Jurnal Klinik dan Riset Kesehatan (JK-RISK) – RSUD dr. Saiful Anwar Malang adalah jurnal open-access resmi yang dikelola oleh tim Penelitian dan Pendidikan Rumah Sakit Umum Daerah dr. Saiful Anwar Malang, Jawa Timur, Indonesia. JK-RISK menerbitkan satu volume yang terdiri atas tiga nomor setiap tahunnya yang diterbitkan pada bulan Oktober, Februari, dan Juni. JK-RISK hanya akan menerbitkan jurnal berbahasa Indonesia. Jurnal Klinik dan Riset Kesehatan – RSUD dr. Saiful Anwar Malang menerbitkan tulisan-tulisan ilmiah hasil penelitian, laporan kasus, tinjauan pustaka dalam bidang kedokteran dan kesehatan yang belum pernah diterbikan di jurnal manapun. Pernyataan kode etik publikasi JK-RISK merupakan pernyataan kode etik yang melibatkan pengelola jurnal, editor, mitra bestari serta penulis dalam proses publikasi jurnal ilmiah. Abstract

Moisturizer is the main component of daily basic skin care especially if there is a change in the epidermal barrier and reduced water content in the epidermis layer. When the amount of water drop below 10%, the skin become dry. One of herbal plants that affect skin hydration is Centella asiatica. C. asiatica is known as Pegagan in Indonesia and belongs to the Umbelliferae/Apiaceae family. Extracts from the leaves of C. asiatica have been reported to have potential as antioxidants, antimicrobial agents, collagen synthesis agents, and even act as wound healing properties. The active components of C. asiatica include pentacyclic triterpenes, particularly asiaticoside, madecassoside, asiatic acid and madecassic acid. C. asiatica is used as an active compound in skin care preparations due to its hydrating, antioxidant, anti-inflammatory, anti-cellulite and anti-aging activities. A formulation containing a concentration of C. asiatica extract showed efficacy in increasing skin moisture by increasing the hydration state of the skin surface and reducing the occurrence of transepidermal water loss (TEWL). This literature review aims to add insight about Centella asiatica and its mechanism of action that can be used as a moisturizer for the skin. Centella asiatica can be used as a cosmetic formulation that can treat dry and sensitive skin. More research is needed to identify and determine the chemical compound C. asiatica which is responsible for various therapeutic activities, especially as an effective, safe, easy and inexpensive moisturizer.

Abstract

Background: Osteosarcoma is the most common primary bone malignancy. Although the primary tumor has been resected, the patient remains at high risk of metastases. The incidence and clinicopathological characteristics of osteosarcoma need to be studied. Aim: To obtain epidemiological data that can help in diagnosis and management of osteosarcoma. Methods: This research was conducted in a retrospective descriptive manner. The data obtained from medical records at the Anatomical Pathology Laboratory, RSU Dr. Saiful Anwar Malang in 2018-2020 which met the inclusion criteria and exclusion criteria. The variables studied included gender, age, location, subtype, grade, Huvos assessment. Results: There were 68 cases of osteosarcoma that met the criterias. Most of the patients were male (52.94%). The age of the patients was spread from 8 years to 79 years. With average age was 24.56 years, the median age was 16 years, and the most in the age range of 11-25 years. The most common location was the lower extremity (82.35%). The most subtype was osteosarcoma NOS (94.12%). The most cases were high grade (94.12%). From all of cases, 3 cases were huvos graded, 1 case were grade 1, and 2 cases were grade 2. Conclusion: Most Osteosarcoma patient are men, most often in the age range of 11-25 years. The most common location is in the lower extremities, the most subtype is osteosarcoma NOS, the most grade is High grade. Most of the cases were not carried out by huvos grading.

Abstract

Corneal ulcer is an infectious condition of the cornea that involves the epithelial layer to the stroma. Steroid use to treat corneal ulcer without indication is still a massive issue especially in primary-level healthcare facility (PLHF) and may potentially cause various complications. Steroids can delay epithelial healing and induce perforation, and also neutrophil inhibition that will lead to infection exacerbation. A holistic understanding of corneal ulcers, especially the approach to diagnosis, both clinically and diagnostics work up, and effective management for patients are very important to understand, especially in PLHF. This case report aims to provide knowledge regarding the steroid management of corneal ulcers and periosteal graft use in perforated corneal ulcers. . Male, 39 year old, comes to RSUD Dr. Saiful Anwar Malang (RSSA) with chief complaints of redness, pain, and white lesion that progressively widened in left eye. Patient had a history of trauma caused by inflorescence one month ago. White lesion was first only a pinpoint lesion but progressively widened. Patient have sought medical advice to PLHF and secondary-level healthcare facility (SLHF), then referred to RSSA with potential complication occurring. Approach to diagnosis holistically and management with correct indication is important to prevent corneal ulcer perforation especially due to steroid use.

Abstract

Background: Primary cutaneous aspergillosis (PCA) is a rare invasive fungal infection of the skin that can be life-threatening. This disease is caused by Aspergillus spp and belongs to the deep mycosis (subcutaneous) group. Case report: A 63-year-old man came with complaint of wound that feels itchy, painful and exudative on right lower leg since 5 years ago. Dermatological examination showed multiple erythematous papules and plaques with verrucous surface partially covered in brownish yellow crust accompanied by xerosis and some ulcers with pus. The proposed differential diagnoses are Tuberculosis verrucosa cutis, Chromoblastomycosis and Mycetoma. The result of tissue culture showed Aspergillus niger. Biopsy result supports the diagnose of deep mycosis. The patient then diagnosed as primary cutaneous aspergillosis, treated with pulse dose Itraconazole and improved clinically within 2 weeks. Conclusion: Atypical manifestation in this patient gives diagnostic challenge for dermatologist. The importance of establishing diagnosis in patient with deep mycoses are to identify the cause, determine the appropriate treatment and prevent cimplication.

