Abstract

Telemedicine services can be a diversion of most offline or classic consultations into online consultations through telemedicine services. The use of telemedicine continues, especially through telemedicine services provided by the Ministry of Health for patients who need it, such as about prevention of certain diseases, or patients who have experienced symptoms of certain diseases can consult through telemedicine services. This observational analytical study with a cross-sectional design uses a quantitative approach aimed at analyzing factors related to the level of satisfaction of telemedicine service users in Aceh Province. Data collection used a googleform questionnaire that was distributed using social media for 2 weeks. The sample in the study was the population of Aceh who met the inclusion criteria totaling 468 people. Sampling used the accidental sampling technique. Data analysis used a logistic regression test. factors related to the use of telemedicine application services in Aceh Province are the use of the JKN mobile application (OR=1.61; 95%CI=1.06-2.54; p-value=0.025), income < Minimum Wage (OR=1.64; 95%CI=1.13-2.37; p-value=0.008), and 4G/LTE internet network (OR=1.69; 95%CI=1.01-2.84; p-value=0.046). The most dominant factor related to the use of telemedicine is non-BPJS Kesehatan insurance users (AOR=2.42; 95%CI=0.53 – 11.05). It is expected that respondents should use the telemedicine application counseling facility provided for control and encouragement in supporting and assisting efforts to recover quickly and prevent disease. Respondents also pay attention to the type of application used because each telemedicine application has its own advantages and disadvantages.

Abstract

COVID-19 is a global pandemic that threatens everyone's health. The increase in confirmed COVID-19 cases continues to occur, including in Aceh Besar Regency. Public health intervention efforts have proven effective in reducing the incidence of COVID-19 and community leaders can be involved in these public health intervention efforts. Public health interventions are expected to cause changes in health behavior, but these behavioral changes require health beliefs. This study is a qualitative study with an exploratory approach to determine the beliefs of community leaders towards COVID-19. The number of informants in this study was 8 community leaders who were selected by purposive sampling. Data collection was carried out by conducting in-depth interviews. Data analysis in this study was carried out before entering the field, while in the field and after finishing in the field. The results of the interviews showed that the health beliefs of community leaders towards COVID-19 were influenced by the sources of information obtained, beliefs about COVID-19, the attitudes of community leaders towards people around them who were infected with COVID-19 and knowledge about the dangerous impacts of COVID-19. Good knowledge will increase the trust of community leaders and improve the attitudes of community leaders to participate in the COVID-19 vaccination which is one of the preventions of COVID-19. Community leaders are also involved in helping health workers provide information about efforts to prevent and handle COVID-19. It is hoped that community leaders will play an active role in efforts to prevent and handle COVID-19 by increasing knowledge and attitudes about COVID-19 which can be obtained through online and print information media. In addition, community leaders are expected to work together with health workers in preventing COVID-19.

Abstract

The low adoption rate of the Mobile JKN application in the operational area of BPJS Kesehatan Mataram Branch represents a major obstacle. The application was developed to improve accessibility and efficiency in digital health services, yet its usage remains low. Objective: This study aims to analyze the factors influencing user acceptance of the Mobile JKN application using the Unified Theory of Acceptance and Use of Technology (UTAUT) framework. Methods:This quantitative study employed a cross-sectional study design. The study population included all users of the Mobile JKN application within the BPJS Kesehatan Mataram Branch operational area, with a sample of 399 respondents selected through simple random sampling. Primary data was gathered using structured questionnaires, while secondary data was sourced from BPJS Kesehatan records. The questionnaire was validated with a significance level (p-value < 0.05) and reliability tested with Cronbach’s Alpha (≥ 0.7). Data analysis was conducted using the Chi-square test for bivariate analysis and regression analysis for multivariate analysis. Results: The results revealed significant relationships between user behavior (use behavior) and performance expectancy (p < 0.001), effort expectancy (p < 0.001), facilitating conditions (p < 0.001), hedonic motivation (p < 0.001), and price value (p < 0.001). However, social influence (p = 0.361) and habit (p = 0.628) were not significantly associated. The results of the multinomial logistic regression test identify that effort expectancy is the dominant factor influencing user behavior. Conclusion: This study concludes that perceived economic value, ease of use, and infrastructure support are critical for enhancing the adoption rate of Mobile JKN. Policymakers need to strengthen user education, simplify the application interface, and enhance technological infrastructure as well as integration with information systems in healthcare facilities to promote broader utilization.

