Abstract

Elderly patients face various surgical risks, including complications, slower recovery, prolonged hospital stays, functional decline, and even death. Early ambulation is beneficial in reducing these risks in older adults. However, elderly patients often lack motivation to ambulate due to insufficient understanding and family support, leading to longer bed rest. Aim : To evaluate the impact of early ambulation as an intervention to enhance postoperative recovery in elderly patients. Methods: This is a systematic review study with a PICO approach. Article searches were conducted using online databases such as EBSCO, PubMed, ScienceDirect, SpringerLink, and Scopus with the keywords (Elderly OR Older Adult) AND (Post Operative OR Post Surgery) AND (Early Ambulation OR Early Mobilization) AND (Reduce Length of Stay) published between 2014–2024 and The 5 relevant articles were found. Results: Five studies indicated that the implementation of early ambulation programs in elderly surgical patients not only reduces length of stay but also improves functional capacity and decreases the incidence of complications such as pneumonia, atelectasis, and venous thrombosis. Conclusion: The studies show that early mobilization reduces hospital stays, accelerates organ function recovery, and provides psychological benefits, such as reduced postoperative anxiety.

Abstract

The elderly often experience a decline in balance, functional ability, and mobility, increasing their susceptibility to falls and serious injuries. This study aimed to identify commonly used methods for assessing fall risks in the elderly through a literature review. Methods: The review focused on individuals aged >60 years and included studies assessing fall risk factors such as physical weakness and motor reaction time improvement. Cross-sectional studies and full-text journal articles were selected from four databases (ScienceDirect, ProQuest, PubMed, and SAGE Research Methods) covering the period from January 2019 to December 2024. The inclusion criteria involved studies focusing on fall risk factors, while exclusions included COVID-19-related studies, abstracts, reviews, discussion papers, letters to editors, and articles without full texts. Results: A total of 10 journal articles involving 15,274 elderly respondents were reviewed. Demographic data included age, gender, marital status, economic status, environment, weakness, medical history, and fall history. Various assessment methods were identified, with the most frequently used being fall questionnaires (26%), MMSE (17.3%), balance and fall scales (13%), physical activity assessments (8.6%), motor reaction time tests (8.6%), and BMI evaluations (8.6%). The data showed that methods with higher usage percentages are commonly employed to determine fall risks in the elderly. Conclusions: Fall risk assessments utilize diverse methodologies, including physical tests, mental health evaluations, and interviews. Notably, fall risk measurement methods have proven effective in preventing falls in this population.

Abstract

Urinary incontinence is defined by the international continence society as a condition where uncontrollable urine output is a social or hygienic problem in the elderly and can be objectively proven. This study is looking for a model of gymnastic movements that are suitable for overcoming incontinence in the elderly, in the first year looking for gymnastic movements and getting a Vasten Puki Yoga gymnastics model that can be used as one of the effective interventions to control incontinence, which has been tested on elderly people in 5 elderly homes and get significant results without finding difficulties, which means that the Vasten Puki Yoga gymnastics movement can be used in the elderly. and in the second year the Vasten Puki Yoga gymnastics movement was used for research at Dr. Moewardi Surakarta Hospital. This study used a pre-experimental research design, namely one group pretest-postest. The population of this study were elderly people who experienced incontinence in nursing homes and poly Geriatric Hospital Dr. Moewardi Surakarta A total of 80 respondents with total sampling technique. The results of the Paired Sample T-test showed an average difference between pre-test and post-test of 8.30 with a significance value (2-tailed) of 0.000 <0.05. Yoga Vasten Puki exercise is an exercise designed with light movements for the elderly to reduce the frequency of micturition, because it can enable or activate the pelvic muscles. Yoga Vasten Puki exercise can be effective if done regularly at least twice a day and the movement is repeated 3 times for 4 weeks to reduce the frequency of micturition.

Abstract

To investigate the effect of cognitive behavioral therapy in patients with psychosis and schizophrenia. CBT plays a crucial role as an effective therapy in addressing psychotic disorders and schizophrenia by altering cognitive and behavioral patterns, reducing psychotic symptoms, enhancing self-control abilities, addressing cognitive dysfunction, and improving the overall quality of life of patients.Method: A systematic review of the effectiveness of psychological interventions in treating psychosis and schizophrenia focused on cognitive therapy has been conducted. The electronic databases Springer, Science Direct, Proquest, and Clinical Trials were searched using a combination of different search terms. Data were independently extracted by one author, and a synthesis of the results was offered. We identified articles fulfilling inclusion criteria: articles that investigate and report the role of CBT in managing and reducing symptoms and disabilities related to schizophrenia. The majority of research has been cross-sectional and has shown that increased cognitive biases in comparison to healthy controls are associated with psychoses in schizophrenia. The search date was conducted in February 2024, and the search was re-run in March 2024 for new entries.Result: 10 papers were included and reported the effectiveness of cognitive behavioral therapy approach interventions in reducing psychosis. Ultimately, no reliable factors were definitively linked to better outcomes after cognitive therapy. Many potential influences were either not confirmed in other studies or based on weak research. Conclusion: Additionally, studies often examined these factors independently, ignoring their potential interconnectedness (like age and education). Moving forward, researchers need well-designed studies to explore the true impact of these potential factors on treatment response. Understanding these factors can help determine who benefits most from cognitive behavioral therapy and ultimately lead to better outcomes.

