Abstract

 Background: Oral lichen planus is a chronic inflammatory condition that only involves in the oral cavity and is mediated by the cellular immune system. The etiology of OLP is unclear but autoreactive T lymphocytes are considered to play an important role in the development of OLP. Factors like stress and psychological anxiety have been associated with OLP but their roles in the development of OLP is yet explored. Purpose: To describe stress-induced OLP immunopathogenesis and therapeutic potential. Reviews: In the early stages, the mechanism of OLP involves the expression of keratinocyte antigens or exposure to an antigen in the form of self-peptide or heat shock protein (HSP). HSP90 is the most expressed heat shock protein in the basal layer of keratinocytes and plays a role in recruitment of cellular immune cells through the production of cytokines due to TLR2/4 and CD91 activation, inflammatory cell migration due to 4 integrin activation, and increased antigen presentation due to HSP90-peptide binding to MHC class I/II. Stress as a physiological response triggers the release of the hormone cortisol from the adrenal cortex and catecholamine hormones such as epinephrine/ adrenaline and norepinephrine/noradrenaline from the adrenal medulla. Catecholamines increase the migration of T lymphocyte cells through the interaction of integrins and integrin ligands on the endothelium through the expression of 2-integrin after binding to adrenergic receptors on the cell membrane. Conclusion: Oral lichen planus is a chronic inflammatory condition caused by various factors where stress increases the migration of T lymphocyte cells on the side that expresses self-peptides and antigens through the interaction of immune cells with catecholamines. Topical nonselective beta blockers can be supporting therapy in reducing pain and size of OLP lesions.

Abstract

 Background: Peri-implantitis is one of many factors that can cause implant failure, with common cases ranging from 1%-47% and the highest incidence ranging from 10.7%- 47.2%. Mechanical debridement (MD) is currently the standard for peri-implantitis treatment. However, MD has limitations in the removal of infected tissue. Moreover, the rough texture of the implant's surface and bacteria adhesion and colonization increases the difficulty in performing MD. To overcome these limitations, adjunct therapy is needed to increase peri-implantitis treatment effectiveness. One of those adjunct therapies, photodynamic therapy (PDT), is used to destroy bacterial cells and significantly reduce inflammatory cell infiltration around the implant. Purpose: To describe the effectiveness of PDT as an adjunct therapy to MD in periimplantitis treatment through narrative review. Review: PDT is effective in reducing the number of bacteria, plaque index (PI), bleeding on probing (BOP), probing depth (PD), crestal bone loss (CBL), and excessive proinflammatory cytokines (IL-6, IL-1β, TNF-α) in patients. However, the effectiveness of PDT can be influenced by several factors, including patients' conditions, such as diabetes and smoking habits, types of photosensitizers used, and exposure time. Conclusion: PDT is an effective adjunctive therapy to MD in peri-implantitis treatment since it can improve clinical parameter values, significantly reduce P. gingivalis, and decrease proinflammatory cytokines.

Abstract

Background: Molar incisor hypomineralization (MIH) is a global dental condition. Early intake of antibiotics can increase the risk of MIH. Purpose: To assess the knowledge of physicians and dentists on MIH and its associations with antibiotics exposure during early childhood. Methods: This cross-sectional study surveyed the above health professionals utilizing an electronically distributed questionnaire. The chi-square test was used to compare differences in levels of knowledge between the study groups. Results: There were 335 participants in the study; general physicians (n=79), pediatricians (n=98) and dentists (n=158). A significantly lower proportion of general physicians and pediatricians had knowledge of MIH compared to dentists (19% and 18% vs. 82%, respectively, P<0.001). There was no statistically significant difference between all groups regarding their knowledge about the association between antibiotics prescribed during the first four years of life and MIH development (P=0.07). Conclusions: Physicians and pediatricians lacked knowledge about the dental condition of MIH. Most study respondents did not know the association between frequent antibiotic intake during early childhood and the development of MIH. Since medical practitioners are more likely to prescribe antibiotics and have a greater impact on early childhood health, raising awareness of MIH and its relationship with antibiotic exposure in early life among medical practitioners is essential.

