Abstract

Hyperemesis gravidarum (HG) is a pregnancy complication characterized by excessive nausea and vomiting, which can cause dehydration, electrolyte balance disorders and weight loss. The length of stay for HG patients in hospital can vary. This study aims to analyze the factors that contribute to the length of treatment for HG patients at Raja Ahmad Tabib General Hospital Tanjungpinang, Indonesia. This research was conducted with an analytical observational design with a retrospective approach. Data was collected from medical records of HG patients treated at Raja Ahmad Tabib General Hospital Tanjungpinang in the period January 2020 to December 2022. This was done using multiple linear regression. The dependent variable is length of treatment (days). The independent variables analyzed were maternal age, parity, severity of HG, nutritional status, and accompanying complications. A total of 100 HG patients were included in this study. The average length of treatment is 3.5 days. The results of the analysis showed that the factors that contributed to the length of treatment for HG patients were maternal age (p=0.02), parity (p=0.04), and severity of HG (p=0.001). Maternal age, parity, and severity of HG are factors that contribute to the length of treatment for HG patients at Raja Ahmad Tabib General Hospital Tanjungpinang.

Abstract

Introduction: Bullous pemphigoid is the most common bullous-type autoimmune disease. Hypovitaminosis D is often associated with autoimmune disease and found in 86,7% bullous pemphigoid patients. Vitamin D supplementation in bullous pemphigoid patients ought to ameliorate its clinical symptoms. Case presentation: A-58 years old male patient with bullous pemphigoid and acute kidney injury admitted with a chief complaint of infected blisters all over the body. The patient had been treated for 1 week with worsening swallowing pain and low intake. On the 12th day of hospitalization, the patient was given calcitriol supplementation of 0,25 mcg per day. There was an improvement in clinical symptoms shown on autoimmune bullous skin disorder intensity score (ABSIS) from 57 on inpatient day 7 to 17.5 on inpatient day 20. Patient discharged on the 20th day of hospitalization with no emerging blisters and the ability to swallow minced food by mouth. Conclusion: Calcitriol supplementation of 0,25 mcg per day for 10 days helps to improve the ABSIS score in bullous pemphigoid patient with acute kidney injury. Administration of calcitriol is considered safe and well tolerated.

Abstract

Introduction: Radiation enteritis is damage to the small and large intestines caused by radiation therapy for regional cancer treatment. The incidence is around 90% in patients undergoing radiation in the pelvic and abdominal regions. This study aimed to report the effect of probiotics on radiation enteritis in endometrial cancer patients. Case presentation: A 57-year-old morbidly obese woman diagnosed with endometrial cancer underwent chemoradiation therapy. On the 21st out of 25 radiation sessions and the 2nd chemotherapy session, the patient visited the nutrition clinic at the Radiation Oncology Therapy Center with a complaint of diarrhea for the past 5 days. The diarrhea occurred approximately 10 times a day, with each bowel movement being approximately 50 ml and yellow in color. The patient was then treated with anti-diarrheal therapy, probiotics (Lactobacillus acidophilus, Bifidobacterium longum, and Streptococcus thermophilus) 2 times a day, zinc 20 mg once a day, and prescribed a low-fat and lactose-free diet. After monitoring on the 21st day, the frequency of liquid bowel movements decreased to 2 times a day, and the appetite improved. Conclusion: Patients with radiation enteritis who are given probiotics may experience a reduction in symptoms of acute diarrhea. Direct administration of probiotics increases beneficial bacteria in the digestive tract.

