Abstract

STEMI with onset≤12 hours, necessary to take Primary Percutaneous Coronary Intervention (pPCI), but if there is no facility, another therapy is fibrinolytic, to improve blood flow in the coronary artery and myocardial function, thus reducing infarction expansion. This is a retrospective study in STEMI patients onset≤12 hours, had performed successful fibrinolytic with Streptokinase (SK) and Alteplase (tPA) from January 2015 to August 2017. TIMI flow was assessed by coronary angiography. There were 54 patients who had performed successful fibrinolytic therapy with SK and tPA; each group had 27 patients. In the SK group, there were nine patients (33.3%) with TIMI flow 2 and 18 patients (66.7%) with TIMI flow 3. While in the tPA group there were 11 patients (40.7%) with TIMI flow 2, 16 patients (59.3%) with TIMI flow 3, (p=0.573). There is no difference in TIMI flow between SK and tPA on STEMI patients after success fibrinolytic therapy.

Abstract

The safety of MSG on the fetus has not been confirmed yet. The objective of this research is to determine the effect of tea of torbangun leaves on fetal development disorder of mice. The present study is experimental with a Completely Randomized Design (CRD). Data obtained were then analyzed by ANOVA, with p<0,05 as the level of significant. The comparison of mean values ± SEM increase number of live fetuses group C1-T1 (4,60 ± 0,93; 11,40 ± 0,87), p=0,018. The comparison of mean values ± SEM decrease number of embryo re-sorption percentage group C0-C1 (00,00 ± 00,00; 36,74 ± 15,13), group C1-C2 (36,74 ± 15,13; 00,00 ± 0,00), group C1-T1 (36,74 ± 15,13; 00,00 ± 0,00), and group C1-T2 (36,74 ± 15,13; 15,00 ± 22,36), p=0,020. This study concluded that T1 group is giving the best effect on increasing life fetuses and decreasing embryo re-sorption percentage.

Abstract

WHO recommends the supplementation of 200,000 IU vitamin A into 400 mg albendazole to cut down its reinfection. A Randomized Controlled Trial was conducted in SD N 384 Sikapas, North Sumatera from April - June 2016. Inclusion criteria were children with ascariasis. Sample divided into intervention and control group. Kato-Katz was used to identify egg worms and faecal egg count intensity (FECs). P value < 0.05 was consider significant. After three months follow up, there were 51 and 56 samples in intervention and control group with average age 9.6 years (SD 1.99) and 9.7 years (SD 2.20). Prevalence of ascariasis was 75.5%. There was no significant difference in Ascaris lumbricoides reinfection but instead of in FECs intensity. The NNT was 22.31. The addition of vitamin A in albendazole was not suggested according to this study.

Abstract

To investigate the association of MMP7 and VEGF expression with ovarian cancer, benign ovarian cysts, and normal ovaries. This study was analytical research with case-control design. In the study, IHC expression of MMP7 and VEGF was carried out on paraffin block of ovarian cancer tissue, benign ovarian cyst, and normal ovary. In 40 subjects with ovarian tumors, 17 patients were found with positive MMP7 expression. In the control group, no subjects were found with positive MMP7 expression. There was a significant relationship between MMP7 expression and ovarian tumor incidence. Meanwhile, in 40 subjects with ovarian tumors, 21 patients were found with positive VEGF expression. In the control group, no subjects with positive VEGF expression were found. There was a significant relationship between VEGF expression and ovarian tumor incidence. There was a significant relationship between MMP7 and VEGF expression with ovarian tumor incidence.

Abstract

The responsiveness of neoadjuvant chemotherapy in Triple Negative Breast Cancer (TNBC) needs determination to prevent overtreatment with chemoresistance regimen. A total of 60 consented patients from Haji Adam Malik and Bunda Thamrin Hospital were included in this cohort prospectie study. Patients with heart, kidney, liver disease, history of surgery, chemotherapy, or hormonal therapy were excluded. Apoptotic indexes were taken from all subjects before and after neoadjuvant chemotherapy. After 3 cycles of neoadjuvant chemotherapy, 31 subjects (51.7%) did not show clinical response while 29 subjects (48.3%) had clinical response. There was no significant difference of apoptotic index before and after neoadjuvant chemotherapy (5.47+1.38 vs 5.52+1.08 pg/mL). There was also no significant relationship between apoptosis index and clinical response (p=0.993). This study showed that apoptotic index fails to be neoadjuvant chemotherapy response predictor in triple negative breast cancer. Further research with larger samples is needed to confirm this result

Abstract

Exercise is non-pharmacological management for Diabetes Mellitus (DM) type-2. Previous study found that both eerobic and interval training improved insulin resistance. The aim of this study was to analyze the chronic effect various type of exercises to fasting blood glucose (FBG) and insulin resistance in DM type-2 model rats. It was an experimental study, twenty male Wistar rats, age 8 weeks, weight 150-180 gram as the object. Rats were given high fed diet for 4 weeks then injected streptozocin dose 30 mg/kgBB in citrate buffer pH 4.5 i.p, and 45 mg/kgBB at the following week. Groups were divided into i.e moderate continous training (MCT), severe continous training (SCT), slow interval training (SIT), and fast interval training (FIT). All groups were treated with ran on the treadmill three times a week for 8 weeks. Fasting blood glucose and fasting insulin were checked before and after intervention. Insulin resistance was determined by calculating HOMA-IR. Data analized with paired t-test (p<0,05). The results shown that all group significantly decreased fasting blood glucose and insulin resistance (p <0,05) after eight weeks exercise except insulin resistance in MCT group. Fasting blood glucose and insulin resistence post-test was found lowest in SIT groups in this study. In conclucions chronic effect of aerobic continous and aerobic interval in various intensity can decrease fasting blood glucose and insulin resistance in DM type-2 rat model. Slow interval training was the best exercise model to decrease insulin resistance.

