Jurnal Klinik dan Riset Kesehatan (JK-RISK) – RSUD dr. Saiful Anwar Malang adalah jurnal open-access resmi yang dikelola oleh tim Penelitian dan Pendidikan Rumah Sakit Umum Daerah dr. Saiful Anwar Malang, Jawa Timur, Indonesia. JK-RISK menerbitkan satu volume yang terdiri atas tiga nomor setiap tahunnya yang diterbitkan pada bulan Oktober, Februari, dan Juni. JK-RISK hanya akan menerbitkan jurnal berbahasa Indonesia. Jurnal Klinik dan Riset Kesehatan – RSUD dr. Saiful Anwar Malang menerbitkan tulisan-tulisan ilmiah hasil penelitian, laporan kasus, tinjauan pustaka dalam bidang kedokteran dan kesehatan yang belum pernah diterbikan di jurnal manapun. Pernyataan kode etik publikasi JK-RISK merupakan pernyataan kode etik yang melibatkan pengelola jurnal, editor, mitra bestari serta penulis dalam proses publikasi jurnal ilmiah. Abstract

Physalis angulata is a plant from the Solanacea family that has been used as a traditional treatment for various diseases in various countries including Indonesia. Various phytochemicals have been isolated from Physalis angulata which contain carbohydrates, fats, minerals and vitamins and secondary metabolites, namely physalin, withangulatin, saponins, tannins, flavonoids and others. This plant has various pharmacological activities such as anti-inflammatory, immunomodulating, antiproliferation, antioxidant, antibacterial, and others that support extracts of leaves, fruit, stems, and roots with different phytochemicals. Since time immemorial Physalis angulata is also a traditional medicine for a variety of skin disorders, such as wounds, ulcer and inflammation. Currently, research on P. angulata for skin disorders has been developed such as adjuvant therapy for scleroderma, accelerated wound healing and irritant contact dermatitis. Given the vast pharmacological activity of Physalis angulata, this plant is highly considered a therapeutic modality for a variety of skin diseases. For this reason, research on safety and types of extracts is still needed to determine the active ingredients that are useful for each disease.

Abstract

Metastasis has been literally defined as the spread of cancer from the main body part when the cancer starts to induce other parts of the body. Approximately, one third of patients with advanced renal cell carcinoma (RCC) have presented bone metastases which are often osteolytic causing substantial morbidity, such as: pain pathological fractures, spinal cord compression and hypercalcemia. Additionally, the presence of bone metastases in RCC is also associated with a poor prognosis. This paper reports a case presentation regarding embolization in a case of a 25-year-old female patient with a pathological diagnosis of a pathological fracture at the L1 level dt MBD Process Renal Cell Carcinoma prior to spinal stabilization surgery. Embolization hence plays an important role in treating hypervascular bone metastases, providing effective means of treating hemorrhagic conditions by minimizing the bleeding supply to tumor masses. In sum, embolization could be performed either as a definitive treatment or as an adjunct to subsequent surgical management.

Abstract

Cardiopulmonary exercise test (CPET) is a non-invasive modality used to assess a person's exercise capacity. An assessment of the gas exchange mechanism that occurs in the breath-to-breath stage of respiration will describe gas exchange at the cellular level. Monitoring of gas exchange is a parameter in assessing abnormalities that cause disruption of aerobic metabolic processes in the body. These parameters in CPET assist clinicians in sorting and assessing the factors that cause dyspnea or fatigue during activity, whether disorders on the respiratory system, cardiovascular system, or muskuloskeletal side. In addition, CPET is also a modality in determining the prognosis and feasibility of heart transplantation in clients with heart failure. Peak VO2 assessment 14 mL/kg/min gave a better picture of cumulative survival in heart failure patients, and O2 pulse 10 mL/beat showed better event-free survival.

Abstract

Vitamin E has many functions for the human body. The role of vitamin E in the skin includes being an antioxidant, maintaining skin moisture, protection against sunlight, and the process of wound healing. Sources of vitamin E can be obtained from vegetable oils, nuts, seeds, meat and milk. Vitamin E deficiency can result from food intake or absorption disorders. Impaired absorption of fat such as cystic fibrosis, Crohn's disease, can cause deficiency because the carrier of vitamin E is fat. This can also affect the protective function of cell membranes against free radicals to influence organ systems. Vitamin E therapy can be both topical and systemic. Topical vitamin E used as a component of skin products has anti-inflammatory and antiproliferative effects in concentrations ranging from 2 to 20%. Vitamin E in oral form is D-α tocopherol, with an oral dose of between 50 IU and 1000 IU per day.

