Abstract

Manajemen perioperatif pada pasien gagal ginjal kronis (GGK) yang disertai dengan penyakit penyerta krisis hipertensi merupakan tantangan klinis yang kompleks. Team anestesi harus mempertimbangkan dengan matang manajemen perioperatif, dalam pemilihan obat anestesi, dan manajemen cairan selama operasi, terhadap efek penurunan fungsi ekskresi ginjal pada pasien penyakit gagal ginjal kronik. Seorang laki-laki dengan gagal ginjal kronis dan hemodialisis rutin 1 minggu 2x dan sudah menjalani hemodialisis selama 60 th, di jadwalkan ORIF clavicula kanan, riwayat hipertensi. Status fisik diklasifikasikan sebagai ASA IV manajemen anestesi dilakukan dengan general anestesi dengan pemasangan LMA (laringeal mask airway) dan obat-obatan yg diberikan SA 0,25 mg, fentanyl 100 mcg recofol 50mg, pemeliharaan sevoflurane 2%, N20 + oksigen dengan kosnsentrasi 2 lt/menit dengan perbandingan (50;50) obat intra operasi ondan 4 mg, santagesic 1 gr dan terpasang nikardipine kontinu 3 mg/jam. Operasi ORIF berlangsung selama 45 menit. Selama prosedur, hemodinamik pasien terpantau stabil dengan rata-rata nadi 86-89 x/menit dan tekanan darah terkontrol 130-149 mmHg dengan nicardipin kontinyu 3 cc/jam (3 mg/jam). Tidak ada kendala intra operasi, tekanan darah tertinggi 146/84 mmHg. Pasca operasi pasien dipindahkan di ruang pemulihan. Perioperative management of patients with chronic kidney disease (CKD) accompanied by concomitant hypertensive crisis is a complex clinical challenge. The anesthesia team must carefully consider perioperative management, the selection of anesthetic drugs, and fluid management during surgery, taking into account the effects of reduced renal excretory function in patients with chronic kidney disease. A male patient with chronic kidney disease undergoing routine hemodialysis twice a week for 60 years was scheduled for right clavicle ORIF. His physical status was classified as ASA IV. Anesthesia management was performed using general anesthesia with the placement of an LMA (laryngeal mask airway) and the administration of the following medications: SA 0.25 mg, fentanyl 100 mcg, recofol 50mg, maintenance with sevoflurane 2%, N2O + oxygen at a concentration of 2 L/min with a ratio of 50:50. Intraoperative medication: 4 mg of ondan, 1 g of santagesic, and continuous administration of 3 mg/hour of nicardipine. The ORIF surgery lasted 45 minutes. During the procedure, the patient's hemodynamics were monitored and remained stable with an average pulse of 86-89 beats per minute and controlled blood pressure of 130-149 mmHg with continuous nicardipine 3 cc/hour (3 mg/hour). Highest intraoperative blood pressure was 146/84 mmHg. Postoperatively, the patient was transferred to the recovery room.

Abstract

Objective: This article aims to unravel the endemicity of Dengue Hemorrhagic Fever (DHF) in West Nusa Tenggara Province through an analysis of social and environmental determinants from an epidemiological perspective. This study seeks to provide a comprehensive understanding of the factors that maintain the dengue transmission cycle in the region. Methods: This study used a narrative literature review approach, searching publications from Google Scholar, PubMed, ScienceDirect, DOAJ, and official reports such as the NTB Health Profile, Ministry of Health DHF surveillance data, and Riskesdas. Literature was selected based on its relevance to the topic of DHF endemicity, social, environmental, behavioral, and vector dynamics determinants. The analysis was conducted using narrative synthesis using an environment-based disease epidemiology approach and the Social Determinants of Health framework. Results: The study results indicate that DHF endemicity in NTB is influenced by the interaction of social, environmental, and behavioral factors. Poor sanitation conditions, household water storage practices, population density, low 3M+ PSN behavior, and limited health literacy are the dominant factors. Furthermore, seasonal rainfall, climate change, and inequitable health services contribute to the continued cycle of transmission. Urban areas such as Mataram City and West Lombok have a higher incidence due to population density and mobility. Implications: Efforts to control dengue fever endemicity in West Nusa Tenggara (NTB) require an integrated, multisectoral approach, including strengthening environmental management, community-based mosquito breeding sites, health education, improving water and sanitation infrastructure, and strengthening epidemiological surveillance systems. Limitations: As a literature review study, this research relies on the availability and completeness of secondary data. Variations in data between sources can affect the consistency of interpretation, and no quantitative statistical analysis was performed to measure the strength of the relationship between variables.