Abstract

Leprosy is a chronic inflammatory disease caused by Mycobacterium leprae. Chronic infection can cause anemia of chronic disease which is characterized by normochromic normocytic morphology. This study was conducted to assess the relationship between erythrocyte index levels (MCV, MCH, and MCHC) with the morphology of anemia that occurs in leprosy patients before treatment. A cross-sectional study using secondary data from medical records and management information system applications at Sanglah Hospital. The variables of this study were age, sex, hemoglobin levels, and erythrocyte index levels (MCV, MCH, and MCHC). Data were collected after permission was granted and ethical clearance was completed. Of the 156 study samples, 106 patients were diagnosed with leprosy who had not received therapy and 50 patients were not leprosy. The mean age of the patients was 39.15 ± 12.218 years and male patients were more common than female patients. There was a positive correlation between anemia, normal erythrocyte index levels, and the leprosy group. Following are the p-values, prevalence ratios, and confidence intervals; anemia [p= 0.017; PR(95% CI)= 1,458(1.034-2.056)], MCV, MCH, and MCHC [p= 0.020; PR(95% CI)= 1,268(1.010-1.591); p= 0.015; PR(95% CI)= 1,293(1,021-1,639); p= 0.009; PR(95% CI)= 1,297(1,036-1,624)]. The results of the analysis were statistically significant and clinically important. Leprosy patients had anemia before therapy with normochromic normocytic anemia morphology in accordance with anemia of chronic disease.

Abstract

Asthma is one of the main diseases that cause patients to require hospitalization. WHO records 235 million cases of asthma worldwide each year. In Indonesia, the number of asthma sufferers is 4.5% of the total population or 12 million. At Grha Permata Ibu Hospital, there are 113 asthma sufferers out of a total of 9,654 patients. Asthma causes the airways to narrow because they are blocked by mucus/phlegm. Therefore, appropriate treatment such as pharmacological management with nebulization therapy & non-pharmacological chest physiotherapy and effective coughing techniques are needed to clear the respiratory tract of obstructing phlegm. Objectives to determine the application of effective post-nebulization clapping & coughing techniques to overcome airway clearance in asthma sufferers. The type of research is quantitative descriptive. The methods used by the author are interviews, physical examinations, & observations. The instruments used are informed consent sheets, assessments, observations & questionnaires. Before the intervention, all respondents experienced problems with airway clearance with clinical shortness of breath, coughing and difficulty expelling phlegm. After the intervention, the results showed that the clapping technique & effective coughing post nebulization were able to reduce shortness of breath, as many as 66.6% of respondents said that shortness of breath was reduced, 33.3% of respondents said they were not short of breath and 73.3% of respondents were able to expel phlegm. Implementation of clapping and coughing techniques effectively after nebulization to overcome airway clearance in asthma sufferers.

Abstract

IL-6 has been shown to enhance collagen synthesis and prolong the fibroblast proliferative phase, potentially contributing to uncontrolled keloid growth. However, despite its frequent mention in the literature as a crucial factor in keloid formation, studies specifically exploring the molecular mechanisms of IL-6 in fibroblast regulation and its interaction with other cytokines within keloid tissue remain limited. This study aims to further investigate the role of IL-6 in keloid matrix formation. This study is an analytical study conducted cross-sectionally. The samples used were patients diagnosed with keloids histopathologically based on microscopic criteria. Clinical data were taken from medical records and microscopic and immunohistochemical assessments using paraffin blocks available in the anatomical pathology laboratory of hospitals in Jambi city. There were 36 cases of keloids, aged between 12 and 42 years, mostly in women (66.7%), occurring in the trunk area (77.8%) 30 cases of recurrence (83.3%). We found 24 cases (77.8%) with strong IL-6 immunoexpression. There were 3 cases (8.3%) with no immunoexpression. There was a strong correlation between IL-6 immunoexpression and gender (p=0.03), age(0.04) and recurrence (p=0.02). The role of fibroblast cells is very important for the process of collagen formation in wound healing. The role of fibroblast cells is very important for the process of collagen formation in wound healing, where interleukin-6 in various theories and studies supports the process of collagen formation.

Abstract

Stunting is one of the prevalent public health issues in society, affecting the quality of life of children under five and the nation's future productivity. Objective: This study aims to describe the factors associated with stunting among children under five in Sukamentri Village, under the jurisdiction of Guntur Public Health Center, Garut City. Method: This research employs a descriptive quantitative design with a cross-sectional approach. The study sample comprises 26 stunted children aged 0–59 months and their parents from 21 families in Sukamentri Village, selected using total sampling. The research was conducted from August to September. Data analysis includes univariate analysis with frequency distribution. Results: The findings reveal that the majority of stunted children are female (57.69%) and aged 0–36 months (65.38%). Most mothers of stunted children have a high school education level (38.10%), monthly income exceeding the UMK Garut Regency (76.19%), and access to health insurance, predominantly BPJS (85.71%). Regarding sanitation facilities, almost all families have private toilets (95.24%), dispose of feces in drains (57.14%), and discharge wastewater into rivers or drains (85.71%). Immunization among stunted children is incomplete (53.85%), most attend integrated health posts (73.08%), and consume snacks 1–2 times daily (69.23%), with 80.77% being packaged foods. Conclusions: These findings highlight the need for multidimensional interventions to reduce stunting prevalence, including nutrition education, sanitation improvement, and strengthening family economic resilience. A holistic approach involving cross-sectoral collaboration is necessary to create a supportive environment for optimal child growth and significantly reduce stunting prevalence.