Abstract

Mindfulness technique and lavender aromatherapy in reducing anxiety levels in cervical cancer patients undergoing chemotherapy. This research method uses a quasi-experimental design with a one-group pretest-posttest approach. A total of 49 respondents who experienced mild to moderate anxiety were involved in this study. The level of patient anxiety was measured before and after the intervention using the Hamilton Anxiety Rating Scale (HARS) questionnaire. The interventions provided included mindfulness technique training and lavender aromatherapy during chemotherapy sessions. The results showed a significant decrease in patient anxiety levels after the intervention, with the average anxiety level dropping from 43.40 to 34.26. The results of the paired sample t-test statistical test showed a p value <0.05, indicating that mindfulness and lavender aromatherapy interventions were effective in reducing anxiety in cervical cancer patients undergoing chemotherapy. Based on these findings, mindfulness and lavender aromatherapy interventions can be integrated as complementary therapies in cancer patient care to reduce anxiety during the treatment process. This study supports Calista Roy's adaptation theory, which emphasizes the importance of environmental interventions in helping patients adapt emotionally and physically to the disease. Suggestions for educational and health institutions are to integrate mindfulness techniques and lavender aromatherapy into nursing curricula and clinical practice to improve the well-being of cancer patients. These techniques can also be applied independently by the community as a simple way to manage everyday anxiety.

Abstract

Malignant melanoma (MM) is a rare skin cancer that is increasing constantly. It accounts for the most cause of death of all skin cancer. High accuracy in MM staging is crucial to give exact information about the prognosis and facilitate the use of the most effective therapy available. The gold standard for skin cancer diagnosis is tissue biopsy, therefore precisely detecting the primary tumor site is crucial. It has high risk of dissemination to regional lymph nodes and visceral organs. Lymph node involvement and distant metastasis are the key prognostic value of MM. Nuclear medicine has been used to detect various primary and metastatic cancer including skin cancer. The aim of this literature review is to discuss about the role of nuclear medicine specifically positron emission tomography (PET), single-photon emission tomography (SPECT), and planar gamma imaging for the diagnosis of MM. We gather literature searches and books related using combined keywords such as “gamma camera”, “SPECT”, “PET”, “malignant melanoma”, “histopathology”. The data we use are limited to 10 years of this review article. Nuclear medicine technique, such as lymphoscintigraphy using gamma camera or SPECT/CT is recommended for early stage of MM, while PET/CT is recommended and have high sensitivity for advance stage of MM and evaluating disease recurrence. Utilising nuclear medicine to aid in acurate diagnostic of MM will give opportunity of a better outcome.

Abstract

Acute myocardial infarction (IMA) is a leading cause of morbidity and mortality worldwide, so early diagnosis is needed to detect the disease. High sensitivity troponin assessment is a biomarker examination that can diagnose Acute Myocardial Infarction. The purpose of this study is to determine the effectiveness of the use of High Sensitivity Cardiac Troponin and as an early diagnosis of Acute Myocardial Infarction (IMA). This study is a systematic review study of 4 databases, namely Scopus, PubMed, EBSCO Host, and Sage Journals, with the keywords "high sensitivity cardiac troponin" AND "acute myocardial infarction". The results of the study followed the protocol and rules of Preferred Reporting Items for Systematic Reviews (PRISMA) and used the JBI tool to assess the quality of the articles to be analyzed. The inclusion criteria in this study are articles taken in the last 5 years using the Randomized Control Trials (RCT) method and observational studies. A literature search yielded 452 research articles, of which 10 were included after several selections in a systematic review and were eligible for analysis. The ten articles used random comparison and observation research methods. All articles show that high sensitivity cardiac troponin has high sensitivity and specificity in supporting the diagnosis of acute myocardial infarction. This shows that high sensitivity cardiac troponin is one of the biomarker examinations that has high sensitivity in identifying patients with a high risk of acute myocardial infarction. This finding is expected to be a reference for health workers to determine the initial diagnosis of Acute Myocardial Infarction (IMA).
