Muhammadiyah Journal of Geriatric (MuJG) merupakan jurnal ilmiah berkala, open-access, dengan skema double-blind peer-reviewed yang diterbitkan oleh Fakultas Kedokteran dan Kesehatan, Universitas Muhammadiyah Jakarta. MuJG mempublikasikan informasi mengenai area klinis, preventif, kuratif, pengembangan terapi, basic sciences research dan aspek sosial pada penyakit yang berkaitan dengan geriatri dan gerontologi. Tujuan utama dari jurnal ini adalah untuk membentuk forum publikasi, edukasi, pertukaran pendapat, dan untuk mempromosikan kegiatan penelitian dan publikasi secara global. MuJG merupakan open journal yang berfokus pada ilmu kedokteran geriatri dan gerontologi, basic sciences (biokimia, genetika, biologi molekuler, mikrobiologi & parasitologi, imunologi, serta ilmu kedokteran dasar lainnya tentang geriatri dan aging), penyakit degeneratif dan penyakit-penyakit yang sering dijumpai pada pasien geriatri. MuJG menerima beberapa jenis format manuskrip diantaranya artikel penelitian (original article), literatur review (review article), dan laporan kasus (case-report). Paper yang diterima dapat berbahasa Inggris maupun Bahasa Indonesia.

Abstract

Latar Belakang: Diabetes melitus (DM) merupakan penyakit metabolik yang disebabkan oleh resistensi insulin, akibatnya akan terjadi ketidakseimbangan gula dalam darah sehingga mengakibatkan peningkatan kadar konsentrasi glukosa. Moringa oleifera Lam sebagai terapi komplemen mempunyai efek antidiabetik dan antihiperglikemik serta mengevaluasi fruktosamin sebagai salah satu alternatif indikator diabetes Tujuan: mengetahui pengaruh ekstrak etanol daun kelor (Moringa oleifera Lam.) terhadap kadar fruktosamin pada tikus diabetes melitus. (Rattus norvegicus) diinduksi oleh streptozotocin-nicotinamide. Metode: Penelitian ini menggunakan metode eksperimental laboratoris dengan true experimen post test only dengan menggunakan 30 ekor tikus yang dibagi menjadi 6 kelompok dan diberi pemberian ekstrak etanol kelor (100, 200, 300 mg/kgBB) dan metformin sebagai kontrol positif, tidak ada pemberian untuk kontrol negatif dan normal. kelompok. Hasil: Rata-rata kadar fruktosamin terendah pada kelompok perlakuan yang diberi ekstrak daun kelor adalah 300 mg/kg BB dengan nilai 280,145 nmol/L. Uji one-way anova menunjukkan adanya perbedaan yang signifikan (p<0,05). Hasil post hoc test menunjukkan adanya perbedaan yang signifikan antara kelompok perlakuan dan kelompok negatif (p<0,05). Simpulan: Ekstrak etanol daun kelor dengan dosis 300 mg/kgBB paling efektif menurunkan kadar fruktosamin pada tikus model diabetes melitus.

Abstract

Background: Tuberculosis (TB) is a global health issue, with over 10 million cases worldwide, and Indonesia ranks as the second-highest country with TB cases. The comorbidity of diabetes mellitus (DM) increases the risk of TB, especially in elderly patients. TB patients with DM are challenging to treat, have low cure rates, and increase the risk of multi-drug resistance TB. Purpose: To understand the characteristic overview of elderly patients with drug-susceptible pulmonary tuberculosis along with type 2 diabetes. Method: This descriptive research utilized surveys with primary data from interviews with elderly patients at RSUD R Syamsudin SH and secondary data from the hospital, covering January 2022 to October 2023. Results: Out of 33 respondents, 20 were male and 13 female. Most (81.8%) had education up to high school, and 72.7% were unemployed. According to BMI, 36.4% had normal weight. Regarding drug adherence, 75.8% supervised it. All respondents had positive chest X-rays for TB, and 87.9% tested positive in molecular rapid tests. Random blood sugar levels showed 12 patients below 200 mg/dl. Among those tested for HbA1c, 78.8% did not undergo the exam, and 6.1% had levels <8%. Conclusion: TB DM predominantly affects elderly males. Most respondents were high school graduates, unemployed, and had normal BMI. They often took on the role of supervising drug adherence. In the Glucose Ad Random, more individuals had levels above 200, and among those tested for HbA1c, many exceeded 7%. All patients showed TB-consistent chest X-rays, with most testing positive in rapid molecular tests.