Abstract

The diagnosis of Lucio's leprosy is difficult because cutaneous manifestations are often atypical. Patients often come in severe conditions accompanied by Lucio phenomenon. A 33 years old man had a major complaint of purplish blue spots and ulcers accompanied by blisters for the past four days. The patient's eyebrows and eyelashes have been lost for the past year, but there were no prior histories of painless skin lesions and nerve impairment. Dermatological examination showed multiple stellate purpuric patches scattered almost throughout the body. There were also numerous bullas and erosions covered by dark necrotic areas, mainly on extremities. These involved about 40% of his Body Surface Area (BSA). The slit skin smears revealed BI 6+ and MI 1%. Hematoxylin and Eosin histopathology examination revealed a granulomatous inflammatory reaction accompanied by vasculitis. While Ziehl Neelsen's staining revealed abundant acid-fast bacilli scattered in the dermis area, some infiltrated vascular endothelial walls supported a Lucio phenomenon. The patient also had severe anemia, neutrophilia, hyperkalemia, and hypoalbuminemia. The extent of skin lesions and septic conditions complicate the management and lead to high levels of morbidity and mortality. Lucio's phenomenon is a severe leprosy reaction that is difficult to recognize with necrotizing erythematous cutaneous lesions as characteristic clinical manifestations, especially on the extremities. Extensive involvement of the lesion and sepsis were the main obstacles that were encountered during this case management. The administration of multidrug therapy for multibacillary leprosy (MDT-MB) WHO regimens are the primary therapy that can be combined with systemic corticosteroids.

Abstract

Background: Sinonasal neoplasms are uncommon. Adenoid cystic carcinoma (ACC) is the 2nd commonest sinonasal malignancy, characterized by an indolent and slow growth but relatively high local recurrence and distant metastasis rate. Surgical management followed by adjuvant radiation therapy is the mainstay of treatment for sinonasal ACC. Surgical approach is modified according to the tumor size and location.In general, prognosis of sinonasal neoplasms is poor. Factors that contribute the overall prognosis such as type of histopathological findings, absent of perineural or perivascular invasion and distant metastases, site of tumour, age of patient and treatment modality. Aim: To report a case of nasal ACC with regional metastases treated with surgical management followed radiation therapy. Case Report: A 70-years-old man came to the ENT clinic at RSUD dr. Saiful Anwar Malang with right nasal obstruction and history of right epistaxis for 12 months. The patient underwent surgical using lateral rhinotomy approach with Moure’s incision, histopatological specimen revealed a cribiform type ACC. Conclusion: Surgical management and adjuvant radiation therapy is the gold standard for sinonasal ACC Longterm evaluation is a mandatory according to the high incidence of local recurrence and distant metastases of sinonasal ACC.

Abstract

Introduction: Pneumonia is an infection of the lungs (alveoli) caused by bacteria, viruses, or fungi Method: Retrospective study conducted in Saiful Anwar Hospital on January - May 2019. Study samples are sputum, bronchoscopy specimen, or pleural fluid from pneumonia patients from the inpatient ward, outpatient care, and intensive care unit. Data was organized by Microsoft Excel 2016. Result: Pneumonia is more frequent in male (58%) than female (42%) with average of age 56-75 years old. Microbial Profile results are the most common isolate of sputum from Inpatient Ward is Klebsiella pneumoniae ss. pneumoniae Piperacillin/Tazobactam sensitive (100% sensitivity); from Outpatient Care is Klebsiella pneumoniae ss. pneumoniae Amikacin sensitive (100% sensitivity); from Intensive Care Unit is Klebsiella pneumoniae ss. pneumoniae Ceftazidim sensitive (100% sensitivity). Conclusion: The most common isolate of bronchoscopy specimen from Outpatient Care is Klebsiella pneumoniae ss. pneumoniae Ertapenem sensitive (100% sensitivity) while from the Intensive Care Unit is Pseudomonas aeruginosa. The most common isolate of pleural fluid from Inpatient Ward is Pseudomonas aeruginosa while from the Intensive Care is Escherichia coli. Antimicrobial drug sensitivity test could not be conducted on the isolate of bronchoscopy specimen from Outpatient Care, pleural fluid from Inpatient Ward as well as Intensive Care Unit because the isolate number is less than 30.