Abstract

Background. Tuberculosis is one of the world's health problems, with larger mortality rate than HIV/AIDS. In order to control this disease, a new and inexpensive diagnostic method is needed. Oral samples, such as buccal mucosa epithelium, can be an alternative specimen, other than sputum, in the diagnosis of tuberculosis. Purpose. This study aimed to analyze the presence of Mycobacterium tuberculosis on buccal swabbing of tuberculosis patients. Method. Study was conducted on 18 new pulmonary tuberculosis patients in TB DOTS unit of Dr. Soetomo General Hospital. Each subject was swabbed on the buccal mucosa of the oral cavity for subsequent examination of PCR, AFB, and culture to detect Mycobacterium tuberculosis in the oral cavity. Result. The results showed that Mycobacterium tuberculosis was detected from 2 study subjects (11%) using PCR, whereas in AFB and culture examination methods no Mycobacterium tuberculosis was detected on buccal mucosa of the oral cavity. Conclusion. This study showed that Mycobacterium tuberculosis was detected on buccal mucosa swab of tuberculosis patients based on PCR technique, but this method is less suitable in diagnosis of pulmonary tuberculosis.

Abstract

Background: During the coronavirus disease-19 (COVID-19) pandemic, many people were afraid to go to the dentist, which affected their dental and oral health conditions. Dental cavities may hamper children's growth and development; if this condition continues, it will affect the children's dietary intake, which may impact their quality of life and, if allowed to, cause stunting. Although in this condition, we still have to take preventive precautions with dental health education in school-age children. This required increasing dental health knowledge and understanding of dental disorders, particularly. Purpose: To describe oral and dental health knowledge improved following dental health education at Muhammadiyah 4 Elementary School, which is located in Pucang, Surabaya City, East Java. Methods: Two surveyors were responsible for data collecting. This study's sample size was 105 people ranging from third to fifth grade. The seminar covered dental and oral health via a hybrid method. The participants were asked to fill out a questionnaire form for the pre-and post-test to determine the improvement of oral and dental health knowledge for the dental health education evaluation. Results: Most oral and dental health participants understood the presentation on oral and dental health education. The post-test percentage was higher (89.14%) than the pre-test rate (60.29%). Conclusion: The program findings demonstrated that oral and dental health education with a hybrid method effectively increased elementary school children's understanding of dental and oral health status.

Abstract

Background: Losing teeth as a result of tooth extraction is a concern since it can affect speech impairment, temporomandibular junction (TMJ) dysfunction, masticatory dysfunction, and psychological disorders, specifically aesthetics. A fixed denture or a removable denture must be made to replace missing teeth in order to preserve the quality of life of the patient. Purpose: to describe the improvement in oral and dental health knowledge that resulted from dental health education about how to care for dentures in the nursing home. Methods: The data collection was done by two surveyors. The sample size for this study was 50 individuals over the age of 50. Under the direction of Jagir Health Centre, Pelangi and Lestari nursing home to educate the elderly about the instruction on how to take care of dentures. For the pre- and post-tests, the participants were required to complete a questionnaire in order to assess how their knowledge of oral and dental health had changed. Results: The presentation on oral and dental health education was mostly grasped by the participants. In comparison to the pre-test rate (58%), the post-test percentage (98%) was higher. Conclusion: This study showed that enhancement of elderly awareness of their oral and dental health condition through dental health education was a successful approach.

Abstract

Background: Oral cancer is one of the most aggressive and invasive cancers with high metastatic potential. Oral cancer is cancer with the 11th highest number of cases in the world. Oral cancer is treated with chemotherapy and radiotherapy. However, this therapy causes side effects in the form of damage to normal cells in the surrounding tissue. Pomegranate extract contains polyphenols which may be great for inhibiting the development of oral cancer. Purpose: This article presents a systematic and comprehensive review of the potential of pomegranate extract as a natural product to inhibit the development of oral cancer. Review: Pomegranate extract was obtained by ethanol extraction using maceration method. The main content of pomegranate is polyphenolic compounds such as punicalagin, tannins, flavonoids, and ellagic acid. This compound reduces ATP formation, shortens the subG1 phase, and increases apoptosis. At the microcellular level, pomegranate extract can inhibit the activity of MMP-2 or MMP-9 to produce anti-proliferative, anti-angiogenesis and pro-apoptotic processes of cancer cells at concentrations of 25 and 50 μg/ml. Anti-proliferative and pro-apoptotic effects are produced through mitochondrial damage mechanisms. Exposure after 72 hours can reduce oral cell viability greater than exposure after 24 hours. Conclusion: Pomegranate extract is likely has four mechanisms to oral cancer: inhibiting the invasion, migration and growth of oral cancer cells, increasing oral cancer cell apoptosis and regulating antioxidant genes. So that this material can be used as a candidate for oral cancer therapy.