Abstract

Unsafe act is a worker's failure to follow correct work procedures, giving rise to the potential for work accidents. Human factors with unsafe act are the cause of 85% of work accidents. This research aims to determine factors related to unsafe act, namely K3 knowledge, K3 supervision, K3 training, and fatigue in office construction project workers by PT. X in North Jakarta. This type of research is quantitative with a cross sectional study design. The research sample used a total sampling of 72 workers. Research variables were analyzed using the Chi-square test. The highest proportion of respondents were workers who behaved safely (53%), had good K3 knowledge (66%), good K3 supervision (65%), had attended K3 training (54%), and experienced severe fatigue (54%). Bivariate results show a relationship between K3 knowledge (p-value = 0.020), K3 training (p-value = 0.001), and fatigue (p-value = 0.004) with unsafe act. There is no relationship between K3 supervision (p-value = 0.731) and unsafe act. In conclusion, K3 knowledge, K3 training, and fatigue are related to unsafe act in workers.

Abstract

Physical activity is crucial for children, and it is also useful in the prevention of obesity. In today's modern lifestyle, children are becoming more inactive, which puts them at risk of being overweight and obese. Studies have shown that physical activity and dietary changes can help reduce weight in obese children. Therefore, researchers are interested in studying the relationship between physical activity and nutritional status in children. This cross-sectional study was conducted in February-March 2024 at 5 Ciherang Elementary Schools. Children who meet the inclusion criteria will be asked questions about physical activity using the IPAQ questionnaire, followed by a measurement of body weight and height, and then nutritional status will be interpreted using the BMI per age chart from CDC. Two hundred forty-six children met the inclusion criteria, and it was found that there was no significant difference in the mean value of IPAQ between nutritional status groups based on BMI/Age (p-value = 0.844). In this study, it was found that there was no significant difference between physical activity duration and nutritional status in children.

Abstract

Childbirth anxiety is a common phenomenon experienced by primigravidae and can have a negative impact on the birthing process and the health of the mother and baby. Husband's assistance during childbirth has been proven to help reduce childbirth anxiety. This study aims to analyze the role of the husband's assistance on childbirth anxiety in primigravida at the Sikakap Health Center, Mentawai Islands, Indonesia. This research uses an analytical observational design with a cross-sectional approach. Data was collected from 100 primigravidas who gave birth at the Sikakap Health Center, Mentawai Islands, Indonesia. Data were analyzed using the Chi-square test and logistic regression. The results of the study showed that there was a significant relationship between the husband's assistance and childbirth anxiety in primigravida (p<0.001). Primigravidas who were accompanied by their husbands during childbirth had a lower risk of childbirth anxiety compared to primigravidas who were not accompanied by their husbands (OR=0.23, 95% CI: 0.09-0.51). In conclusion, Husband's assistance has been proven to reduce childbirth anxiety in primigravida. Therefore, efforts need to be made to increase husbands' participation in childbirth assistance at the Sikakap Health Center, Mentawai Islands, Indonesia.

Abstract

Keloids are abnormal cutaneous wound healing responses extending beyond the borders of the initial wound, usually appearing pink-purplish to hyperpigmented nodules or plaques with a hard consistency, irregular shape, uneven border, and smooth shiny surface. Most often occur on the chest, shoulder, upper arms, earlobes, and cheeks. This case report aims to compare a case of keloid treated with intralesional triamcinolone acetonide (TAC) alone with intralesional triamcinolone acetonide-5-fluorouracil (TAC + 5-FU) combination injection. A 21-year-old Minahasa male complains of growing pruritic scars in the back area and right and left upper arms since five years ago. Physical examination of the right and left upper arms revealed multiple hyperpigmented nodules and plaques, irregularly shaped, smooth, and shiny surfaces with defined borders and varying sizes. A clinical diagnosis of keloid was made. Treatment was initiated with weekly intralesional TAC alone on the left upper arm vs. intralesional TAC + 5-FU combination injection on the right upper arm. The evaluation was made based on the clinical and modified Vancouver scar scale. One of the most commonly used therapeutic options for keloid is TAC. However, the combination of TAC + 5-FU may be opted for due to its mechanism through the corticosteroid mechanism of action in conjunction with the antimetabolite activity of 5-FU. The combination may yield a more effective and faster outcome with fewer side effects. Intralesional combination TAC + 5-FU injection may be a therapeutic option for keloid with minimal side effects.