Abstract

Background: Energy-Protein High (EPH) diet given as nutrition support in infection cases, including Covid-19. Low diet acceptance impact the patient’s recovery; but it can be solved by modified the diet menu. KAYAZI (Makanan Kaya Gizi) as innovation of EPH diet menu can be applied too for covid-19’s patients. Objective: Knowing the effect of KAYAZI as EPH diet variation on food acceptability in terms of food waste and dietary satisfaction in PUS at General Hospital dr. Saiful Anwar Malang. Method: Quasy Experimental with purposive sampling. Total 33 samples for each groups (standard-TKTP diet; KAYAZI diet) based on inclusion-exclusion criteria. Food waste data obtained from observing and filling in Comstock visual form. Dietary satisfaction data obtained from filling out Likert-scale questionnaire in hedonic-scale test. Data processed by software SPSS ver.16, with hypothesis testing nonparametric Mann-Whitney test (p value <0.05). Result: percentage of food waste from KAYAZI was smaller (10.5%) than TKTP diet (25.03%); the mean value of KAYAZI satisfaction level was higher in all assessment categories than the TKTP diet. From the Mann-Whitney, p value <0.05 on food waste and dietary satisfaction for parameters: aroma, taste, color, shape, food containers and cutlery. Meanwhile, dietary satisfaction on parameters of texture and way of food presenting, the value of p> 0.05. Conclusion: The provision of KAYAZI as diet variation affects food acceptability in term of food waste and dietary satisfaction for parameters: aroma, taste, color, shape, food containers and cutlery. But it has no effect in food texture parameter and way of food presenting.

Abstract

The prevalence of sensorineural hearing loss was around 0.07% to 5.2% and increased with age. Cohlear implant was one of the modality to improve hearing function. Cochlear implant produce the electrical impulse needed for hearing. External sound was captured by the processor to be delivered to internal implant. In general, the implant was indicated for severe bilateral sensorineural hearing loss which cannot be improved with other hearing aids. The implantation procedure was relatively safe with minor and infrequent complication of vestibular disorder. The other complication was mostly iatrogenic. Despite the rare complication, the improvement of hearing and quality of life after the implantation depend on the patients, relatives and multidicsciplinary team. Cochlear implant was recommended for sensorineural hearing loss. Apart from the successful implantation, the hearing improvement tremendously depend on the patient, patients’ relatives and multidisciplinary team.

Abstract

Background: The diagnosis Candidiasis spp in patients with lung Cancer is difficult. It is related to immunocompromised status. Antimannan IgM and IgG biomarkers are used to diagnose candidiasis. The aim of this study is to determine the correlation between chemotherapy status, absolute neutrophyl count to antimannan IgM and IgG. Methods: The study design was a cross-sectional correlative analytic which included 37 patients in inpatient ward of Dr. Saiful Anwar Hospital Malang. The inclusion criteria patients aged >18 years with lung cancer who had established anatomical pathology, both non-small cell lung cancer and small cell lung cancer, and on sputum culture examination Candida spp grew with or without the growth of colonies of fungal species nor bacterial colonies. Exclusion criteria included lung cancer patients with HIV infection and autoimmune disease. Of the 37 patients who had received chemotherapy and had not received chemotherapy. The absolute neutrophil count and IgM and IgG antimannan were examined. Data chemotherapy status, absolute neutrophil count were analyzed in relation to IgM and IgG antimannan antibodies. Results: This study showed that there were not correlation between chemotherapy status with antimannan IgM (p=0.585; r=0.089) nor IgG (p=0.124; r=0.245). However, there was a positive, moderate correlation between absolute neutrophil count and antimannan IgM (p=0,042; r=0,362), but there is no correlation between absolute neutrophil count and antimannan IgG (p=0,094;r=0,277). Conclusion: There is positive moderate correlation between IgM antimannan with absolute neutropil count but no correlation with IgG antimannan. There is no correlation between IgM and IgG antimannan with chemotherapy status.