Abstract

Objective: This article aims to analyze the social determinants contributing to the incidence of tuberculosis (TB) in West Nusa Tenggara (NTB) Province using an epidemiological and health systems approach. This study was conducted to understand how social, environmental, behavioral, and health service capacity factors interconnectedly influence the transmission and sustainability of TB cases in the region. Methods: This study employed a narrative literature review method through a search of Google Scholar, PubMed, DOAJ, ScienceDirect, and official reports such as the NTB Health Profile, Tuberculosis Information System (SITB), Riskesdas (Basic Health Research), and WHO documents. Literature was selected based on its relevance to the social determinants of TB, TB epidemiology in NTB, and environmental and health system risk factors over the past 10 years. The analysis was conducted using a narrative synthesis approach based on the Social Determinants of Health framework and infectious disease epidemiology theory. Results: The study indicates that TB in NTB is influenced by a combination of socioeconomic factors such as poverty, overcrowding, poor nutrition, poor sanitation, and substandard housing conditions. Behavioral determinants such as delayed self-examination, stigma, and irregular medication adherence contribute to worsening transmission. From a health system perspective, findings indicate challenges in case finding, accurate contact tracing, surveillance quality, and long-term treatment adherence. Geographic variation and disparities in access to services between Lombok and Sumbawa exacerbate epidemiological disparities and impact TB case detection outcomes in NTB. Implications: TB control in NTB requires a multisectoral approach that combines social interventions, residential environmental improvements, strengthening food security programs, health education, and strengthening the health system through increased primary care capacity, active surveillance, and community-based patient support. Integrating health policies with the housing, social, and education sectors is key to sustainable case reduction. Limitations: This study relies on secondary data that varies across sources and does not conduct statistical analysis or meta-analysis, so the strength of relationships between variables is not presented quantitatively.

Abstract

Objective: This article aims to analyze the social determinants influencing stunting in West Nusa Tenggara (NTB) Province using an epidemiological perspective and the WHO's Social Determinants of Health (SDH) framework. This analysis is expected to provide a comprehensive overview of the social factors contributing to the high prevalence of stunting in NTB. Methods: This research is a narrative review conducted through a literature search in Google Scholar, PubMed, DOAJ, and ScienceDirect databases, as well as official reports such as Riskesdas, SSGI, the NTB Health Profile, and national policy documents. The articles analyzed were selected based on their relevance to the topic of social determinants of stunting in the past 10 years. A literature synthesis was conducted to identify key themes of social determinants contributing to stunting in NTB. Results: The analysis indicates that stunting in NTB is influenced by a number of interrelated social determinants, particularly poverty, low maternal education, limited access to clean water, poor sanitation, inadequate parenting and feeding practices, early marriage, and unequal access to health services. Environmental factors such as drought and household food security contribute to exacerbating children's vulnerability to chronic malnutrition. These social determinants form a complex network of upstream and downstream factors that influence child growth in NTB. Implications: These findings emphasize that efforts to reduce stunting in NTB must be carried out through a multisectoral approach that focuses not only on direct nutrition interventions but also on improving socioeconomic factors, sanitation, health access, women's education, and policies to prevent child marriage. Upstream interventions targeting structural factors need to be strengthened to create long-term impact. Limitations: As a narrative review study, this research relies on the quality and comprehensiveness of the available literature. Specific data for NTB in recent years still vary across sources, requiring careful interpretation. This study did not include quantitative analysis or meta-analysis, thus not providing an estimate of the strength of the relationship between variables.