Abstract

Introduction: Staphylococcus aureus infection can cause pyoderma. These days there is a shift in Staphylococcus aureus sensitivity to antibiotics that is marked by the existence of Methicillin-Resistant Staphyloccus aureus, so alternative therapy that is herbal medicine might be needed. There are a lot of medicinal plants in Indonesia, but until today the usage is still minimum. One of the plant genus that is already being used is allium that had been known to have antimicrobial activity. The genus allium that is consumed a lot by Indonesians especially people of North Sumatra is bawang batak (Allium chinense G.Don.). This study is conducted to assess the antimicrobial activity of bawang batak bulbs in several concentrations to MRSA. Methods: This research is an experimental research with agar diffusion method. The extracts were divided into six groups: extracts in concentration level of 6.25%, 12.5%, 25%, 50%, 100% and a negative control (DMSO). The sample size is four per group with total sample of 24. Results: The results showed that the inhibition of bacterial growth occured at the concentration levels of 50% and 100% with inhibition zone diameters of 8.695 mm and 10.545 mm respectively. Extracts with concentration levels of 25%, 12.5% and 6.25% did not show any inhibition. Conclusion: Bawang batak bulbs extract can be used to inhibit the growth of MRSA

Abstract

Many of the teenagers who enter pregnancy would cause a decline in health and nutritional status. Delivery at an early age was associated with a greater health risk for the mother. To assess the profile of teenage pregnancy in H. Adam Malik Hospital Medan year 2011-2015. This study was conducted with descriptive study design using secondary data from medical records. There were 128 patients who became pregnant in their teens. Then the data were tabulated and presented in the form of frequency distribution table and were analyzed descriptively. Most pregnant women came to the hospital for parturition (60.1%) followed by preeclampsia / eclampsia (14.8%). The way of delivery of pregnant adolescent women was vaginal (52.3%) followed by cesarean (36.7%). There was no significant relationship between adolescent age of pregnant and newborn weight. There was no significant relationship between the age of the adolescent and method of delivery. Most teenage pregnancy occured between 17-19 years. Most pregnant teenagers delivered vaginally, and most of the newborns were normoweight

Abstract

Indonesia is one of the few countries in the world with very high burden of tuberculosis (TB). Tubercular pleural effusion (Pleural TB) is the most common form of extra-pulmonary TB, however the remains a common clinical challenge. This research from May 2017 through September 2017 a total of 42 patients with exudative lymphocytic pleural effusions suspected to pleural tuberculosis were enrolled in this study. Adenosine deaminase (ADA) and Gene X-pert were examined from pleural fluid. Diagnosis was made clinically or based from sputum/pleural fluid culture. The result of this research showed out of all participants, 64,3% (27/42) had ADA in positive level (>40 IU/L) and 31% (13/24) had Gene X-pert positive Mycobacterium Tb. The Gene X-pert sensitivity was 40.7% with specificity 86.6%, PPV and NPV were 44.8% and 57.1% respectively. Kappa Compatibility study show that Gene X-pert point was 0.227 (p 0.066). The usefulness of Gene X-pert to diagnose pleural TB is limited by its poor sensitivity. Gene X-pert cannot be used as single diagnostic tool for tuberculous pleural effusion

Abstract

Although brain tumors only 1.4% of all tumors, high fatality rate made these tumors need special attention. In North Sumatera, there is no data on brain tumors patients profile. Objective: To determine brain tumor patients’ profile in North Sumatera, Indonesia. Method: A descriptive hospital-based study with primary data which taken from September–December 2012. Result: Of 75 brain tumors patients surveyed in 10 hospitals in North Sumatera 38 (50.7%) patients were male and 37 (49.3%) patients were female. Mean of age was 51.45 (11–87) years old. Most of the subjects were housewifes, 26 (34.7%) patients. The most common cause that brought these patients to see doctors was headache 32 (42.7%), followed by decreased level of consciousness 17 (22.7%). Clinical manifestations found in these patients were headache 67 (89.3%), dizziness/vertigo 41 (54.7%), convulsion 22 (29.3%), vomitting 32 (42.7%), motor dysfunction 46 (61.3%), sensory dysfunction 21 (28%), and cognitive decline 21 (28%). Only 7 patients (9.3%) had history of tumor in his/her relatives. Eighteen patients (24%) were treated surgically and 8 (10.7%) were given radiotherapy. 71 patients were alive (94.7%) when discharged from the hospitals due to various reasons. Head CT Scan/MRI showed primary tumors in 56 (74.7%) patients. Of these primary tumors 25 (44.6%) patients were meningioma and 19 (33.9%) were astrocytoma. Of 19 (25.3%) patients with secondary tumor, most common primary tumor where found in the lung 11 (57.9%). Conclusion: Sex the patients were equally distributed with mean of age was 51.45 (11–87) years old. The most common cause that brought these patients to seek for treatment were headache. Most of these patients were treated conservatively. The most common head CT Scan/MRI findings showed primary tumors.
