Abstract

Evaluating the quality of healthcare services is crucial for improving patient satisfaction and building long-term trust in institutions within the BPJS Kesehatan ecosystem, particularly in regional healthcare facilities. This study aims to empirically analyze the simultaneous and partial effects of service quality dimensions based on the modified SERVQUAL framework (tangibles, reliability, responsiveness, assurance, and commitment) on the satisfaction of BPJS Kesehatan outpatient participants at Balimbingan Hospital, Simalungun Regency. A quantitative observational study with a cross-sectional design was conducted in March 2026. A total of 208 outpatient respondents were recruited via convenience sampling. Data were collected using a modified structured questionnaire with 25 items that had been validated and tested for reliability. Statistical analyses included univariate analysis, chi-square tests for bivariate analysis, and multiple logistic regression with the backward-elimination method for multivariate modeling (α = 0.05). Bivariate analysis showed that responsiveness (p = 0.001), reliability (p = 0.000), assurance (p = 0.002), and commitment (p = 0.000) were significantly correlated with patient satisfaction, whereas the physical aspect did not show statistical significance (p = 0.060). The final-stage multiple logistic regression model identified responsiveness and commitment as the only remaining significant predictors. Commitment emerged as the dominant determinant most influential on patient satisfaction (p = 0.003; OR = 4.781; 95% CI = 1.721 to 13.283), followed by responsiveness (p = 0.013; OR = 3.714; 95% CI = 1.322 to 10.428). Staff commitment (affective dedication) and operational responsiveness (prompt care) are critical catalysts in enhancing outpatient satisfaction at Balimbingan Hospital. Hospital management should redirect resource allocation toward professional soft skills development, empathy training, and triage optimization to maintain patient loyalty and competitive advantage.

Abstract

Digital transformation has become a priority for health systems in the Industry 4.0 era, yet its benefits are not distributed evenly across patient groups. Dr. Pirngadi General Hospital, Medan, has implemented an online queueing system through the Mobile JKN application; however, its utilisation remained low, reaching only 9.16% of outpatients (4,709 of 51,419) during January–September 2025.To analyse the implementation of the Mobile JKN online queueing programme in outpatient services using Rogers’ Diffusion of Innovations framework (relative advantage, compatibility, complexity, trialability, and observability), with particular attention to factors that may exclude vulnerable users. A qualitative case study was conducted from February to April 2026. Twelve informants (eight outpatient Mobile JKN users, one outpatient nurse, one administrative coordinator, one IT staff member, and one BPJS Kesehatan person-in-charge) were selected purposively until data saturation. Data were gathered through in-depth interviews, direct observation, and document review, and analysed using thematic analysis. Trustworthiness was established through source and technique triangulation. In terms of relative advantage, the system reduced physical queues and paper logistics and shortened administrative processing through automatic population of the hospital information system (SIMRS). Compatibility was supported by integration via Bridging System v2.0, but a misfit emerged for elderly and non-smartphone users. Complexity was the dominant barrier, centred on account activation requiring phone credit and an internet connection, and on low digital literacy among older patients. Trialability and observability were adequate, monitored through the quality rate (QR) indicator reported every 3–7 days; the QR initially fell below the >85% target but improved with continuous evaluation. The programme delivers clear efficiency gains, but its low uptake reflects a digital divide driven by patient demographics and technological constraints rather than by system design. Targeted digital-literacy support and assisted-registration services for technology-vulnerable groups are required to make the innovation genuinely inclusive.

Abstract

This study explores gender perspectives in English language learning among informatics students in a vocational higher education context. The research aims to examine how gender influences students’ participation, confidence, and use of technology in learning English. A qualitative approach with an exploratory design was employed to gain in-depth insights into students’ experiences and perceptions. Data were collected through semi-structured interviews, classroom observations, and documentation involving 12 students selected through purposive sampling, consisting of six male and six female participants. The data were analyzed using thematic analysis to identify key patterns and themes. The findings reveal that gender influences students’ engagement in English language learning, particularly in classroom participation, confidence in language use, and preferences in utilizing digital learning tools. Female students tend to demonstrate higher participation and confidence in communicative activities, while male students are more likely to experience anxiety and engage less in verbal interaction. In terms of technology use, male students tend to adopt a more instrumental approach, whereas female students prefer interactive and skill-based learning strategies. However, the study also finds that these differences are not absolute, as individual and contextual factors significantly shape students’ learning experiences. This study contributes to the literature by providing a qualitative understanding of gender dynamics in a vocational and technology-based educational setting. The findings suggest the importance of adopting inclusive and gender-responsive teaching practices to support diverse learner needs in English language classrooms. Keywords: Gender Perspectives, English Language Learning, Informatics Students, Classroom Participation
