Abstract

A 7-year-old boy of Java origin, visited the doctor with progressive weakness everytime he ate food containing Monosodium Glutamat (MSG). Progressive weakness began from his legs and spreaded to the arms as well (sometimes also to the neck). He could neither walk nor do anything for 5–6 hours then began to resolve spontaneously.. We suspected the diagnosis of hypokalaemic periodic paralysis upon the history of episodes of flacid paralysis and low serum concentration of potassium (< 3,5 mmol/L) during the attacts. The clinical examination showed that this boy is in good general and nutritional conditions; electrocardiogram, laboratory blood count, urinalisis, thyroid, liver, kidney function, and ANA test were normal as well. The family history of flacid paralysis was negative. We have promptly administered potassium orally and his condition was improved progressively including less degree of flacid paralysis (the weakness). A genetic testing, electromyography (EMG), muscle biopsy and another examination has not performed.

Abstract

Acute coronary syndrome (ACS) is the multisystem of coronary artery disease. The clinical manifestation of ACS is acute myocardialinfarction (AMI). About 90% coronary attack is caused by thrombus occlusion in coronary artery. The suspect of thrombosis can bediagnosed definitely by Angiography, but it is invasive. D-dimer used to measure thrombosis abnormality and fibrinolisys. The result oftheir correlation between D-dimer with ACS is still controversived. The aim of this research is to analyse, the different between plasmaD-dimer level ACS with stenosis ≥ 50% and stenosis < 50%. This study used a cross sectional design. The study consist of seventeenspecimen ACS with stenosis ≥ 50% and 17 specimen with stenosis < 50%. Plasma D-dimer level was measured with quantitativeagglutination latex method. An independent t-test statistical analysis is used in this study. The average Plasma D-dimer of ACS withstenosis ≥ 50% is 960.2 ± 404.99 µg/L thus ACS with stenosis < 50% is 300.3 ± 128.75 µg/L (p = 0.00). The plasma D-dimer levelof ACS with stenosis ≥ 50% is more significant than ACS with stenosis < 50%.

Abstract

Coronary Artery Disease has a high prevalence and is frequently occurred and associated with the high mortality and morbidity. Dyslipidemia is one of the risk factors of Coronary Heart Disease (CHD). ApoB contained in very low-density lipoproteins (VLDL), intermediate-density lipoproteins (IDL), LDL and small dense LDL (sd-LDL), with one molecule of apoB in each particle. Apo A-I is the major apolipoprotein in HDL particles. The ratio of apoB/apoA-I is a balance between apoB-containing particles and potentially atherogenic apoA-I that is antiaterogenik. This study is carried to know the determination whether there are differences between apoB/ apoA-I ratio in patients dyslipidemia with ACS and non ACS. The research used a cross-sectional study design with patients dyslipidemia subjects suffering Acute Coronary Syndrome (ACS) and non ACS who enter to the Laboratory of Pathology Clinic at Dr. Moewardi Hospital between July and November 2011. To determine the pattern of data distribution, the researchers used Kolmogorov Smirnov test. For the analysis of differences in mean apoB/apoA-I ratio in the two population groups is used the T test, using a computer program, with the significance level p<0.05, 95% confidence interval. From 74 samples examined the mean age is 56.42 year old. This patients consisted of 33 males (44.6%) and 41 women (55.4%). All subjects are grouped into two groups, dyslipidemia ACS and non dyslipidemia ACS. The results showed apoB/apoA-I ratio significantly different in patients with dyslipidemia with ACS and non ACS. The mean apoB/apoA-I ratio of women and men subjects in both groups, including groups at high risk of myocardial myokard and higher than the cut-off ratio of apoB/ apoA-I (men 0.9 and women 0.8). It can be concluded that the apoB/apoA-I ratio of women and men subjects in both groups, included the high risk category for infarct myokard although lipid abnormalities are still not demonstrated to the risk of infarct myokard.

Abstract

The clinical manifestation of dengue virus infection is often not clear, varies widely from mild to severe. Exposure of dengue virus which serotype is different from a previous infection is a risk factor for the severe manifestation of dengue virus infection. Dengue hemorrhagic fever is classified into four degrees of severity based on clinical manifestations and laboratory results. Real-time RT-PCR Dengue can detect dengue virus serotype in early dengue virus infection. The aimed of this study was to prove the correlation between dengue virus serotype and degree of severity in adult patients. This study was a cross-sectional observational design done in February until July 2016. Subjects consisted of 100 dengue virus infection patients. Serum of the patients was examined using Real-time RT-PCR Dengue (Simplexa™ Dengue). It was shown that from 46 patients with DENV-3 serotype was 63%, DENV-2 serotype 17.4%, DENV-1 serotypes 17.4% and mixed infection of DENV-1 and DENV-3 serotype 2.2%. There was not any DENV-4 serotype. Dengue Hemorrhagic Fever (DHF) stage I was 47.8%, DHF stage II was 30.4%, DHF stage III was 10.9% and Dengue Fever was 10.9%. There was not any DHF stage IV. There was not enough evidence that DENV-3 correlated with the degree of severity (p= 0.510). Based on this research, DENV-3 serotype was the dominant serotype prevalent at the Dr. Soetomo Hospital. There was no correlation between viral dengue serotype and severity in dengue adult patients in this study.

