Abstract

Financing for Sectio caesarea (SC) procedures within the INA-CBGs system is determined based on specific medical indications. When discrepancies arise between INA-CBGs reimbursement rates and hospital service rates, hospitals may experience financial losses. This study aims to describe INA-CBGs tariffs across various severity levels by considering the characteristics and clinical conditions of delivering mothers. This research employed a descriptive quantitative design. The researcher observed 142 claim data entries for complicated deliveries involving SC procedures, and the data were analyzed descriptively. The results showed that most patients were aged 26–30 years (34.51%), had a medical indication of previous SC (27.46%), were treated in class III wards (54.93%), had a length of stay of four days (66.90%), had a primary diagnosis of O34.2 (31.69%), and were categorized as severity level I (76.06%). The most common secondary diagnoses were a history of SC (12.37%) and anemia (12.37%). Hospital tariffs varied considerably, and the deficit between INA-CBGs rates and hospital rates reached approximately 40 million rupiah. Hospitals should implement strict internal validation of claims, ensure complete documentation, strengthen inter-unit coordination, and provide regular training for staff on updated claim regulations.

Abstract

The number of newly detected HIV/AIDS cases in Surakarta City has increased annually, with 425 cases reported in 2022, 453 in 2023, and 323 cases recorded up to August 2024. This study aimed to map the spatial distribution of HIV/AIDS cases to identify high-risk transmission areas, analyse spatial patterns and trends, and support more effective planning and evaluation of HIV programmes. Using a descriptive ecological design, the study employed secondary data from the Surakarta City Health Office and local AIDS-focused NGOs. Spatial analysis was conducted using overlay and buffering techniques in QGIS. The results showed varying case distributions across sub-districts, represented by colour gradients from dark red (highest case numbers) to white (lowest). Health service availability includes seven hospitals and seventeen primary health centres providing HIV testing, while 107 hotspot areas were identified among key populations, including MSM, transgender individuals, female sex workers, and people who inject drugs. Health service coverage was found to be evenly distributed across all sub-districts. The study recommends that the Health Office and AIDS-related NGOs continue spatial mapping efforts to better identify high-risk transmission areas and strengthen HIV prevention strategies in Surakarta City.

Abstract

Delays in the return of inpatient medical records at Hospital X reached 25.01%, which is far below the required standard of 100% completion within 2×24 hours. This study aims to analyze the factors contributing to these delays using a qualitative approach. The main informants were medical record officers, and data were collected through interviews, observations, and documentation, with validity ensured through source and method triangulation. The findings show that predisposing factors include mismatched educational background, limited knowledge, and staff attitudes related to the completeness of signatures from attending physicians and nurses. Enabling factors involve the long distance between inpatient wards and the medical record unit, as well as the absence of elevator facilities for transporting records. Reinforcing factors include the lack of penalties for delayed returns and insufficient understanding of the physical form and standard operating procedures for medical record return. Improvement efforts include staff training, socialization of medical record completeness requirements, procurement of dedicated transport facilities for records, and regular revision and dissemination of standard operating procedures. The hospital is advised to strengthen monitoring, improve inter-unit coordination, provide efficient record transportation systems, and implement a reward and punishment mechanism to enhance the timeliness of inpatient medical record returns.

Abstract

Medical record officers at Clinic X experience difficulties in determining disease and procedure codes because coding is performed manually, which often results in claim delays due to codes that are insufficiently specific and do not comply with BPJS regulations. Therefore, this study aims to design a UI/UX for the clinical coding module of the Clinic Information System (CIS) at Clinic X, Malang Regency, to ensure faster and more accurate code generation. This study employed a Research and Development (R&D) approach using Figma, involving three medical record officers as informants. The results indicate that the UI/UX design of the clinical coding module consists of five pages: a login page, a coding dashboard page, a coding page, a coding edit page, and a coding report page. The UI/UX interface design covers navigation, page titles, language clarity, icon consistency, colour schemes, visual appearance, layout, font type and size, and overall page structure, all of which were found to be appropriate. The CIS not only functions effectively from a technical perspective but also enhances the comfort and efficiency of healthcare personnel in managing medical data and services.