Abstract

Background: Elderly people experience a natural decline in body function (aging process) over a long period of time. One of the problems that can occur in the elderly is pressure sores. The worldwide incidence of decubitus ulcers ranges from 1 to 56, with 60 to 90% occurring in people over 65 years of age. Skin care to prevent pressure sores is a back massage using VCO (Virgin Coconut Oil) as a moisturizer. Purpose: This study aimed to determine the effect of VCO (Virgin Coconut Oil) back massage therapy on the risk of pressure sores in the elderly at the Welas Asih Home, Tasikmalaya Regency. Method: The research design used was a quasi-experimental one-group pretest and posttest using quantitative methods. This study used the Braden Scale to measure the risk of pressure ulcers in respondents. The sample for this study consisted of 18 elderly people from Panti Welas Asih who met the inclusion criteria. This test was carried out twice a day for four consecutive days, from 3 May to 8 May 2023. An analysis was performed to determine the impact of this research using a paired sample t-test. Results: obtained p-value = 0.000 < α. α value = 0.05. Conclusion: Back massage therapy with VCO is effective in preventing the risk of pressure ulcers in the elderly at Panti Welas Asih.

Abstract

Background: The pre-elderly and elderly endure several changes in the function of their body systems as they get older. Changes can have an impact on parts of pre-elderly and elderly people's lives, including sexual activity. Purposes:  The purpose of this study was to determine the description of the sexual activity of pre-elderly and elderly women who visit the General and Elderly Examination Ward of the UPTD Mengwi II Public Health Center. Methods: This study is a quantitative descriptive study involving 88 samples selected through the Purposive Sampling technique. Data were collected using the Female Sexual Function Index (FSFI) questionnaire. Result: The results of the study showed that the sexual activity of pre-elderly women had an average value of 56.33 which was considered not to have sexual dysfunction, while the sexual activity of elderly women had an average value of 53.79 which was considered to have sexual dysfunction (cut off point of 55). Age (p=0.020), education (p=0.003), disease history (p=0.000), menopause (p=0.000), and perception (p=0.000) are all associated with pre-elderly women's sexual engagement. Age (p=0.020), education (p=0.001), disease history (p=0.000), menopause (p=0.000), and perception (p=0.000) are all associated with senior women's sexual activity. Conclusion: Based on the research results, it was found that most of the low values in this study that led to sexual dysfunction were influenced by factors of medical history, the elderly who had mostly experienced menopause, and the understanding that engaging in sexual activity at an advanced age was considered taboo by the respondents.

Abstract

Background: The context of this study is that older adults face a wide range of health issues, of which pain is a common and serious one that is frequently linked to functional impairment. People 65 years of age and older have joint discomfort twice as frequently as young adults. Finding out how much joint discomfort the elderly at Posyandu Pelangi Nusantara endured was the study's main goal. Purposes: The aim of this study is to determine the prevalence of joint pain based on anatomical location among the elderly at Posyandu Pelangi Nusantara. With an average age of 65, the 66 senior respondents in this descriptive study were found in Posyandu Pelangi Nusantara, Darmo District, Surabaya. Methods: It was conducted using the simple random sampling method. The NRS (Numerical Rating Scale) is the tool used in this study. The NRS, a widely used instrument for pain assessment, has demonstrated high inter-rater reliability (Cronbach’s alpha > 0.9) and validity in previous studies. Result: According to the study's findings, 64% of participants reported experiencing pain, 41% reported having knee discomfort, 38% lower back pain, 15% shoulder pain, and 6% neck pain. This is in line with other studies showing that the majority of elderly respondents experience joint pain at a moderate level. This may occur due to several chronic conditions experienced by the elderly, such as high uric acid levels or excessive physical activity, which can increase the intensity of joint pain. To enhance the quality of life for the elderly, more study may be done to determine the causes of pain and develop strategies to lessen these concerns. Conclusion: Most of the research participants, who were the elderly residents of Posyandu Pelangi Nusantara, complained of joint pain. Knee joint pain is the most common complaint. Suggestions for future research include recommending community-based health intervention programs and conducting further studies to explore the factors causing pain.