Abstract

Proteinuria is a predictor of stroke in diabetic and non diabetic patients, but remains poorly documented from the available researchstudies. The aim of this study is to determine. Proteinuria in stroke (Hemorrhagic and non Hemorrhagic) patients which suffer diabeticand non diabetic. A cross sectional study had been done in stroke patients, diabetics and non diabetics who were hospitalized at theWahidin Sudirohusodo Hospital Makassar during the period of May to July 2010. Urine samples were analyzed using a dipstick method.There were 30 samples of non Hemorrhagic stroke (NHS) with diabetics: 71.4% with proteinuria; Hemorrhagic stroke (HS) with diabetics:87.5% with proteinuria (p = 0.55). Non diabetic HS: 100% with proteinuria; Non diabetic NHS: 40% with proteinuria; (p=0.04).Proteinuria in stroke could occur either in diabetic or non diabetic subjects, but proteinuria was higher in HS the existence of diabetes.Further research is needed on the role of proteinuria in stroke in terms of diagnostic or prognostic values.

Abstract

Dengue Haemorrhagic Fever (DHF) may result acute liver failure. Although these cases are rare but is more common in children, and early identification of these infections is important to reduce the morbidity as well as mortality. The aim of this study was to know the determinations and the correlation of liver functions with the grade of Dengue Haemorrhagic Fever in children. This study was conducted by cross sectional, using data from the medical records of DHF patients who had liver function tests (AST, ALT, PT, aPTT and albumin) in the Laboratory of Clinical Pathology Dr. Wahidin Sudirohusodo Hospital Makassar from the period of July 2008 to June 2010. AST, ALT and albumin were tested using Cobas Integra 400, PT and aPTT were tested using Humaclot VA, and the data were analyzed with Spearman correlation test. There were fifty-five samples: 26 (47.3%) were males and 29 (52.7%) were females. We found 15 (27.3%) grade I, 13 (23.6%) grade II, 16 (29.1%) grade III and 11 (20%) grade IV patients. In this study are found that AST and ALT increased and there is a tendency of extending aPTT value but there was no significant correlation with the grade of DHF. The mean of albumin was lower in grade IV, there was a significant correlation between albumin and the grade of DHF (r=–0.375, p<0.05). In conclusion, there was a significant correlation between albumin and the grade of DHF. It is suggested that the routine albumin examination in DHF patients should be carried on to know whether a prevention of complications is needed to reduce the possibility of mortality.

Abstract

Dyslipidemia is one of the common disorders in the diabetes mellitus (DM) patients causes cardiovascular disorders. Interleukin-12 (IL-12) is an important inflammation cytokine and elevated in type 2 DM (T2DM) which may contribute to inflammation of atherosclerotic plaque formation. Hyperglycemia, hyperlipidemia, hyperinsulinemia and oxidative stress that lead to endothelial dysfunction of atherosclerotic. The aim of this study to determine the correlation of lipid profile with IL-12 in T2DM. This study was an analytical study with cross-sectional design in 30 patients T2DM meet the inclusion and exclusion criteria and conduct blood tests at the Central Laboratory Installation of Dr. M. Djamil Hospital Padang in September 2016-August 2017. Level of lipid profile performed by the enzymatic colorimetric method. Sandwich ELISA used to measure the level of IL-12 in Biomedical Laboratory Medical Faculty of Andalas University. Spearman correlation was used, significant if p<0,05. The mean levels of total cholesterol, high-density lipoprotein, (HDL), triglycerides and low-density lipoprotein (LDL) in T2DM were  204.2 ± 42.8 mg/dL; 46.8 ± 9.3 mg/dL; 57 ± 53.4 mg/dL; 132.97 ± 41.0 mg/dL. The mean level of IL-12 in T2DM is 160.15 ± 99.05 pg/mL. Spearman correlation test of total cholesterol, (HDL), triglycerides and (LDL) with IL-12 are r= 0.228 (p=0.225); r= -0.234 (p=0.212); r= 0.415 (p=0.031); r= 0,215 (p=0.291) and analysis linear regression showed a moderate correlation between the levels triglycerides and Il-12 with r=0,512 and p<0,05. There is a moderate positive correlation between triglycerides, weak between total cholesterol and LDL with IL-12 and negative correlation between HDL with IL-12 in T2DM.