Abstract

Counselling services are an important component of higher education service delivery. Poltekkes Kemenkes Semarang provides counselling services; however, consultation outcomes have not yet been integrated into the clinic’s Electronic Medical Records (EMR), resulting in incomplete patient examination histories and difficulties for psychologists in delivering care. Therefore, the development of an EMR-integrated telepsychology system is required. This study aims to develop a telepsychology prototype integrated with EMR using a prototyping research method. Informants included clinic managers, administrative staff, and psychologists. The research stages comprised information system requirements analysis, data requirements analysis, and user interface design. The telepsychology user interface prototype was developed using Figma. The results show that the developed prototype is able to meet user needs, particularly by enabling psychologists to access psychological examination data efficiently. The telepsychology system supports online counselling services and allows psychologists to view complete patient examination histories directly through the system. In addition, telepsychology records can be accessed by general practitioners as supporting information for referral decisions to psychiatrists. The Primary Clinic of Poltekkes Kemenkes Semarang is encouraged to further enhance the telepsychology application by improving video call features and providing mental health information and self-care tips to support campus clinic services.

Abstract

Stunting is a condition with long-term impacts on cognitive function and productivity, thus requiring accurate early detection to support effective interventions. This study aims to develop a machine learning–based stunting prediction application using the Support Vector Machine (SVM) algorithm, integrated into mobile and web-based systems. The development method follows the CRISP-DM framework to ensure a structured data mining process and incorporates the Behaviour Change Wheel (BCW) to encourage positive user behaviour change. A dataset of approximately 121,000 records obtained from Kaggle was used for model training. Evaluation using black-box testing demonstrated that the application achieved excellent system, information, and service quality, with a benefit score of 98%. The application was shown to improve diagnostic efficiency, enhance the accuracy of stunting risk identification, and support the acceleration of early intervention efforts in primary healthcare centres (Puskesmas). Its implementation is expected to contribute to reducing stunting prevalence and advancing the digital transformation of primary healthcare services. It is recommended that future development focus on optimising healthcare worker training, improving the user interface, and expanding system integration and service coverage to maximise application effectiveness.

Abstract

The main challenges in developing a national health data system in Indonesia include the fact that more than 80% of healthcare facilities remain undigitalized, data fragmentation, and limited regulations on data standardization and interoperability, which are not yet aligned with the Ministry of Health’s 2024 mandate. Fulfilling this mandate requires continuous and real-time access to medical records, as well as data completeness, consistency, accuracy, and the prevention of work duplication among Electronic Medical Record (EMR) users. Intelligence Quotient (IQ), encompassing intellectual, emotional, and spiritual intelligence, significantly influences employee performance and hospital quality. This study aims to develop an IQ model using the End User Computing Satisfaction (EUCS) approach to measure job satisfaction and performance in hospitals. Data were collected through a cross-sectional questionnaire and analyzed using Structural Equation Modeling–Partial Least Squares (SEM-PLS) and Importance Performance Matrix Analysis (IPMA). The results show that IQ has a direct effect on net benefits and an indirect effect through EUCS and satisfaction, with an R-square value of 71% for satisfaction, indicating a strong influence. Efforts to enhance IQ should focus on improving problem-solving abilities, self-awareness, self- and relationship management, flexibility, resilience, and adherence to regulations.

Abstract

The accuracy of diagnostic and medical procedure coding depends on the completeness and clarity of information in medical records. A preliminary survey of 59 inpatient records showed that 23 were coded accurately, while 36 were inaccurate. This study aims to analyse factors contributing to inaccurate injury diagnosis coding at RS ‘Aisyiyah Siti Fatimah Tulangan. A descriptive design was used, with data collected through observation and interviews. Observations were conducted on 100 inpatient medical records of injury cases, and interviews involved two coders. The results showed that the fourth-character coding was accurate in 96 records (96%) and inaccurate in 4 records (4%), mainly due to non-specific diagnoses documented by physicians. For the fifth character, 53 records (53%) were coded accurately and 47 records (47%) inaccurately, primarily because the documentation did not specify whether the injury was open or closed. No records (0%) included accurate external cause codes, while all records (100%) were inaccurate, largely due to limited coder knowledge and the fact that coders were not trained medical record professionals. Hospitals are advised to improve clinical documentation, provide regular disease coding training, and strengthen collaboration between coders and physicians to enhance coding accuracy.