Abstract

Basal cell carcinoma (BCC) is the most common skin cancer, with a higher incidence than all other malignancies combined. Although metastases are rare, patients with multiple or frequently recurrent BCC can suffer from major and difficult-to-manage comorbidities. The choice of therapy in cases of skin cancer must be adjusted based on the type of histological lesion, size, location and age of the patient. No single therapy is ideal for all lesions. We report a case of BCC in a woman in a limited health facility without a dermatologist venereology oncology specialist. A 41-year-old woman came with complaints of a lump on her right cheek that had been slowly enlarging for the last 4 years and a week ago complained of itching at the edge of the lump. Local examination revealed a skin-colored nodule with surrounding telangiectasis. Even though the lump previously did not show any symptoms, the patient wanted the lump removed. The doctor decided to perform excision surgery. Histopathology results showed nodular BCC and the tumor had grown to the edge of the tissue, therefore the doctor decided to refer to an oncologist for additional surgery or radiotherapy. In conclusion, multiple BCC in women in limited health facilities is a complex challenge. Collaboration between general practitioners, specialist doctors, and other stakeholders is needed to increase access to quality health services and improve treatment outcomes for patients with BCC in remote areas.

Abstract

The high need for animal protein in Indonesia has not been met optimally. Tempeh, as a source of vegetable protein, has great potential but is less popular with some groups because of its monotonous taste. Temitel, a mixture of tempeh and eggs, is presented as an alternative processed protein source which is expected to increase protein consumption in the community. This research aims to analyze the organoleptics of Temitel as a basis for developing tempeh-based functional food products. This study used a completely randomized design (CRD) with three treatments (tempeh:egg proportions) and three replications. Organoleptic parameters observed include color, aroma, texture and taste. Data were analyzed using the ANOVA test and Duncan's advanced test. The results showed that the proportion of tempe:egg had a significant effect on the organoleptic parameters of Temitel. Temitel with a higher proportion of tempeh has a darker color, stronger tempeh aroma and taste, and a denser texture. Temitel with a higher proportion of eggs has a brighter color, stronger egg aroma and taste, and a softer texture. Temitel is an alternative processed protein source that has great potential for development. The tempeh:egg proportion can be adjusted to produce Temitel with organoleptic characteristics that suit consumer preferences. Temitel is expected to increase protein consumption in society, especially among those who don't like tempeh.

Abstract

Anorectal malformation (MA) without fistula is a form of congenital abnormality that is often found in newborn babies. This study aims to identify the characteristics and clinical features of MA without fistula based on the latest scientific evidence. A systematic search was conducted on the PubMed, Scopus, and Web of Science databases using relevant keywords. Inclusion criteria included observational studies reporting the characteristics and clinical features of MA without fistula in humans. Two independent researchers conducted study selection, data extraction, and study quality assessment using the JBI Critical Appraisal Checklist tool. A total of 25 studies (2018-2024) involving 1548 MA patients without fistula met the inclusion criteria. The majority of patients were male (62%). The average age at diagnosis is 2 days. The most frequently reported clinical symptoms were absence of anus (100%), abdominal distension (78%), and vomiting (65%). The most common classification of MA without fistula was perineal (45%), followed by vestibular (30%) and cloacal (25%). The most frequently performed definitive surgery was perineal anoplasty (55%), followed by posterior sagittal anorectoplasty (PSARP) (35%) and laparoscopic-assisted anorectoplasty (10%). The most frequently reported postoperative complications were anal stenosis (15%), surgical wound infection (10%), and rectal prolapse (5%). MA without fistula is more common in male babies. The main clinical symptoms are absence of anus, abdominal distension, and vomiting. The perineal classification is the most common. Perineal anoplasty is the most frequently performed definitive surgery. Post-operative complications that need to be watched out for are anal stenosis, surgical wound infection, and rectal prolapse.