Abstract

Background: Through the recent years various types of lasers have found use in dentistry. Among them diode lasers have gained a special place in the arena of soft tissue surgery. Purpose: To substantiate the applicability of low wavelength Diode Laser in soft tissue dental surgeries. Cases: Five different cases in which soft tissue dental surgery was indicated, were included: (1) A 21-year-old female patient who underwent frenectomy; (2) A 24-year-old female patient who underwent excision of gingival polyp; (3) A 30-year-old patient who underwent Gingivectomy; (4) A 32-year-old female patient who underwent surgical crown lengthening and (5) A 28-year-old male patient who underwent operculectomy. Case Management: Each of these patients was successfully treated using low wavelength 445nm Soft Tissue Diode Laser. Conclusion: All the patients experienced normal post-operative healing without any complications. The overall outcome in each case was favourable. The use of low wavelength diode lasers is strongly suggested for soft tissue dental surgeries, owing to their ease of use, versatility, minimal thermal damage and good post-operative healing.

Abstract

Background: The salivary glands are the organs that produce saliva and have a role keeping the oral cavity moist and lubricated. Salivary glands regeneration is important for developing treatments on management of xerostomia and hyposalivation because of the irreversible damage of salivary glands. Common causes include aging, polypharmacy, autoimmune disease, including Sjogren's syndrome, and head and neck radiation, in which xerostomia and hyposalivation having a substantial impact on patient health and wellbeing. Purpose: to describe current approach of salivary glands regeneration for management of xerostomia and hyposalivation through narrative review. Review(s): A literature search using Science Direct and PubMed yielded results between 1973 and 2022, and the results were evaluated to identify the most promising approaches and for future research. The keywords of the search strategy were "salivary gland regeneration” and "xerostomia” or "hyposalivation”. Current studies have revealed potential targets in the development of salivary gland regeneration for the management of xerostomia and hyposalivation. Stem cells therapy aims to repair damaged at the cellular level, especially stimulating acini cells, multiplying and differentiating to replace dead or damaged acini cells. Bioengineering is through isolated acini cells cultured on many modified cell line and administration of bioactive compounds. Gene therapy aims to rebuild functional water channels and neutralization of inflammatory mediators. Conclusion: Several approaches of salivary glands regeneration that can be taken are stem cells therapy, bioengineering, and gene transfer.

Abstract

Background: Vitamin D deficiency in children is one of the problems most often discussed and received attention from around the world today, especially during the Corona virus disease-19 (Covid-19) pandemic. Vitamin D3 increases the production of vitamin K2 protein and activates a protein involved in bone metabolism. Vitamin K2 increases bone formation by stimulating osteoblast differentiation, regulating mineralization of the extracellular matrix, regulating bone marker gene expression, and inhibiting osteoclastogenesis. Purpose: to analyzed the effect of sun exposure, Ultraviolet-B (UV-B) and vitamin K2 supplementation on fibroblast-like cell as bone formation marker in Wistar rats (Rattus novergicus). Methods: twenty-four samples divided into 4 groups namely control group, UV-B group, vitamin K2 group, combination of UV-B and vitamin K2 group. After 21 days, extraction of lower incisors was done to examine fibroblast-like cell number "‹"‹after treatment. After being decalcified, specimens underwent histological evaluation using Haemotoxylin and Eosin staining to observe the fibroblast-like cell number. Result: Data analysis of fibroblast like cells number expression using one way analysis of variance (ANOVA) test showed a significant difference between sample groups (p<0.05). Conclusion: The combination of UV-B exposure and vitamin K2 administration group increased of fibroblast-like cells in Wistar rats (R. novergicus).