Abstract

Background: Lung malignancy is a lung disease that has the highest mortality rate in the world. An examination to determine the state of lung function is spirometry. Aim: This study aims to determine the distribution of lung function in patients with suspect of lung and mediastinal tumor at Saiful Anwar Hospital Malang based on age, sex, body mass index (BMI), and diagnosis. Methods: This research is a descriptive analytic, cross-sectional approach using medical record data from January 2018 to December 2019 and then the data would be processed by spearman test. Results: The results showed that patients with suspect lung and mediastinal tumors suffered the most in the age range of 51-60 years, the sexes were mostly male, the most diagnoses were lung tumors. Most of the interpretation results of the spirometry examination showed restriction, and normal body mass index. The results of the analysis of the relationship between pulmonary function descriptions had a p-value of 0.814 for diagnosis of suspected lung and mediastinal tumor, 0.003 for gender, and 0.360 for BMI. Conclusion: It can be concluded that there is a significant relationship between pulmonary function and gender and there is no significant relationship between pulmonary function with the diagnosis and BMI.

Abstract

Tranexamic Acid, a synthetic lysine-analogue antifibrinolytic, was first patented in 1957. In the beginning, It is indicated for use in certain conditions with abnormal bleeding or bleeding tendencies in which local or systemic hyperfibrinolysis is considered to be involved. Recently its use has been increasing in the field of dermatology especially for use in chronic urticaria and angioedema, as well as melasma. Tranexamic acid is an inhibitor of plasmin, and it is a synthetic derivative of the amino acid lysine that reversibly blocks binding sites on the plasminogen molecule, inhibiting the plasminogen activator from converting plasminogen to plasmin thereby preventing degradation of the fibrin clot. Tranexamic acid can also reduce the release of plasmin-mediated vasoactive peptides such as bradykinin and histamine so that it can help control the symptoms of urticaria and angioedema. Tranexamic acid has an important role in melasma by reducing plasmin activation in UV-exposed skin, thereby inhibiting the release of arachidonic acid and prostaglandin E2, which in turn will inhibit the melanogenesis process. Tranexamic acid has been tried topically, orally, and intradermally in the treatment of dermatological disorder with minimal adverse effects. However, more randomized trials are needed to fully elucidate the exact mechanism of action, ideal route, frequency, and duration of administration of the drug, along with its potential to treat dermatological disorders.

Abstract

Akne vulgaris merupakan masalah yang sering kali menjadi alasan pasien berobat. Akne vulgaris dapat mengganggu kualitas hidup penderitanya karena dapat menurunkan rasa percaya diri, terutama pada derajat berat. Terapi yang ada saat ini seperti retinoid dan antibiotik sering kali memicu masalah lain seperti iritasi akibat penggunaan retinoid dan resisten akibat penggunaan antibiotik topikal jangka panjang. Saat ini telah banyak dilakukan penelitian untuk mencari terapi alternatif pilihan untuk akne vulgaris, salah satu alternatif yang sedang dikembangkan adalah dengan menggunakan ekstrak dari tanaman Artemisia atau lebih banyak dikenal dengan Mugwort. Tanaman ini banyak digunakan sebagai tanaman obat tradisional karena memiliki berbagai aktivitas, seperti antioksidan, anti bakteri, anti jamur, dan anti parasit. Peran anti bakteri pada ekstrak tanaman Artemisia dapat menghambat pertumbuhan berbagai bakteri seperti Pseudomonas aeruginosa, Staphylococcus aureus, dan Propionibacterium acnes. Penggunaan ekstrak tanaman ini dilaporkan memiliki efek samping yang lebih minimal dibandingkan dengan penggunaan bahan kimia. Pemanfaatan ekstrak tanaman Artemisia pada terapi akne vulgaris dapat dipertimbangkan karena kemampuan anti bakteri yang dimilikinya.

Abstract

Background: Atopic dermatitis is a non-communicable skin disorder characterized by intense itching. It is relapsing, recurring, and can have a negative impact on the quality of life of patients and their family members. Atopic dermatitis occurs most often in early infancy and childhood. Atopic dermatitis can occur if there is dysregulation of IgE resulting in a breakdown of the epidermal barrier. There are several factors that trigger the occurrence of atopic rheumatism in children, which can come from environmental factors or medical history. Aim: For this reason, this study aims to analyze the similarity of output regarding the magnitude of risk factors for atopic dermatitis in children. Methods: This study was analyzed using a meta-analysis using PRISMA, where there were 7 of 8654 articles that met the criteria for analysis. Meanwhile, the data presentation was carried out using a forest plot with a random effect statistical model. Results: The presence of pets, children living in environments that have pets (cats or dogs) are known to increase the risk of atopic dermatitis by 5 times compared to children living in environments that do not have animals. pet. The health history factor seen from exclusive breastfeeding, where babies who are exclusively breastfed are significantly less likely to experience atopic dermatitis by 12.5 times compared to babies who are not exclusively breastfed. Conclusion: Not giving exclusive breastfeeding and pets affect atopic dermatitis in children (p<0,05).
