Abstract

Currently, hypertension is one of the most common diseases suffered by peoples. Hypertension is characterized by an increase in blood pressure above normal limits. The large number of hypertension sufferers is caused by unhealthy eating patterns, therefore, to prevent hypertension, it is necessary to carry out a hypertension diet by providing DASH diet education. The study aims to determine the effect of DASH diet education on diet compliance in hypertension sufferers. This study is a quantitative study with a quasi-experimental design of two groups pre and post test with group control. The sampling technique used purposive sampling with a total of 48 samples divided into intervention groups and control groups. The variables in this study were the provision of DASH diet education and diet compliance. The instruments used were the DASH diet education SAP, animated videos and Diet Compliance Questionnaires. The DASH instrument is a valid and reliable measuring tool. Then analyzed using the Mann Withney. The results of the Mann Withney analysis in the intervention group showed an Asymp. Sig. value of 0.000 (<0.05) which means that there is an effect of DASH diet education on hypertension diet compliance. In the control group, there was no change in the pre-post test data, so the Asymp. Sig. value did not appear in the analysis results, thus it can be concluded that there is no effect of providing DASH diet education on dietary compliance of hypertension sufferers. Based on the research results, DASH diet education can increase compliance with hypertension diet in respondents. It is expected that respondents can understand the substance of DASH diet education and continue to implement it in everyday life in order to undergo a hypertension diet program so that blood pressure remains under control.

Abstract

Effective leadership behavior in hospitals can influence nurses’ performance, including job expectations. Positive job expectations can contribute to improving the quality of health services. In addition, organizational commitment is essential as a mediating factor that can connect leadership behavior with work expectations. However, this relationship has not been studied in depth in the context of hospitals. Therefore, this study aims to analyze the influence of leadership behavior on nurses’ work expectations through organizational commitment at Jember Lung Hospital. This study used a quantitative approach with a cross-sectional design. The total sample size was 146 nurses working at the Hospital of Paru in Jember. Data were collected using a questionnaire consisting of three questions: leadership behavior, organizational commitment, and work expectations. The data were analyzed using a logistic regression analysis to explore the influence between the variables studied. This study indicated that leadership behavior positively influences organizational commitment (p < 0.00). In addition, leadership behavior also has a positive effect on organizational commitment (p < 0.001). Organizational commitment mediates the relationship between leadership behavior and work expectations with a value (p < 0.00), which indicates that organizational commitment is a significant mediator in the process. The influence of mediation was 0.000, indicating a substantial contribution of mediation to work expectations. Positive leadership behavior can increase the commitment of nursing organizations and job expectations. This study suggests that hospital management needs to pay attention to leadership behavior to improve nurses’ motivation and work expectations, ultimately impacting the quality of health services.

Abstract

Caring for dementia patients is a demanding task requiring physical, emotional, and psychological resilience. Family caregivers' ability to provide optimal care is significantly influenced by their level of self-efficacy. Higher self-efficacy is associated with better caregiving capabilities and lower stress levels. This study aimed to analyze the relationship between self-efficacy and the ability of family caregivers to provide care for dementia patients. Using a quantitative, cross-sectional design, data were collected through the Caregiver Self-Efficacy Scale (CaSES) and caregiving ability from 50 family caregivers. Data analysis involved descriptive statistics and Gamma correlation. The results showed that 50% of caregivers had high self-efficacy, and 48% demonstrated good caregiving abilities. Statistical tests revealed a significant positive relationship between caregiver self-efficacy and caregiving ability (p = 0.006, r = 0.453). Caregivers with higher self-efficacy demonstrated better caregiving skills.

Abstract

Stroke is a chronic disease that affects the quality of life and ability of daily living activities (ADL) of patients, especially in ischemic stroke patients. In Indonesia, stroke is the third largest cause of disability and death, 25.8 percent of sufferers experience total or partial paralysis. This causes sufferers to have difficulty in carrying out daily activities or Activity of daily living (ADL) and independence of self-perception (Self dependency). The purpose of this study was to determine the relationship between Activity Daily Living (ADL) and Self Dependency in non-hemorrhagic stroke patients. This study was conducted in December 2024 in the stroke unit of RSUD dr. Loekmono Hadi Kudus using a descriptive analytical design with a cross-sectional approach. The population in this study was 250 with a sample of 106 patients selected using a purposive sampling technique. Data were collected through a questionnaire that included a measurement instrument for the level of dependence in the form of a questionnaire and a measuring instrument for calculating ADL (Barthel index). Data analysis using the Gamma Correlation test. The results of the analysis using the Gamma Correlation test statistical test with the SPSS program obtained a p-value of 0.001 < (a = 0.05) with a correlation coefficient (0.963) meaning that there is a very strong relationship between daily living activities and independence in non-hemorrhagic stroke patients in the Stroke Unit of Dr. Loekmono Hadi Kudus Hospital. Independence in cases of non-hemorrhagic stroke patients in the Stroke Unit of Dr. Loekmono Hadi Kudus Hospital mostly experienced moderate dependence while patient independence was in the category of requiring severe dependence assistance.

