Abstract

Background: A smoking habit mostly has spread to almost groups of societies in Indonesia. This habit tends to increase among children and adolescents. Rights to access fresh air free from smoking have been widely discussed at various world meetings. Therefore, it is necessary to have an in-depth study to increase non-smoking areas in public areas.Objective: This study is to develop a need assessment on policy makers and related stakeholders applying a non-smoking area program (in Indonesia it is called KTR/Kawasan Tanpa Rokok) at District Office of Dukun, Magelang Regency, Central Java.Methods: This study was an observational study with a qualitative approach. Its respondents were determined purposively focusing on the Head of Dukun District, some officials of the District, and its visitors.Results: This study found that a majority of respondents understood effects and impacts of smoking to their health. They requested an issuance of official letter for a non-smoking area. The Head of the District has issued a smoking ban for its officials and employees. This issuance was important to increase comfort of its visitors, employees, and officials. In addition, it could make the Head easier to develop derivative regulations.Conclusion: A defined regulation is necessary to apply the non-smoking area program. A regulation from the Regent is highly suggested, but decisions from related agencies can be considered sufficient. The government and related stakeholders should immediately make a local regulation so that the program can be widely applied in the Dukun District.

Abstract

Background: The Research is motivated by the high rate of maternal mortality in the city of Padang. One reason is the delay in the decision making of referral. The pattern of referrral by a midwife decision making is a complicated process that starts with the process of problem identification, problem-solving alternative search, and evaluation of these alternatives.Objective: The purpose of this study is to identify background reference maternity in cases of maternal mortality regarding decision making patterns of midwives in the city of Padang. Methods: This study was a qualitative study using phenomenology. Data collection was done in the work area of Andalas, Kuranji, Pauh, and Nanggalo from May-August 2016. The selection of informants used snowball sampling technique. Qualitative data were collected through in – depth interviews, and secondary data were collected through document review. Analyzed using a fixed comparison method according to Glaser and Strauss.Results: The result of the study shows that the identification of risk factors performed during antenatal care makes midwives decide to refer to hospitals. However, the referral was rejected by the hospital because there was no emergency alert to the mother. When an emergency occurs during labor, the midwife immediately refers to the patient but tends to be in an unstable patient state. Sothe midwife is blamed for not indicating on time. The decision patterns of midwives in referring maternal mothers to maternal mortality cases are influenced by screening of emergency risk factors, the principle of referral and post-referral impacts.Conclusion: Limitations of authority, identification of risk factors, and post-referral impacts make the midwife utterly wrong in making a referral decision.

Abstract

Migraine headache is one of the most frequent primary headaches that occur in children. It is estimated that 6 million children and adolescent in the United States suffer from migraine. Migraine can affect the quality of life and it can cause a financial and social burden. Migraine is often ignored by not only the parents and teacher but also the doctor. The diagnosis of migraine in children is challenging. Precise diagnosis and management are needed to avoid further impact. In this review, it will be discussed how to diagnose and to establish recent management of migraine in children.

Abstract

Background: Sausage is  ready to eat meals for children, adolescents, and older adults. The meat contained in a beef sausage might not be consistent to the one listed on the label. Fourier Transform Infrared (FTIR) spectroscopy combination with a chemometric method is one of the most used methods  to detect wild boar fat with rapid and consistent results. Results of analysis can classify fatty acid composition contained. Objective: The aim of this study is to determine the fat profile in sausage samples and differentiate the grouping of wild boar fat and beef fat in sausage preparation. Methods: The study was designed by making 7 different variations of wild boar reference sample concentrations, which were 100%, 75%, 65%, 50%, 35%, 25% and 100% beef. Six other samples were gathered from various street vendors. The results were analyzed using FTIR spectroscopy combined with chemometric with Partial Least Square (PLS) and Principal Component Analysis (PCA). Result: The results of analysis using Horizon MBTM apps showed optimal wave number within 1250-900 cm-1. The results of calibration with equation y= 0.994x + 0.334 and the value of (R2) determination was 0.998, and root mean square error of calibration (RMSEC) was 1.22%. The results of validation using parameter value of root mean square error of cross validation (RMSECV) 2.68%, and root mean square error of prediction (RMSEP) was 0.11%.Conclusion: PCA result showed that five from six samples possibly have same physical and chemical properties similar to fatty beef sausage.

Abstract

Background: Diabetes mellitus patients may have a different response tosimilar medication. Therefore, individualized treatment and monitoringof the therapy is needed to ensure the patients’ response. Alternativemedication or dose adjustment should be considered in order to achievethe therapeutic goal.Objective: The aim of this study was to find out patients’ response tomedication variation or dose adjustment.Methods: This study used descriptive design in PKU MuhammadiyahHospital and Panti Rapih hospital period October-December 2014.Participants are all patients who got pharmacological therapy for treatmentof diabetes mellitus type 2, including oral antidiabetics and/ insulin andmet the inclusion criteria. The response of therapy was checked 2 times,during check-in and check-out.Results: Analysis of response variation showed that individualizedresponse existed, in which positive response occurred in 10 patients andthe other had a negative response. The changes were dose adjustmentand medication alteration or medication addition. From 6 patients whoneeded medication adjusment, 5 patients had a positive response, andothers had a negative response. Whereas from 6 patients whose dosage ormedication was not changed, 5 of them had a positive response while onehad a negative response.Conclusion: Individualized treatment is needed to achieve a therapeuticgoal. Individualized treatment can be done through dose adjustment andmedication alteration, or medications addition.

