants' nutritional status. .