Abstract

This study aims to determine the effect of Profitability, Leverage, Sales Growth on Tax Avoidance in food and beverage sub-sector manufacturing companies listed on the Indonesia Stock Exchange (IDX) in the 2019-2022 period. The method used in this study is a descriptive method with a quantitative approach. With a total sample population of 16 companies using a purposive sampling technique. Data collection and analysis methods used in this study used the Statistical Product and Service Solution (SPSS) 26 program. The results of this study indicate that Profitablility has an effect on Tax Avoidance, Leverage does not have effect on Tax Avoidance, Sales Growth has an effect on Tax Avoidance.

Abstract

Negara Indonesia merupakan negara yang berdasarkan atas Pancasila, yang salah satu sila-nya adalah Sila Ketuhanan Yang Maha Esa. Prinsip ini meminta seluruh warga negara untuk bertuhan, tanpa memaksakan agama tertentu untuk diyakini. Hal ini menyebabkan Indonesia menjadi negara yang penduduknya memeluk berbagai macam agama. Perpindahan agama pun menjadi sesuatu yang banyak terjadi di masyarakat. Penelitian ini merumuskan tentang hak waris yang dimiliki oleh orang yang telah murtad, berdasarkan hukum islam dan hukum perdata nasional.

Abstract

The purpose of this study is to analyze the supply chain at Coffee Shop Ordinary in Sukabumi and identify waste in the business process. The method used is a qualitative approach with Value Stream Mapping (VSM) as the main analytical tool. The object of the study is Coffee Shop Ordinary, a business that has been operating for 29 years and serves students in Sukabumi. Data were collected through semi-structured. Observations, operational document analysis, and semi-structured interviews. The results of the value stream mapping indicate that the total lead time is approximately 45 minutes per order, consisting of 5 minutes of waiting time, 20 minutes of added activities (VA), such as preparing and serving coffee, and 20 minutes of non-added activities (NVA), which include order queues and equipment setup. Process Cycle Efficiency (PCE) was calculated at 44%, which indicates that Coffee Shop Ordinary's supply chain management is still below the ideal target of approximately 60% for a cafe business, so there is an opportunity for lean improvement

Abstract

Diabetes Mellitus can occur in all age and socioeconomic strata. The number of metabolic diseases worldwide has increased by 25.0%. This study aimed to examine the effect of health education using peer group discussions on dietary adherence and oral antidiabetic medication intake in diabetes mellitus patients at the Fatukanutu Community Health Center, Kupang Regency. The research design used in this study was a one-group pre-post test design. The aim was to determine the effect of health education using peer group discussions on dietary adherence and oral antidiabetic medication intake in diabetes mellitus patients at the Fatukanutu Community Health Center, Kupang Regency. The independent variable in this study was the provision of health education using peer group discussions on the diabetes diet and taking antidiabetic medication, while the dependent variable was the dietary adherence of diabetes mellitus patients and taking antidiabetic medication at the Fatukanutu Community Health Center, Kupang Regency. The sample size was 44 people. Results: The results of the study showed that most of the respondents were male (52.3%), aged over 55 years (54.5%), had a junior high school education (50.0%) and worked as self-employed (27.3%) and retired (27.3%). The Level of Diet Compliance and Taking Oral Anti-Diabetic Medication in Diabetes Mellitus Patients at the Faukanutu Health Center, Kupang Regency Before Health Education Method Peer Group Discussion. The results of the study before the implementation of the intervention showed that most of the respondents (75%) did not comply with dietary compliance and most of the respondents (84.1%) did not comply with medication compliance. The Level of Diet Compliance and Taking Oral Anti-Diabetic Medication in Diabetes Mellitus Patients at the Fatukanutu Health Center, Kupang Regency After Health Education Method Peer Group Discussion. The results of the study after the implementation of the intervention showed that most of the respondents (81.8%) complied with dietary compliance and most of the respondents (79.5%) complied with medication compliance. The Effect of Health Education Using Peer Group Discussion on Diet Compliance and Oral Antidiabetic Medication in Diabetes Mellitus Patients at the Fatukanutu Community Health Center, Kupang Regency. Conclusion: Based on the Wilcoxon test results with a p-value of 0.000 (<0.05), it can be concluded that the health education intervention using peer group discussion has an effect on improving dietary compliance and medication adherence in diabetes mellitus patients at the Fatukanutu Community Health Center, Kupang Regency