Abstract

Early Postoperative pain remains a major clinical challenge that can delay recovery, increase healthcare costs, and diminish patients’ quality of life. Despite advancements in surgical techniques and analgesic protocols, many patients still experience moderate to severe pain in the immediate postoperative period. One key non-pharmacological intervention—early mobilization—has been shown to reduce pain intensity, enhance functional recovery, and minimize opioid use. However, the integration of early mobilization into routine postoperative care is frequently hindered by various systemic and patient-related barriers. These include anxiety about pain during movement, lack of staff training, and inconsistent implementation strategies across healthcare settings. As a result, the full benefits of early mobilization remain underutilized, particularly in vulnerable patient populations. This scoping review aimed to explore the clinical effectiveness, implementation challenges, and contextual factors affecting early mobilization in postoperative pain management. A systematic search of PubMed, Proquest, and ScienceDirect databases was conducted for peer-reviewed studies published between 2020 and 2025. Using the Joanna Briggs Institute (JBI) methodological framework, 35 studies out of 870 screened were included in the final analysis. The findings revealed that initiating mobilization within 48–72 hours after surgery significantly reduced pain and accelerated recovery, especially in orthopedic patients. However, patients undergoing abdominal or thoracic surgery experienced delayed mobilization due to incisional pain and mobility limitations. The review also identified key gaps in current research, particularly regarding the long-term effects of early mobilization on chronic pain, recovery sustainability, and its application in elderly or high-risk populations. Addressing these issues requires structured protocols, multidisciplinary involvement, and the adoption of digital health technologies to enhance implementation and accessibility.

Abstract

Chronic Kidney Disease (CKD) affects over 10% of the global population (>800 million people) and increases the risk of cardiovascular diseases such as coronary artery disease, heart failure, and stroke. One of the primary treatments for CKD is hemodialysis, with arteriovenous (AV) shunt access used in approximately 92% of procedures. AV shunt installation is a surgical procedure often associated with increased patient anxiety. In addition to pharmacological treatment, non-pharmacological approaches such as Progressive Muscle Relaxation (PMR) can be effective in reducing anxiety. This study aimed to determine the effect of PMR on the anxiety levels of patients undergoing pre-operative AV shunt installation. A quasi-experimental pretest-posttest design with a control group was used, involving 70 respondents at Jember Klinik Hospital, divided equally into intervention and control groups (n=35 each). Sampling was conducted using sequential sampling. The State-Trait Anxiety Inventory (STAI) was used to measure anxiety levels, and data were analyzed using the Wilcoxon Signed Rank Test. Results showed a significant decrease in anxiety in the intervention group after receiving PMR. The median anxiety score in the experimental group decreased from 42.66 (pre-test) to 29.57 (post-test), moving from moderate to mild anxiety. In contrast, the control group showed no significant change (pre-test median = 38.06; post-test = 40.03). The Wilcoxon test yielded a p-value of 0.000 in the intervention group, indicating a significant difference before and after the intervention. These findings support the use of PMR as an effective complementary therapy to reduce anxiety in pre-operative CKD patients undergoing AV shunt surgery.

Abstract

Electroconvulsive Therapy (ECT) remains one of the most effective interventions for severe psychiatric disorders. However, despite its established clinical efficacy, ECT continues to be associated with stigma, fear, and misinformation, which significantly impacts patient satisfaction and adherence. Nursing care plays a critical role in shaping patients' perceptions and overall satisfaction with ECT. This literature review explores the influence of nursing education on patient satisfaction throughout the ECT process, highlighting how well-trained nurses contribute to improved communication, reduced anxiety, and enhanced patient-centered care. A systematic literature search was conducted using databases such as EBSCO, ScienceDirect, ResearchGate, IEEE Access, and DOAJ, applying the PRISMA framework to identify, select, and analyze relevant studies. Articles published in English from 2019 to 2025. Findings demonstrate that structured nursing education programs focusing on procedural knowledge, therapeutic communication, and stigma reduction significantly enhance patient satisfaction across pre-treatment, intra-treatment, and post-treatment stages. Educated nurses are better equipped to deliver accurate information, address concerns, and foster trust, ultimately improving the overall patient experience. This review underscores the importance of integrating ECT-specific content into nursing curricula and ongoing professional development to ensure that psychiatric nursing practice aligns with evidence-based, patient-centered care principles.

Abstract

Self-care is a crucial aspect of COPD management, as it can improve quality of life. However, many COPD patients still face challenges in self-care due to various factors, one of which is social support. The purpose of this research is to explore in-depth the support and barriers to self-care in COPD patients. This study was a descriptive qualitative research with a phenomenological approach through in-depth interviews. Participants consisted of 6 people from RSUD Dr. Soedarso Pontianak selected by purposive sampling technique. The inclusion criteria were COPD patients diagnosed for approximately six months, able to participate in interviews, with adequate understanding of their condition, and residing in Pontianak. The exclusion criteria were patients with comorbid acute or chronic illnesses, those experiencing COPD exacerbations, or not residing in Pontianak. The research instruments used were a voice recorder (mobile phone), in-depth interview guidelines, and stationery. Data analysis used the Miles & Huberman analysis method. The study produced three main themes, namely the initial experience and process of self-care, support and expectations in self-care and barriers to self-care. This study shows that the experience of COPD patientsn self-care is influenced by supporting and inhibiting factors. Supporting factors include family role support, social environment, quality and expectations of health services that help patients in maintaining health and managing symptoms. Inhibiting factors, such as environmental influences, physical limitations, limitations in medication management and lack of social support and stress that worsen the patient's condition which hinders the patient's self-care process.