Abstract

Dengue haemorrhagic fever (DHF) is still a problem in Indonesia. It is a syndrome that in most severe form may threaten the patient’slife, primarily through increased vascular permeability due to endothelial dysfunction leading to shock. Von Willebrand factor (vWf) is ablood glycoprotein involved in haemostasis and present in the blood plasma. The vWf is reported as one of dysfunction endothelial marker.However, there is limited information about the kinetics and contribution of vWf in the pathogenesis of DHF in adult patients. In thisstudy, a serial level of vWf was measured as kinetic of plasma vWf. It is expected that, the evidence based medicine will give contributionin the management of DHF patients. Also, in the future a study will be conducted especially about the prediction of shock to know thekinetic of plasma von Willebrand factor in adult dengue haemorrhagic fever patients. A cross-sectional repeated–measurement study wasconducted from October 2007 up to January 2008 in the Department of Clinical Pathology at the Sardjito General Hospital Yogyakarta.Subjects who met the eligible criteria were selected i.e. adult patients hospitalized in the Department of Internal Medicine diagnosed asDHF based on WHO criteria and antibody anti-dengue detection. Serial measurement of plasma vWf was determined on days five (5),seven (7) and 15 using enzyme linked fluorescent assay (ELFA) principle. The resulting data was shown graphically and the differencein levels of vWf among the three groups of time was analyzed by Friedman’s test. The study results showed an increase of vWf on dayfive /5 (218.48 %), followed by 187.08 % on day seven (7). Interestingly, there was a sharp increase of vWf on day 15 (233.80 %). Inaddition, there were statistically significant different levels of vWf among those three groups (p = 0.00) in adult dengue haemorrhagicfever patients with the von Willebrand kinetic factor showing a fluctuation pattern. There is an increased level of vWf on the fifth (5)day but a decrease on the seventh (7) day. However, there is a sharp increase in the convalescence phase.

Abstract

The reference values of laboratory tests are affected by factors such as instruments and methods of tests, which are always beingdeveloped. For these reasons, each laboratory is recommended to determine their own reference values. To determine the reference valuesof complete blood count in healthy adult people, and compare them to the reference values which is taken from the references. A crosssectional study was conducted on 200 healthy adult people, aged 18-60 years, selected during blood donation. The eight haematologicalparameters were estimated using Sysmex Xt-1800i at Dr.Wahidin Sudirohusodo Hospital. The data were analyzed statistically by SPSS11.5 programs. The reference values of leukocyte, erythrocyte, haemoglobin, hematocrit, MCV, MCH, MCHC and platelet were: 4400 to10000 /µL; %:4.2–6.2 × 106/µL; &:3.8–5.5 × 106/µL; %:12.5–17.3 g/dL; &:11.8–15.4 g/dL; %:38.1–50.4 %; &:31.1–49.7 %; 80,1to 94,3 fL, 25,9–31,9 pg, 31,4–35,2 g/dL, and %:171.2–405.1 × 103/µL; &:191.8–441.5 × 103/µL, respectively. Significant differenceswere observed in the MCH and platelet values (p< 0,005). The values found in this study were similar to the reference value commonlyused in our laboratory, except for the MCH value which was lower and the platelet value which was higher..

Abstract

Infection in ICU patients can lead to a septic condition with clinical signs similar to Systemic Inflammatory Response Syndrome (SIRS). The high risk of death and high cost of sepsis is the reason to find an early marker in diagnosing sepsis. Blood culture can givea result in 1-3 days, so C reactive protein, procalcitonin and presepsin which are fast and accurate are needed to find a septic condition in SIRS patients. The aim of this study is to determine the diagnostic value of CRP, PCT and presepsin of sepsis with blood culture as the gold standard., The samples were collected from 32 clinically SIRS patients in the Dr. Kariadi Hosiptal, Semarang. The PCT level was measured using ELFA method, CRP level by PET IA method, while presepsin level by CLEA method. The determined area was under curve (AUC) and the cut off level was determined by 2×2 table to find out the sensitivity, spesificity, positive predictive value, negative predictive value and likelihood ratio of CRP, PCT and presepsin as well. The AUC of PCT, CRP and presepsin was 0.78 (cut off 4.314 ng/mL); 0.673 (cut off 10.245 mg/L) and 0.814 (cut off 1134.5 pg/mL). The presepsin level had a higher sensitivity (90%) than PCT (80%) and CRP (70%). PCT specificity was 72.73%, presepsin and CRP specificity each was 68.18%. Based on this study, AUC and sensitivity of presepsin level were found higher than the PCT and CRP level.