Abstract

Background: The elderly are individuals aged ≥60 years who will experience functional changes in their body systems, including the musculoskeletal system, and put them at high risk of suffering rheumatoid arthritis. This disease causes joint pain and often affects the elderly's ability to carry out daily activities, thus having an impact on the elderly's level of independence. Purposes: This study aimed to determine the relationship between pain levels and the independence of elderly people with rheumatoid arthritis in UPTD Puskesmas Mengwi II. Methods: This research is a quantitative study that used a correlational design and a cross-sectional approach. The sample was 67 elderly people with rheumatoid arthritis who were selected by the Purposive Sampling technique. Data were collected using the Numerical Rating Scale and Barthel Index, which were analyzed using the Spearman Rank test. Result: The results showed that the average score of pain level in elderly people with rheumatoid arthritis was on a scale of 4 (moderate pain), with an average independence score of 75.37 (mild dependence). The results of bivariate analysis showed that there was a relationship between pain level and the independence of elderly people with rheumatoid arthritis (p=0.000; r= -0.591). Conclusion: the pain level of rheumatoid arthritis impacts the elderly’s activities, so the nursing services can optimize pain management so that elderly people will not experience problems in carrying out their daily activities independently.

Abstract

Background: The Elderly, as a final stage in the development of human life, will face various health problems. Some physical and psychological body functions will decline. Mental health problems that are often experienced by the elderly include anxiety. Elderly people with anxiety symptoms require special attention through handling these symptoms. If not, these symptoms can harm the psychosocial condition of the elderly. Non-pharmacological treatments that are used as alternatives to overcome anxiety such as progressive muscle relaxation and the butterfly hug. Purposes: This study aims to analyze the difference in effectiveness of progressive muscle relaxation and the butterfly hug on elderly anxiety. Methods: This research design is quantitative with a quasi-experimental approach. The sampling technique was purposive sampling, and a total of 66 respondents were selected according to the predetermined restriction criteria and divided into two intervention groups. Data normality test using the Shapiro-Wilk, hypothesis testing using the Wilcoxon, and then a different test using the Mann-Whitney. Result: The results of data analysis of respondent characteristics showed the dominance of age in both intervention groups, namely, age 60-74 years. In the progressive muscle relaxation group, as many as 26 elderly people (78.8%), and in the butterfly hug group, as many as 19 elderly people (57.6%). Female gender (100%). The statement of the Wilcoxon test in each group indicated that the significance value was 0.001 (p)=0.001 0.005. Conclusion: This study shows the results that there is no significant distinction between progressive muscle relaxation and the butterfly hug on anxiety in the elderly.

Abstract

Background: Over the past few decades, the incidence of patients experiencing acute kidney injury (AKI) has increased, necessitating inpatient care. The prognosis of AKI patients is influenced by the management they receive, requiring swift and accurate intervention to prevent poor outcomes. Current AKI diagnostic biomarkers are considered slow, affecting the timeliness of treatment. This study aims to investigate the correlation between the neutrophil-to-lymphocyte ratio (NLR) and AKI as an early biomarker that has the potential to predict AKI. Objective: This study aims to determine whether there is a correlation between NLR and the stage of AKI using the AKIN criteria and the NLR cutoff for each stage of AKI. Methods: This study is a cross-sectional study. Subjects were selected using consecutive sampling. Data were collected using secondary data from the medical records of PKU Muhammadiyah Purbalingga Hospital. Results: We conducted 101 study subjects, there were 49 male and 52 female, with an average age of 62.3 years. Cardiovascular comorbidities were the most common among AKI patients, with 44 subjects affected. A significant correlation was found between NLR and the stage of AKI, as follows: a statistical analysis using ordinal logistic regression (p=0.044), odds ratio (1.03), and multivariate AUC test (77%), cut off (17.7) with specificity (71%) and sensitivity (75%). Conclusion: There is a correlation between NLR and the stage of AKI using AKIN criteria at PKU Muhammadiyah Purbalingga Hospital.

Abstract

Background: Osteoarthritis (OA) is the most common form of arthritis in society throughout the world and is the main cause of disability in older adults. This disease has a slow development time with common main complaints such as pain, swelling and deformity in the affected area. This can result in limited movement and is a major cause of disability. Women are known to be more susceptible to developing OA, while advanced age is also a risk factor for developing OA. Purposes: This research is aiming to know the prevalence of OA in elderly women with and without obesity using the BMI calculation within the period of 2022. Methods: This research using a numerical analytic observation from secondary data of medical records with cross-sectional research design. Result: The study finds from the total of 196 patients, 133(67.9%) of them were obese whereas 63 (32.1%) weren't in obese condition. Conclusion: Therefore, that there was a significant difference between the average number of elderly women with and without obesity.