Abstract

Background: The Prevalence of LBP in Indonesia continues to increase, while the exact cause cannot be identified. The facts show that most of LBP is caused by occupational health problems rather than organic diseases, which one of them is the habit of wearing high-heeled shoes. Besides having an aesthetic function, high-heeled shoes also give negative effects on health. Objective: To evaluate the association between the usage and duration of high heels with LBP, as well as evaluating disability due to LBP in female salespersonMethods: The study used analytic observational study with cross-sectional design that includes 123 sales promotion girls in one of department stores, Cengkareng, West Jakarta which was carried out from November to December 2015. The data was collected through the interview which includes characteristics of the subjects, complaint history of low back pain (LBP), measurement of heel heights, and anthropometry data. The intensity of pain is assessed by Visual Analogue Scale, whilst the degree of disability is assessed by using Oswestry Low Back Pain Disability Questionnaire. Data analysis is performed by using Fisher’s test with a significance level p<0.05 software SPSS for Windows version 21.Results: The majority of 20 to 25-year-old sales promotion girls with a normal nutritional status wear 5-7 cm-high-heeled shoes for more than one year. About 68% of them complain of low back pain (LBP), where 11% of subjects were obtained with suspected disability due to LBP complaint. The heel heights is not correlated with LPB complaints, but the working period of sales promotion girls is (p=0.000).Conclusion: There is no correlation between high heel shoes with low back pain. There is a relationship between the working period with low back pain.

Abstract

Backround: Diabetes mellitus (DM) is a degenerative disease predicted by WHO to increase in Indonesia from 8.4 million in 2000 to around 21.3 million in 2030. Diabetes mellitus is closely related to dyslipidemia which can increase the risk of coronary artery disease (CAD). Current DM treatment is using oral hypoglycemic drugs, insulin, or a combination of both. Many previous studies have not examined the effect of these therapies on lipid profiles, specifically total cholesterol and triglyceride levels.Objective: To determine the differences in changes in total cholesterol and triglyceride levels between the use of insulin monotherapy and combination therapy of insulin and metformin in patients with type II diabetes mellitus in Sleman Yogyakarta Hospital from January 2010 toJune 2012.Methods: This study was an observational analytic study using a retrospective cohort study method with samples (n = 64). Samples were taken by non-probability consecutive sampling, carried out in DM patients inpatient or outpatient at Sleman Yogyakarta Hospital for the period of January 2010 - June 2012. The samples were divided into two groups, insulin monotherapy group and insulin and metformin combination therapy group. The data taken was the total cholesterol and the first triglyceride level, then the second total cholesterol and triglyceride data were made after four months of getting therapy.Result: Analysis of statistical data using the Mann Whitney test for changes in total cholesterol levels in the two treatment groups showed p = 0.166, whereas for changes in triglyceride levels in the two treatment groups it was found that p = 0.840.Conclusion: There were no significant differences in changes in total cholesterol and triglyceride levels between the two treatment groups.

Abstract

Background: Acne is a skin disorder characterised by inflammation, accompanied by a blockage of the oil glands in the skin. One of the causes of acne is Staphylococcus aureus bacteria. Many plants have been widely studied as an antibacterial agent, including Garden Balsam Leaf (Impatiens balsamina L.) which could be formulated in a peel-off gel preparation.Objective : The purpose of this research was formulating an antibacterial peel-off gel mask with a variation of viscosity enhancing agent hydroxypropyl methylcellulose (HPMC), as well as testing its antibacterial activity on Staphylococcus aureus.Methods : HPMC 1-2% was formulated in peel-off gel mask of Garden Balsam leaf extract. Physical properties such as viscosity, pH, spreadability, and drying time were tested, while antibacterial activity test was conducted by the agar-diffusion method.Results : The viscosity of the entire formula was > 20,000 cP, pH 6, drying time > 20 minutes, and spreading power > 6 cm. Also, the peel-off gel mask also showed antibacterial activity on Staphylococcus aureus with average inhibition zone were > 10 mm, which indicated strong antibacterial inhibitory power.Conclussion : The concentration variation of HPMC had a significant effect on all physical properties test. HPMC 1.5% in the preparation showed the most optimum results.

Abstract

Cysticercosis occurs in porcine naturally. A human can be infected by cysticercosis if they swallow Taenia solium's egg inadvertently. These eggs can be found in vegetables or unboiled water. These eggs hatch and later will become larva that becomes cysts in muscle or brain, known as cysticercus cellulose. Neurocysticercosis is an infection of the central nervous system caused by Taenia solium at its larval stage. Indonesia is a developing country with a high incidence rate of neurocysticercosis, with the highest amount of cases found in Bali and Papua. Neurocysticercosis boasts a broad array of clinical symptoms, from silent disease to a severe, debilitating illness that leads to death. The diagnosis of neurocysticercosis itself presents a great challenge, due to the unspecific clinical symptoms, in-pathognomonic radiographic findings and serologic studies with low specificity and sensitivity. The management of neurocysticercosis consists of a combination of antiparasitic agents and supportive therapies, ranging from steroid to surgeries that can be done under certain circumstances.

Abstract

Fatal Complications and high amputation rate were reported in popliteal artery rupture cases. Diagnosis delay and revascularization failure within 6 hour of Golden Period are related to amputation and death, even when specialist intervention is available. We report a patient with popliteal artery rupture and compartment syndrome, which was successfully underwent artery repair surgery after 42 hours of delayed revascularization. This case illustrates that delayed revascularization might produce an acceptable outcome in a particular circumstance.
